Large-Breed Dog Preventive Care: A Checklist for Joints, Heart Health, and Weight

The most effective preventive plan for a large-breed dog is simple in principle but consistent in practice: keep growth and adult body condition lean, use controlled exercise to protect developing joints, give FDA-approved heartworm prevention year-round with testing every 12 months, and make each veterinary examination a full review of nutrition, mobility, parasites, vaccines, dental health and age-related risks. Puppies, seniors and dogs with chronic disease usually need visits more often than once a year.

Why large dogs need a deliberate preventive-care plan

Large and giant breeds place substantial force on their bones, joints and cardiovascular system. Hip dysplasia is a developmental disorder seen most often in large-breed dogs, and risk reflects several interacting factors: heredity, nutrition, growth rate and exercise. Excess body fat adds orthopedic strain and is associated with osteoarthritis and other disease. Heartworm infection can produce severe lung disease, heart failure and permanent damage.

Prevention cannot remove inherited risk, but it can reduce avoidable stress and identify problems earlier. The plan should change as the dog grows, is neutered, becomes more or less active, gains or loses weight, or develops new symptoms.

Keep puppy growth and adult weight lean

Choose food for the expected adult size

If a puppy is expected to weigh at least 70 lb as an adult, American Animal Hospital Association (AAHA) obesity-prevention guidance recommends a diet formulated for large-breed growth. The 70-lb threshold refers to predicted adult weight, not the puppy’s current weight. Ask your veterinarian to confirm the prediction and the appropriate food rather than switching on the basis of breed name alone.

Large-breed puppy food is intended to support steady skeletal development, not the fastest possible growth. Rapid or excessive growth can increase orthopedic risk. Do not add calcium, high-calorie toppers or growth supplements unless the veterinarian has identified a specific need; an unbalanced addition can undermine an otherwise complete diet.

Measure the whole calorie budget

At every examination, ask for a body-condition score and a daily calorie target. The target must include meals, treats, dental chews, toppers, table food and food used to give medication. AAHA recommends a nutritional assessment regularly, ideally at each examination.

  • Weigh or measure the main ration instead of filling a bowl by eye.
  • Record treats and extras for several days so the veterinarian can see the real intake.
  • Recalculate portions when growth, neutering status, activity or body condition changes.
  • Use the dog’s body condition, not appetite alone, to decide whether portions need adjustment.

A lean dog should have ribs that are easy to feel beneath a thin covering, a visible waist when viewed from above and an abdominal tuck from the side. Your veterinarian’s scoring system is the reference point; agree on the desired score and how often to recheck it.

Why preventing excess weight matters

Every additional pound increases the load carried by already stressed hips, elbows, knees and spine. Weight reduction is harder after arthritis or pain has limited activity, so preventing gain during growth and early adulthood is easier than treating its consequences. If your dog is gaining steadily despite measured portions, bring the food log to the clinic rather than making a drastic diet change without guidance.

Protect developing joints with safe activity

Use controlled exercise during growth

Growing puppies need movement to develop muscle and coordination, but forced or repetitive high-impact exercise can overload immature joints. Favor several short, controlled sessions over endurance work. Let the puppy set the pace for walking, use play on surfaces with good traction, and stop when coordination deteriorates, the puppy lags, or stiffness persists afterward.

Avoid repeated long runs, forced jogging, frequent jumping from heights and sharp, high-speed turns on slippery floors while the skeleton is developing. Exercise should build strength without causing pain or prolonged stiffness. Your veterinarian can tailor activity to age, breed, body condition and any orthopedic findings.

Recognize signs that need an orthopedic examination

  • Lameness, even if it appears only after exercise
  • Stiffness when rising after rest or following activity
  • Reduced range of motion or pain when a hip is moved
  • A “bunny-hopping” gait while running
  • Reluctance to climb stairs, jump, play or get into a vehicle

Intermittent signs are easy to miss in the examination room. Record a video of the gait at home, on a walk and when the dog rises from rest, then show it to the veterinarian.

How hip dysplasia is evaluated and managed

Hip dysplasia is diagnosed and staged with a physical examination and imaging. Radiographs are commonly used; stress radiography may be appropriate in selected cases for diagnosis or treatment planning. The veterinarian may recommend weight reduction, limiting exercise on hard surfaces, physical therapy and veterinary anti-inflammatory treatment for an affected dog. Do not start human pain medicine or an over-the-counter joint product without veterinary advice.

No supplement has been established as a universal way to prevent hip dysplasia. The practical prevention priorities remain lean growth, appropriate nutrition, safe conditioning and prompt assessment of abnormal movement.

Provide year-round heartworm protection and testing

The core heartworm schedule

The American Heartworm Society recommends FDA-approved heartworm prevention throughout the year and testing every 12 months. Annual testing is still recommended when prevention has been given continuously because missed doses, vomiting, administration errors and imperfect drug absorption can occur, and no preventive eliminates every risk.

Year-round prevention is the single most important step owners can take to reduce the risk of heartworms, according to the American Heartworm Society (2018). Give each dose on the prescribed schedule and keep a record of the product, date and dose.

Puppies younger than 7 months

A puppy younger than 7 months can start prevention without a heartworm test. The American Heartworm Society advises testing six months after the initial visit, testing again six months later, and then testing annually. This schedule accounts for the time between infection and when a test can reliably detect disease.

Adults starting or restarting after a lapse

An adult dog beginning prevention for the first time, or restarting after a gap, needs veterinary-directed testing. Do not assume that a dog looks healthy enough to skip the test: early infection may have few visible signs, and giving some preventives to an infected dog requires a specific veterinary plan.

