How to Use a Toilet Footstool Position for Easier Bowel Movements

To use a toilet footstool, sit normally, place both feet on a low, stable stool, and let your knees rise while your feet remain supported. Settle into a comfortable position, relax your pelvic and abdominal muscles, allow enough unhurried time, and avoid forceful straining. The stool is optional: it may make the posture more comfortable for some people, but it is not a proven cure for constipation.

Stop or change the setup if it worsens your balance, causes pain, or makes you feel unsafe. Persistent constipation or warning symptoms require medical advice rather than reliance on posture alone.

What the toilet footstool position does

A toilet footstool raises your feet while you remain seated. That changes the angle between your trunk, pelvis and thighs toward a more crouched or squatting posture without requiring you to squat on the toilet itself. The practical aim is comfort and easier positioning, not a guaranteed medical effect.

Keep both feet supported and your weight securely on the toilet seat. The stool should not turn the toilet into an unstable perch, and you should never stand or squat on the seat. If you cannot transfer safely, have poor balance, or have a condition that makes bending difficult, ask a clinician or occupational therapist whether this arrangement is appropriate.

Some people like the feeling of having their knees higher because it resembles a squatting posture. Others notice no difference, or feel more cramped when their knees rise. Both responses are reasonable. A footstool is an optional positioning aid, not a test you must pass and not a substitute for evaluation of constipation.

How to use a toilet footstool: a safe sequence

  1. Choose a stable surface. Use a footstool that does not rock, slide or tip on your bathroom floor. A broad, non-slip base is more important than a particular brand or shape.
  2. Set it in front of the toilet before sitting. Leave enough room to place both feet fully on the platform. Do not place only your toes on the edge.
  3. Check the floor. Move loose rugs, wipe up water and make sure a bath mat cannot slide under the stool. A stool that shifts when you press down with one foot is not safe to use.
  4. Sit as usual. Lower yourself onto the toilet seat and keep your weight centered. Do not try to balance in a squat on the seat.
  5. Place both feet on the stool. Raise your feet so your knees are higher than they are when your feet rest on the floor. Adjust the stool until the position feels supported rather than cramped.
  6. Keep your legs supported. Your feet should rest flat or securely on the platform, with no need to reach, twist or balance on an edge. If one foot cannot fit comfortably, use the floor instead.
  7. Relax instead of pushing hard. Let your pelvic and abdominal muscles soften. Breathe normally and allow the bowel to empty without repeated, forceful bearing down.
  8. Give yourself enough time and privacy. NICE bowel-habit guidance emphasizes a comfortable, private environment, sufficient time and techniques that avoid straining. Rushing can encourage forceful pushing.
  9. Respond to discomfort immediately. Lower your feet to the floor or remove the stool if you feel dizzy, unstable, numb, short of breath or painful. A posture aid should not make you less safe.
  10. Finish without prolonged sitting. When you are done, return your feet to the floor, stand carefully and clean the stool if necessary. Remaining on the toilet for a long time is not required for the method to work.

A quick safety check before each use

Before settling in, confirm that the stool is directly in front of you, both feet can reach it, and you have a clear route to stand. The platform should not catch on the toilet base or bathroom fixtures. Test it while seated with light pressure before committing your weight. If you need to hold a wall, towel rail or unstable cabinet to rise, seek a properly installed grab bar or professional equipment advice instead.

Finding a comfortable height and arrangement

There is no evidence-based universal stool height. The right setup is the lowest, most stable support that raises your knees comfortably without forcing your hips, knees or ankles into pain. A stool that is too low may make little noticeable difference; one that is too high can crowd your abdomen, make standing harder or reduce balance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Arrangement What it offers Important limitation
No stool Simple, familiar and usually the most stable option. You do not get the raised-knee posture; that is not automatically a problem.
Low, stable stool Raises both feet and may make a seated, semi-squatting posture more comfortable. Comfort varies; it has not been shown to improve constipation outcomes for everyone.
Higher stool Raises the knees further for some body shapes. Can make balance, hip flexion and standing more difficult. Do not assume higher is better.
7-inch or 9-inch stool used in a trial Provides two tested examples for comparison. The 2023 trial found no meaningful difference between those heights in its measured outcomes, so neither is a universal recommendation.

Check the floor for water, bath mats or loose rugs that could slide. If the stool moves when you press down with one foot, replace it or use no stool. People with limited leg strength may need a grab bar, a raised seat or professional equipment assessment; a footstool alone does not solve transfer risk.

