Neanderthal Health and Medicine: What Fossils Can—and Cannot—Tell Us

Neanderthals were neither uniformly sick nor medically modern. Fossils show that some individuals survived serious injuries, dental disease and degenerative damage, while dental calculus preserves clues to foods, microbes, smoke and plant compounds. Together, those traces support a strong case for practical care within Neanderthal groups and possible self-medication in one particularly informative case. They do not reveal pain, a healer’s diagnosis, a complete diet or the exact treatment used.

The key is to separate an observation from the medical interpretation built on it: a healed fracture is direct evidence of bone repair, but only indirect evidence that another person nursed the injured individual.

What counts as evidence of Neanderthal health?

A skeleton is a selective medical record. Bone can remodel after a fracture, infection or long-term mechanical stress, but many illnesses affect only soft tissue or kill before repair leaves a mark. Teeth and dental calculus add different kinds of evidence: cavities, abscesses and wear are visible in the tooth itself, while trapped particles, chemical compounds, ancient DNA and oral microbes can preserve information about food and health.

  • Directness: a visible lesion is more direct than a molecular signal interpreted from residue.
  • Specificity: a named pathogen or plant compound is more specific than a general stress marker.
  • Representativeness: one individual cannot stand for all Neanderthals or all regions.
  • Alternative explanations: a plant residue may be food rather than medicine, and survival may reflect care without proving deliberate treatment.

Preservation and ecology also matter. Cold, dry or mineral-rich settings preserve different materials than warm, wet settings, and a steppe diet should not be treated as a universal Neanderthal menu.

What bones reveal about injury, disease and survival

Healed trauma proves survival, not a treatment plan

Antemortem trauma is injury that occurred while an individual was alive. When the edges of a fracture or lesion remodel, the person survived long enough for biological repair. This is valuable evidence of resilience, but it does not identify who helped, what treatment was attempted or whether recovery was complete.

Shanidar specimens preserve antemortem injuries and trauma-related degenerative joint disease. Their healed lesions show survival after serious physical damage. The bones do not record every act of nursing, the duration of assistance, or the motives of anyone who provided it.

Degenerative change records load over time

Arthritic or other degenerative changes can reflect age, repeated activity, altered movement after an injury, or several factors at once. In a person who survived trauma, degeneration may indicate that an old injury changed how the joint was used. It is not, by itself, a diagnosis of a particular occupation or a measure of suffering.

Dental disease is visible but incomplete

Tooth wear, calculus, abscesses and other lesions can reveal local oral problems. A dental abscess indicates infection in or around a tooth, yet the skeleton cannot tell us whether pain was relieved, whether an infected tooth was treated, or how long symptoms lasted. Absence of a lesion is also weak evidence: disease may have affected tissues that did not preserve or may have been fatal before bone reacted.

Did Neanderthals care for sick or injured people?

The best answer is an inference grounded in survival patterns, not a directly observed medical procedure. A person with substantial impairment who lived long enough for healing may have been able to obtain food, avoid danger and remain with a group despite reduced mobility. That does not make individual caregiving the only explanation—an injury could have been less disabling than it appears, or recovery could have occurred quickly—but repeated cases make social support plausible.

A review by Penny Spikins, Andy Needham, Lorna Tilley and Gail Hitchens (2018) examined survival with serious injury or impairment. Its authors argue that Neanderthal medical treatment and healthcare was widespread and part of a social context of strong prosocial bonds. That conclusion should be presented as a behavioral interpretation: the fossils support knowledgeable, organized assistance more strongly than they reveal a named remedy, procedure or professional healer.

What “healthcare” can mean in this context

Healthcare need not resemble a modern clinic. Assistance could have included sharing food, protecting an injured person, helping with movement, keeping someone near a fire, cleaning a wound, or using locally known plants. The evidence does not let us assign a frequency to each behavior or identify who performed it.

Dental calculus: a microscopic archive

Dental calculus is hardened plaque. Because it can trap microscopic particles and biomolecules, it sometimes preserves a wider behavioral record than bone. The record is still tiny and uneven: calculus represents only a fraction of what entered an individual’s mouth, and preservation varies by person and site.

Plant foods and cooking

Karen Hardy and colleagues reported cooked plant foods in calculus and described their results as “the first molecular evidence for inhalation of wood-fire smoke and bitumen or oil shale and ingestion of a range of cooked plant foods” (2012, Naturwissenschaften). Their findings demonstrate that plant consumption and cooking can be detected chemically, not that every detected plant was medicinal.

A 2016 paleoethnobotanical review recorded evidence for 61 plant taxa from 26 families across 17 archaeological sites. That breadth shows dietary flexibility across places and occupations. It does not mean a single Neanderthal ate all 61 taxa or that the plants were available in every season.

Smithsonian Human Origins Program educational material reports starch granules from legumes, dates and wild barley in approximately 40,000-year-old Neanderthal plaque. Starch evidence can reveal a food category even when larger plant remains are absent, but it remains a sample of oral deposits rather than a complete dietary inventory.

