Chatbots can produce fluent, detailed answers that contain wrong facts, invented quotations, fabricated studies or citations, and outdated technical details. Before you rely on an answer, identify the claims that matter, open the underlying sources, and check that those sources support the exact wording, number, date and context.
This verification habit is faster and safer than trying to judge truth from a chatbot’s confidence. The workflow below shows what to check, how to ask for better evidence, when to qualify or discard an answer, and why provenance signals cannot substitute for fact-checking.

Why a confident chatbot answer can be wrong
Large language models generate likely sequences of words. They are optimized to produce useful-looking responses, not to guarantee that every sentence is true. When information is missing, ambiguous or outside the model’s reliable knowledge, a system may still complete the answer instead of stopping. OpenAI describes this persistent problem as hallucination; the National Institute of Standards and Technology (NIST) uses the term confabulation for false or misleading generated material.
An answer can therefore be grammatically polished, specific and apparently authoritative while being false. A citation icon, footnote or linked title does not change that. The cited item may not exist, may be misattributed, or may discuss a different claim from the one in the answer.

Common forms of incorrect output
- Wrong facts: an incorrect definition, date, name, specification or sequence of events.
- Invented sources: nonexistent papers, reports, court decisions, URLs or publishers.
- Distorted quotations: words attributed to a person or document that do not appear in the original.
- Misused statistics: a real number presented with the wrong population, date, geography or measurement.
- Outdated instructions: a menu label, command, API behavior or policy that has changed.
- Overconfident interpretation: a plausible conclusion presented as settled when the question is ambiguous or evidence is mixed.
OpenAI’s Help Center advice is concise: use ChatGPT as a first draft, not a final source, and always verify quotes, data, technical information and references to external documents. The same principle applies to every consumer chatbot.
Decide what must be checked
Do not spend equal effort on every sentence. Separate creative suggestions and opinions from factual claims that could change a decision, cause harm or be repeated as true.
| Claim type | Why it deserves checking | What to verify |
|---|---|---|
| Dates, prices, laws and policies | They change and are often region-specific. | Publication or effective date, jurisdiction, edition and current status. |
| Statistics and measurements | A real figure can be misleading outside its original context. | Population, sample, method, unit, time period and the publisher’s definition. |
| Quotations | Language models can invent or subtly alter wording. | The exact words in the original speech, transcript, book or document. |
| Technical instructions | One wrong flag or version assumption can break a system. | Official documentation for the exact product, version and operating environment. |
| Medical, legal, financial or safety advice | An error can have serious consequences. | Authoritative guidance and, when appropriate, a qualified human professional. |
| Claims about external studies or reports | Fabricated references can look credible. | Whether the work exists, who published it and whether it supports the precise claim. |
For low-stakes brainstorming, fiction or formatting, verification may be unnecessary. For a consequential decision or published statement, treat every material factual assertion as a checkpoint.

A step-by-step workflow for fact-checking an AI answer
- Extract the decision-relevant claims. Copy the sentences that affect what you will do, buy, publish, configure or believe. Break compound sentences into individual claims. A sentence saying that a feature launched in a certain year and is available in a particular country contains at least two claims.
- Ask for evidence, but do not treat the response as proof. Request the source title, publisher, publication date, URL and the passage supporting each claim. ChatGPT may offer web search or other tools depending on the plan, and tool availability varies. A source list is a lead for checking, not a substitute for checking.
- Open every important citation yourself. Confirm that the page, paper or document actually exists and is attributable to a credible publisher. Do not rely on a citation that appears only in the chatbot’s text, and do not assume a plausible-looking URL is genuine.
- Match the source to the exact sentence. Read the relevant passage in context. A source can be authentic yet fail to support the chatbot’s wording. Check whether the source states a fact, reports someone’s opinion, describes a limited experiment or merely mentions the subject.
- Preserve scope and qualifiers. Record the source’s population, geography, date, edition, sample, units and uncertainty. If a study measured one group in one year, do not rewrite its result as a universal or current rate.
- Verify quotations word for word. Search the original text or transcript, then compare spelling, punctuation and surrounding context. If the exact wording cannot be found, remove quotation marks and describe the idea only if another reliable source supports it.
- Check volatile facts against current primary sources. For software behavior, regulations, product specifications, schedules and prices, prefer the responsible organization’s current documentation or notice. A model’s training knowledge may be old, and a previously correct answer can become wrong after an update.
- Seek independent confirmation when stakes are high. Use another authoritative source or ask a qualified expert to review the point. This is not a requirement to collect a fixed number of links; there is no universal source count that guarantees accuracy. The goal is adequate, relevant support for the particular claim.
- Resolve contradictions explicitly. If credible sources disagree, identify the date, jurisdiction, definitions or methods that explain the difference. State the disagreement instead of selecting the answer that sounds most certain.
- Qualify, omit or escalate unsupported claims. If you cannot establish a consequential statement, do not fill the gap with a guess. Mark it as uncertain, remove it, or obtain advice from an appropriate human professional.
How to ask a chatbot for answers that are easier to check
Prompting cannot make a model infallible, but it can expose assumptions and produce a more useful audit trail.
Require claim-level sourcing
Ask: List each factual claim separately. For every claim, provide the original publisher, document title, publication date and a direct link. Quote the supporting passage and say when the source does not support the claim. This format makes gaps visible instead of burying them in a long bibliography.

