Nasal Insulin Gel: Is It an Alternative to Insulin Injections?

No established nasal insulin gel is documented here as an available replacement for prescribed insulin injections. Nasal insulin has been studied, including in a small trial, but that evidence does not show that people can safely switch to it. Do not stop or change insulin without guidance from your diabetes-care clinician.

What nasal insulin gel research shows

Researchers have tested ways to deliver insulin through the nose, including formulations designed to form a gel. A 2005 study used carbopol and hydroxypropyl methylcellulose and assessed insulin release in vitro as well as effects in animals and healthy volunteers. It is formulation research, not evidence that a nasal gel is an approved or marketed treatment. The 2005 study described the approach as painless, but that author statement is not a current clinical recommendation or regulatory conclusion.

A small trial in adults with type 1 diabetes

A 2001 six-month crossover study included 16 adults with type 1 diabetes and compared nasal insulin gel with subcutaneous insulin. At month six, reported HbA1c was 8.3% during the nasal period and 8.6% during the injection period; the authors said the difference was not statistically significant. With only 16 participants, this historical result does not establish that the treatments are equivalent or that nasal gel is a safe substitute.

Tolerability was a concern: four participants withdrew because of nasal burning or persistent sinusitis, and another participant had purulent sinusitis at month six. The authors concluded that “Other galenic forms should be investigated to improve nasal tolerance and bioavailability.” The trial report therefore offers both a limited efficacy observation and a clear warning about the formulation studied.

Is a nasal insulin product available?

The cited studies do not establish an approved, currently available nasal insulin gel for routine diabetes treatment. A 2010 pharmacokinetic paper discusses Nasulin, an intranasal recombinant human insulin spray with an absorption enhancer, in the context of clinical development; it does not identify it as a marketed consumer alternative. The Nasulin paper is evidence of development research, not proof of present availability.

These sources are not an exhaustive regulator-by-regulator check for every country. If you have heard of a product locally, ask your diabetes-care clinician or pharmacist and check the relevant medicines regulator rather than assuming a research formulation is an approved treatment.

Do not confuse nasal glucagon with nasal insulin

Baqsimi is a nasal powder containing glucagon, not insulin. The FDA label indicates it for severe hypoglycemia; it is an emergency treatment, not routine insulin coverage and not a way to replace insulin injections. Its label describes a single-use intranasal device. See the FDA label for Baqsimi.

How nasal research fits among insulin delivery routes

Insulin injections remain distinct from experimental nasal formulations. The FDA’s consumer information also describes inhaled insulin as a delivery route, but inhaled insulin is not nasal gel and should not be confused with it. FDA consumer information on insulin and other injectable diabetes medicines does not establish nasal gel as a replacement.

What to do if injections are difficult

Tell your diabetes-care clinician what makes injections hard, whether that is pain, needle anxiety, practical access, or another issue. They can review appropriate treatment and delivery options for your situation. Do not stop, reduce, or substitute prescribed insulin on the basis of nasal-gel research; changing insulin without a care plan can put glucose control at risk.
