How to Write a Risk Acceptance Record an Executive Can Sign

A signable risk acceptance record is a bounded decision to live with residual risk—not a declaration that the risk has disappeared. It should let an authorized executive answer, quickly: what exposure is being accepted, why acceptance is reasonable now, what authority supports the decision, which conditions apply, and when the decision must be reviewed.

What the executive is actually accepting

Risk acceptance applies to the exposure left after existing controls and feasible treatments are considered. NIST defines acceptable risk as residual risk that remains within the organization’s risk appetite and tolerance. The record therefore needs to distinguish the original (inherent) exposure from the current (residual) exposure.

  • Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
  • Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
  • Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
  • 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.

Define the boundaries around that sentence: the system or process covered, locations and data involved, excluded components, business purpose, assessment date, and assumptions. An executive cannot approve an undefined exposure.

Build the decision header first

Put the facts needed for routing and authority at the top of the record.

  • Record ID and date: Use a unique identifier and the date the decision is requested.
  • System or process: Name the service, application, facility or business activity.
  • Business owner and risk owner: Identify who operates the activity and who is accountable for managing the exposure.
  • Classification: Mark the record’s handling category if your organization uses one.
  • Requested decision: State that approval of a specifically described residual risk is requested, including its validity period.

Give the signer a one-paragraph executive summary

Keep the summary short enough to read before the detail. State the exposure, the consequence if it materializes, the current residual rating, and the decision requested. Include the business need that makes immediate elimination impractical, but do not use business urgency as a substitute for an assessment.

For example: “The customer-support platform retains a documented access-control gap affecting privileged accounts. Existing controls reduce the exposure to a medium residual rating, but a limited number of accounts remain outside the target control. Replacing the integration this quarter would interrupt a critical service. Approval is requested through 30 September 2027, subject to the controls and review triggers below.”

Document inherent and residual risk

The assessment section should show how the rating changed and how confident you are in it.

Element What to record
Inherent likelihood and impact The exposure before implemented controls, using your organization’s approved scale and definitions.
Implemented controls Preventive, detective and corrective controls that are operating now, with evidence or testing dates.
Control gaps Missing, partial, ineffective or unverified controls that leave exposure behind.
Residual likelihood and impact The likelihood and consequence after current controls; show the resulting rating and its position against risk appetite and tolerance.
Confidence and uncertainty Data limitations, assumptions, threat uncertainty, control-test limitations and dependencies that could make the estimate wrong.

Do not hide uncertainty inside a single score. Explain which assumptions would change the rating and what evidence will test them.

Show why acceptance is proportionate

Acceptance is one possible risk response. NIST describes the response choices as “Accepting, avoiding, mitigating, sharing, or transferring risk.” Record the alternatives considered and the trade-off that led to acceptance.

Option Questions for the decision record
Mitigate Which additional controls would reduce residual risk, at what cost and effort, and by when?
Avoid Could the organization stop the activity, remove the feature or change the process, and what mission or revenue would be affected?
Share or transfer Can a contract, insurer, partner or service provider assume part of the exposure, and what risk remains with the organization?
Accept Why is the remaining exposure within tolerance now, given business need, feasibility, timing and reversibility?

Compare options using residual-risk reduction, effect on mission or revenue, implementation cost and effort, time to deploy, reversibility, third-party dependency, authority required and monitoring burden. A brief table is usually clearer than several paragraphs of justification.

Turn acceptance into enforceable conditions

A signature should not end the control work. List conditions that must remain true for the acceptance to be valid.

  • Mandatory controls: Identify compensating or baseline controls that cannot be removed.
  • Actions: Assign each remediation or evidence task to a named owner with a due date.
  • Monitoring evidence: Specify logs, test results, vulnerability reports, incident metrics, access reviews or other evidence to collect.
  • Escalation thresholds: State the result that requires notification or suspension, such as a control failure, incident, material increase in likelihood or impact, or an overdue corrective action.
  • Reporting cadence: Set the schedule and recipient for status reports.

Conditions should be testable. “Improve security” is not a condition; “complete quarterly privileged-access review and retain the signed review record” is.

Identify the authority and the person who can sign

Name the accepting executive or authorizing official and cite the policy, delegation or role description that grants authority for this system, data class and risk level. NIST states: “The explicit acceptance of risk is the responsibility of the authorizing official and cannot be delegated to other officials within the organization.” A risk analyst, system administrator or project manager can prepare the record or recommend a decision, but should not sign unless the organization’s authority framework makes that person the authorizing official.

If authority is unclear, stop the approval route and resolve it before signature. A technically complete form signed by someone without authority does not create a valid organizational decision.

  • Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft

Set a duration, expiry and reapproval triggers

Do not make acceptance open-ended by default. Include an effective date, an expiry date or scheduled review frequency, and the events that force an earlier review. Authorization decisions should convey terms and conditions plus a termination date or time-driven authorization frequency.

  • Material change to the threat, vulnerability, system scope, data, architecture or business use.
  • Control failure, missed action deadline or evidence that a control is less effective than assumed.
  • Incident, near miss or new intelligence that changes likelihood or impact.
  • Change in law, policy, contract, risk appetite or executive authority.
  • Expiration of a compensating control, supplier arrangement or insurance coverage.

State what happens at expiry: reapproval, conversion to a new record, mitigation completion, or termination of the activity. A review date is not the same as an automatic renewal.

  • Printed name
  • Title and organizational unit
  • Signature or approved electronic signature
  • Signature date

Attach or link the assessment evidence, control tests, action plan and authority reference so the signer can verify the basis for the decision without searching across systems.

Maintain the record after signature

Store the signed record with the risk-register entry or assessment record. Accepted findings remain documented; they are not closed merely because an executive signed. Monitor changes to the risk factors, collect the specified evidence, track action owners and record later review, modification, renewal or termination decisions. If a trigger occurs, escalate it under the stated conditions rather than waiting for the calendar review.

A practical pre-signature checklist

  • The risk statement names the threat or condition, affected asset or process and business consequence.
  • Scope boundaries and assumptions are explicit.
  • Inherent and residual likelihood and impact are shown with confidence and uncertainty.
  • Implemented controls, gaps and compensating controls have evidence.
  • Alternatives and their cost, feasibility, timing and mission trade-offs are recorded.
  • Conditions have owners, due dates, evidence, thresholds and reporting cadence.
  • The signer is the authorized official under a cited policy or delegation.
  • Effective date, expiry or review date and event-driven triggers are present.
  • The approval wording covers the exact residual risk, scope and conditions.
  • The signed record is linked to the risk register and monitored after approval.

Some governance teams use a spreadsheet with separate acceptance, authority and expiry-log sheets; the format is secondary to making the authority, exposure, conditions and end date unambiguous.
