Inner speech vs speaking aloud: how the brain processes each one

Inner speech and speaking aloud use overlapping language and speech-planning machinery, but they are not the same brain event. Inner speech generates words without intentionally moving the larynx, tongue and lips to make an audible sound. Speaking aloud adds full motor execution, breathing, acoustic output and feedback through the ears.

The brain also predicts the sensory consequences of its own actions. A motor command can send an efference copy (also called a corollary discharge) to sensory systems, allowing the brain to label an event as self-generated. That prediction operates during overt speech and may also support some forms of inner speech, but inner speech is a diverse family of experiences rather than one uniform neural process.

What is the difference between inner speech and speaking aloud?

Inner speech is covert verbal cognition

Inner speech is the experience of using words without intentionally producing an audible utterance. You might silently rehearse a sentence, narrate a task, ask yourself a question, read in your head or simulate another person’s reply. The words are represented in a verbal format, yet there is no deliberate articulation that produces sound in the environment.

“Silent” does not mean that the brain is inactive. Depending on the type of inner speech, it can recruit language, speech-planning, motor-prediction and auditory-imagery systems. Some people experience a vivid voice with a strong sense of pitch and identity; others experience a more abstract stream of words. These differences matter because the neural pattern varies with the phenomenology.

Speaking aloud adds execution and feedback

Overt speech converts a linguistic plan into coordinated movement. Respiratory muscles provide airflow, the larynx generates a vocal source, and the tongue, lips, jaw and palate shape that source into recognizable speech. The resulting sound travels through the air to the ears, where auditory pathways analyze the acoustic feedback while the speaker is still talking.

Overt speech therefore combines at least four operations: constructing the message, planning its phonological and motor form, executing articulation and monitoring the sound that actually occurred. Inner speech can use the first two operations and may simulate the latter two, but it does not require intentional audible articulation or external acoustic feedback.

Where the two modes overlap—and where they diverge

Processing dimension Inner speech Speaking aloud
Motor output Speech planning may be engaged, but there is no intentional audible articulation. Recruitment differs by subtype. Full motor execution coordinates breathing, the larynx, tongue, lips, jaw and other articulators to produce sound.
Auditory input Auditory imagery or perceptual simulation may represent a voice without sound reaching the ears. Real acoustic feedback reaches auditory cortex during and after articulation.
Predictive monitoring A corollary-discharge prediction can help mark the imagined event as self-generated, even without overt sound. The prediction is compared with actual vocal-auditory feedback; the mismatch can be detected while speaking.
Neural uniformity Heterogeneous: monologic, dialogal, elicited and spontaneous forms can recruit different networks. Tasks differ, but every overt utterance includes articulation and sound production.
Clinical self-monitoring A failure to identify internally generated verbal activity has been proposed as one contributor to voices experienced as external in auditory verbal hallucinations. Abnormally weak suppression of self-generated speech sounds is studied as a related self-monitoring disturbance.

The overlap explains why inner speech can feel like “talking silently.” The differences explain why imagining a sentence does not create the same auditory response, muscle activity or timing as saying it.

How the brain predicts its own voice

The efference-copy idea

When the brain prepares an action, motor systems can send a parallel copy of the command to sensory regions. This copy predicts the action’s likely consequences before the sensory event is fully processed. For speech, the prediction can include when a sound should occur and what its acoustic pattern should be.

If the incoming signal matches the prediction, sensory processing can be reduced because the event is expected. If the signal differs, the mismatch receives more attention. This arrangement lets the brain distinguish the consequences of its own movement from an unexpected sound produced elsewhere.

Speaking-induced suppression

Ford and Mathalon’s review describes speaking-induced suppression (SIS) as the finding that sounds generated by a person’s overt speech produce a smaller neurophysiological response in auditory cortex than comparable externally generated sounds. The suppression is not the ear becoming insensitive; it is a predictive comparison in the brain.

During aloud speech, the brain has both a prediction and real acoustic feedback with which to compare it. Inner speech lacks the external sound, so there is no identical auditory signal to suppress. Nevertheless, a simulated sensory consequence can still be part of the experience, and a corollary-discharge mechanism may help maintain the sense that the words originated from oneself.

Inner speech is several phenomena, not one

A 2023 activation-likelihood-estimation meta-analysis by Pratts, Pobric and Yao found evidence for two broad mechanisms. One is a corollary-discharge mechanism that relies on speech-production regions, including left inferior frontal regions in the model. The other is a perceptual-simulation mechanism that relies more on speech-perception regions. The analysis also found that activation of speech-production regions is not observed in all inner-speech subtypes.

Monologic inner speech

Monologic inner speech is a one-speaker stream: silently narrating what you are doing, planning the next step or evaluating an event. It can involve language-generation and monitoring systems without the alternating roles found in a dialogue.