Symptoms that require prompt care

Coughing, reduced exercise tolerance, breathing difficulty, fainting or collapse can indicate serious cardiopulmonary disease, including heartworm disease. Treat breathing difficulty, collapse or sudden weakness as urgent and contact a veterinary service immediately rather than waiting for the next routine appointment.

Decide whether a large dog needs cardiac screening

There is no single test package for every asymptomatic dog

Breed, age, family history and examination findings determine whether an electrocardiogram (ECG), echocardiogram, blood-pressure measurement or another diagnostic test is appropriate. Current guidance does not establish one universal cardiac-screening protocol for every asymptomatic large-breed dog, including a single schedule for screening dilated cardiomyopathy.

Tell the veterinarian about fainting, unusual breathing, exercise intolerance, a new heart murmur, a family history of heart disease or a decline in stamina. Those details may justify targeted testing even when a dog appears normal at home.

Make the yearly examination comprehensive

What to review at each routine visit

Adult dogs generally need at least one examination each year. Puppies, seniors and dogs with chronic disease often need more frequent appointments. A useful visit covers more than vaccines:

  • Body-condition score, calorie target and the complete food-and-treat intake
  • Growth or weight trend and adjustment of portions
  • Core and lifestyle vaccines based on exposure and local requirements
  • Heartworm prevention, annual testing and other parasite control
  • Dental examination and home-care options
  • Mobility, gait, joint range of motion and exercise tolerance
  • Behavior, sleep, anxiety and changes in daily function
  • Reproductive decisions, including timing of neutering where relevant
  • Breed- and age-related risks, plus any need for blood pressure or cardiac diagnostics

A survey reported that 72% of canine patients were screened annually for heartworm and 94% of surveyed veterinarians reported recommending heartworm prevention. The survey year was not specified on the accessed page, so these figures describe that survey population rather than universal compliance.

Bring information that improves the appointment

  • A food-and-treat log, including chews, toppers and medication food
  • A current medication and supplement list with doses
  • The usual exercise pattern, including distance, surfaces and intensity
  • Videos of intermittent lameness, coughing, unusual breathing or difficulty rising
  • Dates of heartworm doses, missed doses, vomiting after dosing or recent travel

Compare preventive-care plans by what they actually include

Two clinics may both advertise an “annual checkup” while offering different follow-up and monitoring. Compare the elements below rather than the label alone.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Care area Minimum question to ask Why it matters for a large-breed dog
Growth and body condition Will you score body condition and set a calorie target, including treats? Lean growth reduces avoidable orthopedic strain and makes weight changes measurable.
Joint protection Will you assess gait, range of motion and exercise tolerance? Early stiffness or lameness can prompt activity changes and imaging before function declines.
Heartworm prevention How will year-round dosing and 12-month testing be tracked? Missed doses and delayed testing can allow infection to progress unnoticed.
Cardiac evaluation Which signs, breed risks or family history would trigger ECG, echocardiography or blood-pressure testing? There is no universal screening package; testing should match the dog’s risk and findings.
Follow-up access How quickly can we recheck weight, mobility or exercise intolerance? Portion changes, rehabilitation and diagnostic results often require adjustment rather than one annual decision.

Use this age-by-age checklist

Puppy period

  1. Estimate adult weight and select a diet formulated for large-breed growth when the prediction is at least 70 lb.
  2. Set measured portions and a body-condition recheck schedule.
  3. Start FDA-approved heartworm prevention; puppies younger than 7 months do not need a test before starting.
  4. Plan testing six months after the initial visit, again six months later, then annually.
  5. Build low-impact, controlled exercise and avoid forced repetitive impact.
  6. Discuss vaccines, parasite control, dental care, behavior and reproductive timing.

Adult period

  1. Attend at least one complete examination every year, with more frequent visits if recommended.
  2. Recalculate calories after neutering, changes in activity or any body-condition change.
  3. Continue year-round heartworm prevention and testing every 12 months.
  4. Report stiffness, lameness, coughing, fainting or reduced stamina rather than waiting for the next annual visit.
  5. Ask whether breed, age or family history warrants targeted cardiac testing.

Senior years or chronic disease

  1. Schedule examinations at the interval your veterinarian recommends; many seniors need more than one per year.
  2. Track weight, muscle, mobility, appetite, water intake and exercise tolerance at home.
  3. Review pain control, physical therapy, hard-surface exercise limits and any needed imaging.
  4. Reassess heartworm, vaccine, dental and parasite plans as health and exposure change.

What to do when something changes

Your dog is gaining weight

Measure every calorie, verify the target with the veterinarian and reduce extras before making a major change to the main diet. Recheck body condition and weight after the interval your clinic sets. Increased thirst, appetite, lethargy or sudden weight change also deserves a medical assessment.

Your dog becomes stiff or lame

Stop forced exercise and avoid hard surfaces and jumping until evaluated. Capture a video, note when the sign occurs and arrange an orthopedic examination. Do not give human anti-inflammatory drugs.

A heartworm dose was missed

Write down the date and product, contact the prescribing clinic and follow its instructions about giving the next dose and testing. Do not double doses or restart an adult dog after a prolonged lapse without veterinary direction.

Exercise tolerance suddenly falls

Reduced stamina, coughing, breathing difficulty, fainting or collapse can be signs of serious heart or lung disease. Seek prompt veterinary care; breathing difficulty or collapse is an emergency.
