Low-sodium salt substitutes can lower sodium, but many replace sodium chloride with potassium chloride. That makes them different from salt-free seasonings and means they are not automatically safe for people with kidney problems, potassium restrictions, or medicines that affect potassium. For eligible adults who already add table salt, the World Health Organization (WHO) conditionally suggests a potassium-containing lower-sodium substitute, while emphasizing that the recommendation does not cover children, pregnant women, people with kidney impairment, or anyone whose potassium excretion may be compromised.

The safest choice depends on what the product contains and on your medical care plan. Read the ingredient list and nutrition panel, check potassium as well as sodium, and ask a clinician or kidney dietitian before using a potassium-based product if you have any potassium-handling risk.

What “low-sodium salt substitute” can mean

People use low-sodium salt substitutes to describe several different products. They are not interchangeable, so identify the category before deciding what to use.

Potassium-containing blends

These products replace part of sodium chloride (ordinary salt) with potassium chloride. They still contain sodium, but less than an equivalent amount of regular salt. The potassium content can be substantial, and the proportions vary by formulation. A “lower sodium” claim does not mean “low potassium.”

Potassium-only replacements

A salt-free potassium chloride product may contain no sodium while supplying a large amount of potassium in each measured serving. “Salt-free” therefore does not mean suitable for people who have been told to restrict potassium.

Salt-free flavorings

Herbs, spices, citrus juice or zest, garlic, onion, vinegar, pepper, and other seasonings add flavor without being a salt substitute. They do not replace sodium with potassium and are the practical route for people who need to avoid added potassium. They are culinary options, not a treatment for high blood pressure.

How potassium-based substitutes lower sodium

Sodium chloride contributes the salty taste and sodium load of table salt. Replacing some sodium chloride with potassium chloride reduces sodium in a given volume of the product. Because potassium chloride tastes different from sodium chloride, formulations and serving sizes differ; no single sodium or potassium value applies to every product.

The change affects discretionary salt added at the table or during cooking. It does not remove sodium already present in bread, sauces, canned foods, cured foods, packaged meals, or restaurant dishes. A substitute cannot make unlimited use of salty foods harmless, and using more of it than the labeled serving can erase the intended sodium reduction while increasing potassium exposure.

What the WHO recommends in 2025—and who is covered

In its 2025 guideline, the World Health Organization states: If choosing to use table salt, WHO suggests replacing regular table salt with lower-sodium salt substitutes that contain potassium (conditional recommendation).

This is a conditional option for adults in the general population who choose to use table salt. It is not a universal instruction to add potassium. The guideline does not apply to children or pregnant women, and it excludes people with kidney impairment or other conditions or circumstances that might compromise potassium excretion. WHO gives potassium-sparing diuretics and potassium supplements as examples of circumstances requiring exclusion.

The recommendation concerns discretionary table salt, not the sodium in manufactured foods or meals eaten away from home. Reducing the amount of table salt and lowering total sodium intake across the diet remain broader strategies.

Who should avoid potassium substitutes or ask first

The question “who should avoid them?” cannot be answered by a brand name alone. The relevant issue is whether your body and medicines can keep blood potassium in a safe range.

People with kidney impairment or chronic kidney disease

Kidneys normally remove excess potassium. When kidney function is reduced, potassium can accumulate. The National Kidney Foundation (NKF) advises: Do not use a potassium-based salt substitute unless your healthcare team says it is safe. Ask your nephrologist, primary-care clinician, or kidney dietitian before buying or using one.

People taking medicines that can raise potassium

WHO excludes circumstances that may compromise potassium excretion. NKF notes that ACE inhibitors, angiotensin-receptor blockers (ARBs), and nonsteroidal mineralocorticoid receptor antagonists (nsMRAs), among other medicines, can raise potassium, particularly in people with chronic kidney disease. Potassium supplements and potassium-sparing diuretics are additional examples cited in the guidance.

Do not assume that every person taking a medicine in one of these classes has the same risk. Dose, kidney function, laboratory results, other medicines, and diet matter. Have a clinician or pharmacist review your complete medication list instead of self-screening from a short list.

People given a potassium or sodium restriction

If your care plan limits potassium or sodium, follow that plan rather than relying on a “low-sodium” front label. Check the product warning and the potassium amount per serving. A potassium-containing blend may conflict with a potassium restriction even when its sodium is lower.

Children and pregnant women

These groups are outside the scope of WHO’s 2025 recommendation. Do not extend an adult, general-population recommendation to a child or pregnancy without advice from the appropriate healthcare professional.

What the evidence shows—and what it does not show

WHO’s evidence review included 26 randomized controlled trials published from 1986 through 2021, involving 34,961 adults and 92 children. Most tested products that replaced sodium with potassium. The trials generally excluded people in whom extra potassium might cause harm, including participants with kidney disease or impaired renal function and those using potassium-sparing medicines.

Because those higher-risk groups were excluded, the trial results cannot establish that potassium substitutes are safe for them. Evidence from a generally healthy study population should not be used to overrule an individualized kidney or medication plan.

Across the trials, compared with regular salt, lower-sodium substitutes reduced mean systolic blood pressure by 4.76 mmHg and mean diastolic pressure by 2.43 mmHg. WHO rates the evidence as moderate certainty. Follow-up ranged from 56 days to five years. These are average differences between groups, not a promise that one person will see the same change.

