How to troubleshoot MCP server connection and permission errors

Most MCP failures are not interchangeable “permission” problems. First identify whether the client is launching a local process over stdio or calling a remote HTTP endpoint, then compare the client message with the server log. HTTP 421 usually points to host validation, 401 to a missing, malformed, or expired bearer token, 403 to missing scopes, and method errors to protocol or capability mismatch.

Use the workflow below in that order. It prevents changing OAuth settings when the request never passed an HTTP host check, and prevents changing network settings when transport already works and the peers disagree about a method or protocol revision.

Start by classifying the failure

Connection troubleshooting is fastest when you classify the layer that failed. An MCP client can fail before an MCP message is sent, while establishing HTTP transport, during authorization, or after the server has accepted the connection and begun protocol negotiation. The same generic client wording can cover very different causes.

What you see Layer to investigate First checks Likely direction
Generic error connecting to a remote server Transport or HTTP request Endpoint, HTTP reachability, transport type, server log Confirm Streamable HTTP or the appropriate older SSE transport, then find the server-side response.
HTTP 421 or “Invalid Host header” HTTP host validation Host header, allowlist, reverse proxy behavior Configure accepted hostnames deliberately; do not treat this as an OAuth failure.
HTTP 401 Bearer-token authentication Token presence, format, expiry, resource or audience Correct the token or complete the OAuth flow.
HTTP 403 or “insufficient scope” Authorization policy Granted scopes versus required scopes Request or grant the required scope; changing transport will not add permission.
Method not found or behavior differs by client/server version MCP protocol compatibility Negotiated revision and method support Use a method supported by both peers or align their protocol revisions.
Local process never starts Process launch Executable path, command, arguments, process output Fix the launcher environment. This is not evidence that the server denied a permission.

The status-code interpretation above follows the current SDK documentation checked on October 13, 2026. SDK behavior and protocol details can change, so apply the advice to the exact client, server, and SDK versions you run.

1. Identify the transport and endpoint before changing settings

Write down exactly how the client is supposed to reach the server. MCP deployments generally fall into two practical categories:

  • stdio: The client launches a local server process and exchanges messages through standard input and output. The command, arguments, executable search path, and process environment all matter.
  • Remote HTTP: The client connects to a URL. The TypeScript SDK documents Streamable HTTP as its remote transport and describes SSE as a fallback for older servers that expose only the older SSE transport.

Do not assume that a URL copied from an older configuration is still the right transport. A server that supports Streamable HTTP may expose a different path or connection sequence from an SSE-only server. Conversely, selecting an SSE mode for a server that expects Streamable HTTP can produce a generic transport error even when the host is reachable.

Record the effective configuration

Capture the command and every argument for stdio, or the complete endpoint URL and transport selection for HTTP. Also note whether a proxy, gateway, or service mesh sits between the client and server. A typo in a path, an unexpected URL rewrite, or a proxy that changes the Host header can look like a server-side permission problem.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • For stdio, verify the executable name, absolute path if one is configured, argument order, working directory, and environment variables.
  • For HTTP, verify the scheme, hostname, port, path, and whether the client is actually sending the transport expected by the server.
  • Record the client and server SDK versions and any configured protocol revision. Version information becomes essential if transport succeeds but a method fails later.

2. Read the server log beside the client error

A client often reports only “server returned an error response” or a generic connection failure. That message may be all the client exposes when the HTTP request is rejected before an MCP JSON-RPC message reaches the application. The server log usually contains the status code, rejected header, authentication challenge, or startup exception that identifies the layer.

Reproduce one failure while watching the server log, then correlate the timestamps. Look for the incoming Host value, HTTP status, authorization challenge, protocol-initialization messages, and process startup output. If no request appears at all, investigate DNS, routing, the endpoint, or the local launcher before changing server authorization.

The Python SDK troubleshooting documentation uses a host-validation case in which the log contains HTTP 421 and “Invalid Host header” while the client sees a less useful transport error. That distinction matters: the peer may be reachable, but the web server has rejected the request before MCP negotiation.

3. Fix HTTP 421 “Invalid Host header” failures

HTTP 421, often described as “Misdirected Request,” and the text “Invalid Host header” point to host validation in the documented Python SDK deployment scenario. The server is checking the hostname in the request against an allowed set and rejecting it. This is an HTTP deployment check, not proof that a bearer token is wrong.

