Identify what is wrong with the answer
Read the response once and classify the failure before changing several things at once. The remedy differs depending on whether Copilot misunderstood the goal, lacked a document, used stale information or failed to access the service.

Why Copilot produces inaccurate or irrelevant answers
It cannot ground the answer in the information you expect
Copilot can give a more general response when it cannot find relevant, accessible material. A file may not be attached, may be stored somewhere your account cannot reach, or may be an older copy. Organizational permissions still apply: Copilot cannot use a company document that you are not allowed to open.

The request leaves too many decisions unstated
“Summarize this” does not tell Copilot which document, audience, length, date range or facts matter. A clear request gives it a narrower target and makes an incorrect result easier to diagnose.

The conversation contains stale assumptions
Long chats accumulate definitions, examples and constraints. If the current task conflicts with an earlier instruction, Copilot may continue following the older context. A new conversation removes those previous prompts.
Account and license context changes available work data
For work or school accounts, Copilot Chat without the relevant add-on relies on public web data. The add-on can enable web and authorized organizational work data for eligible users, but it does not bypass permissions or guarantee factual accuracy. A license check is therefore an access diagnostic, not a general cure for wrong answers.
Use a precise troubleshooting prompt
Start by naming the mismatch and the correction you need. Microsoft recommends giving Copilot the goal, context, source and expected response, then iterating on one defined element at a time.

- State the task. Say exactly what decision, summary, calculation or rewrite you need.
- Define the context. Include the audience, business or personal situation, date range, jurisdiction and any terminology Copilot must use.
- Identify the source. Attach the file or provide its exact name and location. For current facts, identify the latest page or document.
- Set boundaries. Say whether Copilot must use the named source only, may use other sources, or should distinguish sourced facts from assumptions.
- Specify the output. Request a table, numbered steps, quotations with section names, a word limit or a list of unresolved questions.
- Define freshness. Ask it to include the source’s publication or revision date and to flag information older than your cutoff.
For example:
You answered generally, but I need the current policy in the attached file. Use that file only. Quote the relevant section, identify its revision date, explain the rule in plain language, and list any question the file does not answer.
This wording is an example of a focused follow-up, not a guarantee that every response will be correct.
Get Copilot to use the correct file
Attach or identify the source explicitly
Attach the document in the conversation when the interface allows it. If the file is already in an organizational location, name it precisely enough to distinguish it from archived or similarly titled copies. Tell Copilot whether the file is the sole authority and which sections matter.
Check the file version and date
Open the source yourself. Confirm its revision or publication date, effective date and relevant section. An old website or earlier file version can produce an outdated answer even when Copilot cites it. Ask Copilot to show the date it used so you can spot a mismatch.
Separate source facts from interpretation
Ask for direct quotations or section references first, then request an explanation. If the quotation does not support the conclusion, treat the interpretation as ungrounded and revise the question rather than accepting a confident paraphrase.

When the file still cannot be used
- Confirm that you are signed in with the account that can open the file directly.
- Check that the file has finished uploading or syncing.
- Remove ambiguity by using the full file name and, where available, its location.
- Ask Copilot what source it actually used; a refusal or generic answer can indicate that the file was not accessible.
Check work or school account access before rewriting the prompt repeatedly
If the expected answer depends on company files, email, meetings or SharePoint content, verify the account context first. Sign in with the work or school account that owns the permissions, not a personal account. Then confirm that you can open the underlying item outside Copilot.
Available grounding varies by account type and license. Chat without the Copilot add-on may answer from public web data, while the add-on can permit web and authorized organizational data for eligible users. Neither option grants access to content restricted by organizational permissions, and neither removes the need to verify the result.
Do not purchase an add-on solely to correct a factual error. Consider licensing only when the specific problem is that an eligible user needs authorized organizational grounding that the current account does not provide.
Reset context and refine one issue at a time
Start a new chat
- Open Copilot Chat.
- Select New chat.
- Restate the task, source, date requirement and output format in the first message.
- Do not paste unrelated earlier exchanges into the new conversation.
A new chat is useful when the previous thread contains obsolete assumptions. It will not fix a missing permission, an unavailable file or a wrong source.
Ask a focused follow-up in the same chat
If only one element is wrong, identify it directly: “The answer used the 2024 policy; use the 2026 revision attached,” or “Give only the three eligibility conditions and quote each section.” Changing one variable makes it possible to tell whether the revision helped.
Use positive, ordered instructions
Tell Copilot what to do and in what order: “First identify the source and date. Next quote the applicable section. Finally explain the result in five bullets.” Positive instructions are generally clearer than a long list of prohibitions.

