Repeated sign-in or MFA prompts have two broad explanations: a security policy is deliberately requiring reauthentication, or the browser, app, device, or identity provider is losing the state that should keep a session alive. A prompt by itself does not prove a system defect.
Start with identity-provider logs before changing policy. Record exactly when and where the prompt appears, inspect the authentication and Conditional Access details, then test browser persistence, application SSO, device state, and session-policy interactions. This sequence separates an expected security challenge from a broken session or conflicting configuration without weakening access controls blindly.

What repeated sign-in prompts can indicate
Continuous authentication systems evaluate a session over time rather than treating the first login as permanent. They may ask for a password or MFA again when a sign-in-frequency rule expires, risk changes, a device becomes noncompliant, a password is reset, an account is disabled, or a session token is no longer accepted.

The same symptom—“I am asked to sign in again”—therefore needs evidence before remediation.

There is no reliable general statistic showing how often repeated prompts are caused by one category. Treat the behavior as a diagnostic pattern, not proof of a universal prevalence or a single fix.
Capture the pattern before changing anything
Ask the affected person to reproduce the issue once, if safe, and record the context. For a Microsoft Entra investigation, capture the correlation ID shown with the sign-in and use it to locate related events.
- User account and approximate UTC time of each prompt.
- Application, website, or resource being opened.
- Browser or native client, its profile, and the operating system.
- Whether the device is managed, Microsoft Entra joined, hybrid joined, or unmanaged.
- Whether the prompt follows a browser restart, application switch, sleep or reconnect, network change, or a regular interval.
- Whether a password reset, device-compliance change, account change, or risk alert occurred beforehand.
- Correlation ID, user, date, and resource filters used when searching the identity-provider log.
Compare an event that prompted with an event that did not. A repeated row is not automatically a repeated user action: Microsoft Entra logs can contain interactive and non-interactive requests for one authentication.

Read the identity-provider event first
Microsoft Entra sign-in logs
In Microsoft Entra, open the relevant sign-in event and inspect Authentication Details. It shows the authentication methods used, policy results, and information explaining why the request succeeded or failed. Open the Conditional Access tab to identify policies that applied, and check the session-lifetime information for the event.
Filter by correlation ID, user, date, and resource when locating the event, as recommended in Microsoft’s sign-in-log and Conditional Access troubleshooting guidance. Compare the details of adjacent requests instead of assuming every log row represents a separate prompt.

Continuous Access Evaluation events
For Microsoft Entra Continuous Access Evaluation (CAE), related requests can appear in both interactive and non-interactive views. CAE may be marked true on only one request in the sequence. Use the Is CAE Token filter, then inspect the session and policy details around the event. This prevents a legitimate token-evaluation request from being mistaken for a user repeatedly starting a new login.
What the log can establish
| Evidence in the event | What it helps determine | Next check |
|---|---|---|
| Authentication method and result | Whether a password, MFA method, token, or another method was requested and whether it succeeded | Compare with the user’s reported prompt and the next request in time |
| Conditional Access policy results | Which policy required a control or blocked the request | Review that policy’s session, device, risk, and sign-in-frequency settings |
| Session-lifetime information | Whether reauthentication or a session expiration was applied | Compare all applicable session policies, including legacy rules still in use |
| Device information | Whether the request came from the expected device state and client | Check join status, compliance, browser profile, and client-specific SSO |
Test whether browser state is being retained
A browser normally needs the identity provider’s session and device cookies to recognize an existing session. Microsoft Entra’s Stay signed in choice creates a persistent browser cookie. If a profile, privacy setting, cleanup policy, extension, or managed-browser rule removes that cookie, the next visit can look like a new sign-in.
Outdated Drivers Are Slowing You Down