Dialogal inner speech

Dialogal inner speech has an exchange structure, such as asking yourself a question and generating an answer or mentally simulating another person’s perspective. The 2025 narrative review by Dahò and Monzani reports that dialogal forms recruit theory-of-mind networks, which help represent another person’s beliefs, intentions or viewpoint. That does not mean every silent conversation is a social judgment; it means that adopting distinct perspectives can engage networks used for social-cognitive reasoning.

Elicited inner speech

Elicited inner speech is produced in response to an instruction or task—for example, silently repeating a word during an experiment. The same 2025 review reports stronger activation of the left inferior frontal gyrus for task-elicited forms. A constrained repetition task is therefore not a complete model of the spontaneous inner monologue people experience in daily life.

Spontaneous inner speech

Spontaneous inner speech arises without an experimenter’s prompt. The 2025 review associates it with activity in Heschl’s gyrus, a region of auditory cortex. This finding is consistent with a perceptual or auditory-simulation component, but it does not establish that Heschl’s gyrus alone creates an inner voice.

Dahò and Monzani’s review covered 83 studies — PubMed literature review, 2025, published studies from 1990 through 2024. Its classification is useful because it prevents a common error: treating results from one highly controlled inner-speech task as if they described every form of silent verbal thought.

Which brain regions participate?

Speech-production and motor-planning regions

The left inferior frontal gyrus is repeatedly implicated in language production and in some forms of inner speech, especially elicited tasks. Premotor and other motor-related regions can prepare the articulatory pattern even when no audible movement follows. A 2025 meta-analysis reported increased activity in motor-related regions during inner speech and described distinct, inversely coupled audiomotor networks involving ventral premotor and auditory regions.

These observations support a network model. Motor-related activity can represent how a sentence would be articulated, while auditory regions represent what it would sound like. The balance between those components changes with the person’s experience and with the task.

Auditory cortex and perceptual simulation

Auditory cortex processes actual speech sounds during overt speaking and can also participate in internally generated auditory representations. Heschl’s gyrus is an early auditory-cortex region associated with spontaneous inner speech in the 2025 review. Superior temporal regions and other language-related areas are also discussed in models of self-monitoring and clinical voice-hearing.

Activity in auditory cortex during inner speech should not be interpreted as proof that a sound entered through the ears. Brain imaging measures neural activity, not whether an acoustic wave was present in the environment. An internally simulated voice and an external recording can therefore involve overlapping—but not identical—auditory processing.

Theory-of-mind and language networks

When inner speech takes a dialogal form, the brain may represent a speaker and a listener, or one perspective and another. That perspective management can recruit theory-of-mind networks in addition to language systems. The result is still private thought; the extra activity reflects the structure of the imagined exchange rather than an actual person speaking nearby.

Connections between regions

Clinical models also examine communication among auditory cortex, the left inferior frontal gyrus, superior temporal regions and language-related white-matter pathways. The important unit is the interaction among regions, not a single “inner-speech spot.”

What happens during inner speech, step by step?

  1. A message or goal is selected. You decide what to rehearse, evaluate or imagine saying. This can be deliberate, as in planning an email, or spontaneous, as in an unprompted thought.
  2. Linguistic structure is assembled. Words and grammar are organized into a phrase or sentence. The complexity of this stage depends on whether the thought is a fragment, a full sentence or a simulated conversation.
  3. A speech form may be prepared. Production-related regions can specify an articulatory plan, but the extent of this motor preparation varies. Some inner-speech experiences are strongly speech-like; others rely more on perceptual simulation.
  4. A sensory prediction may be generated. A corollary-discharge signal can predict what the imagined utterance would sound like and label it as self-produced.
  5. The representation is consciously experienced. You experience words, a voice, or both. Because there is no intentional articulation, there is no ordinary external sound to feed back through the ears.
  6. Monitoring checks ownership and meaning. The brain evaluates whether the verbal activity fits the expected self-generated pattern and whether it is relevant to the current task or context.

What happens when you speak aloud?

  1. Message and syntax are prepared. The speaker selects a meaning and converts it into linguistic form.
  2. Phonological and motor plans are generated. The sequence of speech sounds and the movements needed to produce them are organized.
  3. Breathing and articulation execute the plan. Respiratory muscles regulate airflow; the larynx and vocal tract shape that airflow into speech.
  4. Acoustic feedback arrives. The produced sound reaches the ears and activates auditory pathways while the utterance is still unfolding.
  5. Prediction is compared with sensation. The efference copy predicts the expected sound. Matching feedback is partly attenuated, producing speaking-induced suppression; a mismatch can produce a stronger error signal.
  6. Corrections are made. If the sound, timing or articulation is not as expected, the speaker can adjust the next movement or repeat part of the utterance.

Inner speech can approximate the first two stages and simulate later stages, but only overt speech includes the complete motor-and-acoustic loop.

Why does your voice sound different in your head?