When a reply is still wrong, work through the same sequence instead of repeatedly adding random instructions:

- Identify the authority. Name the exact attachment, page, policy revision or date cutoff.
- Test access outside Copilot. Open the item with the same account and confirm that it is not an archived or restricted copy.
- Ask for evidence before prose. Request the source title, date and quoted section first.
- Constrain the format. Specify the number of items, headings, columns or word limit you need.
- Compare the second response with the first. If changing the source or date did not change the answer, the source may not be available or the question may still be ambiguous.
- Escalate the right symptom. Use Get Help for a missing icon or connection failure; report a reproducible app fault rather than treating it as a prompting problem.
This sequence also creates a useful record of what you tried if an administrator or Microsoft support team needs to investigate.

Verify every answer before relying on it
Grounding improves relevance and supplies citations, but it is not a guarantee. Microsoft Support states: “Grounding improves accuracy and relevance and gives you citations to verify, but you should still review the sources and confirm critical details before you share or act on them.”
- Open each cited source rather than relying on the citation label.
- Check that the source actually supports the sentence Copilot attached to it.
- Compare the source’s publication, revision or effective date with the date your task requires.
- Look for omitted conditions, exceptions, definitions and geographic or account limits.
- For high-impact decisions, confirm the detail in the authoritative document or with a qualified human.
If Copilot cannot provide a source for a consequential claim, treat the claim as unverified. Ask it to mark uncertainty and list what evidence is missing.
Distinguish answer quality from access and connectivity problems
When to use Get Help troubleshooters
Use the Microsoft Get Help app when the symptom is product access rather than reasoning:

- If Copilot will not connect, run the connectivity troubleshooter. It checks for firewall rules and other blockers.
These tools can restore access; they do not correct a factual error in a response that already loaded.
When to check permissions
If Copilot answers from public information when you expected internal material, test the same file with your signed-in account. An inaccessible item, wrong tenant or wrong account explains missing work context more often than prompt wording does.
Recover the updated personal Copilot app
For personal-account users of the updated Copilot app, Microsoft’s September 2026 guidance separates migration and sync issues from answer-quality issues.
- Confirm that the app or website uses the same account and sign-in method as before.
- Restart the app or refresh the page.
- Read and follow any migration notice shown in the app.
- Allow time for content migration and device synchronization to complete.
- If the problem persists, open the ellipsis menu, choose Send feedback, then Report a problem.
- Include when the issue began, your device, operating system and a screenshot.
App labels and migration behavior can change, so check the current Microsoft support instructions if your screen differs.
Practical examples of targeted corrections
Generic policy answer
Instead of “What is our travel policy?”, ask: “Using the attached 2026 travel policy only, list the reimbursement limits for rail, air and lodging. Quote the section heading, include the policy revision date and mark any category the document does not cover.”

Wrong version of a document
Say: “The answer cites the 2024 handbook. Ignore that version and use the attached 2026 handbook. Compare the two sections on parental leave and identify every changed number.”
Off-topic response after a long chat
Start a new chat and write: “My only task is to turn the attached meeting notes into five action items with owners and due dates. Do not summarize unrelated discussion.”
Current web information
Ask: “Use the latest official page available as of 17 October 2026. Give the publication date, link title and exact section for each claim. If the latest date cannot be established, say so.” Then open the cited pages yourself.
What not to do
- Do not keep adding vague instructions to the same confused conversation.
- Do not assume a more expensive license makes an answer true.
- Do not treat a citation as proof without opening and checking the source.
- Do not upload confidential material unless your organization permits it and the account is appropriate.
- Do not act on legal, financial, medical, security or compliance details until a current authoritative source confirms them.
When to report a problem
Report an issue when a reproducible product failure remains after you have confirmed the account, permissions, source and connection. Include the exact prompt, the response, the source or attachment used, timestamps, account type, device, operating system and screenshots. Remove confidential information that support staff do not need.
For a one-off wrong answer with usable citations, use the verification and prompt-refinement steps first. For a missing Copilot icon, failed connection or migration that never completes, use the matching troubleshooter or in-app report path.
