A retrospective accuracy result is only one piece of evidence. Safe deployment also requires checking the intended use, local patient population, input data, clinician interaction, workflow effects, unintended consequences, governance and continuing performance after launch.
The practical approach below turns those requirements into a documented, risk-proportionate process. It applies to triage, diagnosis, prognosis, risk scoring, treatment-response prediction and administrative tools, while recognizing that each use may need different measures, reference standards and approval criteria.

What clinician validation must establish
A model can perform well on historical data and still fail in a hospital. Historical testing generally cannot show whether required inputs are available at the point of care, whether an alert arrives early enough to matter, whether clinicians understand and appropriately challenge the output, or whether the software creates delays, workarounds and new safety hazards. The World Health Organization (WHO) treats analytical validation, clinical validation, usability, deployment and surveillance as related but distinct parts of the lifecycle.

- Purpose: What decision or task is the system intended to support, and what action may follow?
- Population and setting: Which patients, sites, specialties, shifts and care pathways are in scope?
- Performance: Does the output agree with an appropriate reference standard or comparator for that use?
- Use in context: Can representative clinicians obtain, interpret and act on the output without unsafe friction?
- Control: Who approves release, monitors the system and can pause or roll it back?
There is no universal accuracy threshold or single study design that fits every healthcare AI application. Evidence should be proportional to potential harm, the reversibility of decisions, the vulnerability of the population and the degree to which the tool changes clinical action.
How do we know an AI tool works in our hospital?
Start by writing a one-page intended-use statement before reviewing performance numbers. This prevents a vendor claim for one purpose from being treated as evidence for another.
Define the decision and consequence
State whether the system is for triage, diagnosis, prognosis, risk assessment, treatment-response prediction, documentation, scheduling or another administrative function. Specify the exact decision point, the authorized users, the required action and what happens when the output is unavailable or uncertain. A triage alert, for example, may be judged by timely prioritization and missed urgent cases; a prognostic estimate may require calibration and clinically meaningful outcome horizons. A generic accuracy score cannot represent both.
Describe the local population and workflow
Record the wards, clinics or services in scope; adult, pediatric or other patient groups; relevant comorbidities; language and accessibility needs; and periods when staffing or equipment differs. Map where data originate, when the result appears, who sees it and how it is documented. Include handoffs, weekends, downtime procedures and the route for escalating a disagreement.
Set boundaries and exclusions
List conditions in which the model has not been evaluated, inputs it cannot interpret and decisions it must not make. If a tool is advisory, say explicitly that clinicians retain responsibility for the decision. If the product has regulatory implications, assess them against the intended use and product characteristics; the U.S. Food and Drug Administration (FDA) clinical decision support material is a U.S. policy resource, not a universal international rule.

Build an evidence and data review
Clinicians need enough information to judge whether a result is relevant to their patients, not merely whether a model was published. The FDA’s clinical decision support discussion identifies useful review information: intended use, target patient group, required inputs, algorithmic approach, validation results and patient-specific knowns and unknowns.
Separate training, tuning and test data
Ask the developer to identify which data trained the model, which data were used for tuning and which data were reserved for testing. The FDA glossary states: “For AI-enabled medical products, test data should be independent of data used for training and tuning.” Independence reduces the risk that reported performance reflects memorization or repeated optimization on the same cases.

Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Check whether the test set represents your population and care process. Look for differences in devices, coding practices, laboratory methods, imaging protocols, referral patterns and missing-data rates. A model trained in one health system may encounter different prevalence, documentation habits or workflow timing in another.
Examine inputs, missingness and harmonization
For every required input, document its source, refresh interval, units, allowable range, latency and failure behavior. Systems that combine images, text, laboratory values, vital signs or claims data can be sensitive to mismatched timestamps, incompatible formats and missing modalities. Determine whether the model refuses to score, substitutes a default or silently proceeds when an input is absent. Clinicians should see when an output is based on incomplete information.
Distinguish vendor evidence from local evidence
External validation can establish that a system worked in another population or setting; it does not replace a local evaluation. Request the protocol, population definition, reference standard, subgroup results, exclusions and version number behind every reported result. Publication is useful context, but ONC SAFER guidance cautions that publication alone does not guarantee safety or effectiveness.

