A 2026 case report describes five primary melanomas in situ in a 49-year-old man who said he had injected melanotan II and then melanotan I while also using tanning beds. The authors do not claim the peptide caused the cancers, and nothing in the report can quantify a risk. What it does provide is a serious safety signal about an unapproved product bought online, and that signal is reinforced by regulators in Australia and the United States.
One correction to the headline’s framing: the report does not say the product came from a gym. The man reported buying it from an online vendor.

What the 2026 case report describes
The paper was published online on May 12, 2026 in JAAD Case Reports by Douglas Jaxon Vadner and Sidney Smith. The patient was a 49-year-old White man with Fitzpatrick skin phototype II who presented with several clinically atypical pigmented lesions.
According to his account, he started tanning-bed sessions in December 2025 and, at the same time, began self-administering melanotan II bought from an online vendor. He later switched to melanotan I. He reported that existing moles darkened quickly. Biopsies identified five primary melanomas in situ, each arising in a preexisting nevus.
His wider history also matters for interpreting the case. He reported intermittent tanning-bed use before this period, two earlier blistering sunburns, and testosterone use obtained online. The product’s source, dose and purity were not independently verified, so the melanotan history rests on his report alone.
What a single case can and cannot show
The authors state that causality cannot be established from a single observational case. Several limits follow directly from the report’s design:

- It is one patient. A single case can flag a possible link in time, but it cannot show that melanotan produced the melanomas.
- The exposure is self-reported. Nobody chemically analyzed what he injected, so the identity, strength and purity of the product are unconfirmed.
- Other exposures were present. Tanning-bed use, prior blistering sunburns and testosterone use all complicate attribution.
- No risk figure can be derived. Counting published cases does not produce an incidence rate, and no reliable population-level melanoma risk estimate attributable to melanotan use is available in the published sources to date.
The fair reading is that the report adds a case to a small body of concern and gives clinicians a reason to ask about these products. It does not show how often harm occurs.

A 2014 report in Dermatology described a 20-year-old woman who developed melanoma after self-injecting melanotan II, reportedly to enhance sunbed tanning. It is also a case report, so it neither establishes causation nor sizes any risk. Its authors described melanotan II as unlicensed and incompletely tested, and noted that the extent and types of its adverse effects were unknown.

Taken together, the two reports show a pattern of melanoma cases in people using unregulated melanotan products with UV exposure. They do not show a rate.
What the Skin Cancer Foundation says
In a July 7, 2026 warning, The Skin Cancer Foundation summarized the published safety concerns: case reports of melanoma after melanotan II use, rapid changes in pigmented lesions that can complicate detection, and other reported serious adverse effects. The Foundation says case reports cannot establish direct causation.

The Foundation also reports that melanotan II is not FDA-approved for any use in the United States and that its safety has not been established. It advises against injectable and intranasal tanning drugs. Deborah S. Sarnoff, MD, President of The Skin Cancer Foundation, put the position plainly: “The desire for a tan should never outweigh concerns about health and safety.”

On August 17, 2026, the Therapeutic Goods Administration (TGA) warned consumers against melanotan II tanning products. In Australia, MT-II is prescription-only, is not approved as a tanning agent, and no product containing it is on the Australian Register of Therapeutic Goods or approved for supply.

The TGA’s investigation confirmed MT-II in seized products. For five nasal spray bottles labeled “Pure Tans Triple Strength 30 MG” with a 20 mL volume, it estimated the amount of MT-II at 22 mg to 54 mg per the five bottles, well below and above the labeled 30 mg. The TGA says inconsistent amounts can increase the risk of adverse events. Its findings apply to the products it tested. They do not test the product in the headline or products sold elsewhere online.
A separate TGA enforcement release dated May 21, 2026 reported that 27 infringement notices totaling A$101,412 were issued to a New South Wales individual for alleged unlawful supply. The notices were paid in May. The TGA states that supplying melanotan tanning products without a doctor’s prescription is illegal in Australia, whatever the form of the product.

If a mole changes after using melanotan
The Skin Cancer Foundation advises people with concerns about changing moles, pigmentation or skin-cancer risk to see a board-certified dermatologist. This is general guidance, not a diagnosis.
