Those effects should be mild and brief. Persistent or worsening diarrhea, repeated vomiting, poor feeding, dehydration, blood, unusual sleepiness, breathing trouble, or poor weight gain need prompt pediatric advice. Facial, lip, tongue, or throat swelling, wheeze, or severe breathing difficulty is an emergency.
Probiotics are not one uniform treatment. Effects and risks depend on the exact microorganism (genus, species and strain), dose, formulation, storage and the baby’s medical condition. Products sold as supplements are generally regulated as foods rather than medicines, and no probiotic has been approved by the U.S. Food and Drug Administration (FDA) as a drug or biological product for infants.

Which infant probiotic effects can be normal?
A new probiotic can alter the mix of microorganisms and fermentation in the gut. In a healthy infant, that may produce extra gas or a temporary change in bowel movements. The National Institutes of Health Office of Dietary Supplements (2025) notes that probiotics may cause gas in healthy people and recommends discussing the strain, dose and duration with a health professional.

Gas and flatulence
More burping, passing gas, or a temporarily gurgly abdomen is the most commonly described mild reaction. If the baby remains comfortable, feeds normally, has the usual number of wet diapers and otherwise behaves normally, observation is usually reasonable while you contact the prescribing clinician if the symptom does not settle.

Mild abdominal discomfort
Some babies may draw up their legs, fuss briefly or appear uncomfortable after a dose. Discomfort should not be severe, progressively worse or associated with a hard, distended abdomen, repeated vomiting, lethargy or refusal to feed. Those combinations can indicate an illness unrelated to the probiotic and require medical assessment.
Temporary stool changes
Stool may become more or less frequent, softer, or different in color for a short period. Occasional abdominal discomfort, diarrhea and flatulence are also described in neonatal guidance for premature babies from Worcestershire Acute Hospitals NHS. In any infant, ongoing watery diarrhea, blood or mucus with illness, fewer wet diapers or poor feeding is not a “normal adjustment” to ignore.
No reliable published frequency estimate is available for mild probiotic side effects specifically in healthy, full-term infants. A new symptom should not automatically be blamed on the supplement: reflux, feeding intolerance, allergy, infection and ordinary changes in infant stool can look similar.
Why the same probiotic does not affect every baby the same way
Strain matters
A label that says only “Lactobacillus” or “Bifidobacterium” does not identify the complete product. The genus, species and strain determine the organism being given. Evidence for one strain cannot be assumed to apply to another, even when the names look similar.

Dose and formulation matter
Drops, powders and products mixed with milk can contain different organisms, doses and added ingredients. Viable counts can change during storage, and products sold as supplements do not undergo the same pre-market testing as medicines. The NHS, in a 2022 review, explains that probiotics are generally classed as food rather than medicine, so labels and live counts cannot always be assured.
Storage and handling matter
Follow the package’s storage instructions exactly. Heat, moisture, an opened bottle or an expired product may reduce viability or change the delivered dose. Do not substitute a different product because the package uses a similar marketing term. Ask the clinician or pharmacist to verify the exact product before switching.
How to monitor a baby after starting a probiotic
- Record the product. Write down the full label, including genus, species, strain, dose, other ingredients, lot number and expiration date. Keep the container or a photograph available for the clinician.
- Record the baseline. Note normal feeding, wet diapers, stool pattern, vomiting, temperature and behavior before the first dose. This helps distinguish a pre-existing problem from a change after starting.
- Give only the labeled or prescribed amount. Do not increase the dose to treat colic, diarrhea or reflux, and do not combine multiple probiotic products unless the treating clinician specifically directs it.
- Watch overall condition, not one diaper. A single softer stool or a little extra gas is less important than hydration, feeding, alertness and the direction of change over the following days.
- Contact the pediatric clinician if the change persists or worsens. Ask whether to pause, stop or change the product rather than making a substitution yourself. Bring the symptom record and product information to the conversation.
When to call a pediatric clinician
Call the baby’s pediatric clinician promptly when symptoms are repeated, significant, persistent or getting worse. The following signs deserve medical advice even if a probiotic was started recently:
| Sign | Why it matters | What to do |
|---|---|---|
| Repeated vomiting or forceful/projectile vomiting | Can cause dehydration and may signal reflux complications, obstruction or infection rather than a probiotic effect. | Contact the clinician promptly; seek urgent care if the baby cannot keep feeds down. |
| Significant or ongoing diarrhea | Fluid loss is especially important in young infants. | Ask for same-day guidance, particularly with fewer wet diapers or a dry mouth. |
| Refusal or inability to feed | Reduced intake can rapidly lead to dehydration and may indicate serious illness. | Obtain prompt medical assessment. |
| Fewer wet diapers or other dehydration signs | Suggests inadequate fluid intake or excessive loss. | Call promptly; follow the clinician’s instructions about feeding and fluids. |
| Blood in stool or vomit | Blood is not an expected probiotic adjustment effect. | Seek prompt medical evaluation. |
| Poor weight gain, unusual sleepiness or low energy | May indicate inadequate intake, dehydration or another disease process. | Arrange prompt assessment. |
| Fever, breathing difficulty or a generally deteriorating condition | These findings point beyond a simple gastrointestinal reaction. | Contact a clinician urgently; use emergency services for severe breathing problems. |
Mayo Clinic’s infant-reflux guidance likewise lists poor weight gain, projectile vomiting, green, yellow or bloody vomit, blood in stool, feeding refusal, breathing difficulty and low energy as reasons for medical evaluation. Those warning signs should be evaluated regardless of whether the infant is taking a probiotic.

