The Citadel of Penance is in northwestern Fainweald. To open its sealed main gate permanently, descend into the ravine below it, defeat the Bloodcursed Lithopod, take the Citadel Gate Lever, then return and use the lever at the gate. A separate teleport reportedly drops you inside the Citadel’s prison cells, but it does not unlock the front entrance.
Where to find the Citadel of Penance
Head to the northwestern stretch of Fainweald and look for the Citadel’s sealed main gate. From there, the permanent entrance route leads down the sloping paths into the ravine beneath the gate. The gate’s note points toward a thief in the ravine, and approaching the rocky area with corrupted growths triggers the Bloodcursed Lithopod encounter. How to unlock the main entrance permanently
- Find the sealed gate. Use it as your landmark in northwestern Fainweald.
- Take the ravine paths below the gate. Follow the sloping route down toward the rocky area with corrupted growths.
- Defeat the Bloodcursed Lithopod. The encounter is reported to begin when you approach that area.
- Collect the Citadel Gate Lever. The Lithopod encounter yields the lever.
- Return to the main gate and use the mechanism. The lever opens the entrance permanently, giving you durable front-door access.

Can you enter without opening the gate?
Yes. A reported alternate route uses a teleport object in ruins reached from the One-Legged Wolf area. Interacting with it sends you to the Citadel’s lower prison cells, bypassing the initial lever errand. The guides describe the object’s appearance differently, so do not rely on a precise visual label; look for the teleport in the ruins and confirm what it does in your game.

This shortcut is not a substitute for the lever if your goal is to open the main gate. It reportedly places you inside the prison cells and does not permanently unlock the front entrance. GameSpot and AllThingsHow describe the teleport route.