A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11Okta documents a related case in which a missing device cookie causes the device to appear new and triggers another MFA challenge, even after a user selected a keep-signed-in option. This is an Okta-specific behavior example, not a universal rule for every identity provider.
- Record the browser name, profile, and whether the issue occurs in a private window. Private windows commonly discard state when closed, so compare them with a normal managed profile.
- Sign in once, close and reopen the same browser profile, and test the same resource. Note whether the prompt appears immediately or only after a longer interval.
- Review cookie and site-data controls for the identity provider and application. Check automatic “clear on exit” settings, enterprise cleanup policies, extension behavior, and profile synchronization.
- Repeat the test with extensions temporarily disabled only if your organization’s policy permits it. A difference identifies an extension or profile interaction, not a reason to leave security controls disabled.
- Check whether the same account persists in another approved browser profile on the same device. A profile-specific result points to local state rather than a tenant-wide session rule.
Do not assume that clearing cookies is a cure. Deleting cookies also removes the state that allows a session to persist. Use cache clearing only as a deliberate diagnostic test when corruption is suspected, record the result, and sign in again under controlled conditions. Microsoft’s Edge troubleshooting guidance also identifies stale or corrupted cached credentials, cookies, and authentication tokens as possible contributors.
Check application and device SSO
Separate application tokens
In a documented Microsoft Entra scenario, separate client applications hold separate OAuth refresh tokens instead of sharing one SSO state. Moving between those applications can therefore produce more than one MFA validation. If prompts occur only when switching applications, compare each client’s token and broker behavior rather than treating the browser session as the only suspect.

Joined and hybrid-joined devices
Microsoft Entra joined and hybrid-joined devices receive a Primary Refresh Token that supports SSO across applications. Verify the device’s join and compliance state in the event details and management tools. An unexpected transition to an unmanaged or noncompliant state can legitimately change the authentication path.
Mobile broker behavior
For affected mobile scenarios, Microsoft recommends the Authenticator app as a broker to other federated Entra applications to reduce prompts on the device. This is a Microsoft-platform recommendation; other identity providers and mobile clients use different broker and token arrangements.
Compare session and sign-in policies
Overlapping rules often explain a prompt that seems too frequent to an administrator. In Microsoft Entra, Microsoft states: “In Microsoft Entra ID, the most restrictive policy for session lifetime determines when the user needs to reauthenticate.” Therefore, inspect every applicable policy rather than changing the first rule you find.
Settings to compare
- Sign-in frequency: the interval after which an interactive sign-in can be required again.
- Persistent browser session: whether the browser is allowed to retain the session.
- Remember MFA: whether a previous MFA result can be reused under the configured conditions.
- Legacy session rules: older token-lifetime configurations that may still affect a tenant during migration.
- Application scope: whether one rule covers the resource while another rule covers a different client used in the same workflow.
Microsoft warns that some combinations, including Remember MFA and Stay signed in, can increase authentication requests rather than reduce them. Microsoft also says Conditional Access session management replaced configurable refresh-token and session-token lifetime policies; the retirement date for that feature was January 30, 2021. Confirm the current control in the tenant instead of relying on an old token-lifetime article or a setting that has been retired.
Azure Virtual Desktop special case
Microsoft documents a specific Azure Virtual Desktop failure pattern: the Azure Virtual Desktop application and the Windows Cloud Login application have different sign-in frequencies. One session can expire first and prompt during the user’s desktop experience. For that scenario, compare the two application policies and align their frequencies. Do not apply this exact remedy to unrelated applications without evidence that the same split-policy condition exists.

Decide whether the prompt is security-driven or a defect
Signs of an intended security reauthentication
- The sign-in log identifies a sign-in-frequency or session-lifetime policy.
- The prompt follows a password reset, account disablement, device-compliance change, or changed risk condition.
- The request appears when accessing a resource with stronger requirements than the previous resource.
- The same interval and policy result recur consistently across approved clients.
These cases call for a scoped policy review, not blanket suppression. Reauthentication can be the correct response to a changed account or device state.
Signs of lost session state or policy conflict
- The prompt appears immediately after every browser restart while a persistent session is expected.
- Only one browser profile or one extension set is affected.
- Multiple applications prompt separately despite a joined device that should provide SSO.
- Different applications show different session frequencies for the same user journey.
- The log shows no new user-initiated sign-in, but related interactive and non-interactive requests reveal token or cookie failure.
Verify the prompt’s origin before entering credentials or approving MFA. Users trained to approve every unexpected request can become vulnerable to phishing. Microsoft cautions that triggering reauthentication too frequently can create MFA fatigue and open the door to phishing.