Inspect the hostname that actually arrives

Compare the URL the client uses with the Host header recorded by the server or reverse proxy. They can differ when a proxy terminates TLS, rewrites domains, routes by an internal service name, or forwards a different host value. Check both sides of the proxy: the external hostname presented by the client and the value forwarded to the application.

Configure an intentional allowlist

Configure the server’s host allowlist for the hostname it is meant to serve. Include each legitimate deployment name that clients use, such as the public DNS name and any deliberately supported internal name. Follow the host-validation and deployment guidance for the SDK version in use; the exact setting and syntax are SDK-specific.

Binding a server to a non-localhost interface does not, by itself, define a safe host allowlist. A bind address controls where the process listens, while host validation controls which Host values it accepts. Treat them as separate settings.

Check reverse-proxy behavior

  • Confirm that the proxy forwards the intended Host value instead of replacing it with an internal upstream name.
  • Check whether TLS termination causes the proxy to route by one hostname while forwarding another.
  • Ensure the proxy sends requests to the same path and transport endpoint that the MCP server exposes.
  • After changing the allowlist or proxy rule, restart or reload the component that owns that setting, then reproduce one request and read the new log entry.

The MCP Python SDK troubleshooting documentation summarizes the operational priority this way: “A freshly deployed server that refuses every connection is a Host allowlist until proven otherwise.” Apply that advice to the documented case, while still checking the actual Host value and proxy path rather than blindly permitting every hostname.

4. Diagnose HTTP 401 authentication failures

In the TypeScript SDK authorization guidance, HTTP 401 means the request lacks a usable bearer token. The documented causes include a missing token, a malformed token, or an expired token. Start with the authentication challenge and the token that the client actually sent; do not infer token state from a cached login screen.

Check token presence and formatting

  • Verify that the client sends an Authorization: Bearer credential to the MCP endpoint that requires it.
  • Check for truncation, accidental quotation marks, whitespace, or a token copied into the wrong configuration field.
  • Confirm the token has not expired. A previously successful connection can begin returning 401 when the access token lifetime ends.
  • Make sure the client is sending the token to the intended endpoint, not to a different host or path after a redirect or configuration change.

Validate resource and audience expectations

An otherwise valid token can be intended for a different protected resource. Compare the resource or audience expected by the MCP server with the value for which the client obtained the token. The authorization documentation also points clients to protected-resource metadata. Use that metadata endpoint to discover the server’s authorization requirements rather than guessing an issuer or resource value.

Complete the OAuth flow when the client supports it

If the client is designed to handle OAuth, confirm that an OAuth provider is configured and that the authorization flow was completed before reconnecting. A login at an unrelated service does not necessarily produce a token for this MCP resource. When a flow has been interrupted, clear the failed authorization state according to the client’s normal procedure and authorize the requested resource again.

Do not “fix” a 401 by weakening host validation or switching transports. A 401 is an authorization response generated after the HTTP request reached the protected endpoint; solve the credential, resource, or OAuth configuration first.

5. Diagnose HTTP 403 and insufficient-scope responses

HTTP 403 is different from 401. The TypeScript SDK authorization documentation describes 403 as a token that is accepted but lacks one or more scopes required by the server. In other words, the client is authenticated, but the authorization policy does not allow the requested operation.

Compare granted and required scopes

Read the server’s required scopes from its authorization documentation, protected-resource metadata, or the explicit error response. Compare them with the scopes recorded in the access token or the scopes granted during OAuth. Pay attention to spelling, case, and scope separators; a nearly matching scope is still a different scope.

Request the missing permission

Adjust the client’s requested scope set or the authorization-provider configuration, then obtain a new access token. Updating a configuration file without refreshing an already-issued token will leave the client presenting the old scope set and receiving the same 403.

Keep the distinction clear: 401 asks “is there a valid credential for this resource?” while 403 asks “does that credential include the permission required for this request?” Neither status indicates that the peers use different MCP protocol revisions.

6. Separate protocol and method errors from authorization

When the server accepts the transport and authentication but returns “method not found,” or when behavior changes between client and server versions, investigate MCP protocol compatibility. Confirm the protocol revision each peer supports and the revision negotiated during initialization. Then verify that the method the client calls exists in that revision and is implemented by the server.

Use the order of events as evidence

  1. Establish whether the HTTP or stdio transport connected.
  2. Check whether initialization or negotiation completed.
  3. Identify the exact method name and the revision under which it was called.
  4. Compare that method with the server’s advertised capabilities and the SDK version’s supported protocol behavior.

