Each assay answers a different biological question, so a larger number in one test is not automatically a stronger or more protective immune response.
The key distinction is between immunogenicity (whether vaccination produces a measurable immune response) and clinical protection (whether people avoid infection or disease). Clinical trials need carefully defined stimulation conditions, controls, cell denominators and analysis rules before results from different participants, laboratories or studies can be compared.

What a T-cell assay is actually measuring
Most vaccine T-cell tests start with peripheral blood mononuclear cells (PBMCs), a mixture that includes T cells and other blood immune cells. Investigators expose the cells to vaccine proteins, peptides or other antigens in the laboratory. Responding T cells may release cytokines, produce cytokines inside the cell, bind a specific peptide–major histocompatibility complex (MHC), or divide. The assay readout is therefore a response under defined laboratory conditions, not a direct measurement of protection in the body.

Assays also count different populations. A cytokine test detects cells that perform the measured function during stimulation. A multimer test counts cells whose receptors recognize one selected peptide–MHC complex, whether or not they release a measured cytokine in that experiment. A proliferation test detects division after stimulation. These populations overlap, but they are not identical.
How a clinical laboratory turns a blood sample into a result
A trial protocol should define the laboratory workflow before samples are analyzed. A typical sequence is:
- Collect and transport blood. Record collection time, anticoagulant, temperature and transit conditions. Delays or poor handling can reduce cell viability and alter responses.
- Prepare PBMCs. Separate the mononuclear-cell fraction, count cells and assess viability. The number of usable cells becomes the denominator for later calculations.
- Stimulate parallel wells or tubes. Expose cells to the vaccine antigen or peptide pools at specified concentrations and durations. Keep an unstimulated condition to estimate background and positive-control conditions to show that cells can respond.
- Apply the assay-specific detection system. ELISpot captures secreted cytokines; intracellular cytokine staining (ICS) fixes, permeabilizes and stains cells for flow cytometry; MHC multimers bind matching T-cell receptors; proliferation assays track dye dilution or another division marker.
- Acquire and analyze data. Use predefined counting, gating and response rules. Report the cell denominator, controls, cytokines or markers, stimulation details and any background subtraction.
- Interpret the change from baseline. Compare post-vaccination samples with the participant’s baseline and with the assay’s negative control, while retaining uncertainty from counting, sampling and background measurements.
PBMC quality is a major determinant of assay performance. The World Health Organization notes that cell-based assays require high-quality specimens, are complex and costly, and remain incompletely harmonized. Age, HLA type and demographic representation can also influence which epitopes are presented and how strongly cells respond.
Four main ways trials measure vaccine-specific T cells
ELISpot and FluoroSpot: counting cytokine-secreting cells
In an enzyme-linked immunospot (ELISpot) assay, PBMCs sit on a membrane coated with a capture antibody. After antigen stimulation, a responding cell secretes a cytokine such as interferon-gamma (IFN-γ); the cytokine is captured around that cell and developed as a visible spot. The number of spots estimates the frequency of cytokine-secreting cells under the stated stimulation conditions.
ELISpot is valued for sensitivity and relative simplicity in the WHO description of immuno-monitoring assays. FluoroSpot is a related format that uses fluorescent detection and can measure more than one secreted analyte. Neither format counts every antigen-specific T cell: a cell that recognizes the antigen but does not produce the measured cytokine during the incubation will not generate a corresponding spot. Results depend on antigen dose, incubation time, cell viability, plate development and the rules used to count spots.

