A new or worsening headache, repeated vomiting, unusual sleepiness, confusion, seizure, fever, wound redness, or a sudden neurological change needs urgent medical assessment. Tell emergency staff immediately that the person has a cerebrospinal-fluid (CSF) shunt.
Symptoms overlap, so you cannot safely determine the cause at home. The practical rule is to seek emergency help for severe or rapidly worsening symptoms and contact the neurosurgical team immediately for any new symptom or sign along the shunt or surgical wound.

What a hydrocephalus shunt problem means
A shunt diverts excess CSF from the brain’s ventricles to another part of the body, commonly the abdomen, where the fluid can be absorbed. A problem can occur when the system drains too little, becomes infected, or drains too much.
- Blockage or underdrainage: An obstruction prevents enough CSF from flowing, allowing pressure and fluid to build up again.
- Infection: Bacteria can involve the wound, tubing, valve, or CSF. Infection may occur soon after surgery or later.
- Overdrainage: CSF leaves the ventricles faster than it is produced, potentially causing the ventricles to collapse or blood vessels to tear.
The FDA says blockage is the most common cause of shunt malfunction. The NHS warns that a serious blockage can produce a dangerous buildup of fluid and brain damage. No single symptom reliably distinguishes blockage, infection, and overdrainage in every patient; age, shunt design, valve setting, time since surgery, and the person’s usual neurological baseline all affect presentation.

“When there is reason to suspect that a CSF shunt system is not functioning properly (for example, if the symptoms of hydrocephalus return), medical attention should be sought immediately.” — U.S. Food and Drug Administration
How blockage, infection, and overdrainage compare
| Problem | Typical symptom pattern | Clues at the wound or shunt track | Timing information | How clinicians investigate | Possible treatment |
|---|---|---|---|---|---|
| Blockage or underdrainage | Return of hydrocephalus symptoms: headache, nausea or vomiting, visual or balance problems, drowsiness, altered consciousness, seizure, or another new neurological change. | Often no skin change; swelling or tenderness can occur if another complication is present. | Can happen at any time after implantation. | Neurological examination, comparison with prior status, CT or MRI, and other tests selected by neurosurgery. | Valve adjustment in selected cases; revision or replacement of blocked components; temporary external drainage when urgently needed. |
| Infection | Fever or feeling unwell, headache, vomiting, neck stiffness, painful eyes, irritability, or sleepiness. | Redness, tenderness, swelling, wound breakdown, clear CSF leakage, or drainage along the shunt path. Abdominal pain can occur when the distal catheter drains into the abdomen. | Risk is highest during the first 30 days after surgery according to Southampton; the NHS says infections are more likely during the first few months, but later infections remain possible. | Examination, imaging and laboratory or CSF tests as indicated; urgent neurosurgical review. | Antibiotics and, in some cases, temporary drainage plus shunt removal, replacement, or revision. |
| Overdrainage | Headache is common; symptoms may resemble underdrainage and cannot be classified reliably by sensation alone. | Usually no external sign identifies overdrainage. | May follow a change in drainage conditions or valve setting, but can occur later as well. | Clinical assessment and brain imaging, interpreted with the person’s prior scans and shunt type. | Specialist adjustment of a programmable valve when appropriate; other procedures may be needed for severe complications. |
Because these patterns overlap, a table can guide urgency but cannot provide a diagnosis.
Symptoms of a blocked or underdraining shunt
When a catheter, valve, or connector is blocked, CSF may accumulate and hydrocephalus symptoms can return. Possible signs include:
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errors- Headache that is new, persistent, or progressively worse.
- Nausea or vomiting, especially when it accompanies headache or drowsiness.
- Blurred or double vision, other visual changes, imbalance, or difficulty walking.
- Increasing sleepiness, confusion, altered consciousness, or difficulty waking.
- A seizure or another sudden neurological change.
Symptoms may develop gradually or deteriorate quickly. A person who had improved after shunt surgery can begin to resemble their pre-treatment state. A blockage can raise pressure enough to injure the brain, so waiting to see whether symptoms settle is unsafe.

How blockage may appear in babies
Infants may not be able to report headache. Watch for vomiting, unusual irritability, excessive sleepiness, poor interaction, feeding difficulty, or abnormal head growth. Any concerning change in an infant with a shunt warrants immediate contact with the treating team; emergency deterioration requires emergency services.
Symptoms that suggest shunt infection
Infection can involve the incision, the hardware, or the CSF. Contact the surgical or neurosurgical team immediately for fever, feeling markedly unwell, headache, vomiting, neck stiffness, or painful eyes. Inspect the incision and the path of the tubing for:
- Redness, warmth, tenderness, or swelling.
- Wound separation, pus, or other drainage.
- Clear fluid leaking from the wound, which may be CSF.
- New pain or tenderness along the shunt track.
- Abdominal pain or swelling when the shunt drains to the abdomen.
Babies may show irritability, poor feeding, or unusual sleepiness rather than a measurable complaint. Southampton identifies the first 30 days after surgery as the period of greatest infection risk; the NHS notes that risk is also higher during the first few months. Infection can still occur later, so timing does not rule it out.

Overdrainage means the shunt removes CSF faster than the body produces it. The resulting pressure change commonly causes headache. The FDA warns that severe overdrainage can collapse the ventricles, stretch or tear blood vessels, and lead to a subdural hematoma or slit-ventricle syndrome.

