Modern Assessment Technology in Physiotherapy and Rehabilitation: What It Can—and Can’t—Measure

Digital tools can help physiotherapists assess selected movements and symptoms, monitor activity between appointments, and deliver care by video. They are not one interchangeable form of assessment: whether a result is useful depends on the specific measure, patient group, device, and clinical question. They can complement a physical examination, but the evidence does not support treating technology as a general substitute for one.

What counts as digital physiotherapy assessment?

Digital assessment spans several different activities. A video appointment lets a clinician observe and interact with a patient remotely; an app or wearable can collect information over time; and motion-capture software estimates movement from video or sensors. The tools do not all measure the same thing, and a result that is useful for one task is not automatically valid for another.

Approach What it can contribute Important boundary
Synchronous video assessment Live observation, questions, and selected functional or physical tests Depends on the test, patient, camera view, environment, and communication quality
Digital questionnaires and tests Patient-reported symptoms or function and selected standardized measures Evidence for one questionnaire or test does not establish validity for all measures
Apps and remote monitoring Information about exercise adherence, symptoms, or activity between visits Collected information needs interpretation in the context of the treatment plan
Wearable activity trackers Measures such as step counts and activity feedback A consumer device is not thereby a validated clinical instrument
Markerless motion capture Estimates of movement from video without attaching markers Clinical value remains task- and system-specific; broad diagnostic claims are not established

How does digital assessment compare with conventional face-to-face physiotherapy assessment?

A 2023 systematic review of digital versus conventional face-to-face physiotherapy assessment for musculoskeletal disorders included 10 repeated-measures studies and 193 participants aged 23–62. Reported validity ranged from moderate or acceptable to excellent for certain clinical tests, range-of-motion measures, patient-reported outcome measures (PROMs), pain, neck posture, and management decisions. The review found spinal-posture observations performed poorly; certainty across findings ranged from very low to high. Agreement on diagnosis also fell when only exact diagnosis matches counted. These findings support considering digital methods for particular assessment components, not assuming that a remote assessment reproduces every part of an in-person examination. A broader 2021 systematic review examined 39 studies: 15 in simulated telehealth settings and 24 in real-world settings. It concluded that telehealth assessment appeared valid and reliable for specific types of assessment in limited populations and settings, while identifying a need for further research. What can a video physiotherapy assessment evaluate?

Real-time video can support selected tests when the clinician can see the relevant movement and the test is appropriate for remote delivery. The telehealth assessment review reported evidence for measures including range of movement, muscle strength and endurance, pain, some shoulder and elbow special tests, the Berg Balance Scale, timed up and go, timed stance, six-minute walk, and steps in a 360-degree turn. The evidence came from particular studies and populations; it does not establish that all examination maneuvers can be performed or interpreted reliably over video.

Practical conditions matter. A clinician may need a clear view of the body part or task, adequate room to move, usable audio and video, and a safe way for the patient to perform the movement. The review also noted that visual or audio quality and verbal or nonverbal communication could be challenging. Participants often reported satisfaction, but many preferred in-person assessment when offered a choice. The review describes assessment types and implementation considerations.

What do apps and remote monitoring add between visits?

An app or motion-capture tool can transmit information while someone performs a home exercise or self-management program. A physical therapist can review that information when considering whether a plan needs to change. The American Physical Therapy Association (APTA) describes digital tools as supporting telehealth visits, synchronous or asynchronous monitoring, adherence and outcome insights, assessment, and treatment planning. The data can inform care; it does not make the software an independent examiner. See the APTA foundational paper on digital health.

Can a wearable tracker replace a formal rehabilitation assessment?

No. A wearable may help monitor activity, with step count the most common measure in a 2024 review of commercially available devices. That review included 18 studies and 1,754 patients: six randomized trials, six quasi-experimental studies, and six observational studies. All six randomized trials reported increased physical activity from wearable-driven feedback. Two orthopaedic trials reported that wearable self-directed rehabilitation was non-inferior to traditional physiotherapy in their study populations. Those results do not establish that any consumer tracker is a validated clinical instrument or that self-directed care is appropriate for every person or condition. The authors called for larger, better-designed studies and economic evaluations before widespread adoption. Read the wearable-device review.

If a tracker is considered, treat it as an optional activity-monitoring aid to discuss with the clinician, not as a required purchase, diagnostic device, or replacement for an examination. A step count describes activity; by itself, it does not explain why activity changed or determine what treatment is needed.

How reliable is markerless motion capture?

Markerless motion capture estimates movement without attaching markers to the body. A 2023 review included 65 studies; Kinect was the most frequently used system, and the review noted a more recent trend toward smartphone video. The authors described clinical use as preliminary and the benefits for assessment as inconclusive, despite potential for symptom identification and future screening support. Do not infer diagnostic accuracy or clinical effectiveness for a particular system unless it has been validated for that system, task, and relevant patient group. How should patients and clinicians choose between remote and in-person assessment?

Choose the format around the assessment question, rather than assuming that one delivery method is always best. A practical discussion can work through these checks:

  1. Identify the clinical question. What symptom, movement, function, or outcome needs assessment, and what measure would answer it?
  2. Check evidence for that exact task. Is the tool supported for the relevant measure and patient group, and are its results valid and reliable enough to inform care?
  3. Ask whether the result will change a decision. Consider whether the information can be interpreted alongside symptoms and history and whether it is likely to affect the treatment plan.
  4. Review access and preferences. Discuss patient or family preference, cultural needs, confidence using technology, accessibility, and any barriers to taking part remotely.
  5. Check the setting and safety. Consider the room, camera angle, bandwidth, communication quality, and whether the patient can perform the task safely without in-person assistance.
  6. Discuss privacy and costs. Consider how information is handled, along with direct and indirect costs for the patient and clinician.

The APTA’s 2024 clinical practice guideline supports telerehabilitation for examination and intervention when clinicians and patients use shared decision-making about service options, costs, barriers, and facilitators. The guideline authors conclude: “Overall, with shared decision-making between clinicians and patients to inform patients of service delivery options, direct and indirect costs, barriers, and facilitators of telerehabilitation, the evidence supports the use of telerehabilitation by physical therapists for both examination and intervention.” Read the APTA clinical practice guideline.

Does video telerehabilitation improve attendance or exercise adherence?

A 2024 systematic review comparing real-time video telerehabilitation with in-person physiotherapy estimated attendance was 8% higher with video (95% confidence interval −1 to 18) and exercise adherence was 9% higher (95% confidence interval 2 to 16); satisfaction was similar. The review rated certainty very low to low, reflecting inconsistency and risk of bias. These estimates concern attendance, adherence, and satisfaction; they do not establish that video delivery definitively improves clinical outcomes. Read the video telerehabilitation review.
