Medical Device Service Skills: What HTM Teams Need to Build

Closing the medical-device field service knowledge gap takes more than sending technicians to courses. Healthcare technology management (HTM) teams need task-specific training, supervised practice, reliable service information, technical support, tools and parts—and a way to verify that people can perform assigned work safely. U.S. workforce survey findings point to pressure from recruiting, training and changing device technologies, while the FDA warns that poor-quality servicing can contribute to device malfunctions and adverse events.

What the knowledge gap is—and what the evidence can establish

There is no single, verified statistic for a universal medical-device service knowledge gap. The available U.S. evidence instead describes connected pressures: difficulty recruiting and planning succession, insufficient training identified as a career challenge, and work that increasingly involves complex devices, software, cybersecurity, and integration with IT, electronic health records (EHRs), and connected platforms.

AAMI and TechNation’s 2025 State of HTM survey is a workforce snapshot, not a census of every hospital or service organization. AAMI’s 2026 account says 947 HTM professionals participated. Respondents worked in varied settings, and smaller community hospitals may have particularly small HTM teams. The results identify reported concerns; they do not show that every technician or organization has the same capability gap.

Reported figure How to interpret it
46% of 757 respondents who answered the career-path question had transitioned into HTM from another field. AAMI’s account of the 2025 survey; the percentage applies only to respondents who answered that question.
8,800 additional biomedical equipment repairers by 2033. AAMI relays this U.S. Bureau of Labor Statistics projection; it is a projection, not a count of current vacancies.
113 institutions with Biomedical Technology/Technician programs (CIP 15.0401). AAMI relays a College Navigator count attributed to the National Center for Education Statistics. Directory counts can change.
16 graduates and 60 people currently enrolled. Counts AAMI reported for its two-year BMET Apprenticeship Program in its 2026 account; enrollment and completion figures are time-sensitive.

These figures provide context for workforce development, not proof that a particular course, certification, or apprenticeship closes a knowledge gap. AAMI also reports concerns about OEM restrictions, including cases where service documentation, parts, or software access may be limited. This is a reported structural concern, not a claim about every manufacturer or device. The survey findings and attributed figures are described in AAMI’s account of the 2025 State of HTM.

Why the gap matters to safe servicing

The FDA defines servicing as “the repair and/or preventive or routine maintenance of one or more parts in a finished device, after distribution, for purposes of returning the device to the safety and performance specifications established by the original equipment manufacturer (OEM) and to meet its original intended use.” The agency says timely, cost-effective, quality maintenance and repair matter to the continued quality, safety, and effectiveness of marketed devices. Poor-quality servicing may lead to poor performance, malfunction, and adverse events; cybersecurity is also an important servicing consideration.

For an HTM team, capability therefore means more than knowing how to replace a component. A technician may need to recognize when a fault involves a network connection, software configuration, cybersecurity control, or interaction with another clinical system—and know when to stop, escalate, or obtain authorized technical support. The right boundary depends on the device, assigned task, available procedures, and the technician’s demonstrated competence.

Servicing and remanufacturing are not interchangeable

The FDA distinguishes servicing from remanufacturing. Its May 2024 final guidance helps determine whether activities performed on a particular device are likely to constitute remanufacturing and discusses remanufacturers’ regulatory responsibilities. The distinction is not decided solely by whether work is performed by a hospital, an independent service provider, or an OEM: the activities and their effects on the specific device matter. Organizations should assess proposed work against current FDA guidance and applicable requirements rather than assume that all repair or modification falls into one category.

Build capability around assigned devices and tasks

Start with the work the team actually performs, rather than a generic list of courses. Inventory devices, service tasks, risk and complexity, the information and tools available, and the people authorized and prepared to do each task. The resulting map should identify not only training needs but also unsupported work that should be escalated or assigned elsewhere.

