The right delivery system depends on the payload, target cells, required duration of action, need for repeat dosing, immune history, safety margin, manufacturing process and route of administration. Lipid nanoparticles (LNPs) are synthetic carriers that can deliver RNA and other nucleic acids. Viral vectors—especially adeno-associated virus (AAV)—use engineered viral particles to deliver genetic material and can provide long-lasting expression, but they bring different payload, immunity and manufacturing constraints.
Comparing the platforms as if they were interchangeable shipping containers leads to poor decisions. A therapy intended to silence a gene may need a very different carrier from one intended to replace a missing gene or edit DNA. The useful question is not “Which platform is better?” but “Which platform fits this payload, tissue, duration and clinical risk profile?”

What the two delivery platforms are
Lipid nanoparticles are synthetic carriers
An LNP packages nucleic acid inside a particle made from lipids rather than a virus. A common formulation uses four functional components: an ionizable or cationic lipid, a helper lipid, cholesterol and a PEG-lipid. The proportions and chemical identities determine particle size, stability, biodistribution, cellular uptake and release of the payload. The Nature Reviews Genetics review of RNA delivery systems describes these components as the basis of many clinically used lipid structures.

After uptake, formulation chemistry is intended to promote intracellular release, often called endosomal escape. This is not a single standardized technology: changing the ionizable lipid, helper lipid, PEG-lipid, particle size or administration route can change where the particles travel and how efficiently cells receive the payload.

Viral vectors use engineered virus particles
A viral vector is a modified virus used as a genetic delivery vehicle. The vector is engineered to remove or disable disease-causing functions while retaining the ability to enter selected cells. “Viral vector” covers several families, including AAV, lentiviral and adenoviral systems; their biology and risk profiles are not identical. AAV is the most relevant example for many in-vivo gene-therapy discussions, but conclusions about AAV should not be presented as conclusions about every viral vector.
AAV vectors use a protein capsid and a packaged genetic cassette. Different capsids and serotypes have different tissue tropisms, meaning they tend to reach some tissues more readily than others. Vector design, dose and route still influence the actual distribution. The 2024 review in Signal Transduction and Targeted Therapy notes that AAV is non-pathogenic, rarely integrates into the host genome and can sustain long-term transgene expression, while also discussing immune responses, high-dose adverse effects, genotoxicity and manufacturing challenges.
How do lipid nanoparticles compare with viral vectors?
| Decision factor | Lipid nanoparticles | Viral vectors, especially AAV |
|---|---|---|
| Carrier | Synthetic lipid formulation, commonly combining an ionizable or cationic lipid, helper lipid, cholesterol and PEG-lipid. | Engineered viral particle; capsid and vector design influence cell entry and tissue tropism. |
| Typical payloads | siRNA, mRNA and other nucleic acids. Payload choice and formulation determine whether the effect is silencing, protein production or another mechanism. | Genetic cassettes for gene addition and related strategies, subject to the vector’s limited cargo capacity. |
| Expression duration | Many RNA effects are transient, although duration varies with the payload, chemistry, formulation and biology of the target. | AAV can support long-term transgene expression; durable expression is not automatically desirable for every therapy. |
| Tissue delivery | Biodistribution and cell targeting are major formulation and route-design challenges. | Natural or engineered tropism can help reach particular tissues, but precise targeting remains difficult and depends on capsid, dose and route. |
| Immune and safety issues | Avoids viral proteins, but lipids, dose, biodistribution and innate immune activation can still create toxicity or other concerns. | Pre-existing or treatment-induced immunity can limit use; high doses have been associated in the literature with concerns such as hepatotoxicity, thrombotic microangiopathy, neurotoxicity and genotoxicity. |
| Repeat dosing | Repeat administration can be a design attraction, but feasibility depends on the formulation, interval, indication and patient response. | Antibodies and other immune responses can make repeat administration difficult, though the practical answer depends on the specific vector and clinical setting. |
| Manufacturing | Requires reproducible lipid synthesis, nucleic-acid loading, particle production, purification, characterization and scale-up. | Requires reproducible production of a potent, appropriately characterized viral vector; capacity, yield, purification and consistency remain major challenges. |
This comparison describes platform tendencies, not guarantees. A specific LNP product or viral vector can behave differently from the general category.