How to decide whether the fit is right

After placing both feet on the platform, ask whether your knees are comfortably raised, your abdomen feels unrestricted, and you can breathe and relax normally. You should be able to remove your feet without a sudden loss of balance. A comfortable arrangement can be repeated; an arrangement that causes hip, knee, ankle or back pain should be abandoned rather than tolerated in hopes of a stronger effect.

Using no stool is also a legitimate choice. Guidance that mentions sitting or squatting does not require every person to buy equipment, and the direct trial did not establish a superior commercial height. Compare arrangements by safety and comfort first, not by how extreme the knee angle appears.

What the clinical evidence actually shows

The randomized footstool study

Trieu and colleagues reported a randomized study in 2023 involving 41 patients referred to a tertiary neurogastroenterology unit. Ninety-three percent were female, and the mean age was 52 years with a standard deviation of 14 years. Each participant completed balloon-expulsion tests in randomized order with no stool, a 7-inch stool and a 9-inch stool.

The stools progressively narrowed the measured spine-to-femur angle, confirming that the equipment changed posture. However, the study found no improvement in balloon-expulsion time compared with no stool, no difference between the 7-inch and 9-inch stools, and no significant change in the measured subjective ratings. The authors concluded that the recommendation to use a stool during evacuation cannot be applied to all patients with constipation.

This was a small study in a referred specialist population, and balloon expulsion is a simulated clinical test rather than an ordinary bowel movement at home. Its result does not prove that a stool never feels helpful, nor does it prove that a stool works for people outside that study. It does show why claims that a particular height will cure constipation or work for everyone go beyond the evidence.

What the study does and does not answer

  • It shows that footstools alter seated posture.
  • It does not establish a medically superior height.
  • It does not demonstrate faster or easier everyday bowel movements for the general population.
  • It does not demonstrate that one stool height is preferable to another.
  • It does not test whether a stool prevents hemorrhoids, treats a diagnosed bowel disorder or replaces medical treatment.
  • It does not tell you whether your personal sense of comfort will improve; that is something you can judge safely for yourself.

Comfort is different from treatment

A person may report that a raised-knee position feels easier even when a clinical measurement does not improve. Conversely, a posture can feel awkward without indicating a disease. Keep those observations separate from claims about treating constipation. If the stool helps you relax and does not compromise safety, you may continue it as a comfort measure. If it does nothing, there is no medical obligation to keep using it.

What professional guidance says about posture

NICE

NICE guidance on adult faecal incontinence recommends “encouraging people to adopt a sitting or squatting position where possible while emptying the bowel.” The page also emphasizes enough time, a comfortable and private toilet environment and methods that avoid straining. Because the guidance addresses faecal incontinence management rather than a footstool-specific constipation trial, use it as bowel-habit advice, not as proof that a commercial stool improves outcomes.

NIDDK

The U.S. National Institute of Diabetes and Digestive and Kidney Diseases says that during bowel training, people can “try to relax your muscles or put your feet on a footstool to make yourself more comfortable.” That wording presents the stool as an option for comfort. NIDDK separately discusses fiber, fluids when increasing fiber, physical activity, bowel training and clinician advice when self-care is not enough.

World Gastroenterology Organisation

The World Gastroenterology Organisation includes responding promptly to the urge to have a bowel movement and adopting a squatting position among proper toilet habits that may facilitate evacuation. Its material also covers fiber and clinical treatment options. A posture suggestion should therefore be treated as one habit among several, not as a stand-alone constipation treatment.

Indian consensus guidance

An Indian consensus statement describes an Indian-style squat as more physiological but also notes that published evidence supporting posture advantages is limited. Its posture discussion should be read with that qualification. It is not evidence that a Western toilet footstool has a universally beneficial height or outcome.

Pair posture with proven bowel-habit basics

If constipation is the concern, a footstool addresses only the position used on the toilet. NIDDK’s broader self-care advice includes several other areas to discuss and apply safely:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Fiber: Increase dietary fiber when appropriate. If you increase fiber, take fluids alongside it as advised, because adding fiber without adequate fluid can worsen discomfort for some people.
  • Fluids: Maintain the fluid intake recommended for your health situation. People with heart, kidney or other conditions that restrict fluids should follow their clinician’s limits rather than copying general advice.
  • Physical activity: Regular movement can support bowel function when it is safe for you. Choose activity appropriate to your mobility and medical conditions.
  • Bowel training: Use a predictable, unhurried routine and allow enough time. Responding to the urge rather than repeatedly delaying it is part of the toilet-habit guidance described by the World Gastroenterology Organisation.
  • Medicines: Review medicines that may contribute to constipation with a clinician or pharmacist. Do not stop a prescribed medicine on your own; NIDDK advises discussing medication changes with a health professional.

