How measles, a cold and flu typically differ
The pattern over time can offer clues, but symptoms overlap and are not a reliable way to tell an individual illness apart. This comparison reflects U.S. CDC guidance; it is not a diagnostic tool. Measles can be atypical, including in some people with weakened immune systems who may not develop a rash, and other infections can resemble it.

Measles symptoms and their usual sequence
Early illness
Measles symptoms usually begin 7–14 days after contact with the virus. The early illness commonly includes high fever, cough, runny nose and red, watery eyes. Fever can exceed 104°F. These time ranges describe usual patterns, not a guarantee for any one person.
Koplik spots and rash
Koplik spots may appear inside the mouth about 2–3 days after symptoms begin. The rash typically starts 3–5 days after the first symptoms, as flat red spots around the face or hairline, then moves down the body. Spots may merge, and fever can spike above 104°F when the rash appears. The CDC clinical overview describes the rash as usually appearing about 14 days after exposure.

The rash is an important clue, but do not wait for it before seeking advice: measles can spread before it appears, and some people with measles may not develop a rash.
What points more toward a cold or flu?
A cold
A cold is usually milder and often features a runny or stuffy nose, sneezing, cough or sore throat. Mild aches or headache can occur, and older children and adults usually have a low-grade fever or none. Cold symptoms often peak within 2–3 days and usually last less than a week, though duration varies.

Flu
Flu often starts suddenly. It can cause fever or chills, cough, sore throat, a runny or stuffy nose, muscle or body aches, headache and fatigue. Fever is not universal. Compared with a cold, fever, aches, chills, fatigue and headache tend to be more common or pronounced with flu; sneezing and a stuffy nose are more common with a cold.

What to do if measles is possible
- Call a healthcare provider immediately. Explain the symptoms and any known exposure or relevant travel. Call before going to a clinic or hospital so staff can arrange assessment without exposing other people.
- Limit contact with others. Measles is airborne. The CDC says people with measles can spread it from four days before through four days after the rash appears, and virus may remain in the air for up to two hours after an infected person leaves a space. CDC advises a sick person to stay home until the fifth day after rash onset and avoid contact with people at home who are not sick.
- Seek emergency care for warning signs. Trouble breathing or breathing faster than normal is an emergency warning sign. Have someone call ahead before arrival if measles may be involved.
- Leave diagnosis and reporting to the health response. CDC guidance says laboratory confirmation is essential for sporadic measles cases and outbreaks, and suspected cases should be reported to the local health department. A clinician can guide testing and next steps; local contact pathways may differ outside the United States.
Why prompt attention matters
Measles can cause ear infections and diarrhea, as well as serious complications including pneumonia and encephalitis; it can be fatal. Higher-risk groups for serious complications include children younger than five, adults older than 20, pregnant people and people with weakened immune systems.
In CDC figures, ear infections occur in about 1 out of every 10 children with measles, and diarrhea is reported in less than 1 out of 10 people with measles. These U.S. population-level figures do not predict an individual’s outcome.

How vaccination affects risk
MMR vaccination is the best protection against measles. CDC says two doses are about 97% effective at preventing measles and one dose about 93% effective. These are CDC estimates, not a guarantee for an individual; if you may have been exposed, contact a healthcare provider even if you are vaccinated.
