Copilot Notebooks can miss a file or produce an unreliable answer for reasons that have little to do with the language model itself. The usual causes are notebook scope, your permission to open the source, unsupported or incorrectly added content, reference-selection limits, stale versions, and differences between Microsoft 365 Copilot and Copilot Chat experiences. Work through those checks in order, then verify every important claim against the cited material.
What Copilot Notebooks actually uses
A Copilot Notebook answer is grounded in the references available to that notebook and to the person asking the question. It is not an automatic search of every file, email, Teams chat or web page that your account could theoretically reach. Microsoft Support describes notebook references as the content selected for that notebook; content merely existing somewhere in a shared site or drive is not guaranteed to be used.

Even an attached item may not influence a particular response. Copilot can select only a subset of available references for a question or generated artifact. A file can therefore appear in the References list while being absent from the evidence used for one answer.

Fast triage: identify the failure before changing anything
Use this order. It separates a missing-source problem from an answer-quality problem and avoids changing several variables at once.
- Scope: Is the expected file, page, link or location listed in the notebook’s References area?
- Access: Can the same account that is asking the question open the item directly?
- Type and method: Is it a supported source, and was it added through the appropriate upload, search, link or OneDrive option?
- Selection: Could the item be outside the applicable reference limit or lose a relevance ranking to other files?
- Prompt: Did you name the exact source and request a citation or section?
- Validation: Do the cited passages actually support each important statement?
- Account and rollout: Are licensing, administrator settings or a gradual feature rollout changing what this account can use?
Ask the same question again only after recording what changed. That makes it possible to tell whether the issue was scope, permissions, selection or interpretation.
Step 1: confirm the notebook scope
Inspect References, not your general file list
Open the notebook and inspect its References list. Look for the exact filename, page title, URL or location you expect. A document visible in OneDrive, SharePoint or another shared location is not automatically a notebook reference. Likewise, a file mentioned in a chat is not necessarily available to the notebook.

Depending on the experience and source, Microsoft documents upload, search, link and OneDrive methods. Choose the method that points to the actual item rather than assuming a broad folder or site reference will include every descendant file.
Add the exact file when its inclusion matters
For a decision based on one contract, report, spreadsheet or policy, add that individual file directly. This removes ambiguity caused by relevance ranking and makes later citation checking easier. If the source has several versions, add the current version and identify its date in the prompt.
Understand inherited locations
A folder, site or other shared location can expose many accessible files without forcing Copilot to use all of them. Microsoft’s “How Microsoft Copilot Notebooks works” guidance says that up to 300 of the most relevant accessible files may be selected from a shared location. Relevance is determined for the particular request, so an expected file can be left out even when you can open it and the location is listed.

Step 2: test permissions as the asking user
Notebook grounding is limited to content the user has permission to access. Open the source directly in the same browser session and account used to ask Copilot. Confirm that you can read the complete item, not merely see its name in a folder.
- Sign in with the account shown in the Copilot experience.
- Open the file, page or link directly.
- Check that the content loads without a request-access dialog, expired sharing link or sign-in loop.
- For a shared notebook, test each linked source while signed in as the recipient, not as the notebook owner.
In a shared notebook, recipients may need separate permission to linked files. Sharing the notebook does not necessarily share every underlying document. Ask the owner to grant access to the source, or add a version that the intended audience can open. Do not treat a filename displayed in the notebook as proof that everyone can read its contents.
Step 3: check source type and how it was added
Supported Microsoft content
Microsoft documents references such as Word, PowerPoint, Excel, PDF, Loop, Copilot Pages and OneNote pages. Availability can differ by product surface and account, so use the source options shown in your notebook rather than assuming every format behaves identically.
Web links have additional conditions
Web references must meet the applicable public-access or organizational-source requirements. An authenticated or paywalled external page may not work in the documented reference flow. A link to one page also does not guarantee that linked subpages, downloads or an entire site become references. If the material is important, place it in a supported file that the notebook’s users can open, or add each required page through an available supported method.
Verify the item after adding it
After adding a source, return to References and confirm that it appears as the expected title or file. Open it from the notebook if that option is available. A failed import, an old link or a similarly named file can otherwise look like a successful addition.

Step 4: account for reference limits and relevance selection
Limits depend on the notebook experience. Microsoft Support documents the following figures for 2026; they are product limits, not accuracy or performance scores, and Microsoft can change them.

Count references in the notebook and identify whether they are individual files or inherited through a location. If a large collection is unavoidable, ask about a narrow subject and name the expected file. A smaller, focused reference set gives Copilot fewer opportunities to blend unrelated versions or topics.
Step 5: make the prompt identify the evidence
A vague question invites broad selection and synthesis. State the source, the task and the level of evidence you require.
- Name the exact file or page title: “Use the 2026 benefits policy PDF titled ‘…’.”
- Limit the source when appropriate: “Use this file only; do not infer a rule from other references.”
- Ask for a location: “Give the page, heading or quoted passage supporting each requirement.”
- Specify the version or date when multiple editions exist.
- Separate extraction from interpretation: first ask for the stated figures, then ask what they imply.
Microsoft’s grounding guidance recommends adding a file or link and using an instruction such as “Use this file only.” It also cautions that pointing Copilot at an old version can produce an outdated answer. If the prompt names a file that is not in References, add it first; a title in the question does not grant access or force retrieval.