What should clinicians test before an AI tool goes live?
Use a representative, supervised evaluation that tests both the output and the way people use it. Include physicians, nurses, allied health professionals, pharmacists or technicians who will encounter the system, plus informatics, operations, privacy, security and patient-safety participants as appropriate.
Test the complete user journey
- Access: Verify that the right role can open the result at the intended point in the electronic health record or other interface.
- Input readiness: Confirm that required data are present, current and correctly mapped for ordinary cases, edge cases and downtime recovery.
- Presentation: Check that the output, confidence or uncertainty, timestamp, model version and explanatory context are understandable without hiding important limitations.
- Interpretation: Observe whether clinicians can distinguish a prediction from a diagnosis, recommendation or required action.
- Action: Record what users do next, including acceptance, verification, override, escalation and no action.
- Documentation: Ensure the decision and any override can be recorded without duplicating work or creating ambiguous responsibility.
- Failure handling: Simulate missing inputs, delayed results, contradictory data, outages and clearly unsafe recommendations.
Use realistic cases and representative users
Include routine cases, borderline cases, rare but high-consequence conditions and patients whose characteristics differ from the development cohort. Involve clinicians with different levels of experience and different roles in the pathway. A system that appears intuitive to a specialist may be confusing to a covering clinician or triage nurse.
Measure workflow and human factors
Record whether the output arrives early enough, how long it takes to review, how often users seek additional information, and what work is displaced. Look for alert fatigue, automation bias, overreliance on a confident-looking score, delayed treatment while users verify the result, and inequitable access to follow-up. Retrospective analytical validation cannot answer these questions; they require observation or prospective evaluation in the intended context.
Choose endpoints that match clinical risk
Write the evaluation question and analysis plan before examining results. Select endpoints that represent the intended use and the consequences of error.
Model-level measures
Depending on the task, useful measures may include sensitivity, specificity, positive and negative predictive value, discrimination, calibration, false-alert rate, time-to-result or agreement with a defined reference standard. Report uncertainty and clinically relevant subgroups rather than one pooled number. The appropriate reference standard may be an expert panel, a laboratory result, an imaging interpretation, a later confirmed diagnosis or a documented outcome; it should be justified for the use case.

