The honest way to use AI is to treat it as a drafting assistant, not an authority. Ask it to distinguish verified information from assumptions, identify what it does not know, and ask for missing context instead of guessing. Then check every material claim against the cited primary source and, for consequential work, have a qualified person review it.
You cannot make a model incapable of inventing facts. You can, however, build a workflow that makes uncertainty visible, catches unsupported claims, protects sensitive information, and stops an attractive draft from being mistaken for evidence.

What honest AI use means
Honest use is not a special tone or a disclaimer added after an answer. It is a process for matching the model’s confidence to the evidence available. A useful answer should tell you what is established, what is inferred, what depends on an assumption, and what still needs checking.
Crashes, No Sound, or Screen Glitches?

OpenAI’s Help Center describes these errors as hallucinations and warns that confidence is not a measure of reliability. A polished paragraph is therefore a draft to inspect, not proof that the underlying claim is true.

The practical objective is not to eliminate every error. It is to prevent an unverified output from being used as if it were verified, especially when the result could affect someone’s health, money, legal position, safety, employment, education, or reputation.
Why an AI can sound certain while being wrong
An answer may be generated from patterns that resemble a good response without the model having reliable evidence for the particular date, country, product version, or person you mean. Missing context makes this worse. A question such as “What is the deadline?” has no dependable answer until the jurisdiction, program, and relevant year are known.
There is also a built-in pressure toward answering. In its explanation of hallucinations, OpenAI notes that evaluation incentives can reward guessing over abstaining. Its Model Spec favors indicating uncertainty or asking for clarification when an answer may be wrong. That means a responsible user should explicitly permit the model to stop, ask, or say that the evidence is insufficient.
Recommended Free Tools
Do not look for a single universal hallucination percentage. Error rates vary by model, task, language, prompt, tool access, and evaluation method, and no authoritative figure from the available guidance supports quoting one number for all uses. Qualitative controls and claim-by-claim verification are more defensible.
Start by defining the task and its risk
Before asking for an answer, specify the conditions that determine whether an answer is valid. This takes less time than repairing a confident answer built on the wrong assumptions.

- Task: Say whether you want a summary, calculation, draft, explanation, options, or a source-finding plan.
- Audience: Identify the reader’s expertise and what they need to decide or do.
- Date: Give the relevant “as of” date. Laws, prices, software behavior, public-office holders, and safety guidance can change.
- Geography and jurisdiction: Name the country, state, province, regulator, or market.
- Version or edition: Include the software release, contract version, standard edition, or publication revision when it matters.
- Stakes: Mark the request as low, medium, or high consequence. A brainstorming list and a medication decision should not use the same approval threshold.
- Available evidence: Provide the documents, links, data, or excerpts the model is allowed to use, and say when it must not fill gaps from general knowledge.
If any of these facts are missing and could change the answer, ask a clarifying question before requesting a conclusion. A good clarification is narrow and actionable: “Which country and tax year do you mean?” or “Do you mean version 4.2 or 5.0?”
Use a prompt that makes uncertainty visible
Instead of asking for an authoritative-sounding response, require an evidence map. This does not make the output true, but it gives you places to check and makes unsupported confidence easier to spot.
Answer this task for {{audience}} as of {{date}} in {{jurisdiction}} and {{version}}: {{task}}. First list the facts you can support from the supplied material. Then list assumptions, unresolved uncertainty, and the questions that would change the answer. For every material factual claim, provide the exact source link and explain what part of the source supports it. If the evidence is missing, outdated, or conflicting, say so and do not guess. End with a short verification checklist and identify whether qualified human review is needed.
For a question with missing context, use a stop condition:
Before answering, ask up to three clarifying questions if the date, location, version, or intended use is unspecified. Do not choose a likely interpretation silently.
For calculations, ask for the inputs, formula, units, rounding rule, and an independently checkable working. For summaries, tell the model to separate what the document says from its interpretation. For creative work, factual accuracy may be less important, but invented quotations, credentials, historical claims, or references still need checking.
A repeatable workflow for honest answers
Use the following sequence whenever an output contains facts another person might rely on.