Intracellular cytokine staining (ICS): identifying responding subsets
For ICS, cells are stimulated and treated so newly produced cytokines remain inside them. Fluorescent antibodies then label cytokines and surface or intracellular phenotype markers. Flow cytometry identifies individual cell populations, allowing investigators to ask whether CD4 or CD8 T cells, for example, produced IFN-γ, interleukin-2 (IL-2), tumour necrosis factor-alpha (TNF-α), or combinations of these cytokines.
This multiparameter resolution is the main advantage of ICS: a study can report both function and phenotype in the same event-level data set. The trade-offs are more complex staining, instrument setup, gating and analysis, along with substantial laboratory labor. The WHO source notes that some users consider ICS less sensitive than ELISpot. A result can also change when analysts alter compensation, gating boundaries, viability exclusions or the definition of a positive polyfunctional cell.
MHC multimers and tetramers: enumerating cells with a chosen specificity
Peptide–MHC multimers, including tetramers, are reagent complexes designed to bind T-cell receptors that recognize a particular peptide presented by a particular MHC molecule. Fluorescent multimers allow flow cytometry to count those cells directly. This is primarily a specificity and enumeration readout, not a test of cytokine release.
The selected peptide and HLA/MHC allele determine which cells can be detected. A multimer panel aimed at one epitope will miss responses directed at other vaccine epitopes, and a person who does not carry the relevant HLA type may not be measurable with that reagent. Multimer-positive cells may be functional, exhausted, quiescent or otherwise heterogeneous; additional markers or functional assays are needed to characterize what they do.
Proliferation assays: asking whether cells divide
Proliferation assays follow cell division after antigen exposure. A common flow-cytometric approach labels cells with a fluorescent dye such as CFSE; each division dilutes the dye, producing successive intensity peaks. The assay can estimate the fraction of cells that proliferated and, with appropriate markers, identify the responding subset.

Division is a different biological dimension from immediate cytokine secretion or multimer binding. Some cells may proliferate without releasing the cytokine selected for an ELISpot or ICS panel, while others may produce cytokine during a short stimulation but not divide during a longer culture. The Clinical and Laboratory Standards Institute ILA26 standard addresses proliferation methods alongside ICS, multimers and ELISpot, including specimen handling, quality control, validation, interpretation and troubleshooting.

How the assay choices compare
| Method | Primary feature measured | Cell-subset detail | Main strengths | Important constraints |
|---|---|---|---|---|
| ELISpot | Secreted cytokine from responding cells | Usually limited unless cells are further characterized | High sensitivity and comparatively simple processing | Counts only cells producing the measured cytokine; spot counting and background affect estimates |
| FluoroSpot | Two or more secreted analytes, detected fluorescently | Generally limited subset resolution | Can examine multiple secretions in one well | More complex fluorescence interpretation; still dependent on stimulation and secretion thresholds |
| ICS with flow cytometry | Intracellular cytokines plus phenotype markers | High; CD4, CD8 and other gated populations can be separated | Multiparameter functional and phenotypic information | Labor-intensive, instrument- and gating-dependent; some consider it less sensitive than ELISpot |
| MHC multimer or tetramer | Binding to one peptide–MHC specificity | Flow cytometry can define phenotype, but specificity is restricted to the reagent | Direct enumeration of selected antigen-specific cells | Requires suitable peptide and HLA/MHC coverage; binding alone does not demonstrate function |
| Proliferation assay | Antigen-induced cell division | Flow cytometry can identify dividing subsets | Measures expansion, a functional dimension not captured by secretion or binding | Depends on culture duration, dye performance, viability and division thresholds |
There is no universally best assay. Selection should match the biological question, available sample volume, expected HLA coverage, laboratory capability and the need for comparability across sites.
Why two valid assays can give different answers
Assays differ in sensitivity, background, antigen presentation and the subset they detect. A study of eight participants receiving a melanoma vaccine compared tetramers, cytokine flow cytometry and ELISpot. Tetramer measurements were highest, followed by cytokine flow cytometry and ELISpot, and the study found no significant concordance among methods for peptide-specific CD8 T-cell counts. The result demonstrates that methods can disagree; it is not a universal ranking. The cohort was small and came from a cancer-vaccine setting. See the American Association for Cancer Research report for the study details.
Other sources of disagreement include:
- Antigen design: Whole proteins, overlapping peptide pools and single epitopes stimulate different processing and presentation pathways.
- Stimulation conditions: Concentration, incubation time, co-stimulatory reagents and culture medium alter which cells respond.
- Background signal: Unstimulated cells can release cytokines or show nonspecific staining. Subtracting that background introduces its own measurement error and can reduce statistical power.
- Sample quality: Cell loss, freezing and thawing, viability and transport time affect counts before the assay begins.
- Denominators and thresholds: A percentage of CD8 cells, a percentage of all viable lymphocytes and spots per million PBMCs are not interchangeable. Positive-response thresholds and minimum event counts also differ.
- Analysis choices: Flow-cytometry compensation, gating hierarchy, doublet exclusion and definitions of polyfunctionality can change the reported value.
The review Analyzing cellular immunogenicity in vaccine clinical trials discusses these design and interpretation issues in the context of clinical studies.
What a good trial report must disclose
A percentage or spot count has little meaning without its measurement context. Trial reports should state:

