Ketamine can cause short-lived nausea, drowsiness, dizziness, dissociation, altered perception, anxiety, headache, fatigue and temporary changes in blood pressure or heart rate. Those effects may occur during or soon after a supervised treatment, but they are not automatically harmless. Trouble breathing, collapse, a seizure, inability to wake the person, an irregular heartbeat, serious allergic symptoms or rapidly worsening illness require emergency help.
The medicine and setting matter. Ketamine injection (given intravenously or intramuscularly by a trained clinician) is not the same treatment context as monitored esketamine nasal spray. Repeated exposure also creates different urinary and liver risks from the brief sensations that can occur during one session.

Which side effects can occur during or soon after treatment?
Official information describes a range of possible effects, rather than a single list that every patient will experience. The Oxford Health NHS Foundation Trust leaflet, version 3.1 dated March 10, 2026, describes its own specialist-service experience; its observations should not be treated as universal rates for every dose, indication or formulation. Nausea, vomiting and injection-site discomfort
Nausea or vomiting can occur with ketamine injection, and the injection site may hurt. Tell staff promptly if vomiting is severe, you cannot keep fluids down, or you develop choking, breathing difficulty or marked weakness. Do not assume that a symptom is routine simply because nausea is listed as a possible effect.
Drowsiness, dizziness and fatigue
Sleepiness, dizziness and light-headedness are possible during or after treatment. The Oxford service notes that some patients feel tired for the rest of the day. Drowsiness can impair judgment and coordination even when the person feels mentally clearer, so follow the discharge instructions and arrange a safe way home.
Dissociation and altered perception
Dissociation can involve feeling detached from your body or surroundings; altered perception may include unusual sights, sounds or a changed sense of time. Anxiety can accompany these experiences. In a supervised service, staff can observe you and respond if the experience becomes distressing, prolonged or medically concerning. Severe confusion, agitation, uncontrolled movements or hallucinations should be reported immediately rather than “waited out.”

Headache and ringing in the ears (tinnitus) are listed in the Oxford service information. Blood pressure or heart rate can rise temporarily. Your clinician should decide whether a reading or symptom is acceptable for your treatment plan; do not use a home measurement to overrule the clinic’s instructions.

When is a ketamine reaction an emergency?
Call your local emergency number for severe breathing difficulty, collapse, a seizure, inability to awaken the person, or any rapidly worsening emergency. Emergency numbers and ambulance pathways differ by country. If the person is unconscious or having trouble breathing, do not drive them yourself; follow the dispatcher’s instructions.
Serious warning signs after ketamine injection
MedlinePlus identifies the following as serious effects requiring immediate medical attention:
- Difficulty breathing or swallowing.
- Hives, or swelling of the lips, tongue, throat or eyes, which can signal a serious allergic reaction.
- Dizziness with collapse, an irregular heartbeat or other severe cardiovascular symptoms.
- Severe confusion, agitation, hallucinations or uncontrolled muscle movements.
- Difficulty urinating.
- Symptoms that are rapidly intensifying instead of settling.
For breathing trouble, collapse, seizure or inability to wake, use emergency services immediately. For other serious symptoms, contact the treating clinician or emergency service promptly according to the written plan you were given; do not delay because the treatment was supervised.

Serious warning signs after esketamine nasal spray
MedlinePlus lists breathing problems, extreme sleepiness, fainting, chest pain, shortness of breath, sudden severe headache, vision changes and seizures as symptoms requiring immediate attention or emergency treatment. Esketamine treatment is designed to occur in a facility with observation before, during and for at least two hours after dosing. Tell the onsite team as soon as a symptom starts. If a serious symptom appears after discharge, use the emergency instructions from that facility or call local emergency services.

