MRI and PET answer different questions, so neither is universally “better” for suspected Alzheimer’s. MRI shows brain structure and can reveal atrophy, stroke, tumor, hydrocephalus, or other causes of cognitive symptoms. PET uses an injected radioactive tracer to map glucose metabolism or molecular changes such as amyloid plaques and tau. A clinician generally starts with a full history, examination, cognitive testing, medication and laboratory review, then uses structural imaging when appropriate. PET is added when uncertainty remains and its result could change diagnosis, treatment eligibility, or care planning.
MRI and PET at a glance
| Question | MRI | PET |
|---|---|---|
| What it measures | Anatomy: brain volume, tissue structure, blood products and some vascular changes | Tracer-defined biology: glucose use (FDG), amyloid plaques, or tau accumulation |
| Main role | Looks for atrophy and alternative structural explanations for symptoms | Supports or refutes specific molecular or metabolic patterns when diagnosis remains uncertain |
| Radiation | No ionizing radiation | Yes; the amount depends on the tracer and protocol |
| Key safety issue | Strong magnetic field, changing gradients and radiofrequency energy; implants and metal require screening | Intravenous tracer, radiation precautions, and tracer-specific preparation |
| Can it be normal in early Alzheimer’s? | Yes. Early disease may not produce visible atrophy | A negative or non-characteristic result can occur, and every scan needs clinical interpretation |
| Can it identify another cause? | Often useful for stroke, tumor, hydrocephalus and other structural disorders | Metabolic patterns may help distinguish disorders, but PET is not a general substitute for structural imaging |
RadiologyInfo describes MRI as using “a powerful magnetic field, radio frequency pulses and a computer to produce detailed pictures” of internal structures. The National Institute on Aging (NIA) defines PET as using “small amounts of a radioactive substance, called a tracer, to measure specific activity — such as energy use — or a specific molecule in different brain regions.” Those underlying technologies explain why the scans complement rather than replace one another.

What an MRI contributes
It maps structure and atrophy
A dementia MRI provides high-resolution views of brain tissue. Radiologists assess overall volume, the hippocampal and medial temporal regions, parietal areas, white-matter changes, old or recent infarcts, microbleeds and other structural findings. In later Alzheimer’s, reduced volume is often most apparent in temporal and parietal regions. The pattern, severity and symmetry matter more than a single measurement.

An MRI report may describe mild, moderate or marked volume loss, but atrophy is not synonymous with Alzheimer’s. Normal aging, vascular disease, prior injury and several other neurologic disorders can alter brain volume. The scan therefore contributes evidence to a diagnosis rather than serving as a stand-alone test.

It searches for alternative explanations
Structural imaging can reveal a tumor, subdural collection, hydrocephalus, a large or strategically placed stroke, extensive vascular injury or another lesion that changes the treatment plan. It can also identify patterns that make a non-Alzheimer’s dementia more plausible. This exclusion role is one reason clinicians commonly obtain MRI before ordering a molecular PET study.
A normal MRI does not rule out early disease
Visible atrophy may lag behind symptoms. A person with mild cognitive impairment or early Alzheimer’s can have an MRI that is read as normal or nonspecific. Conversely, an abnormal scan does not prove that Alzheimer’s is causing the current symptoms. Cognitive testing, symptom progression and other medical information determine how much weight to give the images.
MRI safety and contraindications
MRI does not use ionizing radiation, but the scanner’s static magnetic field can move ferromagnetic objects or interfere with implanted devices. Before the appointment, tell the facility about pacemakers, implanted stimulators, aneurysm clips, cochlear implants, infusion pumps, metal fragments (especially in the eye), shrapnel, surgical hardware and any history of metal work. Facilities verify the exact device model and its MRI conditions; “MRI-compatible” is not a blanket designation for every setting.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsThe changing gradients and radiofrequency energy can produce loud sounds and tissue heating. Ear protection is provided, and motion can reduce image quality. Claustrophobia may require a wider scanner, coaching, a mirror, an anxiolytic prescribed in advance or, in selected cases, sedation. Sedation means arranging a ride and following fasting and medication instructions from the facility.
Some examinations use gadolinium contrast, but contrast is not automatically required for a dementia MRI. If it is proposed, the clinician should explain what question it addresses and review kidney function, prior contrast reactions, pregnancy status and the specific agent. Kidney-related precautions depend on the agent and the patient’s renal function.

