Use production feedback as evidence for a controlled evaluation loop, not as a list of orders. Capture representative traces, classify the underlying failure, convert recurring patterns into reproducible test cases, run the proposed change against those cases and a broad regression set, then roll it out gradually while monitoring for drift. A single complaint or memorable transcript should trigger investigation; it should not, by itself, justify changing a general agent behavior.
Why production feedback is valuable—and dangerous
Deployed agents encounter goals, data, tools and edge cases that an offline benchmark cannot fully predict. Feedback can reveal failures that were absent from development tests and can keep an evaluation set aligned with actual use. It can also be misleading: users report unusual incidents more readily than ordinary successes, and a vivid failure may be caused by a temporary tool outage, ambiguous instructions or bad upstream data rather than by the agent policy you are considering changing.

OpenAI’s production-evaluation work describes two distinct uses for real traffic: discovering previously unknown behaviors and estimating how often known behaviors occur in representative, de-identified traffic when user settings permit that use. Treat those as different jobs. Exploration benefits from unusual examples; incidence estimates require a sampling method that does not overrepresent complaints.

A 2026 peer-reviewed study, Measuring Agents in Production, examined 20 case studies and surveyed 86 practitioners across 26 domains. In that sample, 70% relied on prompting off-the-shelf models rather than weight tuning, 74% depended primarily on human evaluation, and 68% of systems executed no more than 10 steps before human intervention. These figures describe that study’s deployed systems, not a universal rule. They do show why prompt, tool and workflow changes deserve the same disciplined testing as model changes, and why reliability must be judged over repeated cases rather than one successful demonstration.

The feedback-to-improvement loop
- Collect context. Preserve the user goal, relevant inputs and outputs, tool calls and results, final outcome, versions and any human intervention.
- Triage the event. Separate agent reasoning errors from tool, data, policy, environment and reviewer problems. Group similar reports and rank them by impact and recurrence.
- Check prevalence. Sample ordinary traffic and compare the incident with users, tasks and time windows beyond the original report.
- Create evaluation cases. Turn a confirmed failure mode into a reproducible task with expected behavior and an appropriate grader. Add successful and nearby counterexamples.
- Test the change broadly. Run the candidate prompt, model, tool or workflow on new cases plus a protected regression set. Inspect task success, correctness, safety, cost and latency separately.
- Roll out and monitor. Use a canary or controlled comparison where possible, then watch production behavior as traffic, tools and models change.
Keep an immutable copy of the original trace and record which decision each later test or change was based on. That chain makes it possible to distinguish an actual improvement from a change that merely avoids one known example.
1. Capture feedback with enough context to reproduce it
A complaint such as “the agent gave the wrong answer” is not an evaluation case. The case becomes useful when another reviewer can understand the task, replay the relevant conditions and judge the result without guessing what the user meant.
Minimum trace fields
- Intent and constraints: the user’s goal, required format, deadlines, authorization and any stated exclusions.
- Conversation state: the messages and retrieved context that were actually visible to the agent, including system and developer instructions that your access policy allows you to retain.
- Actions: every tool request, arguments, returned data, error, retry and intermediate decision. Include timestamps or step order so multi-step behavior can be reconstructed.
- Outcome: the final response, whether the task was completed, what the user accepted or corrected, and any downstream business or safety effect.
- Configuration: model and prompt versions, tool and data-source versions, routing rules, retrieval settings and relevant feature flags.
- Human involvement: when a person edited, approved, rejected or took over, and the reason recorded at the time.
Do not assume that every production transcript may be reused for training or evaluation. Permitted use depends on deployment settings, contracts, consent, retention rules and applicable law. Minimize personal data, restrict access, redact identifiers where they are not needed and document the purpose for which a trace is retained. OpenAI’s production-evaluation example uses de-identified traffic when settings permit model-improvement use; that condition should not be generalized to another deployment.

Store the original complaint, the trace and the reviewer’s interpretation as separate fields. The complaint tells you what felt harmful or surprising. The trace tells you what occurred. The interpretation should remain revisable until someone checks the evidence. This prevents a reviewer’s early theory from being mistaken for the observed fact.
2. Triage reports by failure mode and severity
Before editing a prompt, cluster events. A useful taxonomy prevents a model fix from being applied to a problem caused elsewhere.

