Large-Scale Genetic Testing for Common Disease Risk: What It Can—and Can’t—Tell You

Large-scale genetic testing can measure many DNA variants at once, but a broad scan does not automatically predict whether you will develop a common disease. The result depends on what the test measures and how the findings are interpreted. For common, multifactorial diseases, a polygenic risk score (PRS) estimates risk; it is not a diagnosis, and it must be considered alongside factors such as family history and clinical screening guidance.

What does “large-scale gene scanning” mean?

It describes a family of approaches that examine many genetic variants in one test. Next-generation sequencing (NGS) is a measurement technology: it can sequence large sections of a person’s genome. Whole-exome sequencing (WES) focuses on the exome, while whole-genome sequencing (WGS) examines the genome more broadly. A disease-specific panel targets a selected set of genes or variants. These approaches differ in what they measure; none, by itself, guarantees a meaningful prediction of common-disease risk. The U.S. Food and Drug Administration describes NGS as an advance that can rapidly sequence large sections of a genome in clinical precision medicine (FDA, “Precision Medicine”).

A PRS is different: it is an interpretation method, not a sequencing technology. It combines information from many variants to estimate a person’s relative or overall likelihood of a particular outcome, according to the score’s design. A scan may provide genetic data that could be used in an analysis, but the score and its evidence determine what the result can reasonably say.

How do sequencing and risk interpretation differ?

Approach What it does What it does not establish by itself
WGS or WES Sequences broad portions of DNA; WGS covers the genome more broadly, while WES focuses on the exome. That every measured variant is clinically meaningful, or that the test provides a validated common-disease prediction.
Disease-specific genetic panel Examines selected genes or variants relevant to a defined question. Risk outside the selected targets, or a comprehensive picture of common-disease risk.
Polygenic risk score Uses many variants to produce a statistical estimate for a particular disease or outcome. A diagnosis, certainty about an individual’s future, or a guarantee that using the score improves health.

The distinctions matter because “more variants” is not the same as “more useful.” FDA guidance on NGS-based in-vitro diagnostics addresses analytical validation—whether a test reliably detects what it claims to measure—but accurate measurement alone does not prove that a score predicts disease well or changes care for the better (FDA, April 2018).

What can a result say about a common disease?

Common diseases usually arise through a mix of genetic and non-genetic influences. A PRS can indicate that someone’s estimated genetic risk is higher or lower than a reference or comparison group, depending on the particular score and how it is reported. It cannot say with certainty whether that person will develop the disease. The American College of Medical Genetics and Genomics (ACMG) describes PRS results as statistical predictions of increased clinical risk, not diagnoses. A low score does not rule out significant risk (ACMG, 2023).

Population fit also matters. A score developed or validated in a population unlike the person receiving it may have poor predictive value for them. Do not assume a score performs equally across ancestry groups unless there is evidence for that score in the relevant population. No single accuracy figure applies to all broad genetic scans: performance depends on the named test or score, disease, population, and intended use.

How to judge whether a test is meaningful

Four separate questions help distinguish a technically impressive scan from a clinically useful one. The FDA and the National Human Genome Research Institute (NHGRI) discuss test validation and evidence; ACMG’s population-screening guidance also emphasizes whether screening connects people to beneficial care.

  • Analytical validity: Does the laboratory detect the variants it claims to measure accurately and reliably? This is a question about the assay and laboratory performance, not whether a detected variant predicts disease.
  • Clinical validity: Does the variant or score predict the specific disease or outcome claimed, for the population and use case in question? NHGRI’s report highlights the importance of association evidence, replication, and testing in a representative population (NHGRI, “Promoting Safe and Effective Genetic Testing in the United States”). A statistically significant association does not by itself show that a variant causes disease: a common polymorphism may be associated with increased risk without being necessary or sufficient for the disease.
  • Clinical utility: Does acting on the result help someone make a better decision about diagnosis, treatment, management, or prevention? A valid prediction does not automatically demonstrate that using it improves health outcomes (USPSTF technical brief, 2021).
  • Ethical, legal, and social implications: How will consent, privacy, family implications, access, and equity be handled? These are part of the ACCE framework described in the USPSTF technical brief.

Screening is not the same as diagnostic testing

Screening may be offered to people without a specific prior reason to suspect a disease. Diagnostic testing is generally prompted by symptoms, family history, or other evidence that raises the likelihood of a condition. A risk result from screening should not be treated as a diagnosis or substituted for an appropriate clinical evaluation. ACMG says population-screening programs can benefit people only when risk identification is linked to evidence-based risk-reducing care (ACMG, 2021).

Broad scans can reveal other kinds of findings

Exome or genome testing may also identify possible monogenic disease risks, carrier status, or pharmacogenomic findings—information about how genes may affect responses to medicines. These are distinct from polygenic risk estimates for common disease. They have different evidence standards and care pathways, so a finding in one category should not be interpreted as if it were another. ACMG’s guidance on elective exome and genome testing discusses these possible categories (ACMG, 2021).

Questions to ask before acting on a result

  • What exactly was measured: WGS, WES, a targeted panel, or a PRS based on existing data?
  • Which disease and outcome does the result address, and what evidence supports that use?
  • Has the score been evaluated in a population relevant to me?
  • What would change in my care because of this result, and is that change supported by evidence?
  • How should the result be considered alongside my family history, age, health history, and established screening recommendations?
  • What consent, privacy, family, and follow-up interpretation arrangements apply?

ACMG advises that genetic risk results be interpreted in context rather than used alone (ACMG, 2023; ACMG, 2021). A clinician can help determine whether a finding calls for follow-up; a genetic counselor may also be relevant when the result, family implications, or testing choices need explanation.
