Researchers attach a polymer coating—most often methoxy polyethylene glycol (mPEG)—to the cell surface. In laboratory and animal studies, this has reduced some forms of immune recognition. The cells are not literally invisible, however, and the approach is not an available blood product: Canadian Blood Services says it remains at the laboratory-testing stage and has not been used in a patient.
Why red blood cell antigens matter
Red blood cells carry surface molecules called antigens. If a transfused cell carries an antigen the recipient lacks, the recipient may have antibodies that recognize it or may develop them after exposure. ABO and Rh antigens are especially important, but they are not the only ones that matter. People who have received repeated transfusions can develop antibodies to other red-cell antigens, making compatible units harder to find.

Routine transfusion care therefore relies on blood typing and compatibility testing. Immunocamouflage is being studied as a possible way to reduce recognition of selected antigens; it does not establish that blood matching or other safeguards can be skipped.

In the experimental process, researchers chemically attach a polymer such as mPEG to the red-cell membrane. The resulting surface layer can create steric and charge shielding around antigens, making it harder for antibodies to bind or for immune cells to recognize the modified cell. This is cell processing in a research setting, not a method for treating or preparing blood outside specialized laboratories.

“Go undetected” is shorthand for reduced detection in particular experiments. It does not mean every antigen is hidden from every antibody or immune pathway.
What studies have found
Anti-D assays: polymer size and concentration mattered
A 2015 study by Li and colleagues tested polymer-modified red cells with human anti-D antibodies, using flow cytometry and monocyte monolayer assays. In the reported in-vitro experiments, 20 kDa mPEG at grafting concentrations of at least 1.5 mM reached the paper’s stated clinically significant protection threshold: monocyte index values were at or below 5%. These are laboratory assay results, not patient outcomes, and the concentration and polymer size were important to the result. Read the 2015 study in the American Journal of Hematology.

Animal transfusion models showed reduced antibody responses
In a 2011 mouse study, mPEG-modified cells were associated with a greater than 90% reduction in anti-sheep-red-cell IgG after a primary transfusion, and greater than 80% reductions after two transfusions. Those figures refer to a specific antibody, species, and experimental model. They do not establish safety or effectiveness in people. Read the 2011 study in Science China Life Sciences.
PEG and PEOZ showed different experimental trade-offs
A 2014 laboratory comparison found mPEG more effective than PEOZ at reducing antibody binding and phagocytosis, while PEOZ had less effect on red-cell morphology. These findings describe different properties in the experiments; they do not identify a material selected for clinical use. Read the mPEG–PEOZ comparison in Biomaterials.

Why researchers are exploring the approach
For patients with antibodies against multiple red-cell antigens, locating compatible donor units can be difficult. In principle, masking selected antigens might expand options in situations where compatible blood is scarce. The 2015 anti-D paper also discussed a possible future role in easing D-negative inventory constraints in regions where D-negative blood is rare.
That is a proposed application, not evidence that modified D-positive blood can safely replace D-negative units in practice. The studies summarized here do not establish population-level risk reduction or clinical-trial outcomes.

Is immunocamouflaged blood available to patients?
No. Canadian Blood Services describes immunocamouflage as a laboratory-stage technique and says it has not yet been used in a patient. Its discussion of possible future relevance for hard-to-match patients, including compassionate-use potential recognized by Health Canada, is not evidence of a currently authorized product or routine transfusion option. Read the Canadian Blood Services explainer.

For now, immunocamouflage is a promising preclinical research approach—not “universal blood,” a proven substitute for standard matching, or a product blood banks can use routinely.