Long-term and repeated-use risks are different
Brief perceptual or gastrointestinal effects during a monitored session should not be confused with complications associated with repeated or long-term exposure. The FDA-approved ketamine injection label describes serious urinary and liver problems, including in people receiving recurrent dosing and in cases of abuse.
Urinary-tract injury and obstruction
The FDA label reports cystitis (bladder inflammation), reduced bladder capacity, ureteral stenosis, ureteral obstruction and hydronephrosis. An obstruction can impair kidney function and may require emergency intervention. Warning symptoms can include painful or frequent urination, urgency, blood in the urine, reduced urine output, inability to urinate or severe lower-abdominal or flank pain.

Contact the prescribing team urgently; if you cannot urinate, have severe pain, fever, vomiting or rapidly worsening symptoms, seek emergency care.

Liver injury and sclerosing cholangitis
The FDA label also describes liver injury, including ketamine-induced sclerosing cholangitis, a serious disease affecting the bile ducts. For a recurrent-dosing plan, clinicians should obtain baseline liver-function tests and decide how to monitor them. Suspected sclerosing cholangitis requires immediate discontinuation of ketamine and specialist assessment.
MedlinePlus lists possible liver-injury warning symptoms such as upper-right abdominal pain, dark urine, yellowing of the skin or eyes, an unexplained rash, flu-like symptoms, loss of appetite, nausea or vomiting and unusual lack of energy. Report these promptly to the clinician managing your treatment; jaundice, severe abdominal pain, confusion, fainting or rapid deterioration warrants emergency evaluation.

- Stay for the observation period. The administering clinician decides when your breathing, alertness, blood pressure and other observations are adequate for discharge.
- Have a responsible way home. Dizziness, dissociation and drowsiness can persist after you leave, even if the session itself felt manageable.
- Do not drive or operate machinery for at least 24 hours after injection. MedlinePlus gives this minimum because drowsiness can continue. Your clinician may impose a longer restriction based on your response or other medicines.
- Follow the exact written plan. Use the phone number supplied by the clinic for persistent vomiting, severe anxiety, unusual behavior, urinary symptoms or any symptom that worries you.
- Escalate quickly if danger signs appear. Breathing difficulty, collapse, seizure, inability to awaken, chest pain, severe allergic swelling or rapid worsening is an emergency, not a symptom to monitor alone.
Do not transfer the 24-hour driving instruction, observation period or other advice to a different product without confirming it with the clinician responsible for that medicine. The route, dose, indication and coexisting conditions change the risk assessment.
How ketamine injection and esketamine monitoring differ
Both treatments can involve sedation, dissociation and blood-pressure changes, but they are not interchangeable. Ketamine injection information describes intravenous or intramuscular administration by a specially trained provider with careful monitoring during and after administration. Esketamine nasal spray information specifies a facility-based dose and observation before, during and for at least two hours afterward.

Because the official instructions differ, use the discharge sheet and emergency number for the exact formulation you received. Advice found for an injection should not automatically be applied to a nasal product, and vice versa.
When to call the treating team instead of waiting
Contact the clinic promptly when an effect is severe, lasts longer than the team predicted, returns after initially improving, or feels unusual for you. Examples include persistent vomiting, worsening headache, distressing or prolonged dissociation, marked anxiety, tinnitus that does not settle, new urinary symptoms, yellowing of the eyes or skin, dark urine or upper-right abdominal pain.

This information helps the clinician distinguish a transient treatment effect from an adverse reaction or another illness. Do not take an extra dose, use someone else’s medicine or stop a recurrent-dosing plan without medical direction, except where emergency clinicians instruct otherwise.
What is and is not known about frequency
No single population-wide percentage reliably covers ketamine injection, esketamine, every dose, every indication and every treatment setting. The NHS leaflet provides context-specific information, not a universal incidence rate. A symptom being listed as “possible” does not predict that it will happen to you, and a symptom not listed in a short summary still deserves attention if it is severe or rapidly worsening.
The safest interpretation is contextual: transient nausea, drowsiness, dizziness, dissociation, altered perception, anxiety, headache, fatigue, tinnitus or temporary vital-sign changes can occur under supervision; breathing, cardiac, allergic, neurologic, urinary and hepatic warning signs need immediate clinical assessment at the level of urgency described above.
