Sildenafil, tadalafil, vardenafil and avanafil can be effective, but they can cause dangerous blood-pressure changes when combined with nitrate medicines and can interact with several common heart, prostate, blood-pressure, HIV, antifungal and antibiotic treatments.
Use only the medicine and dose prescribed for you, never take more than one tadalafil ED dose in 24 hours, avoid unverified online products, and seek urgent care for an erection lasting more than four hours, sudden vision or hearing loss, severe dizziness or fainting, chest symptoms, or a suspected overdose.

Why a medication review matters more with age
Before taking any medicine for erectile dysfunction, get your healthcare professional’s OK. That is the central advice in Mayo Clinic’s 2026 patient guidance. Erectile difficulty can be an early sign of a cardiovascular, blood-pressure, diabetes, cholesterol, hormonal or medication problem, so treating the symptom without checking the cause can miss an important health issue.
Age itself does not determine whether a tablet is appropriate. What changes over time is the likelihood of several conditions occurring together and the number of medicines being taken. A prescription that was suitable years ago may now interact with a new blood-pressure drug, prostate treatment, heart medicine or supplement. Kidney and liver function can also affect how medicines are handled. A current review is therefore more useful than relying on an old prescription or a friend’s experience.

Bring a complete medication list
Before an appointment, write down every prescription, over-the-counter product, vitamin, herbal preparation and recreational substance you use. Include the strength, how often you take it and when you last took it. Bring the medicine containers or a current pharmacy printout if possible. Tell the clinician about any medicine for chest pain, urinary symptoms, blood pressure, pulmonary hypertension, HIV, fungal infection or an infection treated with antibiotics.
Tell the clinician about relevant health changes
Mention chest discomfort, fainting, unusually low blood pressure, recent heart or stroke problems, diabetes, high cholesterol, hormonal symptoms, kidney or liver disease and any previous reaction to an ED medicine. Do not stop a heart or prostate medicine on your own to make an ED pill possible. The prescriber must decide whether a change, a different ED treatment or a non-pill option is safer.
How the main ED medicines differ
Sildenafil, tadalafil, vardenafil and avanafil are phosphodiesterase type 5 (PDE5) inhibitors. They support an erection in response to sexual stimulation; they do not create continuous sexual arousal by themselves. The medicines are not interchangeable by tablet count or timing, so follow the label for the specific prescription.

The interaction rules you must know
Nitrates: the non-negotiable warning
Do not combine a PDE5 inhibitor with nitrate medicines used for angina unless a clinician who knows both treatments explicitly directs a safe plan. This includes nitroglycerin, glyceryl trinitrate, isosorbide mononitrate and isosorbide dinitrate. The combination can produce a dangerous fall in blood pressure.
If you develop chest symptoms after taking an ED medicine, seek emergency care and tell the emergency team exactly what you took and when. Do not take a nitrate for the chest symptom unless the treating professionals know about the ED medicine and instruct you to do so. Carrying an up-to-date medication list can prevent a dangerous assumption during an emergency.
Alpha-blockers and blood-pressure medicines
Alpha-blockers, often used for urinary or prostate symptoms, and some antihypertensive medicines can lower blood pressure further when combined with a PDE5 inhibitor. That does not mean you should stop either treatment. It means the prescriber needs the names, doses and timing of both medicines before choosing an ED treatment and dose.
CYP3A4 inhibitors and other interacting medicines
Medicines that inhibit the CYP3A4 pathway can change PDE5 inhibitor levels. The interaction check should also cover medicines for pulmonary hypertension, HIV, fungal infections and some bacterial infections. Tell the clinician about short courses as well as long-term prescriptions; a temporary antibiotic or antifungal can matter even if your regular medicines have not changed.

Grapefruit and alcohol
Follow the product guidance about grapefruit or grapefruit juice, because the advice can differ by medicine. Avoid excess alcohol. Heavy drinking can worsen dizziness and blood-pressure effects and can make it harder to judge whether a symptom is a medicine reaction or an emergency.
A safe-use procedure for a prescribed tablet
- Confirm the prescription. Check the medicine name, strength, directions and whether it is for use as needed or on a regular schedule. Never use someone else’s tablets or combine two ED medicines.
- Recheck interactions before the first dose. Compare the prescription with your current medication list, including nitrates, alpha-blockers, blood-pressure medicines, pulmonary-hypertension drugs, HIV medicines, antifungals, antibiotics and supplements. Ask a pharmacist or prescriber if anything has changed since the prescription was written.
- Take only the prescribed amount. Do not increase the dose because the first attempt was ineffective, and do not repeat a dose early. For tadalafil used for ED, never exceed one dose in 24 hours. Ask the prescriber what to do if it does not work rather than experimenting.
- Use the correct timing. Sildenafil and vardenafil generally work best when taken about an hour before sex. Avanafil may work in about 30 minutes depending on dose. Tadalafil can provide a window of up to 36 hours, but the longer window is not a reason to take additional tablets.
- Account for food. Tadalafil may be taken with or without food. A large or high-fat meal can delay sildenafil absorption, so follow the instructions supplied with your prescription if timing matters.
- Limit alcohol and avoid unapproved combinations. Do not add “herbal Viagra,” bodybuilding products or another ED tablet. Products marketed as natural can contain undisclosed prescription ingredients.
- Record what happened. Note the time, dose, food, alcohol, effect and any symptoms. This gives the prescriber useful information for a safe adjustment; it is not a reason to take an extra tablet the same day.
Side effects that need attention
Stop sexual activity and seek medical advice promptly if you develop severe dizziness, fainting, chest symptoms or a reaction that feels serious. Tell the clinician the exact medicine, dose and time taken. Do not drive yourself if you are faint, confused or severely dizzy.
Priapism: an erection lasting more than four hours
An erection lasting more than four hours is priapism and requires urgent medical assessment, even if it is not painful. Mayo Clinic and NHS guidance identify the four-hour threshold. Waiting for it to resolve can permanently injure erectile tissue. Tell emergency clinicians which ED medicine you used and when; do not take another dose or try to treat the erection yourself.

