An organization can turn near-miss reports into institutional memory by capturing the context of each event, classifying it consistently, assigning follow-up, and making related cases easy to retrieve. AI may help with indexing, suggested classifications, and finding similar reports—but it should support, not replace, human judgment. Evidence supports these as promising design choices, not as a proven way to improve safety or performance.

What a near-miss can—and cannot—tell you

A near-miss is a possible failure that did not result in the harm or loss it could have caused. That outcome matters: no harm occurred, but the event does not prove that the hazard was absent or that the controls were reliable. A barrier may have worked, a person may have intervened, or circumstances may simply have been favorable.

Near-misses can also create false reassurance. In a 2008 Management Science paper, Robin L. Dillon and Catherine H. Tinsley found that participants could rate managers whose decisions produced near-misses similarly to managers whose decisions produced successes, while distinguishing both from failures. They also reported that near-miss information could reduce perceived risk and lead participants toward riskier choices. These findings concern the authors’ studies; they are not a universal law about every organization. They do, however, illustrate why an uneventful outcome should not be treated as proof that the underlying risk has gone away.

Why collecting reports is not the same as learning

A report is useful only if it preserves enough context to help someone understand what happened and what should change. A short entry such as “equipment almost failed” may be searchable, but it cannot reliably explain the operating conditions, the control that was expected to work, what actually happened, or why the event matters elsewhere.

To convert an individual report into organizational knowledge, a learning process needs to connect the event to decisions and later action:

  1. Capture the event and its context. Record what was expected, what happened, where and when it happened, the relevant task or process, and the controls involved. Separate confirmed observations from interpretation.
  2. Classify consistently. Use defined categories for event types, contributing factors, controls, and outcomes. Allow narrative detail as well; a category alone can flatten important differences.
  3. Review and assign ownership. Have a responsible person validate the account, assess its significance, and assign any follow-up to an owner with a due date.
  4. Track the response. Preserve the decision, corrective or preventive action, its status, and how the organization checked whether the action was carried out.
  5. Make lessons retrievable. Let people find relevant prior cases by process, hazard, contributing factor, control, location, or other useful context—not just by the words used in the original report.

The point is not to accumulate the largest archive. It is to retain a trustworthy path from an observed event to an accountable response and a record that can inform later decisions.

What structured data and knowledge graphs can add

Categories make reports easier to compare, while links between records can reveal relationships that a list of isolated cases may hide. A 2023 Computers in Industry article by Francesco Simone, Silvia Maria Ansaldi, Patrizia Agnello, and Riccardo Patriarca describes constructing a knowledge graph from industrial near-miss reports and using it for exploratory analysis with refinery data. A graph can represent connections among events, contributing factors, controls, and other entities, making cross-case exploration a plausible application.

That study is not evidence that a knowledge graph—or generative AI—outperforms a conventional database, taxonomy, or skilled analyst, nor that it improves safety outcomes. It supports a narrower point: relationships in near-miss records can be represented and explored computationally. Whether that helps a particular organization depends on the quality of its records, the usefulness of its categories, and how people validate and act on what they find.

Where AI may fit—and where it should not decide

In a proposed near-miss memory system, AI is best treated as an assistant for work that is reviewable against the original record. Possible functions include:

  • suggesting tags or taxonomy categories for a reviewer to accept, edit, or reject;
  • indexing narrative reports so staff can search with ordinary language;
  • surfacing potentially related cases, controls, or contributing factors;
  • drafting a summary that keeps links to the source report and identifies uncertain or missing information.

These are design opportunities, not validated features or demonstrated results of the cited research. The evidence here does not establish that AI can reliably determine root cause, set severity, or choose corrective actions autonomously. Keep accountable people responsible for validating the event record, making risk judgments, and approving responses. A generated answer should never conceal the report it came from or present a plausible pattern as a confirmed cause.