The U.S. Food and Drug Administration reports that millions of MRI scans are performed in the United States each year and that manufacturers, facilities and patients submit approximately 300 adverse-event reports annually. That figure is a system-level report count, not an individual-patient risk rate.
What PET contributes
Amyloid PET: is beta-amyloid present?
Amyloid tracers bind to beta-amyloid plaque deposits. A positive scan supports the presence of amyloid pathology, while a scan showing few or no plaques makes Alzheimer’s less likely as the cause of the current clinical syndrome. Amyloid can also be present in people who never develop Alzheimer’s symptoms, particularly with increasing age. Therefore, a positive result is evidence of pathology, not proof that the pathology explains every symptom.

Amyloid confirmation can affect eligibility decisions for amyloid-targeting therapies, whose labeling, safety requirements and availability must be checked at the time of treatment discussion.
Tau PET: where is abnormal tau?
Tau tracers detect abnormal tau accumulation. Tau distribution generally tracks the spread of Alzheimer-type pathology more closely than amyloid does, so tau PET may help with staging and with questions about clinical severity. The 2025 Alzheimer’s Association/SNMMI criteria describe potential value for selected diagnostic and staging situations, while the NIA notes that tau PET is not commonly used in standard practice. Access, approved indications and interpretation expertise are limited compared with MRI.
FDG PET: how is the brain using glucose?
FDG PET measures regional glucose metabolism. Characteristic areas of reduced glucose use can support an Alzheimer pattern and may help differentiate it from frontotemporal lobar degeneration or Lewy-body dementia when the clinical picture is unclear. FDG PET does not show amyloid or tau directly.

A specialty guideline reports published FDG-PET ranges of 80%–99% sensitivity and 63%–98% specificity for differentiating Alzheimer’s disease from frontotemporal lobar degeneration. For Alzheimer’s versus Lewy-body dementia, reported sensitivity is 70%–92% and specificity 74%–100%. These are ranges from different studies, not a guarantee for an individual. Performance depends on the study population, reference standard, reader expertise, disease stage and clinical context.
How clinicians usually decide which scan to order
- Comprehensive assessment. A clinician takes a history from the patient and an informant, performs a neurologic examination, reviews medications and sleep, depression and sensory issues, administers cognitive testing and orders relevant laboratory tests.
- Structural imaging. MRI is commonly selected to evaluate anatomy, vascular injury and alternative causes. CT may be used when MRI is unavailable or contraindicated, but it provides less soft-tissue detail.
- Resolve a specific uncertainty. If the specialist still needs to distinguish Alzheimer’s from another dementia, FDG PET, cerebrospinal-fluid biomarkers or amyloid PET may be considered. The test should be ordered only when its result is likely to change diagnosis, treatment or planning.
- Specialized staging questions. Tau PET is reserved for selected cases in centers with appropriate tracer access and interpretive expertise.
This sequence is not a rigid rule. A rapidly progressive syndrome, atypical age of onset, unusual neurologic signs or a contraindication to MRI can change the order. The reason for each test should be documented before scheduling it.
Which scan is better for a particular question?
| Clinical question | Usually more informative | Why |
|---|---|---|
| Could a stroke, tumor, hydrocephalus or major vascular lesion explain symptoms? | MRI | Shows anatomy and structural abnormalities directly |
| Is there evidence of amyloid plaque pathology? | Amyloid PET | Uses a tracer designed to bind amyloid |
| What is the regional pattern of brain glucose use? | FDG PET | Maps metabolism and can support differential diagnosis |
| Where is tau accumulating, and could distribution help stage disease? | Tau PET | Visualizes tau, although it is less routine |
| Is there a safe, non-radiation first look at brain structure? | MRI | No ionizing radiation, subject to implant screening |
| Could a molecular result change access to a disease-modifying treatment? | Usually amyloid PET or another validated biomarker, selected by a specialist | Therapy eligibility may require confirmation of amyloid; current criteria and labeling apply |
The “better” scan is the one that answers the unresolved question. Ordering both without a defined purpose adds time, cost and incidental findings without guaranteeing a clearer diagnosis.
Risks, preparation and what the appointments involve
MRI appointment
- Complete the implant and metal screening form accurately. Bring device cards and prior operative records when available.
- Ask whether contrast is planned, whether kidney testing is needed and whether eating, drinking or medication restrictions apply.
- Remove jewelry, watches, removable metal, cards and electronic devices before entering the scanner room.
- Lie still while sequences run. The machine is loud; earplugs or headphones are supplied. A technologist can communicate throughout the scan.
- Tell the technologist immediately if you feel unsafe, overheated or unable to remain still. Motion may require repeating sequences.
Scan duration varies with the protocol and whether contrast is used. The facility should provide an estimate rather than promising a universal time.
PET appointment
- A nurse or technologist places an intravenous line and administers the selected tracer.
- You rest during an uptake period so the tracer distributes. The length depends on the tracer and protocol.
- You then lie still for the scan. The scanner detects emissions from the tracer and creates regional images.
- Follow the facility’s post-scan instructions about hydration, contact with pregnant people or infants and breastfeeding. These precautions are tracer-specific.
For FDG PET, glucose preparation is especially important. Facilities may ask you to avoid strenuous activity, fast for a specified period and report diabetes medicines and glucose levels. Do not change diabetes treatment without explicit instructions. Amyloid and tau protocols have different preparation requirements.