WHO’s 2025 background page reports an estimated global mean sodium intake of 4.3 grams per day in 2019, described as more than twice WHO’s recommended adult level. The page also reports an estimate of 1.9 million deaths each year attributable to high sodium intake. Those global figures describe population burden; they do not predict your personal risk or quantify the effect of one product.

  1. Read the ingredients. Look for potassium chloride. A product containing both sodium chloride and potassium chloride is a blend; one listing potassium chloride without sodium chloride may be a salt-free potassium replacement.
  2. Check sodium per serving. Note the exact serving size, such as one-quarter teaspoon, and compare it with the amount you actually use.
  3. Check potassium per serving. Record the milligrams of potassium, not just the percentage daily value. Several small uses can add up.
  4. Read warnings. Follow statements about kidney disease, potassium or sodium restrictions, and physician approval.
  5. Compare like with like. Evaluate products using the same measured serving and intended use; do not compare a teaspoon of one product with a quarter-teaspoon serving of another.
Product type Sodium Potassium Who may need caution
Regular table salt High sodium; amount varies with serving Usually little or none Anyone limiting sodium
Lower-sodium potassium blend Lower than regular salt, but not zero Can be substantial; check label Kidney impairment, potassium restriction, or medicines affecting potassium
Potassium-only replacement May be zero Can be substantial; check label Anyone with a potassium-handling concern
Herbs, spices, citrus, garlic, vinegar No added sodium unless a packaged blend adds it Not used as a potassium-salt replacement Check blended seasonings for hidden sodium or potassium chloride

The table describes categories, not guaranteed values. Specialty “low-sodium” foods can also use potassium chloride, so inspect those labels too.

Worked example: Morton Lite Salt

Morton describes its consumer Morton Lite Salt as a sodium chloride and potassium chloride blend with 50% less sodium than regular salt. Its published nutrition facts list 290 mg sodium and 350 mg potassium per one-quarter teaspoon (1.4 g). These are manufacturer-provided figures, not independent testing. Confirm the current package because formulations and labels can change.

The manufacturer warns that Lite Salt should not be used by people on a sodium- or potassium-restricted diet unless approved by a physician. That warning means the product is an example of how to evaluate a label, not a universal recommendation. Anyone with kidney disease, a potassium restriction, or a medicine-related potassium concern should obtain individualized approval before use.

How eligible adults can use a substitute responsibly

If you are an adult in the general population, have no potassium-handling restriction, and choose to use table salt, treat a potassium-containing substitute as one measured ingredient—not as permission to use more salt.

  1. Confirm eligibility. Ask a clinician or pharmacist if you have kidney disease, abnormal potassium results, a prescribed potassium limit, pregnancy, childhood use, or medicines that affect potassium.
  2. Choose a product with a clear label. Identify potassium chloride, sodium per serving, potassium per serving, and the package warning.
  3. Measure it. Use the stated serving rather than free-pouring. Keep the container’s measuring spoon or use a standard kitchen measure.
  4. Reduce discretionary salt overall. Use less at the table and in recipes while building flavor with unsalted herbs, spices, acid, garlic, or aromatics.
  5. Audit the rest of the diet. Packaged and restaurant foods often supply more sodium than the salt shaker. Read nutrition panels and ask for lower-sodium preparation when eating out.
  6. Recheck after changes when advised. People with medical risks may need laboratory monitoring or a revised diet plan; do not start potassium products between appointments without asking.

Salt-free ways to add flavor when potassium is not appropriate

Use combinations rather than trying to make one ingredient imitate table salt. Citrus zest and juice brighten vegetables and fish; vinegar adds contrast to beans, salads, and sauces; garlic, onion, ginger, pepper, smoked paprika, cumin, rosemary, thyme, and chili can supply aroma and heat. Toasting spices, browning vegetables, and using unsalted stocks can deepen flavor without a potassium-chloride product.

Check commercial spice blends, seasoning packets, bouillon, and “salt-free” mixes. Some contain potassium chloride, while others contain sodium ingredients despite a healthy-sounding name. The ingredient list and nutrition panel decide which category a product belongs to.

Common mistakes and how to correct them

“Low sodium” means safe for everyone

Correction: Look for potassium chloride and potassium per serving. Lower sodium can come with higher potassium.

“Salt-free” means no medical concern

Correction: A potassium-only replacement may be sodium-free but potassium-rich. Follow your clinician-directed restriction.

The WHO recommendation covers packaged food

Correction: WHO’s recommendation is about discretionary table salt. It does not neutralize sodium already in manufactured or restaurant food.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A product’s percentage claim is a clinical guarantee

Correction: A manufacturer’s “50% less sodium” statement describes that product relative to regular salt under its labeling conditions. It does not predict your blood pressure response or establish safety for excluded groups.

Checking only one medicine

Correction: Potassium risk depends on the complete medication list, kidney function, laboratory values, and diet. Ask a pharmacist or clinician for a personalized review.

Bottom line for choosing what to use

For an eligible adult who already uses table salt, a potassium-containing lower-sodium substitute is an option under WHO’s conditional 2025 guidance, alongside reducing discretionary and total dietary sodium. It is not appropriate to assume safety for children, pregnancy, kidney impairment, potassium restriction, or medicine-related potassium risk. In those situations, use only what your healthcare team approves; salt-free herbs, spices, citrus, garlic, and vinegar can provide flavor without deliberately adding potassium chloride.