Gene silencing
Silencing therapies reduce production of a disease-associated protein rather than adding a replacement gene. Small interfering RNA (siRNA) is one example. Patisiran, used for hereditary transthyretin amyloidosis, is an siRNA medicine delivered with a lipid-based system. Its clinical use demonstrates that lipid delivery can support repeated pharmacologic gene silencing; it is not equivalent to permanent gene replacement.
Transient protein production
Messenger RNA (mRNA) instructs cells to make a protein for a limited period. LNP-mRNA systems are designed to protect the mRNA, enter cells and release it into the cytoplasm, where it can be translated without entering the nucleus. The Nature Reviews Materials review of LNPs for mRNA delivery describes the design and translation issues involved in this approach. How long the protein remains available depends on mRNA stability, cell biology, protein half-life and the formulation—not simply on the label “LNP.”
Gene addition
Gene-addition therapy delivers a functional genetic sequence so cells can produce a needed protein. AAV is prominent in this area because it can provide durable expression in suitable tissues. The tradeoff is a relatively small cargo capacity and the possibility that the therapeutic cassette cannot include a large gene, regulatory element or multiple components in one vector.

Genome editing
Genome editing may require a nuclease, guide RNA, donor template or several components. The delivery system must get the complete editing payload into the right cells at the right time. A transient exposure can be attractive when continued nuclease activity would increase off-target risk, while some diseases may require long-lasting expression of a therapeutic protein after editing. The choice therefore depends on the editing architecture, not just on whether the carrier is viral or nonviral.
Tissue targeting and biodistribution determine real-world performance
A carrier can be potent in a laboratory assay yet fail clinically if too little payload reaches the intended cells. Biodistribution asks where the particles or vectors travel after administration; targeting asks whether enough of that material enters the disease-relevant cells. Both platforms face this problem.
What controls LNP distribution
LNP composition, particle size, surface characteristics, dose, administration route and the chemical structure of the ionizable lipid all affect distribution. Formulations may accumulate in organs that clear nanoparticles efficiently, while other tissues remain difficult to reach. The review “Lipid-based nucleic acid therapeutics with in vivo efficacy” identifies efficacy, biodistribution and toxicity as linked formulation considerations. Improving delivery to one organ can change exposure in another, so tissue selectivity must be measured for the actual product.
What controls AAV distribution
AAV capsid or serotype, dose, route and the patient’s antibodies affect where a vector can go. Engineered capsids may improve access to a target tissue, but a change that increases tropism can also alter off-target exposure or immune recognition. AAV’s ability to express a transgene for a long time is useful only if the vector reaches enough target cells and the expression level remains appropriate.
The 2026 review “Engineering challenges and translational opportunities in emerging gene delivery platforms” treats targeting and translation as central engineering problems across AAV, LNP and other systems. No platform is uniformly best for every organ or disease.
Duration: transient activity versus durable expression
Duration should be selected from the disease mechanism. A transient effect may be preferable when a protein must be periodically adjusted, when a toxic gene product should be suppressed only during a defined window, or when an editing reagent should not remain active indefinitely. In those cases, an LNP carrying RNA can offer a controllable exposure pattern, potentially with repeat dosing.

Long-lasting expression can reduce the need for frequent treatment when a patient needs continuous production of a missing protein. AAV is often considered for this role because episomal vector genomes can support sustained expression in appropriate nondividing cells. However, durable expression creates a different safety obligation: an unwanted level of expression, an adverse immune response or an off-target biological effect may be difficult to reverse. “Long-term” is therefore a design characteristic, not an automatic benefit.
Immune responses and safety are platform-specific, not binary
LNP considerations
LNPs do not display viral capsid proteins, which can avoid some barriers associated with viral-vector immunity. That does not make them non-immunogenic or universally safe. Lipid chemistry, PEG-lipid exposure, impurities, dose and innate immune sensing can affect tolerability. Repeated administration may alter the response to the formulation or reduce effective exposure. The 2017 review “Lipid Nanoparticle Systems for Enabling Gene Therapies” discusses formulation and translation challenges rather than treating all LNPs as interchangeable.
AAV considerations
Patients may have pre-existing antibodies to an AAV capsid, which can reduce delivery or exclude them from a treatment protocol. A new immune response can also limit subsequent dosing. Dose-related toxicity is an important concern because achieving enough vector in a difficult-to-reach tissue may require a high systemic dose. The 2024 AAV review discusses hepatotoxicity, thrombotic microangiopathy and neurotoxicity in the context of high-dose viral therapies, as well as genotoxicity and other safety questions.
Risk depends on the vector design, dose, route, disease, patient and monitoring plan. It is inaccurate to summarize the choice as “safe LNP versus dangerous virus,” or to promise that an LNP can always be redosed or that an AAV can never be administered again.
Repeat dosing and treatment logistics
Repeat dosing is often a major reason to consider a nonviral carrier. If the therapeutic effect naturally fades, an LNP program can be designed around additional doses, provided the formulation remains tolerable and effective. The interval may be determined by RNA persistence, protein turnover, disease progression and immune or laboratory findings. Repeat-dose performance must be demonstrated for the actual product.
AAV’s durability can reduce the theoretical need for redosing, but immune barriers complicate that option. Neutralizing antibodies may develop after the first administration, and the immune system can recognize the capsid on a subsequent dose. Strategies to manage this problem are an active area of development, not a settled property of every AAV therapy.

