Use AI for bounded, support-oriented jobs—such as finding approved resources, delivering structured stress-management exercises, or helping someone prepare for a conversation—while people retain responsibility for listening, clinical judgment, safeguarding and escalation. Do not present a general-purpose chatbot as a therapist, diagnostician, confidential counsellor or crisis service. A responsible programme starts with working conditions, obtains informed worker participation, minimizes data, makes human help easy to reach, and measures both benefits and harm.
Why employee wellbeing cannot be reduced to an app
Work can protect mental health through purpose, connection and financial security, but it can also create or worsen distress through excessive demands, low control, unclear roles, isolation or harmful management. The World Health Organization (WHO) describes work-related mental-health conditions as preventable and its 2022 Guidelines on mental health at work covers organizational interventions, manager and worker training, individual support, return to work and gaining employment. AI belongs inside that broader programme, not in place of it.

WHO’s 2026 workplace facts page reports that 15% of working-age adults live with a mental disorder, 12 billion working days are lost each year to depression and anxiety, and the global economic cost is about US$1 trillion annually, predominantly through reduced productivity. These figures describe the scale of need; they do not show that an AI product improves wellbeing or recovers any of those losses.

What AI can responsibly do—and where the boundary lies
The safest applications have a clear purpose, a limited knowledge base and an obvious hand-off to a person. They provide information or structure; they do not make high-stakes judgments about a worker’s health or employment.
| Potential use | What the system may do | Required human boundary |
|---|---|---|
| Resource navigation | Direct a worker to the employer’s approved employee assistance programme, occupational-health service, policies or external support. | A person must be available when the worker needs interpretation, reassurance, an exception or a confidential conversation. Keep the resource directory current. |
| Structured self-guided exercises | Deliver a defined sequence of evidence-based stress-management content in text or voice. | Present the material as self-help, not treatment. Offer a route to a qualified professional and stop the flow when a person needs assessment or urgent help. |
| Conversation preparation | Help someone organize questions or describe what they want to discuss with a manager, HR contact, clinician or counsellor. | The human conversation remains the decision point. The tool should not draft a diagnosis, determine fitness for work or advise retaliation. |
| Aggregate process insight | With explicit safeguards, summarize recurring themes in voluntary, de-identified feedback so an organization can examine workload, scheduling or access barriers. | Do not identify individual contributors, infer a person’s emotional state, or use wellbeing data for performance, promotion, discipline or selection. |
| Clinical diagnosis, therapy or treatment | Not an appropriate general-purpose workplace use. | Assessment and care belong to qualified clinicians and the worker’s chosen health service. |
| Crisis response | At most, display a prewritten, jurisdiction-specific instruction to contact immediate human or emergency support. | A generative reply cannot be the safety net. A staffed escalation path must be tested before launch. |
WHO and the International Telecommunication Union (ITU) have published an adaptation package for delivering the WHO stress-management guide Doing What Matters in Times of Stress through text or voice chatbots. That package demonstrates a bounded implementation pattern; it is not evidence that an arbitrary generative chatbot is clinically effective or safe.
Start with the work conditions and the support gap
Map the sources of strain
Use interviews, worker representatives, anonymous surveys and existing occupational-safety information to identify excessive workload, low autonomy, unpredictable schedules, role conflict, isolation, discrimination, poor management behaviour and barriers to existing help. Separate problems the employer can fix directly from problems where better navigation or self-guided material could help.
Recommended Free Tools
Define the outcome before selecting technology
Write a plain-language outcome such as “more employees can find the occupational-health appointment process” or “workers can complete an approved stress-management module and understand how to request human support.” Avoid objectives such as “detect unhappy employees” or “predict who will need leave.” The latter invite intrusive inference and employment consequences rather than prevention.
Check whether a non-AI change is better
A clearer policy, additional staffing, manager training, schedule redesign, a confidential phone line or faster clinical referral may solve the identified problem without collecting conversational data. AI is justified only when it adds practical access, consistency or capacity that a simpler intervention cannot provide.
A safe implementation sequence
-
1. Write a narrow use-case specification
State the user, task, approved content, prohibited outputs, languages and channels. Define whether the system gives information, runs a fixed exercise or helps prepare a question. Exclude diagnosis, treatment decisions, employment recommendations, covert monitoring and open-ended emotional counselling.

2. Assign accountable owners
Name the business owner, occupational-health or clinical adviser, privacy lead, information-security contact, worker representatives and the person responsible for urgent referrals. Record who handles complaints, content corrections, outages, harmful outputs and requests to delete data. Accountability must remain with the organization rather than the model vendor or an individual manager.