- Document one complete occurrence. Capture the user, resource, client, device state, time, prompt wording, and correlation ID.
- Locate the identity-provider event. Filter by correlation ID, user, date, and resource. Compare interactive and non-interactive entries.
- Inspect authentication and Conditional Access details. Identify the method, policy result, session lifetime, device information, and CAE status where applicable.
- Reproduce without changing policy. Test after a browser restart, application switch, reconnect, and a controlled wait interval. Record which transition triggers the prompt.
- Test persistence. Check cookies, site-data cleanup, profile management, extensions, cached credentials, and tokens. Test an approved alternate profile only when permitted.
- Compare clients and devices. Determine whether the issue is browser-only, native-client-only, one application, multiple applications, one device, or every device.
- Review SSO and token paths. Check join state, Primary Refresh Token availability, application-specific refresh tokens, and mobile broker use in platform-specific scenarios.
- Reconcile policies. Compare sign-in frequency, persistent browser session, Remember MFA, risk, device, and application scope. Apply the most restrictive effective setting when evaluating Entra behavior.
- Change the smallest justified setting. If a policy is intentionally protecting a sensitive resource, retain it. If a cookie, profile, token, or conflicting policy is responsible, correct that component and retest.
- Validate and document. Repeat the original scenario, confirm the expected prompt interval, and retain the before-and-after event details for audit and rollback.
Use comparison axes to localize the fault
When several explanations remain plausible, compare the behavior along these axes:
| Axis | Useful comparison | What a difference suggests |
|---|---|---|
| Identity provider and session rules | Same user and resource under each provider or policy scope | Provider-specific policy or token behavior |
| Browser versus native client | Same account and resource in both approved clients | Browser cookies/profile or client token handling |
| One app versus multiple apps | Prompt frequency while moving between applications | Separate refresh tokens or app-specific policy |
| Managed/joined versus unmanaged device | Same user and resource on devices with different join states | Device trust, compliance, or SSO availability |
| Persistence across restart | Before and after closing the browser or client | Cookie, cache, profile, or token retention |
| Log claim or policy result | Prompting event versus a successful non-prompting event | Session lifetime, CAE, risk, or Conditional Access difference |
| Account or device-state change | Events before and after password, risk, or compliance changes | Legitimate security reauthentication |
Common mistakes to avoid
- Disabling MFA first: this removes a control before proving that MFA caused the recurrence.
- Clearing all cookies indiscriminately: it can erase the very session state being tested and make the symptom appear worse.
- Counting every log row as a prompt: interactive and non-interactive requests can belong to one authentication sequence.
- Assuming one product’s behavior is universal: Entra, Okta, browsers, native clients, and mobile brokers retain state differently.
- Aligning policies without checking scope: a broad change can weaken protection for resources that intentionally require more frequent reauthentication.
- Approving unexpected MFA requests: verify the originating application and event before responding.

Frequently asked questions
Does a repeated MFA prompt prove that the account is compromised?
No. It can reflect an intended session or risk policy, lost browser state, separate application tokens, or a policy conflict. Check the identity-provider event and verify the prompt’s origin before deciding whether it is an attack or a configuration issue.
Will turning on “Stay signed in” stop all prompts?
No. In Microsoft Entra it creates a persistent browser cookie, but sign-in-frequency, risk, device, application, or account-state requirements can still require reauthentication. Browser cleanup controls can also remove the cookie.
Why do I see several Microsoft Entra sign-in records for one login?
One authentication can generate interactive and non-interactive requests. For CAE, the CAE indicator may appear on only one request. Use the correlation ID and inspect the authentication, session, and policy details together rather than counting rows.
Should I buy a hardware security key to fix repeated prompts?
The documented troubleshooting evidence points to logs, browser state, application tokens, device SSO, and policy configuration. An authentication method such as FIDO2 may appear in a log, but that does not establish that purchasing a physical key will resolve repeated prompts.
Frequently Asked Questions
How often should sign-in frequency be set?
There is no universal interval. Set it according to the resource’s risk, device requirements, and organizational policy, then verify the resulting prompt pattern in sign-in logs.
What if prompts stop after disabling a browser extension?
Treat that as evidence of an extension or profile interaction. Remove or reconfigure the extension through approved administration, retest with normal security controls, and document the identity-provider event before making a permanent change.