Regional diets were different

At Spy, calculus DNA indicated a meat-heavy steppe diet that included woolly rhinoceros and wild sheep. At El Sidrón, evidence included mushrooms, pine nuts and moss. These contrasts caution against statements such as “the Neanderthal diet was mainly meat” or “Neanderthals were primarily plant eaters.” Both may describe particular regions or individuals, not the species as a whole.

How calculus DNA reveals microbes and possible medicines

Oral microbiota track ecology and diet

Weyrich and colleagues sequenced calculus from five Neanderthals and linked regional dietary differences with differences in oral microbiota. Their study also recovered a nearly complete draft genome of Methanobrevibacter oralis dated to approximately 48,000 years ago (Nature, 2017). The result demonstrates that ancient microbial DNA can survive in calculus; it does not imply that a complete oral microbiome can be recovered from every specimen.

The authors wrote that “DNA preserved within dental calculus represents a notable source of information about the behaviour and health of ancient hominin specimens, as well as a unique system that is useful for the study of long-term microbial evolution.” Microbial DNA can identify organisms or evolutionary relationships, but detecting a microbe is not the same as proving it caused symptoms.

The El Sidrón self-medication case

One El Sidrón individual had a dental abscess and evidence of a chronic gastrointestinal pathogen. Calculus also contained signals consistent with poplar and antibiotic-producing Penicillium. The combination is compatible with self-medication: the individual may have consumed plants or materials that contained compounds active against infection or inflammation.

“Compatible with self-medication” is the strongest defensible wording. The evidence does not prove a formulated drug, a conscious pharmacological theory, a measured dose or a successful cure. Poplar could have been eaten for nourishment or taste, and Penicillium may have been present incidentally. The case is persuasive because dental disease, pathogen evidence and potentially bioactive materials occur in the same individual, but alternative explanations remain possible.

How diet studies fit together

Different methods answer different questions, so apparently conflicting results may not actually conflict.

Method What it can show Main limitation
Bone pathology Healed trauma, remodeling and some degenerative or infectious changes Many diseases leave no skeletal trace; fatal illness may end before remodeling
Tooth wear and visible dental lesions Mechanical wear, calculus, abscesses and local oral damage Cannot recover most soft-tissue symptoms, pain or treatment intent
Dental calculus residues Some cooked foods, plant particles, smoke exposure and chemical compounds Tiny, uneven sample; recovered material is not everything eaten
Calculus ancient DNA Plant traces, oral microbes and some pathogens Contamination, preservation and interpretation affect results
Nitrogen isotopes Relative position in food webs; often consistent with substantial animal-protein intake Broad signal that does not list foods or identify medicinal use
Archaeological plant remains Plants available at a site and sometimes consumed or processed there Availability and preservation do not prove an individual ate a plant

Nitrogen-isotope evidence places Neanderthals high in food webs, while tooth wear, starch and calculus reveal regional plant use. The methods are complementary: isotope data provide a broad dietary position; calculus can preserve specific local foods; neither alone reconstructs a complete lifetime menu.

What fossil evidence cannot establish

  • Average health or life expectancy: museum collections are not automatically representative populations. Unless a study defines its sample and uncertainty, do not turn a few fossils into a species-wide prevalence estimate.
  • Pain: a lesion can indicate potential injury or infection, but pain intensity and duration are not preserved.
  • Most soft-tissue disease: many respiratory, gastrointestinal, neurological and skin conditions leave no durable skeletal mark.
  • Exact treatment: healing does not distinguish splinting, rest, analgesic plants, wound cleaning, social provisioning or no deliberate intervention.
  • Complete diet: calculus samples a minute oral archive. A methods review warns that recovered material has little relationship to everything eaten, and salivary amylase can destroy cooked-starch evidence.
  • Intent: finding a bioactive plant does not prove that a Neanderthal selected it as medicine.

A practical framework for evaluating a new claim

  1. Start with the observation. Ask whether the study found a healed fracture, a visible lesion, a chemical residue, DNA or an isotope ratio.
  2. Check the sample. Note the number of individuals, sites, dates and preservation conditions. Five calculus samples cannot establish a population-wide pattern.
  3. Separate identification from causation. A named microbe may be present without being the cause of disease; a plant may be food rather than medicine.
  4. List alternatives. Consider contamination, regional ecology, incidental exposure, ordinary diet and the possibility that an injury was less disabling than its modern description suggests.
  5. State the narrowest conclusion. “Evidence compatible with self-medication” is stronger scientifically than “Neanderthals invented antibiotics,” because it matches what the material can actually support.

What the combined evidence suggests

Neanderthals experienced injuries, dental disease and degenerative damage, and some survived long enough for their bones to heal. Their calculus records varied plant and animal foods, smoke exposure and ancient oral microbes. At least one El Sidrón individual carried a combination of evidence consistent with using materials that could have had medicinal effects. Shanidar and other trauma cases make sustained social support plausible, while the 2018 healthcare review argues that such care was widespread and embedded in prosocial relationships.

Those conclusions are substantial without being romanticized. Fossils can show outcomes and residues; they rarely reveal the decisions, sensations or intentions behind them. The most reliable history of Neanderthal medicine is therefore a layered one: direct pathology first, biomolecular clues second, and carefully qualified behavioral inference last.