3. Conduct a mental-health and worker-impact assessment
Before procurement, examine foreseeable benefits and harms, including stress from being monitored, reduced autonomy, work intensification, exclusion of disabled or non-native-language workers, inaccurate advice and data misuse. WHO’s 20 March 2026 expert workshop recommends impact assessment and continuing monitoring, co-designed with mental-health experts and people with lived experience.
-
4. Co-design with workers
Invite employees and their representatives to test language, tone, accessibility, cultural fit, consent screens, referral wording and failure messages. Explain what participation can and cannot change. Do not treat a one-time consultation as consent; provide a channel for feedback throughout the pilot and after deployment.
-
5. Curate and constrain the content
Prefer an approved library or scripted journey for wellbeing material. Identify the content owner and review date for every resource. Configure the system to say when it does not know, avoid unsupported health claims and direct the user to a person when the request falls outside scope. Test common misunderstandings, hostile prompts, ambiguous symptoms and requests for secrecy.
-
6. Publish the data and employment-use rules
At the first interaction, tell workers that they are speaking with AI, what data is collected, why it is needed, how long it is retained, who can access it, whether a supplier processes it and whether any output can affect employment. State plainly when it cannot be used for performance, attendance, promotion, discipline, redundancy or insurance decisions. Obtain meaningful consent where required and provide a non-AI route to the same support.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy. -
7. Build and rehearse escalation
Put a visible “talk to a person” option on every relevant screen. Configure location-appropriate details for occupational health, the employee assistance programme, a trusted manager or HR contact, emergency services and crisis lines. Run scripted tests for expressions of imminent danger, abuse, confusion, self-harm or harm to others. Verify that a human receives the referral, understands the context the worker agreed to share and responds within a defined time.

8. Pilot narrowly and monitor continuously
Limit the pilot to a volunteer group or a low-risk function, publish the start and end dates, and keep a non-AI comparison route. Review accuracy, accessibility, completion, referral success, complaints, privacy incidents, added workload and signs that workers feel watched or pressured. Pause the system when serious harm, unsafe advice or an unmanageable privacy failure appears; do not wait for a scheduled review.
-
9. Decide whether to expand, change or stop
Use predefined thresholds and worker feedback, not enthusiasm or usage volume alone. Expansion requires evidence that the intended task improved and that harms remained controlled. If a problem cannot be mitigated through content, workflow or governance changes, retire the feature and preserve a reliable human alternative.
Privacy, consent and employment boundaries
Wellbeing conversations can reveal health information even when the user does not type a diagnosis. EU-OSHA notes that systems may infer emotional wellbeing from ordinary-seeming signals such as body language, facial expression and tone of voice. The International Labour Organization (ILO) identifies surveillance, work intensification, reduced autonomy and privacy or data-use concerns as psychosocial risks associated with AI in work.
- Collect the minimum: Do not request camera, microphone, location, contacts or free-form journal entries when a resource link or fixed exercise is sufficient.
- Separate support from management: Keep individual conversations away from line-manager dashboards and performance systems. Aggregate reporting should use thresholds that prevent singling out small groups.
- Explain inference: If any analysis uses language, voice or other signals, describe the inputs, purpose, limits and error modes in terms workers can understand. Avoid covert emotion scoring.
- Set retention limits: Specify deletion dates, backups, access logs and procedures for a worker’s data-access or deletion request. Do not retain transcripts merely because storage is cheap.
- Control secondary use: Prohibit reuse for recruitment, discipline, attendance scoring, productivity ranking, advertising or model training unless a separate lawful and genuinely voluntary basis exists.
- Protect the non-user: A manager’s notes, a colleague’s name or a family member’s health detail can enter a conversation. Design prompts and filters to discourage unnecessary third-party information.
- Provide an alternative: A worker must be able to obtain the underlying support without accepting AI monitoring or creating a conversational profile.
Transparency is not a long privacy notice hidden behind a link. At the point of use, show the identity of the system, its limits, the human route and the consequences—if any—of declining to use it.
How a wellbeing tool should handle a crisis
A system that was not designed and tested for mental-health crisis work must not improvise a counselling response. WHO’s 20 March 2026 workshop called for agreed crisis-referral frameworks and accountability systems and warned that generative AI used for emotional support may lack appropriate design and testing.
- Recognize a possible emergency without diagnosing. Use conservative, reviewed triggers for statements indicating immediate danger, self-harm, harm to others, abuse or inability to stay safe. Treat uncertainty as a reason to offer human help, not as proof that a crisis exists.
- Stop ordinary coaching. Do not continue a breathing exercise, debate the person’s feelings or promise secrecy while an urgent-support message is displayed.
- Give local, actionable options. Show the relevant emergency number, crisis service, occupational-health contact and trusted human channel for the worker’s jurisdiction. If location is unknown, ask only what is necessary or give a general instruction to contact local emergency services.
- Offer a person immediately. Where the organization promises live response, connect or call back through a staffed service. State operating hours and what happens outside them.
- Share only with permission and according to law. Tell the worker what information will be passed to the responder. Emergency exceptions, safeguarding duties and data-protection requirements must be defined by qualified legal and clinical advisers before launch.
- Review every event. Record the minimum needed for safety review, remove identifying data where possible, and examine delays, false reassurance, inaccessible wording and failed referrals. Feed corrections into the system and its procedures.