| Category | Typical signal | First diagnostic question | Likely response |
|---|---|---|---|
| Agent reasoning or instruction failure | The agent ignored a clear constraint, selected an unsuitable action or contradicted available evidence. | Was the needed information visible and unambiguous? | Create a task case and test prompt, model and workflow alternatives. |
| Tool or integration failure | An API timed out, returned stale data, rejected valid arguments or changed its schema. | Would a correctly instructed agent still fail with this tool result? | Repair the integration, add error handling or test the tool contract separately. |
| Data or retrieval failure | The source was missing, outdated, contradictory or ranked below irrelevant context. | Did the agent receive authoritative data for the requested time and scope? | Fix ingestion, freshness, permissions or retrieval evaluation. |
| Ambiguous intent | Reasonable reviewers infer different goals or permissions from the same request. | What clarification would resolve the disagreement? | Add a clarification step or define an explicit default and refusal boundary. |
| Changing environment | The behavior appeared after a model, policy, tool, data or traffic change. | Which version boundary separates working and failing traces? | Compare versions, roll back if necessary and add a change-specific test. |
| Reviewer disagreement | People mark the same output as pass and fail for different reasons. | Is the expected behavior operationally defined? | Calibrate reviewers, refine the rubric and record acceptable alternatives. |
Assign severity using consequences, not emotional vividness. Consider harm, affected users, reversibility, frequency and whether the agent acted without a required approval. Keep low-severity examples when they expose a broad pattern, but do not let a dramatic one-off outrank a recurring failure with greater aggregate impact.
3. Test whether a failure is representative
A complaint is a lead, not a prevalence estimate. Check recurrence across independent users, task types, geographies or business units that your deployment serves, and time windows before and after the incident. Compare the distribution of successful and failed tasks, not only the reports submitted by dissatisfied users.
A practical sampling plan
- Define the population you want to understand, such as all completed support sessions or all approval requests in a specified period.
- Draw a regular sample from that population using stable inclusion rules. Keep a separate queue for escalated complaints and unusual traces so exploratory discoveries are not confused with estimates.
- Stratify when important tasks are rare. For example, review high-risk actions separately instead of allowing a large volume of routine chats to hide them.
- Record denominators: number of eligible tasks, number sampled, number reviewed and number showing the failure mode.
- Repeat the sample after material model, tool, policy or traffic changes. A once-representative set can become stale.
No single sampling interval or error threshold is universally validated. Set review frequency and escalation limits according to the consequences of your agent’s actions, traffic volume and available reviewers. The important controls are explicit inclusion rules, retained denominators and periodic refresh.
Protect against tuning on the same examples
Maintain a protected comparison set that is not inspected for every prompt edit. When a case is used to design a fix, label it as development data; reserve other cases, including later-arriving examples, for confirmation. This is a general evaluation safeguard rather than a source-prescribed split protocol, but it reduces the chance that repeated adjustments simply memorize a small set.
4. Convert a confirmed pattern into an evaluation case
Each case should specify the task, context, expected behavior and grading method. A transcript alone leaves too much room for a grader to reward a plausible-looking but incorrect answer.

Case specification
- Task statement: the user goal and permissions, with irrelevant personal details removed.
- Environment: model, prompt, tools, data snapshot and relevant time conditions.
- Expected behavior: required facts or actions, prohibited actions, and when the agent must ask, defer or escalate.
- Evidence: authoritative records or rules used to determine correctness.
- Grader: deterministic assertions, a rubric-based human review, a calibrated model judge or a combination.
- Failure label: the specific mode and severity that caused the case to be added.
Add at least one successful example and nearby counterexamples. If you are fixing an agent that exposed a private field, include a permitted request that should still succeed and a similar-looking request that must be refused. If you are fixing a tool-selection error, include a case where the tool is required, one where a direct answer is safer and one where the tool returns an error that should trigger recovery.
Match the grader to the behavior
Use deterministic checks for exact fields, authorization decisions, schema validity, citations or tool arguments. Use human review when correctness depends on context, tone, competing obligations or ambiguous intent. Model-based judges can expand coverage, but calibrate them against reviewed examples and inspect disagreements. Anthropic’s guidance recommends combining production monitoring, user feedback, A/B testing, transcript review, systematic human evaluation and task-specific automated grading rather than relying on one score.
5. Build a balanced evaluation set
Your suite should represent deployment, expose known failures and test behaviors adjacent to the proposed change. Keep the categories visible so a single aggregate score cannot conceal a serious regression.
| Set component | Purpose | What to include |
|---|---|---|
| Representative sample | Estimate ordinary production quality. | Regularly sampled tasks with their real distribution, plus documented strata for rare high-impact work. |
| Confirmed failures | Verify that a known problem is addressed. | Reproducible cases from distinct users or contexts, not duplicate copies of one trace. |
| Counterexamples | Prevent an overly broad rule. | Similar prompts where the correct action differs, including permitted and prohibited variants. |
| Stress and recovery cases | Test behavior under realistic disruption. | Missing data, tool errors, long context, conflicting instructions, retries and human handoffs. |
| Protected holdout | Provide an honest comparison after tuning. | Cases kept out of routine iteration and refreshed as the deployment changes. |
Evaluate the whole workflow. An agent can produce a polished final message after choosing the wrong account, calling a tool with excessive permissions or skipping a required approval. Anthropic’s agent-evaluation guidance emphasizes intermediate actions and final outcomes because multi-step behavior is not captured by single-turn text grading.
6. Compare a proposed fix against more than the triggering incident
Candidate changes can include a prompt instruction, tool schema, routing rule, retrieval configuration, model version, approval gate or a combination. Change one meaningful factor at a time when practical; when a coordinated workflow change is necessary, record every component so the result is attributable.