These measures are not interchangeable. A stool may improve comfort while fiber, activity, medication review or treatment addresses a different contributor to constipation. If you are changing several habits at once, note what changes your symptoms so you can describe the pattern accurately to a clinician.

Troubleshooting common problems

The stool feels unstable

Remove it and use the floor until you have a safer solution. Check for wet flooring, a loose bath mat, a narrow base or a stool that flexes under weight. A non-slip platform and clear floor area matter more than an advertised height.

Your knees feel too high or your abdomen feels compressed

Try a lower support or no stool. The goal is a relaxed, supported posture, not the deepest possible squat. If hip, knee or back pain persists, stop and seek advice before trying another height.

You still need to strain

Do not respond by pushing harder or sitting longer. Recheck whether you are rushing, tense or delaying the urge, and consider the broader bowel-habit measures above. Ongoing difficulty warrants medical assessment rather than repeated changes in stool height.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

You feel dizzy or cannot stand safely

Lower your feet to the floor, pause before standing and remove the stool. Someone with falls risk, weakness or recent surgery may need a clinician’s equipment recommendation. Never use a stool that leaves you unable to rise securely.

The stool does nothing noticeable

That is a valid outcome. The randomized trial found no improvement in its simulated-expulsion measures, and comfort effects differ between people. You do not need to continue using the stool if it provides no benefit.

You are tempted to buy a taller model

Do not assume that a higher platform will produce a better result. In the 2023 comparison, 7-inch and 9-inch stools changed posture but did not differ meaningfully in expulsion time or subjective ratings. A taller model may also make transfers less secure.

When constipation needs medical care

Arrange medical advice when constipation persists despite reasonable self-care, keeps recurring, or interferes with daily life. A footstool should not delay evaluation of a medication effect, pelvic-floor problem, bowel obstruction or another condition.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

NIDDK advises seeking care for rectal bleeding or blood in the stool, constant abdominal pain, inability to pass gas, vomiting, fever, lower back pain, unexplained weight loss or other signs of a medical problem. These symptoms deserve prompt attention; severe or rapidly worsening symptoms may require urgent care. Do not attempt to manage them by changing toilet posture alone.

Questions to ask before buying or using one

  • Can I place both feet on it without twisting or reaching?
  • Does it stay firmly in place on my bathroom floor?
  • Can I stand and sit safely with it present?
  • Is the height comfortable for my hips, knees and ankles?
  • Can the surface be cleaned easily?
  • Would a grab bar, raised seat or professional assessment be safer for my mobility?

These questions focus on function and safety. The 7-inch and 9-inch heights tested by Trieu and colleagues should not be treated as purchase prescriptions, and no named height has been shown to be medically superior.

Frequently asked questions

How do I use a toilet footstool?

Place a stable stool in front of the toilet, sit normally, put both feet fully on it and let your knees rise. Keep your weight on the seat, relax your muscles, allow enough time and avoid forceful straining. Remove it if you feel unsafe or uncomfortable.

Does a toilet stool help you poop?

It may make the seated posture feel more comfortable for some people, but it is not a proven cure for constipation. A randomized 2023 study of 41 referred patients found no improvement in simulated balloon-expulsion time with either a 7-inch or 9-inch stool.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What is the best position to poop?

NICE advises a sitting or squatting position where possible, with privacy, enough time and techniques that avoid straining. On a Western toilet, a footstool is one optional way to raise the knees while remaining seated. The safest position is the one in which you can relax, stay balanced and stand securely.

Is a 7-inch or 9-inch footstool better?

No medically superior height has been established. The direct trial tested both heights and found no meaningful difference in expulsion time or measured subjective ratings. Choose based on stability and comfort rather than a promised therapeutic height.

Should I keep sitting until the bowel movement happens?

No. Guidance emphasizes sufficient, unhurried time, not prolonged sitting. Relax, avoid repeated forceful pushing and stop when you are uncomfortable. Persistent difficulty calls for attention to bowel habits and medical advice, not longer toilet sessions.

Can a footstool prevent hemorrhoids?

The trial did not test hemorrhoid prevention, and the available guidance does not establish that a footstool prevents hemorrhoids. Do not buy or use one on that promise. Treat it as an optional comfort aid and avoid straining.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Bottom line

Place both feet on a stable, low stool while seated, keep your knees comfortably raised, relax and avoid straining. Treat the footstool as an optional comfort aid, not a cure. Evidence from a small 2023 randomized trial did not show improved simulated evacuation with 7-inch or 9-inch stools, while bowel-habit guidance supports a comfortable sitting or squatting position when possible. If constipation persists or warning symptoms appear, seek medical care.
