Vitamin D receptor (VDR) drugs such as paricalcitol are being studied for their ability to change the cells and signals surrounding pancreatic tumors—not just to act on cancer cells themselves. In a small 2026 clinical run-in, tumor biopsies showed changes in fibroblasts and immune-cell positioning after paricalcitol was given with chemotherapy. Those findings show biological activity, not that the drug improves survival or response to treatment.
Why target the environment around a pancreatic tumor?
Pancreatic tumors can contain dense stromal tissue and activated cancer-associated fibroblasts (CAFs). These cells interact with tumor and immune cells, and researchers are investigating whether changing those interactions could make the tumor environment less supportive of cancer or alter how immune cells reach tumor cells.

VDR agonism is one proposed way to shift the activity of pancreatic stellate cells and CAFs. Earlier preclinical work helped motivate clinical testing, but the idea is not simply to remove a protective “shield”: stromal and fibroblast populations are varied, and changing their signals can have more than one effect.
What did the 2026 paricalcitol study find?
A 2026 Nature Cancer paper reported a randomized safety run-in involving 36 people with metastatic pancreatic cancer. Participants received first-line gemcitabine and albumin-bound paclitaxel (nab-paclitaxel) with placebo, intravenous paricalcitol, or oral paricalcitol. Researchers collected pretreatment and on-treatment tumor biopsies to examine drug effects in tissue.

In the paricalcitol arms, on-treatment biopsies showed a lower proportion of αSMA-positive fibroblasts, changes in a fibroblast VDR-activation signature, and greater density of CD8-positive T cells, with more of those cells spatially close to tumor cells. The paper also reported varied nuclear VDR expression across participants and detected VDR in tumor, immune, and stromal cells.
The authors reported an exploratory association between VDR expression and tumor response within the paricalcitol arms. That observation is not a validated test for selecting patients, and the run-in was not designed to establish treatment efficacy.

How to interpret the evidence
| Evidence | Agent and setting | What it can show | What it cannot establish |
|---|---|---|---|
| 2026 Nature Cancer safety run-in | Paricalcitol, oral or intravenous, alongside gemcitabine and nab-paclitaxel in metastatic disease | Safety observations and biopsy changes consistent with VDR pathway engagement | Whether paricalcitol improves response rates, survival, or outcomes compared with chemotherapy alone |
| 2020 Scientific Reports laboratory study | Calcipotriol in human pancreatic CAF and T-cell models | How this agent affected fibroblast behavior and immune measures in those cell models | Whether the same effects occur in patients, or whether they apply to paricalcitol |
| Clinical trial registry listings | Paricalcitol in other pancreatic cancer treatment settings | Study protocols, objectives, and planned measurements | Trial results or proof of benefit |
The 2026 study’s primary focus was safety and pharmacodynamic tissue analysis, rather than a definitive test of whether the treatment works. Dana-Farber’s summary likewise notes that the run-in was not intended to measure how well paricalcitol treats pancreatic cancer. A 2025 review reports that the planned phase II portion of the NCT03520790 program was not pursued because of futility based on NAPOLI-3. That development does not negate the run-in’s safety or biopsy findings, but it further limits any claim of clinical benefit.
Why immune effects may not be straightforward
The laboratory findings caution against assuming that every change in the tumor environment is beneficial. In the 2020 Scientific Reports study, calcipotriol reduced CAF proliferation and migration and reduced release of several factors, including prostaglandin E2, IL-6, periostin, and leukemia inhibitory factor. In the same cell-model context, the study reported increased PD-L1 and reduced T-cell proliferation and production of IFN-γ, granzyme B, and IL-17.

These results are from laboratory models, not patient outcomes, and calcipotriol is not interchangeable with paricalcitol. They illustrate why stromal changes and immune effects need to be measured rather than presumed to help.
What is known about safety?
In the 2026 run-in, five participants in the oral paricalcitol arm—42% of that small arm—experienced grade 2–4 hypercalcemia and required dose reduction. This is a safety finding from the study, not an estimate of risk for all patients or all uses of VDR agonists.

Paricalcitol is a pharmacologic VDR agonist. It is not equivalent to an over-the-counter vitamin D supplement, and the cited work does not show that supplements reproduce the effects of paricalcitol or calcipotriol. Do not start or change supplements, prescription drugs, or cancer treatment based on these mechanistic findings; discuss treatment questions with the oncology team.

The National Cancer Institute lists a phase II study of paricalcitol and hydroxychloroquine with gemcitabine and nab-paclitaxel for advanced or metastatic pancreatic cancer. Its objectives include response, safety, survival, and exploratory assessment of fibroblasts and immune cells. NCI also lists a phase Ib neoadjuvant paricalcitol study in resectable pancreatic cancer focused on genetic, immune, and histologic markers. Registry entries describe planned research, not results; check ClinicalTrials.gov or the study site for current status and eligibility.