Overdrainage and underdrainage may feel similar. Do not press, pump, massage, or otherwise manipulate the valve or tubing to test it. If the system has a magnetically adjustable (programmable) valve, a neurosurgeon may change the drainage pressure without open surgery, but only after assessing the person and the device.
When to go to an emergency department
Call your local emergency number or go to an emergency department now for any of the following:
- Seizure.
- Loss of consciousness, confusion, or a major change in alertness.
- Sudden weakness, trouble speaking, new vision loss, severe imbalance, or another abrupt neurological deficit.
- A severe or rapidly worsening headache.
- Repeated vomiting with worsening headache, drowsiness, or other deterioration.
- Marked difficulty waking, breathing problems, or collapse.
- Rapid deterioration in someone with a known or suspected blocked or infected shunt.
Healthdirect describes shunt malfunction or infection as life-threatening. Emergency pathways and telephone numbers differ by country, so use the local emergency service. Tell responders and clinicians that the person has a hydrocephalus shunt, even if the implant was placed years ago.

What to do while arranging care
- Escalate the urgency. Use emergency services for the red flags above. For fever, wound or shunt-track redness, leakage, new headache, vomiting, neck stiffness, or abdominal pain without immediate collapse, call the neurosurgical team immediately and follow its instructions; do not wait for a routine appointment.
- Give a concise timeline. Note when symptoms began, how they changed, recent surgery, injuries, infections, medication changes, and the person’s normal level of alertness.
- Bring device information. Take the shunt or implant identification card, operative paperwork, and a medication list if available. Do not delay emergency transport while searching for documents.
- Do not alter the hardware. Never press or pump the reservoir, squeeze the tubing, or change a programmable setting yourself.
- Protect against deterioration. Keep a drowsy or vomiting person supervised and in a safe position while waiting for help. Follow emergency-dispatch instructions rather than giving food or drink to someone whose alertness or swallowing is impaired.
How clinicians check a suspected shunt problem
Assessment starts with the history and a neurological examination, including comparison with the person’s previous baseline. Clinicians may examine the incision and shunt path, check for fever or abdominal findings, and review the type and setting of the valve.

CT or MRI may be used to look for ventricular-size changes, bleeding, fluid collections, or other causes of symptoms. Imaging is interpreted alongside symptoms and older scans; a scan that looks similar to a prior scan does not automatically exclude malfunction. Additional blood, CSF, or other tests are chosen when infection or another diagnosis is suspected. Only the treating team can determine whether a shunt is functioning adequately.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Treatment depends on the cause
Possible treatment for underdrainage or blockage
A neurosurgeon may revise or replace a blocked catheter, valve, or connector. If the malfunction is severe, temporary external drainage can relieve pressure while definitive treatment is arranged. Some drainage problems can be managed by adjusting a programmable valve rather than operating, but that decision requires specialist assessment.
Possible treatment for infection
Doctors may start antibiotics and obtain tests to identify the organism and extent of infection. Because bacteria can adhere to implanted hardware, treatment sometimes requires temporary CSF drainage and removal or replacement of some or all shunt components. The exact sequence depends on the infection and the patient’s condition.

Possible treatment for overdrainage
When a programmable valve is involved, neurosurgery may increase the pressure setting or otherwise adjust drainage. Severe cases, such as those with a subdural hemorrhage or persistent slit-ventricle syndrome, may require a procedure. Treatment is individualized after imaging and examination.
Best Value
Is a headache after shunt surgery normal?
Headache can occur during recovery, but a new, severe, persistent, or worsening headache is not something to self-diagnose as routine healing. Headache can accompany blockage, infection, or overdrainage, and those conditions require different treatments. Contact the surgical team promptly for a postoperative headache, especially when it occurs with vomiting, fever, neck stiffness, wound changes, drowsiness, visual problems, or a change from the expected recovery pattern. Use emergency services for the red flags listed above.
MRI safety with programmable valves
Some programmable shunt valves use magnets and can have their settings changed by an MRI. Before every MRI, disclose the shunt and the exact valve model to the imaging department. Ask whether the manufacturer requires a setting check or reset immediately after the scan, and confirm that the check has been completed before leaving. Never assume that a previous MRI experience applies to a different valve or model.
Information to keep ready for future emergencies
- Shunt type, valve model, and whether the valve is programmable.
- Date and hospital of implantation or most recent revision.
- The neurosurgical team’s contact details and the person’s usual neurological baseline.
- A current medication and allergy list.
- Copies or photographs of the implant card and operative records, stored where family or caregivers can access them.
These details help emergency clinicians compare current findings with prior records and avoid delays, but they do not replace an examination.
Quick Recap
Key points to remember
- Return of hydrocephalus symptoms can signal blockage and brain-threatening pressure.
- Fever, feeling unwell, wound or shunt-track redness, leakage, neck stiffness, vomiting, or abdominal pain can indicate infection.
- Overdrainage can cause headache and, in severe cases, ventricular collapse, torn vessels, subdural bleeding, or slit-ventricle syndrome.
- Symptoms overlap; only clinical assessment can distinguish the cause and select treatment.
- Seizure, altered consciousness, sudden neurological change, severe worsening headache, repeated vomiting with deterioration, marked drowsiness, breathing trouble, or collapse require emergency care.