The “voice” of inner speech is an internally constructed percept, not a recording arriving from the room. Its pitch, pace, emotional tone and sense of identity depend on the person’s verbal imagery and on which production or perception systems are engaged. Because there is no intentional articulation and no ordinary air-conducted acoustic feedback, the brain is not processing the same signal it would hear after speaking aloud.

This also explains why silently reading a sentence can feel more or less vivid than hearing it spoken. A task that encourages detailed auditory imagery may engage perceptual-simulation systems, whereas a task focused on articulatory rehearsal may rely more on speech-production regions. Neither experience is a universal template for all inner speech.

How does the brain know a thought is self-generated?

Ownership is inferred from agreement among several signals: the thought’s content, the predicted sensory consequences, the timing of the event and the context in which it appears. A verbal event that follows your own intention and matches the expected prediction is more likely to be tagged as self-generated. An unexpected event that does not fit the prediction is more likely to be treated as external.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Predictive monitoring is probabilistic, not infallible. The brain can misattribute the source of an experience when prediction, sensory evidence or contextual interpretation is disrupted. A mismatch does not mean that ordinary inner speech is abnormal; it means that the mechanisms used for source monitoring can fail under particular clinical conditions.

What inner speech research says about auditory verbal hallucinations

In psychosis research, one prominent hypothesis is that a failure of corollary discharge or self-monitoring can make internally generated verbal activity feel externally caused. The 2021 review discusses proposed involvement of auditory cortex, the left inferior frontal gyrus, superior temporal regions and language-related white-matter pathways.

This is a clinical model, not a claim that normal self-talk is a symptom. People without a psychiatric disorder routinely experience inner speech, and ordinary variation in vividness, frequency or tone is not evidence of psychosis. Auditory verbal hallucinations are experiences that are perceived as voices and can involve distress, impaired functioning or a sense of external agency; they require clinical assessment rather than self-diagnosis from a neuroscience article.

The connection with speaking-induced suppression is also a research relationship, not a one-to-one test. Abnormal suppression of self-generated speech sounds has been studied as a possible self-monitoring disturbance, but a laboratory measure cannot by itself explain an individual’s experience.

What do EEG, fNIRS, fMRI and PET studies actually show?

Different methods measure different aspects of processing. EEG tracks electrical activity with millisecond timing. fNIRS measures changes in oxygenation near the cortical surface. fMRI maps blood-oxygen-level changes with finer spatial detail, and PET measures metabolism or selected molecular signals. Because the methods have different strengths, they should not be expected to produce identical maps.

Across these methods, the evidence supports partially overlapping networks for inner and overt speech. It does not support one universal inner-versus-overt signature. A 2023 meta-analysis found distinct activation patterns when inner speech was divided by phenomenology, and the 2025 review likewise emphasized multiple forms and mechanisms.

A developmental EEG/fNIRS study comparing inner and overt speech interpreted differences in terms of motor preparation, phonological processing and auditory-feedback control. Developmental results are valuable for understanding how these systems emerge, but they should not automatically be generalized to every adult task, language or experimental setting.

What the evidence does—and does not—establish

  • Established: Inner speech can engage language and speech-planning systems without intentional audible articulation.
  • Established: Overt speech adds coordinated motor execution and real acoustic feedback.
  • Established: Predictive self-monitoring contributes to the reduced auditory response known as speaking-induced suppression.
  • Supported but qualified: Some inner-speech forms rely on speech-production regions, while others show stronger perceptual-simulation involvement.
  • Supported but qualified: Dialogal, elicited and spontaneous inner speech have different reported network profiles.
  • Not established: A single brain region that creates every inner voice.
  • Not established: One activation pattern that distinguishes all inner speech from all overt speech.
  • Not established: That hearing an inner voice in ordinary self-talk is itself pathological.

A practical way to interpret brain-scan claims about inner speech

  1. Identify the inner-speech task. Silent repetition, spontaneous thought, imagined dialogue and silent reading are not interchangeable.
  2. Check whether articulation was measured. A study may instruct participants not to move, but covert motor preparation can still vary.
  3. Ask whether there was an external sound. Overt speech includes acoustic feedback; inner speech usually does not.
  4. Note the measurement method. EEG timing, fMRI location and fNIRS surface sensitivity answer different questions.
  5. Look for a network result. Activation in one region does not show that the region alone generates the experience.
  6. Separate clinical from everyday findings. Results from people with hallucinations address self-monitoring disturbances in a clinical population and should not be generalized to everyone who thinks in words.

Bottom line

Inner speech is best understood as covert verbal cognition supported by a variable combination of language, motor-planning, predictive-monitoring and auditory-simulation systems. Speaking aloud uses some of the same machinery but completes the loop with respiratory and articulatory movement, real sound and auditory feedback. The brain’s efference-copy system helps identify self-generated speech, yet the exact balance between production and perception depends on whether the inner speech is monologic, dialogal, elicited or spontaneous. Current evidence supports interacting networks—not a single inner-speech center—and clinical findings about disrupted self-monitoring should not be mistaken for a description of normal self-talk.
