The “AI doppelganger” is not a robot or a physical clone. It is a continuously updated model of one person’s metabolism. In Victoria Song’s roughly 100-day account for The Verge, Twin Health’s program combined continuous-glucose-monitoring readings, smart-scale body-composition data, medication details, blood-pressure measurements, meals and activity to predict how her glucose might respond to a food or a walk.
The result looked like a personal, color-coded food database: red for a poor predicted response, orange for a middling response and green for an ideal response for that individual. Those colors are predictions about one person’s glucose response, not universal nutrition grades.

What the AI “doppelganger” actually is
Twin Health describes its digital twin as a real-time model of a member’s metabolism. It continually analyzes data from sensors and connected devices, then produces daily guidance on food, sleep, activity and stress. The model changes as new readings arrive, so it is intended to become more personal than a static calorie target or a generic glycemic-index chart.

A bowl of protein Cheerios with milk was labeled orange when eaten by itself but green when followed by a walk. The example shows why the guidance is contextual: the same meal can receive a different prediction when timing or activity changes.

Which devices and data feed the model?
The digital twin depends on a stream of personal measurements rather than a single app entry. Twin Health’s provider materials identify these connected inputs:
- Continuous glucose monitor (CGM): supplies frequent glucose readings that let the model associate meals and activity with subsequent changes.
- Bluetooth-enabled smart scale: provides body weight and body-composition readings. Twin Health’s health-plan materials describe body-composition data as the first daily input.
- Garmin fitness tracker: contributes activity and related wearable signals.
- Bluetooth blood-pressure cuff: adds blood-pressure readings.
- Member-entered information: meals, medication use and other contextual details are part of the data set Song logged.
A CGM is a medical device, so prescription requirements, approved products, compatibility and access differ by country and by individual eligibility. The same checks apply before buying or connecting any blood-pressure device. A generic Bluetooth smart scale may be a relevant accessory category, but compatibility with a particular Twin Health program should be confirmed; it should not be presented as an official bundle.

How a food prediction is produced
- Collect a baseline: the service receives body-composition, glucose, activity, blood-pressure, medication and meal information.
- Match events to responses: the model compares what happened after particular meals or activities with the member’s prior readings.
- Generate an individualized label: red, orange or green communicates the predicted glucose response for that member.
- Test a change: a walk, different portion, meal combination or timing can alter the next prediction as new data are recorded.
- Deliver broader coaching: Twin Health says the model also supports guidance about sleep, activity and stress, not only food.
This is not a diagnosis and it does not establish that a food is healthy or unhealthy for everyone. It is a decision aid built around an individual’s measured patterns. A green label means “predicted to fit this person’s glucose response” in the system’s model, not “nutritionally complete” or “safe for every medical condition.”
What happened during Song’s approximately 100 days
Song reported that logging changed her behavior and that human coaching supplied accountability. The costs were substantial tracking fatigue, no meaningful weight loss and a medication taper that did not proceed as smoothly as she had hoped.

At follow-up, she reported a nine-point increase in her metabolic-health score, a 70 mg/dL reduction in triglycerides and broad improvement in liver enzymes. Insulin resistance remained, although she said it was reduced. These are her personal results, not a promised outcome for other members. Some laboratory markers were already improving before the experiment, so the account cannot show that the digital twin caused the changes.
What the broader outcome figures do—and do not—show
Twin Health’s health-plan page cites results from a 2025 Cleveland Clinic group in which 71% of participants lowered A1C below 6.5% without glucose-lowering medication other than metformin, alongside an average 27-pound weight loss. The page says individual results vary. Those figures are presented by the company and should be read as results for that cited group, not as a guarantee or a population-wide estimate.

The same page cites a 2024 Scientific Reports retrospective real-world study reporting that 89% of participants reduced A1C below 7 after one year. A retrospective study observes outcomes in an existing program; it does not provide the same causal certainty as a randomized trial. The publication and the company’s attribution should remain clear whenever this number is repeated.

Digital twin versus ordinary meal logging
Both approaches can make eating patterns visible, but they ask for different commitments and produce different kinds of feedback.

| Question | Digital-twin program | Ordinary calorie or meal log |
|---|---|---|
| Data burden | Meal entries plus streams from a CGM and connected devices; Song described frequent tracking fatigue. | Usually relies on manually entered meals and optional weight or activity data. |
| Glucose feedback | Predicts an individual response using CGM history and related inputs. | Normally records food quantities or calories without a continuous glucose stream. |
| Wearables | Provider materials name a smart scale, fitness tracker and Bluetooth blood-pressure cuff in addition to a CGM. | Device integration varies by app and may not include medical measurements. |
| Human support | Song reported coaching and accountability as part of the experience. | May be self-directed unless a separate coach or clinician is included. |
| Medication oversight | Medication information is part of the member data; any change requires appropriate clinical supervision. | Most food logs do not manage medication. |
| Evidence type | Includes one participant’s account, vendor-presented group results and a cited retrospective study. | Evidence depends on the specific app or program; logging alone is not a clinical intervention. |
| Access and privacy | Availability can run through an employer, health plan, provider or regional program, with program-specific data terms. | Often available directly to consumers, but data collection and sharing policies differ by service. |
What to check before joining or buying devices
Confirm the route to access
Twin Health’s materials describe employer, health-plan, provider and regional routes. Public consumer availability, pricing and referral arrangements are not universal. Ask the sponsoring organization which members qualify, which country and devices are supported, what clinical team is involved and what happens if coverage ends.
Review data governance
Before connecting health devices, read the specific program’s privacy notice and data-use terms. Check who can view raw readings, how long records are retained, whether data are used for care or other purposes, and how an account and connected devices are disconnected.
Check compatibility and medical eligibility
Verify the exact CGM model, phone or receiver, smart-scale connection and blood-pressure cuff supported by the program. Prescription rules and regional approvals can prevent a technically compatible device from being usable. Do not alter diabetes or other medication based solely on a color label; medication decisions belong with the prescribing clinician.

- It can identify patterns between your logged meals, activity and measured glucose response.
- It can suggest that changing a walk, portion or meal combination may change the predicted response.
- It can combine glucose with weight, blood pressure, activity and medication context in one coaching workflow.
- It cannot guarantee weight loss, normalize insulin resistance or prove that one intervention caused a laboratory improvement.
- Its traffic-light colors are personal model outputs, not universal rankings of foods.
The practical promise is personalization: replacing a generic rule with feedback based on your own measurements. The practical limitation is effort. The model is only as useful as the data and follow-through it receives, and Song’s experience shows that continuous logging and medication changes can be difficult even when the feedback is actionable.
