The account came from media witnesses, not an official transcript. The state said it followed its protocol; Pike’s attorneys later alleged an IV-placement problem. The cause had not been established in the reporting available as of October 3.
What witnesses said happened
According to a Hindustan Times report drawing on media witnesses, Pike was strapped to a gurney in the death chamber at Riverbend Maximum Security Institution shortly before 7:30 p.m. CDT on Wednesday, September 30. The U.S. Supreme Court had overturned a stay about an hour earlier.
WPLN reporter Catherine Sweeney, who witnessed the execution attempt, said Pike was still breathing “very ragged” after two rounds, lifted her head and looked around. Sweeney said Pike described a spot that was burning and hurting and asked if that was normal. Sweeney added: “None of that was normal.” This is a witness account relayed by a news report, not a published official transcript.
WKRN reporter Tori Gessner separately reported hearing Pike say: “My arm feels like it’s about to bust. One spot is really throbbing.” That reported complaint about her arm is distinct from the question about the burning, painful spot.

Sweeney also said she had not seen two rounds in executions she had previously covered and recalled being told to leave at 8:53 p.m. while Pike was still breathing. These are witness descriptions of what they observed; they do not, by themselves, establish why the attempt did not proceed as intended.

As of the reporting available October 3, there was no final independent finding establishing the cause. The witness descriptions, the department’s account of protocol compliance and the lawyers’ IV-placement allegation are different kinds of evidence and should not be treated as interchangeable.

What Tennessee’s protocol context does—and does not—show
The Tennessee Department of Correction’s public overview says that, after a review, the state completed a revision to a single-drug lethal-injection protocol using pentobarbital in December 2024; executions resumed in May 2025. That general description and the department’s statement about the September 30 procedure provide context, but neither determines what medically happened during Pike’s attempt.
A September 28, 2026 filing by the Tennessee Solicitor General in the U.S. Supreme Court recounted the Tennessee Supreme Court’s scheduling of Pike’s execution for September 30 and summarized the state’s position in her challenge to the protocol. It preceded the attempt and is a procedural account from the state, not a neutral determination of the disputed medical facts.

Pike’s condition and the state’s next steps
The Associated Press reported on October 2, citing a court filing by Pike’s attorneys, that she was unconscious and on a ventilator in hospital. Her lawyers said medical staff were working to save her life and trying to clear the two doses from her system. That report described her condition at that time; it did not establish her eventual outcome.
Hindustan Times reported that Governor Bill Lee ordered a third-party review and paused Tennessee’s remaining scheduled executions for 2026. The review’s findings had not been published in the reporting available by October 3.

Why the incident drew attention
Pike was 50, according to contemporaneous reporting, and had been sentenced to death for the 1995 killing of 18-year-old Colleen Slemmer. The Associated Press reported that Pike was 18 when she and her boyfriend killed Slemmer. Hindustan Times noted that Pike would have been the first woman executed in Tennessee in more than 200 years; the execution attempt was halted, so that description remains conditional.
