MRI Contrast Side Effects: What’s Common and When to Call a Doctor

A brief cool feeling where MRI contrast is injected, mild injection-site discomfort, headache, or nausea can occur. Tell imaging staff immediately about a reaction while you are still at the center. Seek emergency help for trouble breathing or swallowing, or a rapidly worsening reaction; contact your care team about severe, persistent, or unusual symptoms.

MRI contrast usually means a gadolinium-based contrast agent (GBCA) injected into a vein to improve images. Not every MRI uses contrast, and these agents are different from the iodinated contrast used for many CT scans.

What side effects can happen after MRI contrast?

Brief sensations at the injection site

A cool feeling at the injection site—often in the arm—can last a minute or two. The needle may hurt, and bruising can appear afterward. RadiologyInfo describes these as expected experiences.

Other reported effects

MedlinePlus lists burning, pain, warmth or coldness around the injection site, headache, and nausea. The current US prescribing information for Gadavist (gadobutrol), revised February 2026, says its most common adverse reactions, meeting the label’s incidence threshold of at least 0.5%, are headache, nausea, and dizziness. That threshold is not the rate for each reaction and is specific to Gadavist, not a class-wide estimate. A class-wide numeric rate for common or serious GBCA reactions is not established here.

When should I call a doctor after MRI contrast?

  • Get emergency help for difficulty breathing or swallowing. A rapidly worsening reaction also warrants emergency care.
  • Contact a doctor immediately for rash, itching, hives, or dizziness after receiving contrast, as advised by MedlinePlus.
  • Alert imaging staff at once if symptoms begin while you are still at the imaging center.
  • Call your care team about severe, persistent, unusual, or worsening symptoms, including headache, nausea, or injection-site pain. MedlinePlus advises contacting a doctor if listed effects are severe or do not go away, and reporting unusual problems.

Do not rely on a set number of minutes or hours for when a reaction must begin; the patient guidance cited here does not specify a universal onset cutoff.

How long do MRI contrast side effects last?

There is no single duration established for all side effects. RadiologyInfo says the cool injection-site feeling usually lasts a minute or two. For other effects, contact your care team if they are severe or do not go away rather than assuming they will resolve within a particular timeframe.

Can MRI contrast affect my kidneys?

Kidney disease matters because nephrogenic systemic fibrosis (NSF) is a rare but serious condition associated with gadolinium contrast in people with kidney disease. The Gadavist label identifies the greatest risk with chronic severe kidney disease (GFR below 30 mL/min/1.73 m²) or acute kidney injury; those thresholds are from that product’s labeling, not a universal patient rule for every agent.

Tell the imaging team about kidney disease or prior kidney problems, as well as diabetes or high blood pressure when asked. The clinician determines what screening and contrast-agent selection are appropriate. If you are likely to have repeated scans, the radiologist may also consider the agent’s retention profile when selecting a contrast agent.

Does gadolinium stay in the body, and is that harmful?

Gadolinium can remain in the body, including the brain, for months or years. The FDA’s December 19, 2017 safety communication said retention had not been directly linked to adverse health effects in patients with normal kidney function and concluded that the benefits of approved GBCAs continued to outweigh potential risks. Retention is a known phenomenon; that statement does not establish that it causes harm in people with normal kidney function, nor does it prove that every possible risk is absent.

RadiologyInfo also notes that tiny traces may be retained in organs, including the brain, and says no negative effects were known in its patient guidance. For a person expecting repeated contrast-enhanced scans, the care team can weigh the imaging need and the specific agent’s profile.

What should I disclose before an MRI with contrast?

Before the appointment or injection, tell the physician or technologist about:

  • Known allergies.
  • Current medicines and supplements.
  • Kidney disease or prior kidney problems; also disclose diabetes or high blood pressure when requested.
  • Pregnancy, the possibility of pregnancy, plans for pregnancy, or breastfeeding.

The imaging team can use this information to decide whether contrast is needed and which agent and screening are appropriate. Agent choice depends on the clinical question and your circumstances; it is a decision for the radiologist and care team, not a consumer ranking.

Is MRI contrast safe during pregnancy or breastfeeding?

Pregnancy

Tell the imaging team if you are pregnant or could be pregnant. The American College of Radiology’s 2022 statement supports avoiding routine GBCA use during pregnancy because of uncertainty. A clinician may consider it when the diagnostic information is essential and cannot be obtained another way, weighing the individual situation.

Breastfeeding

RadiologyInfo reports that available data and ACR and European Society of Urogenital Radiology guidance support continuing breastfeeding after intravenous contrast. A parent who remains concerned can discuss an optional 24-hour pause with the care team; interruption is not presented as mandatory.

What can I do after the scan?

If you were not sedated, RadiologyInfo says no recovery period is necessary and you can resume your usual activities and diet after the exam. Follow any specific instructions from your imaging team.