Manufacturing and clinical translation
Manufacturing is part of the therapeutic design because a promising carrier must be produced consistently at clinical scale. For LNPs, developers must control lipid identity and purity, particle size distribution, encapsulation efficiency, payload integrity, concentration, sterility and storage stability. Scale-up can change mixing conditions and therefore particle properties. The process must also preserve activity from the manufacturing site through transport and administration.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteAAV production involves vector genome construction, capsid production, purification, removal of empty or defective particles, control of residual impurities and measurement of potency. Yield and batch consistency can be difficult, particularly when a large amount of vector is needed for systemic dosing. Manufacturing capacity and analytical methods influence timelines, cost and the feasibility of commercial supply.
The 2026 platform review and the 2024 AAV review both place manufacturing and translation alongside biology as core challenges. It is not defensible to claim that one platform is always cheaper, faster or easier to scale. Those outcomes depend on the specific formulation, vector, facility, dose and quality requirements.
A practical decision framework for developers and readers
When evaluating a gene-therapy design, work through these questions in order:
- What is the biological objective? Identify whether the therapy silences a gene, supplies a protein, adds a gene, edits DNA or combines several functions.
- What payload must be delivered? Check the size, number and chemical stability of the required nucleic-acid components. An AAV cargo limit may rule out a large gene or multi-component system; an LNP may need a formulation capable of protecting and releasing several RNA species.
- Which cells must receive it? Define the target cell type, organ and percentage of cells that must be reached. Compare biodistribution data for the actual formulation or capsid rather than relying on the platform name.
- How long should activity last? Decide whether transient, repeatable exposure or durable expression best matches the disease. Do not assume that longer expression is always better.
- Can the patient receive another dose? Consider pre-existing antibodies, anticipated immune responses, formulation tolerability, disease progression and the consequences of losing effect.
- What safety margin is acceptable? Examine dose, off-target distribution, innate and adaptive immune effects, organ toxicity, genotoxicity concerns and the ability to stop or reverse activity.
- Can the product be manufactured and delivered reliably? Evaluate scale-up, release assays, storage, route of administration, clinical-site requirements and batch consistency.
This framework compares complete therapy designs rather than abstract labels. It also helps explain why two products using the same broad carrier can have different efficacy and risk profiles.
Examples that clarify the distinction
Patisiran and RNA silencing
Patisiran is an siRNA treatment for hereditary transthyretin amyloidosis. Its purpose is to reduce production of a harmful protein, and lipid-based delivery enables the siRNA to reach the intracellular pathway required for silencing. This is a pharmacologic RNA intervention, not a claim that every LNP provides permanent gene correction.
Best Value
LNP-mRNA medicines
An mRNA LNP delivers instructions for temporary protein production. The payload acts in the cytoplasm and is eventually degraded, while the protein’s persistence depends on its own biology. This approach can be useful when a controlled, repeatable pulse of protein is preferable to a permanently active transgene.
AAV gene addition
An AAV vector can deliver a therapeutic cassette designed for sustained expression in suitable cells. Its small cargo capacity, capsid immunity, dose requirements and manufacturing complexity must be weighed against the value of reducing the need for repeated treatment.
How to interpret evidence and clinical status
Gene-therapy evidence changes quickly. The 2026 comparative review is current to its publication but was available only as a preview. Older reviews remain valuable for mechanisms and design principles, not for a current count of approved products or a definitive market ranking. Approval totals are time-sensitive and depend on how categories are defined, so they should not be converted into a global head-to-head score.

Clinical results also cannot be compared without checking whether studies are in vivo or ex vivo, which route and dose were used, which disease and cell type were treated, and what endpoint was measured. A result for an LNP siRNA medicine does not establish the performance of an LNP-mRNA vaccine or an LNP genome-editing system. Likewise, an AAV result in one tissue or serotype does not describe every viral vector.
The bottom line
Lipid nanoparticles and viral vectors solve overlapping but distinct delivery problems. LNPs offer a flexible synthetic format for siRNA, mRNA and other nucleic acids, with the possibility of repeat dosing but continuing challenges in tissue targeting, toxicity and efficiency. AAV and other viral vectors can provide efficient delivery and, in suitable designs, long-term expression, but limited cargo capacity, immune barriers, dose-related safety concerns and manufacturing demands can constrain their use.
The best choice is the one that matches the payload, target cells, desired duration, immune history, redosing plan, safety requirements and manufacturing reality of a specific therapy. Treat “LNP” and “viral vector” as starting points for analysis—not as complete predictions of benefit or risk.