Clinical and operational outcomes
For implementation questions, add measures such as time to treatment, avoidable escalations, missed urgent cases, length of stay, readmissions, documentation burden, workload, override patterns and reported safety events. These outcomes should be defined before launch and linked to a plausible mechanism by which the AI could help or harm. A statistically strong model may have no clinical value if it does not change a decision, changes it too late or introduces more risk than benefit.
Comparators and reference standards
Compare the AI with current practice, an established score, usual clinician assessment or another accepted comparator when appropriate. Keep the comparison fair: use the same eligibility criteria, time window and outcome definition. If no reliable reference standard exists, document that limitation and use a transparent adjudication process rather than implying certainty.
Decide whether prospective or randomized evidence is warranted
WHO describes evidence choices as risk- and context-dependent. A prospective real-world evaluation may be sufficient for a low-consequence workflow aid, while a tool that drives urgent treatment, restricts access or affects vulnerable patients may need stronger comparative evidence. A randomized study is not automatically required for every system, and it is not a substitute for usability, workflow and safety testing.
Escalate the design when errors are hard to detect or reverse, when the tool changes clinician behavior substantially, when prior evidence is weak or when local performance may differ materially from published results. Define stopping rules for safety signals before the evaluation begins.
Create multidisciplinary oversight and a deployment record
Assign a committee or accountable group with clinical expertise and people able to assess AI performance, data quality, workflow, security, privacy, equity and operations. The ONC SAFER: High Priority Practices guidance (August 2024) recommends maintaining an inventory for AI used in care.
Best Value
Minimum inventory fields
- System name, supplier and exact model or software version
- Authorized purpose, clinical service, patient population and users
- Required inputs, data sources and known exclusions
- Validation, comparison and local evaluation evidence
- Deployment date, owner and responsible operational personnel
- Review cadence, last review date and next scheduled review
- Known limitations, safety risks, user training and downtime procedure
- Change history, approvals and the person authorized to pause or roll back use
Set a release decision before seeing the final result
Define categories such as approve, approve with restrictions, defer for remediation or reject. Specify what evidence is mandatory, what deficiencies require mitigation and who signs the decision. Establish a local pause threshold in advance, but do not copy a threshold from another organization: the appropriate trigger depends on the tool, population and consequences.
Plan monitoring for the entire lifecycle
Validation ends neither at procurement nor at go-live. WHO’s lifecycle framework extends through deployment and post-market surveillance. Real-world performance can change when patient mix, data capture, clinical practice, guidelines, staffing or user behavior changes. The FDA’s 2025 request for comment on measuring real-world performance discusses these changes and should be read as a discussion resource, not final guidance; its listed comment deadline was December 1, 2025.
Monitor inputs and outputs
- Changes in demographic and clinical case mix
- Missingness, coding, device, laboratory or imaging patterns
- Output distributions, confidence values and calibration
- Alert volume, response time, overrides and ignored results
- Safety reports, complaints, near misses and equity signals
- Changes to guidelines, pathways, staffing or surrounding software
Define an investigation pathway
- Capture the signal with patient, time, model version and input context while protecting privacy.
- Triage immediate risk and provide a safe manual or downtime process.
- Reproduce the result and determine whether the issue arose from data, software, model behavior or workflow.
- Notify accountable clinical and technical owners and document the decision.
- Apply mitigation, retraining, configuration change, user communication, pause or rollback as appropriate.
- Re-evaluate after the change and update the inventory, training and review date.
Do not silently substitute a new model or alter thresholds. A material change should trigger documented reassessment because the original validation may no longer apply.
Common validation failures and their fixes
| Failure | Why it is unsafe | Corrective action |
|---|---|---|
| Relying on a retrospective accuracy score | It says little about interaction, timing or unintended consequences. | Add local, preferably prospective, workflow and safety evaluation. |
| Testing only average performance | Subgroups, rare cases or local prevalence may produce different harms. | Predefine relevant subgroups and inspect errors and calibration. |
| Using data that were tuned repeatedly | Reported performance may be optimistic. | Reserve independent test data and document the data lineage. |
| Ignoring missing or delayed inputs | The tool may score cases unlike those in development or fail silently. | Test missingness, latency, harmonization and explicit fallback behavior. |
| Assuming clinicians will use outputs as intended | Automation bias, overrides and workarounds can change risk. | Observe representative users and measure interpretation and action. |
| Treating publication as proof of safety | Peer review does not establish local effectiveness or safe integration. | Review methods, then perform local validation and governance. |
| Launching without an owner or rollback plan | Signals may be missed and unsafe use may continue. | Assign accountability, monitoring, pause authority and review dates. |
A practical pre-deployment checklist
- Intended use: Purpose, users, population, setting, decision point and prohibited uses are written.
- Evidence: Training, tuning and independent test data are identified; external and local evidence are separated.
- Data: Inputs, quality, missingness, timestamps, harmonization and fallback behavior are tested.
- Clinical evaluation: Representative clinicians and operational staff have tested realistic, edge and failure cases.
- Endpoints: Reference standard, comparator, model measures, clinical outcomes and safety reporting are pre-specified.
- Workflow: Timing, workload, documentation, overrides, escalation and downtime are observed.
- Governance: A multidisciplinary group has approved the exact version and authorized scope.
- Operations: Inventory fields, training, support contacts, review cadence and rollback authority are complete.
- Monitoring: Drift, practice changes, user behavior, safety signals and material-change reassessment are planned.
How to communicate results to clinicians
Give users a concise model card or local evaluation brief before training. It should state what the tool does and does not do, who was studied, which inputs are required, how missing data are handled, the reference standard, subgroup limitations, known failure modes, the expected action and the escalation route. Show examples of appropriate use and deliberate non-use. Explain uncertainty in plain language and make the model version visible in the interface or audit record.
Training should be role-specific and include practice with borderline cases, overrides, downtime and reporting a safety concern. Collect questions and revise the guidance when workflow or the model changes.
What a defensible deployment decision looks like
A defensible decision is not “the model is accurate.” It is a documented judgment that this specific version, for this defined population and workflow, has evidence proportionate to its risk; that clinicians can use it as intended; that limitations and fallback procedures are clear; and that the organization can detect, investigate and stop unsafe use. If those conditions are not met, defer deployment, narrow the authorized scope or run another evaluation rather than treating uncertainty as approval.
