Research reviews have found associations between bisphenol A (BPA) exposure and type 2 diabetes and cardiovascular disease, but the evidence does not establish that BPA causes either condition. Much of it comes from observational studies with limits in how exposure and disease were measured. The U.S. Food and Drug Administration (FDA), separately, says its ongoing review supports currently approved BPA uses in food containers and packaging.
What studies have found
Several evidence reviews report associations between BPA exposure and cardiometabolic outcomes. A 2023 umbrella review synthesized 14 reports of systematic reviews with meta-analyses, searching literature through July 12, 2023; it reported associations that included type 2 diabetes and cardiovascular disease. A separate 2024 umbrella review also reported associations with those two outcomes. These reviews summarize earlier studies rather than demonstrate cause and effect. Diabetes findings
A 2015 systematic review and meta-analysis covered 33 observational studies, including five prospective studies. For diabetes, its comparison of participants in the highest versus lowest urinary BPA groups produced a pooled odds ratio of 1.47 (95% confidence interval 1.21–1.80). This is a relative measure of association across the included studies, not a prediction of an individual’s absolute chance of developing diabetes. The review’s authors said more prospective research was needed to clarify possible causal links. Lin et al., 2015.

Cardiovascular findings
A 2020 analysis examined 11,857 adults in U.S. National Health and Nutrition Examination Survey data from 2003–2016 and reported an adjusted association between urinary BPA and cardiovascular disease. Its meta-analysis of five eligible studies included 22,878 subjects and also reported an association. The authors noted that previous evidence had been insufficient; the analysis adds epidemiological evidence but cannot establish that BPA caused disease. 2020 NHANES analysis.

The 2015 review also reported positive associations in five of five cross-sectional studies of cardiovascular disease and two of three cross-sectional studies of hypertension. Those counts describe the studies included in that review, not the proportion of people affected or a current estimate for the population.
Why an association does not prove BPA causes disease
Observational studies can identify patterns, but they cannot by themselves show that an exposure produced an outcome. Other differences between participants may help explain an association, and the timing of exposure and disease can be difficult to establish. A 2014 systematic review described epidemiological findings as inconsistent and highlighted frequent reliance on cross-sectional designs and a single BPA exposure measurement. The 2015 review likewise called for prospective research to clarify causality. Later reviews add breadth to the evidence summary, but they rely on underlying studies and do not remove those limitations. Results also concern different outcomes—such as type 2 diabetes, broader cardiovascular disease, coronary disease, or hypertension—which should not be treated as interchangeable.
How FDA’s assessment fits in
The FDA describes BPA as a chemical used in certain food-contact materials. It says its ongoing safety review supports currently approved uses in food containers and packaging. That is the agency’s regulatory assessment of those approved uses; it is distinct from epidemiological studies reporting associations. It neither proves nor disproves that BPA causes heart disease or diabetes across all possible exposures. FDA: Bisphenol A (BPA): Use in Food Contact Application.

A 2023 review identifies consumption of BPA-contaminated food as a common exposure route. Knowing that exposure can occur through food does not establish that a particular exposure level causes either disease. 2023 review of BPA and cardiovascular or cardiometabolic diseases.
