In the 2024 D&D rules, a creature is Bloodied when it has half its Hit Points or fewer—including exactly half. The threshold has no effect by itself; it is a clear way to signal combat progress or a trigger for another rule or an explicitly designed encounter effect.
What Bloodied means in the 2024 rules
The 2024 Rules Glossary defines a creature as Bloodied while it has half its Hit Points or fewer remaining. The official Basic Rules clarify that Bloodied has no game effect on its own, though another rule or feature can use it as a trigger. Crossing the threshold does not automatically impose a penalty, change a creature’s turn, or reduce its abilities.

For an odd maximum, apply “half or fewer” to whole-number Hit Points. A creature with 45 maximum HP is Bloodied at 22 HP or fewer; 23 HP is still above half. This is arithmetic applied to the definition, not a separate rule.
How to track the threshold during combat
- Set the creature’s maximum HP. Use the monster’s listed Hit Points or roll its listed Hit Point dice expression, as allowed by the 2024 Basic Rules.
- Mark the Bloodied point before play. Note the greatest whole-number value at or below half of maximum HP. For example, mark 22 for a creature with 45 maximum HP.
- Track current HP against that mark. Add a note to your HP tracker, use a VTT marker, or place a token beside the miniature. The 2024 DM toolbox recommends tracking monster conditions alongside HP and suggests tangible reminders such as tokens: Tracking Monster Conditions.
- Choose a cue and apply it consistently. You can say “The ogre is Bloodied,” or narrate the change: “The ogre’s breathing turns ragged and one knee buckles.” A creature need not literally bleed; use details that fit it, such as cracked plating, dimming light, or a faltering stance.
Choose how much information the cue gives
A precise announcement of “Bloodied” communicates that the creature has reached a specific threshold. Evocative description can signal damage without giving players the creature’s exact HP. Either can work, but set expectations: tell the table whether “Bloodied” is always a precise half-HP signal, whether descriptions are approximate, or whether you use both approaches for different purposes.

A 2014 DMG passage is quoted in a D&D Beyond forum search result as advising DMs that they need not reveal exact monster HP and may describe a monster below half its maximum as visibly wounded. It presents “bloodied” as a cue players can use to judge progress and decide when to spend powerful abilities. Three ways to use the threshold
Use it as a descriptive milestone
The simplest option is to make Bloodied a consistent progress cue and leave the creature’s mechanics unchanged. Players receive useful information without needing an exact HP total, and the DM does not have to add a new rule to every encounter.

Use it as an explicit monster or encounter trigger
You can design a feature that activates when a creature becomes Bloodied—for example, it changes tactics, tries to escape, exposes a weak point, or enters a new phase. The threshold itself does not supply that effect. Write down what happens, when it happens, how long it lasts, and whether it can happen again so the table can apply it reliably.

Use it as a morale prompt
Bloodied can be a moment to consider whether an opponent still has a reason to fight. Depending on the creature’s goals and circumstances, it might bargain, flee, or surrender rather than continue to the death. D&D Beyond encounter advice offers surrender at one-quarter of maximum HP as one possible design choice, not a universal rule: D&D Beyond encounter advice. A campaign could use Bloodied as an early warning and a lower threshold as a possible morale point, but the decision belongs to the DM.

Using Bloodied with player features
When a character feature, spell, or encounter effect refers to Bloodied, use the written definition for the applicable rules. Make clear what the characters can observe, and apply only the consequence that the feature actually states. Do not turn the label into an undocumented bonus or penalty.

Bloodied is a health threshold, not a requirement that every creature show red blood. Let the narration communicate harm in a way that fits the creature: a construct may spark or split, an elemental may lose cohesion, and a beast may limp or struggle to breathe. The rules do not mandate a particular description; consistency is what makes the cue useful.