- Define the decision. Write down what the answer will be used to decide or publish, who could be affected, and the consequence of an error.
- Collect the context. Add the audience, date, geography, version, constraints, and source material. Treat missing context as a reason to pause, not an invitation to infer.
- Request separated claims. Ask the model to label known information, assumptions, uncertainty, and open questions. Require it to say when it cannot verify something.
- Demand traceable sources. For factual claims, prefer the original law, regulator, official statistics, standard, court decision, manufacturer documentation, or peer-reviewed paper. A link is a lead, not proof.
- Open every citation. Read the cited page yourself. Confirm that it exists, is the document the model names, and actually supports the sentence attached to it.
- Check fit. Verify the source’s publication or update date, jurisdiction, version, definitions, units, population, and exceptions. A true statement from another country or an older edition can still be wrong for your decision.
- Cross-check important claims independently. Use a second primary or reputable source, a published dataset, or a qualified reviewer. A second answer from the same or another AI is not independent evidence; it may reproduce the same error.
- Escalate high-stakes work. Stop at a clearly labeled draft and obtain qualified legal, medical, financial, safety, employment, or other specialist review before acting.
- Record the review. Keep the model name and version if known, date of generation, prompt or relevant conversation, sources checked, unresolved questions, and human edits. This creates an audit trail and makes later correction possible.
How to fact-check an AI citation
“It has a link” is not the same as “the link proves the claim.” Check the citation at four levels.
| Check | Question | Common failure |
|---|---|---|
| Existence | Does the URL resolve to the named document or organization? | An invented article, altered title, dead link, or unrelated page. |
| Support | Does the cited passage actually establish the sentence? | The source discusses a possibility, while the answer states it as a rule or fact. |
| Scope | Do the date, place, version, population, definitions, and exceptions match? | A national rule presented as universal, or an old software manual used for a current release. |
| Independence | Is there another authoritative basis for a consequential claim? | Several pages repeating the same press release or AI-generated wording. |
Open the source rather than relying on a snippet or the model’s quotation. Search the document for the relevant term, read the surrounding passage, and note qualifications such as “may,” “up to,” “in limited cases,” or “for the 2025 edition.” If the source cannot be opened, mark the claim unverified instead of treating the citation as a credential.
The UK House of Commons Library advises treating AI as an assistant, not an authority. That is a useful rule for citations: the assistant can point you toward evidence, but the evidence must be inspected by a person.
Keep a human in the loop when the consequences matter
OpenAI’s responsible-use guidance says to keep a human in the loop for important work, double-check critical facts, and seek qualified experts for legal, medical, and financial advice. Human review is not a ceremonial sign-off. The reviewer should have the expertise and authority to reject the draft, inspect the sources, and request missing information.
| Use case | Minimum control | When to stop and escalate |
|---|---|---|
| Brainstorming, outlining, or rewriting supplied text | Check that the output follows the brief and does not introduce unsupported facts. | Escalate if the draft adds claims, quotations, or sensitive personal information. |
| Routine factual explanation | Check material claims against current reputable sources and record the date. | Escalate when the answer depends on a regulated rule, safety limit, or rapidly changing information. |
| Legal, medical, financial, employment, or safety decision | Use AI only to prepare questions, organize documents, or produce a reviewable draft. | Always obtain qualified human review before acting or publishing advice as personal guidance. |
| Automated action affecting people, money, access, or rights | Define approval gates, test cases, logging, and a way to reverse or appeal the action. | Do not deploy without accountable human ownership and monitoring appropriate to the risk. |
A reviewer should be able to answer three questions: What exactly is the model claiming? Which source supports each claim? What happens if the claim is wrong? If the answer to any of them is unclear, the output is not ready for consequential use.