Sudden vision or hearing changes
Sudden vision loss or sudden hearing loss after an ED medicine is an emergency. Seek urgent care rather than waiting for a routine appointment, and provide the medication details to the treating team.

Suspected overdose
Contact emergency services or a poison service immediately for a suspected overdose, especially if there is severe dizziness, fainting, chest symptoms or any other concerning change. Keep the packaging available so clinicians can identify the product and strength.

Polypharmacy and changing prescriptions
Medication lists become outdated easily after a hospital stay, a new diagnosis or a change in pharmacy. Repeat the interaction review whenever a medicine is added, stopped or changed. Do not assume that a product bought without a prescription is irrelevant; supplements and “natural” sexual-enhancement products can also alter risk.
Kidney and liver function
Kidney and liver function are part of the choice between ED medicines and may affect the dose or whether a tablet is appropriate. The safe response is a clinical review, not a self-calculated reduction or increase. Ask whether recent blood tests are needed before starting or changing treatment.
Blood-pressure symptoms
Feeling faint, unusually weak or severely dizzy after a dose can indicate excessive blood-pressure lowering, particularly when other blood-pressure medicines or alpha-blockers are involved. Sit or lie down, avoid driving and contact a clinician; seek urgent care for severe symptoms, fainting or chest symptoms.
When tablets are unsuitable or do not work
Oral PDE5 inhibitors are not the only option. Mayo Clinic describes self-injection therapy with alprostadil and other treatments when oral medicines are ineffective or inappropriate. A clinician must teach the injection technique and establish a safe dose. Using more than instructed can cause a prolonged erection, and injections can cause local complications.
Ask for a treatment review rather than taking increasingly large or frequent tablets. The review can consider whether the original diagnosis is correct, whether an interaction or health condition is reducing effectiveness, and whether a non-pill treatment is safer. Do not switch to injections, supplements or another person’s medicine without professional instruction.
How to avoid counterfeit ED medicines online
Unverified marketplaces and products labelled “herbal Viagra” can contain the wrong drug, the wrong strength or hidden prescription ingredients. Mayo Clinic advises checking that an online pharmacy has a physical address, a telephone number, a licensed pharmacist and appropriate licensing before using it. A site that refuses to request a prescription, offers unusually secretive payment or cannot identify a pharmacist should not be trusted.

The FDA issued an alert on May 29, 2026, after finding hidden sildenafil and tadalafil in a product called Erectus Plus. The warning illustrates why a product’s label or “natural” marketing is not proof of its contents. Hidden PDE5 inhibitors are particularly dangerous for people who use nitrates or other interacting medicines because the buyer may not know that a prescription drug is present.
Safer purchasing steps
- Ask your clinician or pharmacist for the exact generic name and strength prescribed.
- Use a pharmacy that requires a valid prescription and clearly identifies its physical address, telephone number and licensed pharmacist.
- Check the package when it arrives: the medicine name, strength, expiry date and manufacturer should be readable and consistent with the prescription.
- Do not take a tablet with missing, altered or unfamiliar packaging. Ask a pharmacist to verify it before use.
- Report suspected counterfeit or hidden-ingredient products to the relevant medicines regulator and seek medical advice if you already took one.
Questions to take to your appointment
- Could my ED be related to a heart, blood-pressure, diabetes, cholesterol, hormonal or medication problem?
- Are any of my medicines nitrates, alpha-blockers, CYP3A4 inhibitors or other interacting drugs?
- Do my kidney or liver tests change which ED medicine is suitable?
- Which option best fits my preferred timing, meals and frequency of sexual activity?
- What symptoms should make me call urgently, and what should I tell emergency clinicians?
- What is the safest alternative if tablets are ineffective or inappropriate?
Keep the answers with your medication list. If you need emergency care, state the ED medicine and the time of the last dose before discussing any chest-pain treatment or other new medicine.