PET involves ionizing radiation from the tracer. The dose and any pregnancy or breastfeeding precautions depend on the radiopharmaceutical, administered activity and local rules. Tell the ordering team about pregnancy, possible pregnancy or breastfeeding before the injection.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Understanding reports and avoiding common misinterpretations
“Atrophy” on MRI is not a diagnosis
Age-related volume loss and vascular changes can coexist with Alzheimer’s. The meaningful question is whether the pattern, degree and progression fit the symptoms and cognitive testing. A radiology report should be interpreted alongside the examination, not read as a verdict in isolation.
A positive amyloid scan is not the same as symptomatic Alzheimer’s
Amyloid positivity establishes amyloid pathology, but it does not prove that amyloid is the sole cause of memory loss, language problems, depression or medication effects. Mixed pathology is common in older adults. Clinicians integrate amyloid status with tau or metabolic findings when available, symptom profile, disease course and other medical causes.
A negative amyloid scan changes probability
Few or no plaques make Alzheimer’s less likely as the explanation for the current syndrome, prompting consideration of vascular, Lewy-body, frontotemporal, medication-related, psychiatric, sleep or other neurologic causes. It does not mean that the person has no cognitive disorder.

Published sensitivity and specificity ranges describe groups studied under particular conditions. A specialist should explain what the result means for this patient rather than applying a single percentage to an individual.
Best Value
Do you need both MRI and PET?
Often, but not always. MRI and PET provide non-duplicative information: MRI checks structure and alternative causes; PET assesses metabolism or molecular pathology. A person with a typical presentation and reassuring structural imaging may not need PET. Someone with an atypical or mixed presentation, an unexpectedly young onset, discordant test results or a treatment decision that requires amyloid confirmation may benefit from PET.
Before agreeing to both, ask three questions:
- What specific uncertainty will this scan resolve?
- How will each possible result change medication, eligibility for therapy, monitoring or care planning?
- Are there less invasive or more available alternatives, such as cerebrospinal-fluid or blood biomarkers, that are validated for the decision being made?
If the clinician cannot identify an action tied to the result, postponing the additional scan may be reasonable while the evaluation is clarified.
Availability, cost and insurance
There is no universal price, waiting time or reimbursement policy for MRI, FDG PET, amyloid PET or tau PET. Charges vary by country, facility, scanner, tracer, contrast use and whether interpretation is bundled. Coverage also depends on the indication, insurer rules and current regulatory policy. Ask the imaging center for the self-pay estimate, authorization requirements, preparation instructions and cancellation policy; ask the treating clinician which diagnosis code and documentation support medical necessity.
PET availability is generally more limited than MRI because tracers require specialized production, delivery and regulatory handling. A referral to a memory clinic or nuclear-medicine center may be necessary. Confirm that the facility performs the specific tracer study ordered rather than assuming every PET center offers amyloid or tau imaging.
Questions to take to the appointment
- What diagnosis or alternative cause are we investigating?
- Why is MRI, FDG PET, amyloid PET or tau PET the best next test for that question?
- Will the result change treatment, eligibility for an amyloid-targeting therapy or care planning?
- Do I have any implant, kidney, pregnancy, breastfeeding, glucose or medication issue that changes preparation or safety?
- Who will interpret the images, and how will the findings be combined with cognitive testing and laboratory results?
- When and how will we discuss the report, including uncertain or incidental findings?
Bottom line for choosing between MRI and PET
MRI is primarily an anatomical test: it detects atrophy and structural causes while avoiding ionizing radiation, but it requires rigorous implant screening and may be normal early in Alzheimer’s. PET is a tracer-based test: FDG maps glucose metabolism, amyloid PET detects plaque pathology and tau PET can provide selected staging information. PET adds radiation, intravenous administration and access constraints. A dementia specialist should order either scan only when the expected result will clarify the diagnosis or change management, and should interpret it with the complete clinical picture.