Use validation, not confidence, as your trust test
Trustworthy use requires more than a persuasive explanation. The National Institute of Standards and Technology describes validation as confirmation through objective evidence. In practice, validation means defining what would count as a correct answer, testing the output against that criterion, and retaining evidence that the test was performed.
- Validity: Does the output answer the specified question rather than a nearby one?
- Reliability: Does it remain acceptable across representative inputs, edge cases, and repeated runs?
- Explainability: Can a reviewer trace important statements to sources, calculations, or explicit assumptions?
- Context-appropriate controls: Are the review, privacy, access, and escalation rules proportionate to the potential harm?
For a small internal draft, a documented spot check may be enough. For a system that makes or recommends consequential decisions, define test cases before deployment, monitor errors, log overrides, and provide a route for human correction. Never treat a high confidence score, a smooth explanation, or agreement between two generated answers as objective validation.
What to say when the answer is unknown
Honest uncertainty is specific. It tells the reader why the answer is uncertain and what would resolve it. Useful wording includes:
- “I don’t have enough reliable information to answer that.”
- “This is uncertain because the source is missing or the facts may have changed.”
- “Here is what I can verify, and here is what remains unverified.”
- “Before I answer, what date, location, or version do you mean?”
- “The available sources disagree. I can summarize the difference, but I cannot select a correct conclusion without the governing document or an expert review.”
- “I can provide a general explanation, not a personal legal, medical, or financial decision.”
These statements are useful only when followed by an appropriate next step: a clarifying question, a source request, a search plan, a calculation that exposes its inputs, or referral to a qualified person. Do not replace “I do not know” with a likely guess merely because the guess sounds complete.
How to reduce made-up facts without pretending they can be eliminated
There is no prompt that guarantees truth. Combine several safeguards instead:
Best Value
- Supply authoritative documents or links and instruct the model to stay within them.
- Ask for a claim-by-claim answer rather than a persuasive essay.
- Require uncertainty labels and a list of missing information.
- Set a date, jurisdiction, and version in the request.
- Tell the model to ask questions before answering when those fields are absent.
- Require exact citations for material claims and reject citations that cannot be opened or do not support the wording.
- Ask for calculations with visible inputs and units, then recompute independently.
- Use a human approval gate before publication, submission, purchase, treatment, filing, or automated action.
Do not ask the model to “sound confident,” “never say you are unsure,” or “fill in plausible details.” Those instructions optimize presentation at the expense of reliability. Likewise, asking for a fictional citation format can make an invented reference look official.
Compare AI answers or tools on evidence, not style
When two systems produce different answers, compare the process behind them rather than choosing the one that reads better.
| Comparison axis | What to inspect |
|---|---|
| Evidence quality | Are claims tied to primary documents, regulators, official statistics, standards, or peer-reviewed work? |
| Source traceability | Can a reviewer open each citation and identify the supporting passage? |
| Recency and fit | Do sources match the required date, jurisdiction, language, population, and software version? |
| Uncertainty and assumptions | Does the answer clearly separate facts, inferences, missing inputs, and unresolved conflicts? |
| Privacy and data handling | What sensitive information is sent, retained, shared, or used for improvement, and what controls apply? |
| Bias and perspective | Could the source selection or framing omit affected groups, opposing evidence, or relevant context? |
| Human review | Is there an accountable person with the expertise and authority to approve or reject the result? |
| Risk fit | Are testing, logging, access controls, appeals, and escalation proportional to the consequences? |
If one answer contains fewer claims but better-supported ones, it is usually the safer starting point. A longer response is not more reliable simply because it includes more detail.

Protect privacy while you verify
Fact-checking does not require exposing someone’s full identity, account details, medical history, legal file, or confidential business material. Before pasting information into a model, remove unnecessary personal identifiers and replace them with neutral labels. Keep the original document in an approved system, and provide only the excerpt or fields needed for the task.
Record what was shared and who can access the conversation when the work involves confidential information. A citation check that creates a new privacy breach is not a successful control. Your organization’s retention, security, and contractual requirements still apply even when the model is used only for drafting.
Disclose AI assistance when the rules require it
Disclosure depends on the setting. A school, employer, publisher, client contract, regulator, or professional body may require you to identify AI assistance, preserve prompts or logs, or obtain permission before using generated material. Follow the strictest applicable rule rather than assuming that editing removes the obligation.
When disclosure is required, be precise: identify what the system helped with, what a person verified or rewrote, and which sources support the factual content. Keep the review record when it may be needed to explain how a document was produced. Disclosure does not substitute for verification, and verification does not cancel a disclosure requirement.

Common failure modes and practical fixes
| Failure mode | Why it fails | Fix |
|---|---|---|
| Accepting a fluent paragraph as fact | Fluency and confidence are not evidence. | Break the paragraph into claims and verify each material one. |
| Trusting a citation without opening it | The reference may be invented, outdated, or irrelevant to the sentence. | Open it, read the supporting passage, and record scope and date. |
| Using a second AI as the fact-checker | Both systems can repeat the same source error or invented detail. | Check the underlying primary source or use a qualified reviewer. |
| Leaving date or jurisdiction implicit | The answer silently selects an interpretation that may not apply. | State the date, location, governing authority, and version; ask before proceeding. |
| Forcing an answer after the model signals uncertainty | Pressure to complete the response encourages unsupported guesses. | Accept the uncertainty and gather the missing evidence. |
| Publishing an unreviewed high-stakes draft | An error can cause legal, medical, financial, safety, or reputational harm. | Use the model for preparation only and require accountable expert approval. |
| Deleting the audit trail | Later reviewers cannot tell what was generated, checked, or changed. | Retain the model/date record, sources, unresolved issues, and human edits where appropriate. |
A compact decision rule
Before you rely on an AI answer, ask:
- What specific decision or publication will this support?
- What facts could change the result?
- Has the model stated its assumptions and uncertainty?
- Can every material claim be traced to an applicable source?
- Has an independent source or qualified person checked the important claims?
- Is the privacy, disclosure, and record-keeping treatment acceptable?
- What is the recovery plan if the answer is wrong?
If you cannot answer those questions, keep the output in draft status. The safest response to an unknown is not a more elaborate guess; it is a clear boundary around what is verified, a request for the missing context, and a practical path to obtain reliable evidence.