Step 6: audit the answer against its citations
For consequential work, validation is part of the task. Microsoft’s validation guidance says: “If a statement can’t be traced back to a source, treat it as unconfirmed until verified.”
- Open every citation attached to an important claim.
- Find the exact sentence, table row or paragraph that supports it.
- Check numbers, dates, names, units, exceptions and conditions.
- Mark claims that combine information from several sources without saying so.
- Look for conclusions that go beyond what the source states.
- Check whether a qualification, dependency or exception was omitted.
- Ask Copilot to list unsupported statements and compare its response with the source, then perform the check yourself.
A citation proves that Copilot found a related passage; it does not prove that the generated sentence is a faithful reading of that passage. If a citation opens but does not contain the claimed fact, report the mismatch and rewrite the answer from the source.
Why an answer can be inaccurate even when the source is present
The expected file was not selected
Reference presence and answer grounding are different states. Ranking, the 300-file shared-location selection, or the applicable notebook limit can leave a present file unused. Direct addition and a source-specific prompt address this cause.

Several documents were blended
When policies, drafts or project notes overlap, Copilot may merge compatible-looking statements. Ask it to identify which source supports each point and to flag conflicts instead of silently reconciling them.
The source is old
An older policy or previous spreadsheet can be readable and correctly cited while no longer describing the current situation. Add the current edition, state its effective date, and ask Copilot to ignore superseded versions unless you are intentionally comparing them.
The response overstates an implication
Language models can turn a conditional statement into a general rule, infer a cause that the document does not establish, or fill a gap with plausible wording. Require quotations or section references for decisions and treat uncited conclusions as unconfirmed.
A qualification was dropped
Limits, regional rules, eligibility conditions and exceptions are easy to lose in a short summary. Ask for a complete list of exceptions and compare it with the source rather than relying on a polished paragraph.

When someone else reports that “the notebook cannot find my file,” record three identities: the notebook owner, the person asking the question and the owner of the linked source. Their permissions and licenses may differ. A source selected successfully for one person may be inaccessible or unselected for another.
Have the affected user open the item directly, then inspect References while signed in to the affected account. If the source is available only to the owner, request access or add a shareable copy. Do not circulate sensitive material merely to solve a grounding problem; use the narrowest permission that lets the intended audience verify the answer.
Account, license and rollout checks
Notebooks and source options depend on license, subscription and administrator settings. Microsoft also notes that features can roll out gradually. If a References option, notebook surface or file type is missing, first confirm:
- Which account and tenant are active in the Copilot app or OneNote experience.
- Whether the organization’s administrator allows the feature and source type.
- Whether the user has the license associated with the notebook experience being tested.
- Whether colleagues are comparing different surfaces, such as Copilot Chat and Microsoft 365 Copilot.
- Whether the feature is still rolling out to that account.
Changing plans is not a guaranteed fix for a missing file or an inaccurate response. Licensing can affect availability, but it does not remove the need to add the correct reference, verify permissions and check citations.
Common symptoms and the most likely next action

A repeatable verification workflow for important decisions
For legal, financial, compliance, health, employment or operational decisions, use a two-pass process rather than accepting the first response.
- Prepare: remove obsolete references, add the exact current files and confirm that every participant can open them.
- Constrain: name the source, version and question; request citations and explicit uncertainty.
- Extract: ask for facts, figures, dates and exceptions before asking for a recommendation or summary.
- Cross-check: open the citations and compare each material claim with the source text.
- Resolve: if sources conflict, ask Copilot to present both statements with dates and owners instead of choosing silently.
- Record: keep the source version and the verified answer together so a later update can be detected.
This workflow does not make generated text infallible. It makes unsupported or stale content visible before it becomes a decision.
What not to assume
- A shared folder or site reference does not mean every file will be read.
- A visible reference does not prove it was used for this response.
- A citation does not prove that the generated wording matches the cited passage.
- Notebook sharing does not automatically grant access to linked files.
- Adding more references is not always better; unrelated or obsolete material can increase blending and selection problems.
- An upgrade or different subscription does not by itself correct permissions, stale content or unsupported conclusions.
When to stop troubleshooting and escalate
Contact your Microsoft 365 administrator or support channel when the same supported source opens for the user, appears in References, is within the applicable limit, and still cannot be selected or cited across repeated, source-specific prompts. Include the account and notebook surface, source type, access result, approximate reference count, exact prompt and citation behavior. Do not include confidential document contents unless your organization’s support process permits it.
If the issue is an answer rather than retrieval, preserve the source version and the generated response, identify the unsupported sentence and document the mismatch. That evidence distinguishes a permissions or rollout problem from normal limitations of generated language.