Score the dimensions that matter
- Task success: was the user’s authorized goal completed?
- Correctness and evidence use: were the answer and actions supported by the right data?
- Safety and policy: did the agent respect permissions, privacy and escalation requirements?
- Robustness: did it recover from tool errors, ambiguity and changed context?
- Cost and latency: did additional steps, retrieval or human review change operational burden?
- Human workload: did the change create more corrections, escalations or inconsistent judgments?
Report each dimension by case category and severity. A single weighted score can be useful for a dashboard, but it should not replace inspection of failures. AWS documentation describes curating, scoring and comparing production traces, then feeding failing traces back into the test set. Apply that practice to both the incident category and adjacent categories that the fix could affect.
Inspect regressions, not just improvements
Suppose a new instruction prevents unsupported answers by making the agent refuse more often. Check whether refusal increased on legitimate requests, whether users receive a useful clarification path and whether the human queue became overloaded. A fix is acceptable only when its benefit remains after these collateral effects are considered.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.7. Keep humans involved where judgment is consequential
Human review is especially important for ambiguous intent, safety decisions, high-value transactions and cases where the reference answer is incomplete. Use a rubric with observable criteria, provide reviewers with the same evidence and periodically measure agreement. Disagreement is diagnostic: it may reveal an unclear policy or a grader that is being asked to make an impossible binary decision.
The MAP study found that human evaluation was the primary approach for 74% of surveyed deployed systems. That statistic describes its sample, but it is a useful warning against assuming that automated scores alone capture reliability. Automate repeatable checks to expand coverage, while reserving human attention for context-sensitive and high-impact outcomes.
The Tool Desk
Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →8. Roll out the change gradually
Offline improvement does not guarantee production improvement. Where feasible, compare the candidate with a control through a limited rollout or A/B test. Keep routing, eligibility and stopping rules documented before looking at outcomes, and avoid exposing high-risk actions to an unreviewed candidate merely to obtain a faster result.
Best Value
Pre-rollout checklist
- The candidate passes newly added cases and the maintained regression set.
- Counterexamples still receive the correct alternative behavior.
- Tool permissions, logging and rollback paths are verified.
- Reviewers know which version they are judging and how to escalate a severe failure.
- Success, regression and safety measures have owners and a defined observation period.
Use a canary for a small, representative slice when the risk warrants it. Keep the previous configuration available for rapid rollback, and preserve traces from both variants so later analysis does not depend on aggregate dashboards alone.
9. Monitor after deployment and refresh the suite
Traffic, tools, policies and user behavior change. Monitor task outcomes, escalation rates, tool errors, latency, cost, safety events, reviewer corrections and the distribution of task types. Compare current slices with the baseline used for approval; a stable average can hide deterioration in a rare but important category.
OpenAI’s cookbook on self-evolving agents describes feedback collection, human and automated grading, model-version comparisons and drift monitoring as parts of iterative improvement. Use those practices as an operating loop, not a one-time launch gate. Set organization-specific alert thresholds based on impact and volume, then review whether alerts still identify meaningful changes after the system evolves.
Refresh without erasing history
Add confirmed new failures, but retain older representative cases so a later optimization cannot silently undo a previous capability. Periodically retire duplicates, update cases whose tools or policies have changed and mark historical cases with the versions under which they were valid. OpenAI’s production-evaluation publication argues that refreshing production-derived evaluations and using broad data can reduce overfitting relative to narrow artificial datasets; it does not guarantee that every production sample will be representative.
Common traps and the corrective move
| Trap | Why it fails | Corrective move |
|---|---|---|
| Patch the most memorable complaint | Vividness is not prevalence, and the incident may have another cause. | Verify recurrence, classify the cause and test representative traffic. |
| Make the evaluation set only known failures | The agent can optimize for the test list while losing ordinary capability. | Keep representative cases, successes, counterexamples and a protected holdout. |
| Grade only the final text | Wrong tools, permissions or intermediate actions may be hidden by a plausible answer. | Evaluate the full trajectory and final outcome. |
| Use one automated judge for every task | Context-sensitive quality and policy decisions are not always reducible to one label. | Combine deterministic checks, calibrated judges and human review. |
| Change several layers without recording them | You cannot tell which component helped or caused a regression. | Version prompts, models, tools and routing; isolate factors when practical. |
| Stop monitoring after launch | Distribution and dependencies drift after approval. | Track outcomes continuously and refresh evaluations with new evidence. |
| Reuse transcripts without governance | Privacy, consent and contractual limits may be violated. | Minimize, de-identify, restrict and document permitted use. |
A compact operating record
For each improvement cycle, maintain one record containing: the trigger traces; prevalence and sampling notes; failure classification; proposed change; affected versions; evaluation-set additions; scores by category; reviewer disagreements; rollout allocation; monitoring results; and the decision to deploy, revise or revert. This record makes the process auditable and prevents the organization from repeatedly rediscovering the same failure.
The central discipline is simple: production tells you what to investigate, evaluation tells you whether a change generalizes, and staged monitoring tells you whether it remains reliable. Keeping those roles separate is how feedback improves an agent without turning every complaint into a brittle global rule.
