Immunotherapy and chemotherapy are not interchangeable treatments for bowel cancer. Chemotherapy uses cytotoxic drugs against cancer cells and can be used around surgery or for advanced disease. Immunotherapy, particularly checkpoint inhibitors, is mainly an option for a defined group of colorectal cancers whose tests show high microsatellite instability (MSI-H) or mismatch repair deficiency (dMMR), especially when disease is advanced or metastatic. The right choice depends on the tumour’s site and stage, molecular results, treatment goal, how quickly a response is needed, expected benefits, risks and practical burden.
What is the difference between chemotherapy and immunotherapy?
The two approaches attack cancer in fundamentally different ways. Chemotherapy delivers anti-cancer drugs that circulate through the bloodstream and directly damage or destroy cancer cells. Immunotherapy does not work as a conventional cell-killing drug; checkpoint inhibitors help the immune system recognise and attack cancer cells that have evaded immune surveillance.

| Feature | Chemotherapy | Immunotherapy |
|---|---|---|
| Basic action | Anti-cancer (cytotoxic) drugs circulate in the blood and destroy cancer cells. | Checkpoint inhibitors help the immune system kill cancer cells. |
| Where it may fit | Some colon cancers before surgery, after surgery to reduce recurrence risk, and advanced or metastatic disease; use differs between colon and rectal cancer. | Particular colorectal cancers, especially advanced or metastatic tumours with MSI-H or dMMR test results. |
| How it is given | Depending on the regimen, by injection, intravenous drip or tablets. | Intravenous infusion; nivolumab may be used alone or with ipilimumab. |
| What determines eligibility | Site, stage, operability, treatment goal, previous therapy and the clinical plan. | Those same clinical factors plus a biomarker showing MSI-H or dMMR in the setting covered by guidance. |
| Speed and timing considerations | May be selected when a faster tumour response is preferred or while genomic results are pending in some advanced cases. | Used when the tumour’s biology and clinical setting make immune checkpoint treatment appropriate. |
These categories overlap in real care. A person may receive chemotherapy at one point and immunotherapy at another, or a team may recommend one rather than the other. The names alone cannot determine the safest or most effective plan.
When is chemotherapy used for bowel cancer?
Before surgery
For some colon cancers, chemotherapy can be given before an operation to shrink a tumour. Whether this is appropriate depends on the cancer’s stage, its location and the surgical plan. “Bowel cancer” covers cancers of the colon and rectum, and treatment pathways are not identical for the two sites.

After surgery
Chemotherapy may be recommended after surgery to reduce the chance that cancer will return. The decision is tied to pathological findings and the overall risk assessment rather than to a single symptom or scan result.
Unresectable or metastatic disease
When colorectal cancer cannot be removed surgically or has spread to distant sites, chemotherapy is one possible systemic treatment. NICE discussions of advanced dMMR/MSI-H colorectal cancer list FOLFOX, FOLFIRI, CAPOX and capecitabine among chemotherapy options, particularly when testing results are delayed or a quicker response is preferred.
Which chemotherapy drugs and regimens are common?
Bowel Cancer UK lists fluorouracil (5-FU), capecitabine, irinotecan, oxaliplatin and trifluridine-tipiracil among commonly used drugs. They can be combined into named regimens or used in other schedules selected by the oncology team.
| Regimen or drug | What the cited patient information identifies | How it may be delivered |
|---|---|---|
| 5-FU (fluorouracil) | Common bowel-cancer chemotherapy drug. | Injection or intravenous treatment, depending on the prescribed schedule. |
| Capecitabine | Common bowel-cancer chemotherapy drug and one of the options listed in NICE’s advanced-disease discussion. | Tablet. |
| Oxaliplatin | Common drug, used in combinations such as FOLFOX and CAPOX. | Intravenous treatment in a combination regimen. |
| Irinotecan | Common drug, used in combinations such as FOLFIRI and FOLFOXIRI. | Intravenous treatment in a combination regimen. |
| Trifluridine-tipiracil | Common bowel-cancer chemotherapy drug listed by Bowel Cancer UK. | The exact schedule and route are set by the treating team. |
| FOLFOX | A named combination regimen. | Regimen-specific administration; the team explains the infusion and any tablet components. |
| FOLFIRI | A named combination regimen. | Regimen-specific administration. |
| FOLFOXIRI | A named combination regimen. | Regimen-specific administration. |
| CAPOX | A named combination regimen involving capecitabine and oxaliplatin. | Combines tablets and intravenous treatment. |
The regimen name does not by itself show which option is best. Doctors consider the tumour’s characteristics, previous treatment, general health, the intended outcome and how burdensome each schedule would be.
When can immunotherapy be an option?
MSI-H and dMMR are central eligibility clues
The clearest distinction in current colorectal guidance is biomarker-based. MSI-H means the tumour has high microsatellite instability, while dMMR means its mismatch-repair system is deficient. These features can indicate that checkpoint inhibition is a suitable treatment strategy in particular clinical settings.
NICE recommends pembrolizumab and nivolumab with ipilimumab as options in specified advanced or metastatic colorectal-cancer settings with MSI-H or dMMR. Its technology appraisal TA1065, published on 28 May 2025, describes nivolumab plus ipilimumab as an option for adults in England with untreated unresectable or metastatic disease, subject to the commercial arrangement stated in that recommendation.