Background subtraction deserves particular care. If an unstimulated value is subtracted from an antigen-stimulated value, random error from both wells carries into the final estimate. High background can turn a biologically plausible response into an imprecise or statistically negative result; low background does not guarantee that the assay is sensitive to every relevant T-cell function.

Cross-site comparisons require more than putting percentages into the same spreadsheet. Laboratories should use a common standard operating procedure, harmonized reagents where practical, controlled specimen collection and transport, shared data templates and the same response definitions. Analysts should document instrument settings, gating and quality-control failures rather than silently changing procedures between batches.
External proficiency panels can reveal whether laboratories produce comparable results on the same blinded samples. Workshop recommendations from the iSBTc-SITC/FDA/NCI immunotherapy-biomarker meeting emphasize standardized assays, full methodology disclosure and proficiency testing where feasible.
The CLSI ILA26 document is a technical reference for specimen collection and transport, sample preparation, quality control, validation, interpretation and troubleshooting. Because standards are revised, laboratories should verify the current edition before treating an older edition as operationally current.

The WHO proposal on immuno-monitoring assays highlights a broader structural problem: most vaccine approvals have historically relied on antibody measurements because they are easier to measure and standardize. In its words, “Most vaccines are currently approved based on data from humoral responses, as these are easier to measure and standardise.” The statement explains why cellular assays often have smaller datasets and less established reference standards, not why T-cell measurements are unimportant.

What a detected T-cell response means for vaccine efficacy
A cellular immunogenicity result supports the conclusion that vaccination triggered a measurable T-cell response under the study’s assay conditions. It does not, by itself, establish reduced infection, fewer symptomatic cases, less severe disease or durable protection. Those outcomes require appropriately designed efficacy analyses, and a validated immune correlate of protection requires evidence linking a defined immune measurement to a clinical endpoint.
A flow-cytometry review describes an HIV vaccine trial with robust ICS-measured T-cell responses but no reduction in infection rates. That example does not prove that T cells can never contribute to protection; it demonstrates why a strong laboratory response cannot substitute for an efficacy endpoint. The review is available at Vaccine applications of flow cytometry.
Early-phase trials also commonly test cellular responses in small donor cohorts. The limited sample size, HLA diversity and demographic coverage can constrain how confidently researchers generalize a result to a larger population. A carefully reported negative assay may reflect low sensitivity or poor sample quality, while a positive assay may represent only the cytokine, epitope or subset that the method was designed to detect.
A practical decision framework for investigators
Choose the assay after defining the question, not the other way around:
- Need a sensitive estimate of cytokine-secreting frequency? Consider ELISpot or FluoroSpot, with controls and spot-counting rules suited to the expected response.
- Need CD4/CD8 identity, cytokine combinations or phenotype? Use ICS when the laboratory can support validated multiparameter flow cytometry and rigorous gating.
- Need the number of cells recognizing one defined epitope? Use an MHC multimer or tetramer, while checking HLA representation and adding functional testing if activity matters.
- Need to know whether cells expand after antigen exposure? Use a validated proliferation design with a defined dye, culture period and division threshold.
- Need comparison across locations? Prioritize one harmonized method, shared controls and proficiency assessment over mixing incomparable readouts.
Many well-designed trials use complementary assays because no single test captures specificity, phenotype, cytokine function and proliferation at once. The combination should be justified in the protocol, with each assay linked to a distinct biological question rather than treated as interchangeable confirmation.