Emergency symptoms: when to call emergency services
Facial, lip, tongue or throat swelling; wheeze; severe breathing difficulty; or rapidly developing cough, vomiting, diarrhea and an itchy rash can represent a serious allergic reaction. NHS guidance describes rapid allergic reactions and states that anaphylaxis requires urgent treatment. Call emergency services immediately for airway swelling or breathing trouble. Do not wait to see whether the next dose makes the reaction better or worse.
Repeated forceful vomiting with marked lethargy, collapse, blue or gray color, or inability to feed is also an emergency. Take the product container with you if it is safe to do so, but do not delay emergency care while looking for label details.
Can probiotics cause sepsis in premature babies?
Yes. Invasive infection linked to a probiotic organism is rare, but it has occurred and can be fatal in highly vulnerable infants. In a 2023 safety communication, the FDA described a preterm infant who developed Bifidobacterium longum sepsis and died after receiving a product identified as Evivo with MCT Oil. The FDA also emphasized that it has not approved any probiotic product as a drug or biological product for use in infants.

Who needs extra oversight?
- Babies born prematurely, especially those with very low or extremely low birth weight.
- Infants who are immunocompromised or seriously ill.
- Babies with a central venous line or another invasive medical device.
- Infants receiving intensive neonatal care or recovering from major illness.
Never start, stop or replace a probiotic for one of these babies without the treating neonatal or pediatric team’s direction. A probiotic that was tolerated by a healthy term sibling is not automatically appropriate for a premature or medically fragile infant.

What the preterm evidence shows
The American Academy of Pediatrics (AAP) said in a 2021 clinical report that evidence did not support routine, universal probiotic administration to preterm infants, particularly those weighing less than 1,000 grams at birth. The report cited variable commercial preparations, contamination recalls, uncertain long-term safety and reported probiotic-associated infections. It also noted that approximately 10% of extremely low-gestational-age neonates had received a probiotic during a neonatal intensive-care stay, while more than 10,000 premature infants had participated in randomized probiotic trials worldwide.
An AAP evidence review published in 2024 reported, in a 60-trial meta-analysis of very preterm infants, a 46% reduction in necrotizing enterocolitis, a 23% reduction in mortality and an 11% reduction in late-onset sepsis. The review rated certainty as low to moderate and found benefit uncertain for extremely low-birth-weight infants. Trial results do not remove the need to weigh product quality and real-world reports of probiotic sepsis and death with the infant’s own care team.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to decide whether an infant should use a probiotic
Before giving a product, ask the pediatrician or neonatal team to document five specifics:
- Exact organism: genus, species and strain, not just a broad category.
- Dose: the amount per serving and how often it should be given.
- Storage: refrigeration, room-temperature limits and handling after opening.
- Duration: when treatment should be reviewed or stopped.
- Purpose: the symptom or diagnosis the product is intended to address and what outcome would count as success.
Availability in a store or an online search for “infant probiotic drops” demonstrates only that a supplement is being sold. It does not establish safety, effectiveness or suitability for an individual baby. Ask whether the evidence applies to your infant’s age, gestational age, medical condition and the exact strain in the bottle.

What to do if mild symptoms appear
If the baby has only mild gas or a brief stool change and is feeding, urinating and behaving normally, continue careful observation and contact the clinician if it does not settle. Do not add antidiarrheal medicines, change formula or introduce another supplement without advice.
If symptoms are substantial, persistent or accompanied by any warning sign, stop treating the event as a routine adjustment: call the pediatric clinician and describe the timing, dose, feeds, wet diapers, stools, vomiting and temperature. The clinician may advise pausing the product, evaluating another cause or arranging an examination. For emergency allergic or breathing symptoms, seek emergency care first and discuss the probiotic afterward.
What “safe” means for healthy term infants
For a healthy, full-term baby, mild gas or a short-lived stool change can occur, but the available evidence does not provide a dependable rate for these effects. Safety also depends on quality control, accurate labeling and matching the strain to the intended use. Because infant probiotic supplements are not FDA-approved drugs or biologics, a reassuring label or a high live-count claim is not the same as proof that a particular product will help your baby.

Bottom line
Mild, short-lived gas, flatulence, abdominal discomfort or a stool change may occur after an infant starts a probiotic. Persistent diarrhea, repeated or forceful vomiting, poor feeding, dehydration, blood, poor growth, unusual sleepiness, fever or breathing problems require prompt medical advice. Swelling of the face or airway, wheeze or severe breathing difficulty is an emergency. Premature and medically fragile infants face a different risk profile, including rare but serious probiotic-associated sepsis, so their care team must make the decision.