The immunotherapies named in the guidance
- Pembrolizumab: a checkpoint inhibitor used in the defined MSI-H/dMMR advanced-disease pathway.
- Nivolumab: a checkpoint inhibitor that may be used alone for relevant bowel cancers.
- Nivolumab plus ipilimumab: a combination option in specified untreated unresectable or metastatic MSI-H/dMMR disease.
Bowel Cancer UK describes these medicines as infusions into a vein. Nivolumab can be given alone or with ipilimumab. The exact infusion interval, duration and monitoring plan are individual clinical decisions and should be confirmed with the hospital team.

Why the cancer’s stage and site change the decision
Colon versus rectum
Colon and rectal cancers are both colorectal cancers, but the anatomy affects surgery and the order in which treatments may be used. A recommendation for a colon tumour should not automatically be applied to a rectal tumour, or vice versa.
Localised disease
When a tumour is potentially removable, the plan may involve surgery with treatment before or after the operation. Chemotherapy has established roles in some of these settings. The cited guidance does not establish a universal immunotherapy role for every localised bowel cancer; eligibility still depends on tumour biology and the specific clinical pathway.
Unresectable or metastatic disease
Advanced disease is where the MSI-H/dMMR distinction is most directly reflected in the NICE recommendations cited here. “Unresectable” means the cancer cannot currently be removed completely with surgery; “metastatic” means it has spread to distant parts of the body. Treatment may aim to control disease, relieve symptoms, prolong life or make another intervention possible, depending on the individual situation.
What tests should be available before a systemic-treatment decision?
MMR and MSI testing
NICE recommends offering colorectal-cancer patients testing for dMMR/MSI at diagnosis. Laboratories can assess mismatch-repair proteins with immunohistochemistry (IHC) or examine microsatellite instability with polymerase chain reaction (PCR). Ask whether the result is recorded as proficient or deficient mismatch repair and whether the MSI result is stable or high; the report’s wording matters.
RAS and BRAF V600E testing
NICE recommends RAS and BRAF V600E testing for people with metastatic colorectal cancer who are suitable for systemic treatment. These results may affect the wider set of medicines a team can consider, even though they do not replace MSI/MMR testing for the immunotherapy question.

| Test or clinical detail | Why it matters to the conversation | Question to ask |
|---|---|---|
| MSI status | High MSI can support checkpoint-inhibitor eligibility in the relevant advanced-disease setting. | “Is the tumour MSI-high, or is it microsatellite stable?” |
| MMR status | dMMR identifies mismatch-repair deficiency and is used in the same treatment pathway. | “Does the report show dMMR or proficient MMR?” |
| RAS | Recommended in metastatic disease suitable for systemic treatment and can inform systemic options. | “What did the RAS result mean for the options?” |
| BRAF V600E | Recommended in metastatic disease and relevant to treatment planning. | “Was BRAF V600E tested, and does it change the plan?” |
| Stage and resectability | Separates peri-operative treatment decisions from treatment for unresectable or metastatic disease. | “Is the cancer localised, resectable, unresectable or metastatic?” |
How clinicians choose between the options
1. Define the immediate goal
First establish what treatment is meant to achieve: shrink a tumour before surgery, reduce recurrence risk after surgery, control advanced disease, relieve symptoms or seek a response quickly enough to enable another procedure. A treatment that is sensible for one goal may not be suitable for another.
2. Confirm the biological information
The team reviews MSI/MMR results and, in metastatic disease, RAS and BRAF V600E results. If genomic results are not yet available, the timing of treatment becomes part of the decision.
3. Discuss response speed
NICE notes that chemotherapy may be used first when MMR testing is delayed or when a faster response is preferred. That does not mean chemotherapy is universally faster or better; it means urgency and the expected clinical course can change the sequence.

NICE’s appraisal reports clinical benefit for nivolumab plus ipilimumab in its defined population. The cited material does not provide a complete, directly comparable table of survival, response rates or adverse-event rates across all bowel-cancer regimens. Ask the oncologist which evidence applies to the exact stage, biomarker result and treatment line under discussion.
5. Match treatment to practical circumstances
Administration route, appointment frequency, travel, time away from work or caring responsibilities, monitoring and access to symptom support can all affect the choice. An infusion-based option and a tablet-containing regimen create different day-to-day demands, even when both are medically reasonable.
Side effects, monitoring and day-to-day burden
It is misleading to describe one class as simply “stronger” or “milder.” Chemotherapy and checkpoint inhibitors have different toxicity patterns, and the risk depends on the exact drug or combination, dose, schedule and the person’s health. Cancer Research UK and Bowel Cancer UK advise discussing likely effects, warning signs and support with the clinical team; the cited sources do not provide a complete direct toxicity comparison suitable for ranking the classes.