Force uncertainty into the open
Ask the model to label statements as established, likely, disputed or unknown, and to list the assumptions behind its conclusion. Request that it say I don’t know when it cannot identify a reliable source. These labels are signals for your review, not guarantees.
Set the time and geography
Include the country or jurisdiction, relevant date, product version, operating system and edition. Then ask the chatbot to identify which facts may have changed since that date. A question such as What is the current cancellation rule? is incomplete without a service, region and effective date.
Ask for a verification plan rather than a verdict
For a complex decision, request the key variables, primary sources to consult, calculations to reproduce and conditions that would change the recommendation. A plan is easier to test than an unsupported yes-or-no answer.
Use a second pass as an auditor
After drafting, provide the answer back to the chatbot with the instruction: Find unsupported claims, invented citations, scope errors, outdated details and ambiguities. Do not defend the draft; report problems and explain what evidence is needed. Treat this as an error-finding aid, not independent validation, because the same model can repeat its original mistake.
How to evaluate a citation
Existence and attribution
Open the link or search the publisher’s site. Check the title, author, organization and date. A citation to a real publisher can still be fabricated if the cited article or quotation does not exist.

Authority for the specific question
Use the source best positioned to answer the claim: an official manual for a command, a regulator for a rule, an original study for its results, or a standards body for a specification. A secondary summary may be useful for orientation but should not silently replace the primary document when precision matters.
Coverage and wording
Highlight the exact sentence in the source that supports the answer. Watch for stronger language in the chatbot’s version: may becoming will, a correlation becoming a cause, or a limited finding becoming a general rule.
Context and limitations
Read the paragraphs before and after the cited passage. Look for exclusions, confidence intervals, sample limitations, version numbers, licensing terms and definitions. Preserve those qualifications in your own notes and writing.
Handling quotations, figures and technical details
Quotations
Search the original speech, transcript, book or filing. Verify the speaker, date and setting, and make sure an excerpt has not been clipped to reverse its meaning. If the primary text is unavailable, paraphrase cautiously or omit the quotation.
Statistics
Trace a number to its original publisher. Record who or what was measured, when, where, how the sample was selected, and what the unit means. Do not present a model-specific benchmark or a narrow evaluation result as a general accuracy rate for all chatbots. No broadly applicable chatbot accuracy or hallucination percentage is established here.

Commands and specifications
Test instructions in a safe environment and consult documentation for the exact release. Confirm required permissions, dependencies, default behavior and rollback steps. A command copied from an old version can fail silently or produce a different result.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why chatbots make up sources
A language model predicts text that resembles patterns in its training and conversation context. It does not automatically query a catalog of real documents every time it writes a citation. When prompted for a reference, it can assemble a plausible title, author and link from partial patterns. OpenAI’s explanation of hallucination connects this behavior with systems being rewarded for guessing rather than acknowledging uncertainty.
NIST warns that fabricated citations can make readers trust a response inappropriately. The practical response is simple: verify that the source exists and then verify that it supports the sentence. Presence of a citation is not evidence of correctness.
Provenance is not fact-checking
Content-provenance tools answer a different question: whether supported signals about an image or audio file indicate how it was produced or handled. OpenAI’s documentation for its verification tools states that a detected signal does not establish that content is accurate or presented in the correct context.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesTherefore, a provenance mark, watermark or origin signal cannot validate a chatbot’s factual answer. You still need to inspect sources, compare claims and preserve context. Conversely, the absence of a provenance signal does not prove that text is false; it only means that the available origin evidence is missing or inconclusive.
Best Value
When to stop using the answer
- The cited source cannot be found or the link resolves to an unrelated page.
- The source exists but does not contain the quoted words, number or conclusion.
- The answer depends on an unstated country, date, version or definition.
- Authoritative sources disagree and the chatbot does not acknowledge the dispute.
- The claim could affect health, safety, legal rights, finances, employment or security and no qualified review is available.
- The model keeps inventing references after you ask it to correct them.
In these cases, replace the unsupported passage with a verified statement, clearly mark the uncertainty, or consult a suitable human expert. Do not use a confident rewrite as a cure for missing evidence.
A practical checklist before you rely on an AI answer
- Have I separated factual claims from suggestions or creative text?
- Which claims could change my decision or cause harm?
- Does each important citation exist and identify a credible publisher?
- Does the source support the exact claim, not merely the general topic?
- Did I verify quotations against the original wording?
- Did I preserve the statistic’s population, date, units and limitations?
- Did I check current documentation for facts that may have changed?
- Did I specify the relevant region, edition, version or jurisdiction?
- Have I mistaken a provenance signal for proof of factual accuracy?
- What will I do if the claim remains unsupported?
What a reliable workflow looks like in practice
Suppose a chatbot says a software feature was introduced in a particular release and cites a documentation page. First, copy the feature, release and availability claims separately. Open the documentation, confirm the page is genuine, and find the version history or release note. Check whether the feature is limited to a plan, operating system or region. If the page is old, look for the current manual. If the source does not mention the claimed behavior, remove it or report that the behavior could not be confirmed. This sequence is more dependable than asking the chatbot to repeat the answer with greater confidence.
For a reported statistic, locate the original study or dataset, then retain its sample and date in your wording. If the chatbot says a study proves a causal relationship but the source reports only an association, correct the claim rather than copying the stronger language. For a quotation, require the original transcript and use the exact wording or omit the quote.

The bottom line for everyday use
AI chatbots are useful for brainstorming, explaining concepts, outlining research questions and producing a first draft. They are not self-authenticating reference works. The safe pattern is to identify consequential claims, inspect primary sources directly, verify exact quotes, figures and technical details, and state uncertainty when evidence is missing. A fluent answer is a work product to audit—not permission to stop checking.