  1. Define the task boundary. For each device type, list preventive maintenance, troubleshooting, repair, software or configuration work, and escalation conditions. Distinguish tasks technicians may perform independently from those requiring supervision, manufacturer support, or another qualified specialist.
  2. Set task-specific competencies. Specify the knowledge and practical performance needed for each task: for example, relevant electrical or electromechanical fundamentals, device operation, applicable service procedures, safe use of tools, and documentation. Add software, networking, cybersecurity, and IT integration competencies where the device and work require them.
  3. Check supportability before assigning work. Confirm that current service information, appropriate technical support, required tools, spare parts, and any needed software access are available. If an essential input is missing, document the constraint and define a safe escalation route instead of treating a training course as a substitute.
  4. Teach, demonstrate, and supervise. Combine foundational instruction with device- and task-specific learning, observed practice, and a documented assessment of performance. Use supervised work to establish readiness for assigned tasks; course attendance alone does not demonstrate that someone can perform them safely.
  5. Reassess when the work changes. Review competencies when devices, software, procedures, integrations, or assigned responsibilities change. Keep training records and service procedures aligned with the equipment actually in use.

Treat supportability as an organizational responsibility

AAMI’s “Flexible Solutions for Device Supportability” guide frames supportability as how effectively and economically a device or system can be supported across its life cycle. The relevant supports include information, training, technical support, tools, and spare parts. That framing helps teams avoid placing the entire burden on individual technicians when a capability problem may also stem from limited documentation, access, or organizational resources.

  • Information: Establish how technicians obtain current service documentation, procedures, and device-specific updates, and how revisions reach the team.
  • Training: Provide learning tied to the equipment and tasks assigned, with time for practical work and supervision.
  • Technical support: Identify who can answer questions or take over when a fault is beyond the team’s documented competence or access.
  • Tools and parts: Make sure required device-specific tools and approved parts are available. Follow device documentation and applicable manufacturer requirements; a generic consumer tool kit is not a substitute for task-appropriate service equipment.

Before acquiring or deploying equipment, HTM leaders can make supportability part of the decision: clarify service documentation and training needs, technical-support arrangements, tools and parts availability, and the work the in-house team is expected to perform. This makes the capability requirement visible before a device enters routine service.

Develop a workforce that can grow with the work

AAMI reports that recruitment of new entrants, impending retirements, limited early-career candidates, and insufficient succession planning are among the workforce concerns identified by survey respondents. It also reports that 46% of the 757 people who answered its career-path question entered HTM from another field. That finding suggests a career-change route exists; it does not establish that every entrant has the same preparation needs.

Organizations can make entry and progression more deliberate by pairing foundational BMET education with structured onboarding, mentoring, supervised practice, and clear competency milestones. Apprenticeships, internships, and partnerships with education or career-pathway organizations are possible routes. AAMI describes its HTM Training Guide as a resource for departments establishing BMET training programs or internships, rather than a step-by-step internship manual. Its HTM resources also cover competency, cybersecurity, servicing, training, and workforce topics.

Certifications can help recognize knowledge of medical devices and clinical practice, as well as repair and maintenance skills, according to AAMI. They are one element of workforce development—not evidence by themselves that an individual is competent for every device or task, or that an organization has resolved its supportability constraints. FDA’s recognized-standards listing describes AAMI TIR 49:2013/(R)2020 as guidance for instructional and training materials for devices used in non-clinical environments, with much of its information also applicable in clinical environments.

Measure whether the capability plan is working

The available sources identify workforce concerns and training resources, but do not establish which intervention measurably closes the gap or improves patient outcomes. Teams should therefore evaluate their own program rather than assume that course completion, certification, or a new hiring pathway proves impact.

  • Track which assigned tasks have documented procedures, required tools and parts, and an identified escalation route.
  • Record task-specific training and observed competency assessments, including any work that remains restricted or supervised.
  • Review service records for recurring faults, delays linked to information or parts access, and issues escalated because the team lacked support or authorization.
  • Use findings to adjust task assignments, training, technical-support arrangements, and workforce planning; interpret changes in the context of device mix, staffing, and workload.

These measures help leaders see where capability is constrained and whether organizational supports are in place. They should be treated as local management indicators, not as validated universal measures of competency or patient-safety improvement.
