In a DEV Community postmortem, author rello says they caught the contradiction by rereading their own policy file—not because a high resume score had made the submissions safe. The episode is a personal account, not an independently audited incident report.
How did the policy conflict happen?
Rello describes Hiring OS as a personal system that collects job listings from ATS APIs, scores roles against a resume, tailors resumes through quality gates, and tracks applications. The named sources include Ashby, Greenhouse, Lever, and YC’s Work at a Startup.

An amendment allowed a daemon to submit an application when the role’s tier and resume-evaluation thresholds passed. But nearby rules in the same document required a human-in-the-loop session and prohibited unattended or autonomous pipelines. Rello says the amendment added no manual submission gate, leaving the new instruction in conflict with the older restriction.

That proximity is the point of the title: the author says the two rules sat just lines apart. It is an example of a contradiction in one person’s policy file, not evidence that all automation systems fail in this way.

Why did a strong resume score fail to answer the important question?
The policy treated passing quality thresholds as sufficient grounds to submit. Rello’s postmortem draws a distinction between two different checks: whether a resume is good, and whether a particular application is safe and appropriate to send without review.
The author reports one application with evaluation scores of 100/100/97 that was rejected the same day. Their tracker attributed the rejection to a work-authorization hard filter. That is a single personal example, not a measure of how often scoring systems miss eligibility issues or predict rejection.

A high score can describe the quality of an output according to the criteria being evaluated. It cannot, by itself, establish that an application meets every employer-specific eligibility requirement or that an external submission should happen without human judgment. Rello’s central question is: “what is a human click still protecting against?”
What did the author change after noticing the contradiction?
Rello says they replaced the conflicting instruction with a policy prohibiting unattended external submissions and limiting evaluation scores to recommendations. The post gives the revised rule this wording:

“Current policy: NO unattended external submission, no exceptions. Eval scores and tier classification may inform a recommendation, never an automatic submission.”
The author also reports checking the system state separately: no submission daemon was active, and a scheduled job only fetched, scored, and scaffolded applications. This is the author’s report; the post does not provide an independent inspection of the system.

What does the reported application history establish?
Rello says they tracked 28 applications over about a week and a half, manually submitting them and logging resume-gate scores. The post’s publication year is not verified, and these figures are the author’s personal records rather than independently validated statistics. They do not establish a general application success rate, the performance of Hiring OS, or the value of automation for other job seekers.

What practical safeguard follows from the postmortem?
Rello recommends scheduling a review when a change removes a human step. As an editorial synthesis of the incident, that review can ask whether a new rule conflicts with existing restrictions and whether passing an output-quality check is being mistaken for permission to take an external action.