Governance records that make AI-assisted memory auditable

NIST’s AI Risk Management Framework 1.0 is voluntary guidance organized around Govern, Map, Measure, and Manage. NIST describes the framework as being revised and reported release of a concept note for a critical-infrastructure profile on April 7, 2026. Its companion Playbook offers suggested practices; neither the framework nor the Playbook is a certification or guarantee of safety.

NIST’s Manage guidance is particularly relevant to an AI-supported incident-learning system. It recommends processes and documentation for tracking, responding to, and recovering from incidents and errors. Examples include records of reported errors, near-misses, incidents, and negative impacts; assessments and responses; system changes and version history; and how repairs were tested and deployed. Applied to this use case, a defensible record should preserve:

These safeguards are practical design recommendations drawn from governance needs. They should not be mistaken for proof that a system is safe, compliant, or effective.

What existing implementations and organizational studies show

A structured software example

DNV describes Aker BP’s introduction of Event Learning Taxonomy (CLUE) within Synergi Life for near-miss and incident case types. The stated aim was to improve consistency and quality in classification and trend analysis by using a taxonomy of contributing factors. This vendor-published case demonstrates an implementation of structured incident-learning software; it is not an independent evaluation, and it does not establish comparative outcomes or AI functionality.

Why context matters across organizations

A 2019 study by Arash Azadegan, Ravi Srinivasan, Constantin Blome, and Kayhan Tajeddini examined responses from 448 organizations in Germany, Switzerland, and Sweden in the context of supply-chain disruption response. The authors reported that near-miss exposure was associated with greater emphasis on procedural strategies and less emphasis on flexible strategies, with industry and regulatory pressures affecting the reported relationships. This finding belongs to that study’s sample and supply-chain context; it does not show that every organization responds to near-misses in the same way.

A 2020 CDC/NIOSH-hosted publication, “Learning from Workers’ Near-Miss Reports to Improve Organizational Management,” addresses worker reports in mining. Its documented subject makes it relevant to organizations considering worker reporting, but its detailed findings should not be inferred from the title alone.

Choosing an approach without assuming AI is the answer

Approach What it can support What the cited evidence establishes
Structured incident-learning platform Case intake, consistent taxonomies, assignment, and follow-up within a dedicated system. DNV’s vendor-published Aker BP case describes CLUE in Synergi Life for near-miss and incident cases; it does not independently establish outcomes or AI capabilities.
Knowledge graph or search layer over reports Connecting records, contributing factors, controls, and events to support cross-case discovery. The 2023 industrial study describes an exploratory knowledge-graph approach using refinery near-miss data; it does not prove superiority or improved outcomes.
AI-assisted retrieval over existing records Potentially helping users search, find related cases, or navigate narrative archives. No cited source establishes its effectiveness for institutional memory; performance must be evaluated in the organization’s own setting.

These approaches are not mutually exclusive, and the evidence does not support ranking them as tested alternatives. When assessing any implementation, examine whether staff can see the source report alongside generated output, correct errors, control access, preserve version history, and track follow-up. Privacy and access controls matter because near-miss records may contain sensitive operational or personal information.

How to tell whether the system is helping

Set evaluation measures before relying on the system. The following are useful operational indicators to consider, not validated benchmarks:

  • Record completeness: whether reports contain the context needed for review and comparison.
  • Ownership and follow-through: whether cases receive an accountable owner and retain a clear action status.
  • Retrieval success: whether users can locate relevant past cases and verify the result against source records.
  • Traceability: whether AI-assisted classifications and summaries can be followed back to their inputs, corrections, and system version.
  • Recurring conditions: whether similar events or conditions appear again after an action, interpreted with care rather than as a standalone measure of system success.

Near-miss counts alone cannot show whether organizational learning has improved: counts may change because reporting behavior or coverage changes. Interpret any trend alongside report quality, follow-up, and the conditions in which events occur. The central test is whether people can use a trustworthy record to make and track a better-informed decision—not whether software can produce a polished summary.