| Decision area | Questions to ask | Evidence to require |
|---|---|---|
| Purpose and benefit | What exact task is supported, for which workers and in what setting? | A measured outcome tied to that task, not a general claim that AI improves wellbeing. |
| Content quality | Who approves health-related material, how often is it reviewed and how are unsupported answers blocked? | Versioned content, review ownership, test cases and correction process. |
| Privacy and minimization | What inputs, transcripts, metadata and inferences are stored, for how long and by whom? | Data-flow map, retention schedule, access controls and deletion procedure. |
| Employment firewall | Can a manager, HR system or supplier use an individual interaction in an employment decision? | Contractual and technical controls, audit logs and a worker-facing prohibition. |
| Transparency | Will workers know they are interacting with AI and understand its limitations? | Plain-language notices, consent screens and examples of uncertain responses. |
| Participation and trust | How were workers, representatives and people with lived experience involved? | Documented co-design, accessibility testing and an ongoing feedback route. |
| Bias and accessibility | Does performance vary by language, disability, culture, accent or communication style? | Disaggregated testing, accessible interfaces and remediation commitments. |
| Human oversight | Who reviews complaints, harmful outputs, content changes and service outages? | Named roles, response times, escalation records and stop authority. |
| Crisis capability | What happens when a user may be in immediate danger? | Jurisdiction-specific scripts, staffed referral routes and results from rehearsal. |
| Net effect on work | Does use reduce friction or add prompts, documentation, pressure and surveillance? | Worker feedback, workload measures and evidence of benefit minus added burden. |
WHO cautions that large language models can produce plausible but wrong health-related answers, reflect bias and fail to protect sensitive information supplied by users. Apply that warning to workplace wellbeing without treating it as a clinical trial of any particular product: fluency is not validation, and a disclaimer does not repair an unsafe workflow.

Measure benefits and harms from the first day
Useful benefit measures
- Whether workers can find the approved service or policy they sought.
- Completion and comprehension of a defined self-guided module.
- Time from a request to a human appointment or response.
- Worker-reported confidence in knowing where to get help.
- Changes in the specific process problem the intervention was intended to address, such as confusion about referral steps.
Safety and trust measures
- Incorrect, misleading, stigmatizing or culturally inappropriate outputs found in audits.
- Unsuccessful, delayed or inappropriate crisis referrals.
- Privacy incidents, unexpected retention, unauthorized access or employment-use violations.
- Differences in access, accuracy, satisfaction and referral success across languages, disability groups and other relevant populations.
- Reports of surveillance, pressure to use the system, increased workload, reduced autonomy or avoidance of human help.
- Requests to delete data, complaints and the time taken to resolve them.
Set thresholds before launch and publish what happens when they are exceeded. A high interaction count is not a success metric if workers are using the tool because human services are unavailable or because managers expect them to do so.
What human support still does
Managers and team leaders
Managers can adjust workload, clarify priorities, notice changes in context and make a respectful invitation to talk. They need training in psychologically safer conversations, boundaries, reasonable adjustments and referral—not access to private AI transcripts.
Occupational-health professionals and clinicians
Qualified professionals assess symptoms, risk, consent, capacity, treatment options and work adjustments. They can integrate medical and workplace context in ways a general-purpose model cannot reliably do.
Employee assistance and peer services
Human counsellors, peer supporters and employee assistance programmes provide confidential listening, continuity and judgment. AI may help a worker locate these services, but it should not make them harder to reach or imply that self-help is an adequate substitute.

Worker representatives
Unions, safety representatives and employee forums can identify power imbalances, test whether consent is meaningful and challenge uses that increase surveillance or shift risk onto workers. Their involvement is part of prevention, not a communications exercise after procurement.

Practical examples of bounded deployment
Example: finding support after a difficult shift
A worker asks where to arrange a confidential appointment. The system identifies itself as AI, displays the approved occupational-health and employee-assistance options, explains operating hours and offers to help prepare questions for a human conversation. It records only the minimum routing information and does not tell the worker’s manager.
Example: a structured stress-management journey
An organization offers the WHO/ITU-adapted stress-management content through a text channel. The opening screen explains that the journey is educational, not therapy; each exercise has a clear stop option; and every stage links to a human service. Completion is evaluated alongside comprehension, referral use, accessibility and reports of distress—not as proof of clinical improvement.
Example: identifying a scheduling problem without profiling people
Workers voluntarily submit anonymous comments about an approved scheduling process. The organization reviews sufficiently large, de-identified groups for recurring operational themes, such as unpredictable shifts, then changes the process and checks whether the problem declines. It does not infer which individual is distressed or attach sentiment scores to personnel records.

WHO’s 2026 expert workshop emphasizes safety, accountability, monitoring and co-design with mental-health experts and people with lived experience. EU-OSHA’s 2025 information sheet summarizes the operating principle: “A human-centred approach, worker participation and a sound OSH prevention and management system in place are essential to balancing the advantages of technology with worker safety, health and well-being, creating safer, healthier and more productive work environments.”
That means AI should make sound support easier to access or deliver while the organization continues to fix harmful conditions, train managers and workers, provide individual help, support return to work and maintain inclusive employment practices. If the system cannot meet those conditions, the responsible choice is to narrow it, redesign it or not deploy it.