Questions to settle before treatment starts
- Which effects are common with this exact regimen, and which require urgent contact?
- What blood tests, scans or other checks are needed before and during treatment?
- Who should be called outside clinic hours if a symptom appears?
- Could treatment be delayed, adjusted or stopped if side effects occur?
- How long is each appointment, and how often will hospital visits be needed?
- Will tablets be taken at home, and what happens if a dose is missed or vomited?
Keep the oncology team’s emergency number accessible. Do not wait for the next routine appointment to report a new or rapidly worsening symptom; the correct threshold for calling depends on the prescribed medicine.

In an ideal pathway, tumour testing is available when the treatment plan is made. In practice, a team may need to decide whether waiting for MMR/MSI information is safe. NICE’s discussion of unresectable or metastatic dMMR/MSI-H colorectal cancer recognises two practical possibilities: start chemotherapy while results are delayed, or choose chemotherapy when a faster response is clinically preferred. Once results arrive, the team can reassess whether immunotherapy is appropriate and whether the sequence should change.
Patients should ask whether a sample has already been tested, whether a repeat biopsy is needed and how long the result is expected to take. A delay is not automatically an error; it must be weighed against symptoms, tumour behaviour and the risks of postponing treatment.
Decision examples: how the same two words can lead to different plans
Example: potentially operable colon cancer
The team may focus on surgery and whether chemotherapy before or after the operation lowers recurrence risk. An immunotherapy discussion would require relevant tumour biology and a guideline-supported indication; the mere fact that immunotherapy exists does not make it a substitute for peri-operative chemotherapy.
Example: untreated, unresectable MSI-H/dMMR colorectal cancer in England
NICE TA1065 identifies nivolumab plus ipilimumab as an option for adults in this defined setting, subject to the recommendation’s commercial arrangement. Pembrolizumab is also described by NICE as an option for specified advanced or metastatic MSI-H/dMMR disease. The oncologist still considers symptoms, treatment urgency, fitness, other test results and the person’s preferences.

Example: advanced disease with MMR testing pending
If a response is needed quickly or the result will not be available promptly, NICE’s discussion allows for chemotherapy options such as FOLFOX, FOLFIRI, CAPOX or capecitabine to be considered first. This is a sequencing decision, not proof that the tumour is unsuitable for immunotherapy forever.
Best Value
Example: MSI-H/dMMR result absent
Without a documented MSI-H or dMMR result, a reader should not assume checkpoint-inhibitor eligibility. Ask the team whether testing was done at diagnosis, whether the pathology report is complete and whether another sample is required.
What the evidence can—and cannot—tell you
The strongest evidence summarized here concerns UK practice, particularly NICE guidance for advanced or metastatic colorectal cancer with MSI-H or dMMR. It does not establish one universal winner between immunotherapy and chemotherapy, a complete toxicity ranking or a single survival figure that applies to every bowel cancer.
NICE’s recommendations apply to NHS England. Other countries may use different approvals, funding rules, sequencing and testing pathways. Confirm the current local guideline and the medicine’s availability where treatment will be delivered.

A practical appointment checklist
- Write down whether the cancer is in the colon or rectum and its current stage.
- Ask whether it is resectable, unresectable or metastatic, and what that means for the goal of treatment.
- Request the MSI and MMR results in the wording used on the pathology report.
- For metastatic disease, ask whether RAS and BRAF V600E testing has been completed.
- Ask which options are recommended now and why one would be given before another.
- Clarify whether a faster response is important and whether waiting for a test result is safe.
- Record the expected benefits, uncertainties, likely monitoring and warning symptoms for the exact regimen.
- Check how treatment is delivered: infusion, injection, tablets or a combination; note appointment frequency and travel requirements.
- Ask who to contact urgently, including outside normal clinic hours.
- Confirm whether the recommendation follows local guidance and whether funding or availability rules apply.
Bottom line for patients and families
Chemotherapy is a drug-based treatment used in several bowel-cancer settings, including some treatment plans around surgery and advanced disease. Immunotherapy is a different mechanism and is most clearly supported for a subset of colorectal cancers with MSI-H or dMMR, particularly in advanced or metastatic disease covered by current guidance. The decision should be made from the tumour’s site, stage, test results, treatment goal, urgency and the practical and safety demands of the exact regimen—not from the treatment label alone.
