My Wife Came Home Late After Every Shift Because of Ultrasound Paperwork—So I Wrote Her a Program

Ultrasound paperwork can follow a clinician beyond the scan itself: images and measurements must be retained, findings documented, and a final report completed and routed. The title describes a personal attempt to ease that burden for someone close to the author, but the program’s features and its effect on her workday are not independently established. The broader lesson is clear: software can organize and prompt documentation, but it does not replace the professional report that belongs in the patient’s record.

Why ultrasound paperwork is more than filling in a form

Ultrasound documentation connects the examination to the clinical record. It helps preserve what was scanned, the relevant images and measurements, the findings, and any communication or follow-up that matters. The American Institute of Ultrasound in Medicine (AIUM), in its 2025-revision practice parameter published January 10, 2026, calls for a permanent, retrievable record of the examination and its interpretation.

That record can include relevant normal and abnormal images, stored in retrievable form, preferably electronically. AIUM says image records should include identifying information, facility details, examination date and time, thermal and mechanical index, and suitable anatomy, laterality, and orientation labels. A retained worksheet should include patient identifiers, the examination date and time, and the people who performed the examination and completed the worksheet. Read the AIUM practice parameter.

The final report still has a distinct professional role

AIUM states: “A signed final report with the ultrasound findings and impression should be included in the patient’s medical record and is the definitive documentation of the study.” The report may include the ordering provider and facility, clinical information and indication, exam date and time, the examination and relevant methods, procedures, measurements, findings, and a conclusion or impression.

Depending on the case, it may also need follow-up recommendations, mention of missing standard views, comparison with prior imaging, or documentation of significant communications. If local standards do not specify report availability, AIUM suggests the final report be available within 24 hours. That is guidance in the parameter, not a universal regulatory deadline.

Reporting responsibilities vary by jurisdiction and clinical setting. A guideline for sonographers in Australia and New Zealand discusses report structure, content, language, examples, and pitfalls, but it should not be read as a universal scope-of-practice rule. See the guideline.

What workflow software can—and cannot—take off someone’s plate

A well-designed ultrasound documentation workflow can reduce repetitive entry and make unfinished tasks harder to overlook. Depending on the system, software may support structured fields, measurement transfer, routing, archiving, and reminders that persist until required documentation is complete. Those are workflow functions, not clinical judgments: software should not be treated as a substitute for interpreting findings, completing the report, or obtaining the required review and sign-off.

Published examples show why workflow integration matters, but they do not establish that one program fits every department. A 2025 retrospective Cleveland Clinic Health System study reviewed 5,013 point-of-care ultrasound examinations across 12 emergency departments in 2023. After an automated EMR in-basket reminder workflow began on June 1, with reminders remaining until a procedure note was completed, procedure-note deficiency fell from 4.44% before the intervention to 1.45% afterward—a 3.0 percentage-point absolute reduction. The result belongs to that system and study design; it is not a promise of the same change elsewhere. Read the study.

A 2023 CHEST conference abstract describes a six-step POCUS process connecting an EMR order, patient verification, image export to PACS, a preliminary report, and attending review and co-signature. It reports an average of 65 documented studies per month over 22 months after implementation, compared with eight in January and February 2021 before the intervention. That is a change in documentation volume, not evidence of time saved or improved clinical outcomes. Read the abstract.

How to judge an ultrasound reporting tool

For a department considering ultrasound reporting software, the central question is whether it fits the existing clinical workflow—not simply whether it offers a form or a reminder. Evaluate the system against the actual equipment, records, responsibilities, and local requirements.

  • Privacy and retention: Does its handling of patient information meet the institution’s security, retention, and jurisdiction-specific requirements?

A 2018 implementation abstract describes mapping 128 fields across 39 templates to transfer measurements into reporting software. It reports utilization of 86–96% among technologists, 92–93% among radiologists, and 95–96% among residents in that implementation. Those adoption figures are specific to the reported project; they do not establish typical time savings or error reduction. Read the abstract.

GE HealthCare’s ViewPoint 6 product sheet describes measurement-transfer and reporting workflows and estimates about five minutes of post-exam documentation per study. That figure is a vendor estimate, not an independently established benchmark. See the product sheet. Vendor descriptions can help identify capabilities to ask about, but they are not independent validation or evidence about the personal program in the title.

What the personal software story can safely claim

The title gives the story its human reason to exist: repeated paperwork was burdensome enough that the author tried to build a practical tool for his wife. The available evidence does not establish the program’s name, features, clinical setting, adoption, or measured effect on her departure time. It would therefore be misleading to claim that the software eliminated late shifts or improved patient care.

The grounded takeaway is narrower and more useful: repetitive documentation is a real workflow problem, and thoughtful software may help with completeness, routing, structured entry, reminders, and archiving. Any tool used in clinical work still needs to fit the organization’s systems and responsibilities, preserve required records, and leave the final professional documentation to the appropriate clinician.