A method mismatch happens after enough connectivity exists to exchange protocol messages. Changing bearer tokens, host allowlists, or proxy settings will not add a method that the server does not implement.

Conversely, do not interpret a 401 or 403 as evidence of an older protocol “era.” The SDK guidance treats authorization responses and protocol-revision evidence as separate diagnostic categories. First obtain a successful authenticated session; only then compare method and revision support.

7. Fix local stdio servers that never start

For stdio, the client must launch a process before any MCP exchange can occur. Check the executable path, command, arguments, working directory, environment, and captured process output. A remote HTTP diagnosis such as host allowlisting does not apply to a process that never spawned.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Understand spawn npx ENOENT

The TypeScript SDK getting-started documentation notes that spawn npx ENOENT means the command is not executable on the process’s PATH. It is a process-launch problem, not a server permission decision. Verify that the runtime containing the command is installed and that the environment used by the MCP client can find it.

  • Check the command name and spelling in the client configuration.
  • Compare the PATH visible to the client with the PATH in your interactive shell; graphical applications and services often inherit a different environment.
  • Use an absolute executable path when the client supports it and when the path is stable for that deployment.
  • Run the configured command with the same arguments and working directory outside the client, then inspect stderr and exit status.
  • Check that arguments are separate argument entries where required, rather than one incorrectly quoted string.

If the process starts and exits immediately, read its stderr and startup log. A missing dependency, invalid argument, or application configuration error is different from an executable that cannot be found. Keep the original process output when reporting the problem; it is usually more actionable than the client’s generic “connection closed” message.

8. Reconnect cleanly after a fix or restart

Once the specific cause is corrected, use the client SDK’s normal connect flow. Close or disconnect the failed transport before creating a new one, and do the same for a transport that finished after the server restarted. A stale session can preserve a closed stream, an old authorization state, or initialization data from a previous server process.

  1. Stop the failed connection attempt and close the transport through the client’s supported API.
  2. Restart or reload the server only after applying the relevant configuration change.
  3. Start a fresh connection rather than reusing a session object tied to the old process or stream.
  4. Watch the server log through transport establishment, authorization, initialization, and the first method call.
  5. If the new attempt fails, classify the new status or log message again; do not assume it is the same failure as before.

A practical decision workflow

Use this sequence for a repeatable incident check:

  1. Identify the path: local stdio or remote HTTP; record command, arguments, endpoint, proxy, client version, server version, and SDK.
  2. Reproduce once: capture the exact client error and the server log entry at the same time.
  3. If there is no server request: check endpoint routing, DNS or network reachability, or the local executable and process output.
  4. If the log shows 421 or “Invalid Host header”: inspect the actual Host value, reverse-proxy forwarding, and the server’s deliberate allowlist.
  5. If the response is 401: inspect bearer-token presence, syntax, expiry, protected resource, audience, and OAuth completion.
  6. If the response is 403: compare required scopes with the token’s scopes and obtain a new token after changing authorization.
  7. If transport and authorization succeed but a method fails: compare protocol revisions, capabilities, and method availability.
  8. Reconnect cleanly: close the old transport and create a fresh session after the targeted change.

Common troubleshooting mistakes

  • Calling every failure a permission error: A request can be rejected by HTTP host validation before MCP sees it, or a local process can fail to launch.
  • Looking only at the client message: The useful status and reason may exist only in the server or proxy log.
  • Allowing every Host value as a shortcut: Configure the names your deployment intends to serve and follow the SDK’s deployment guidance.
  • Reusing an expired token: A successful earlier connection does not prove the current bearer token is valid.
  • Changing transport for a scope problem: 403 requires authorization changes, not an SSE or Streamable HTTP switch.
  • Calling a method before checking revision support: A connected, authenticated peer can still lack that method.
  • Reusing a stale session after a restart: Close the old transport and reconnect through the normal SDK flow.

What to include when asking for help

A useful report identifies the failing layer without exposing secrets. Include the transport type, endpoint shape without credentials, client and server SDK versions, exact status or error text, the relevant server-log lines, whether a reverse proxy is present, and whether the failure occurs during startup, HTTP authorization, initialization, or a particular method call. For 401 and 403 reports, describe the expected resource and scope names but never paste an access token. For stdio reports, include the command and arguments, the process exit status, and stderr, while removing private paths or credentials.
