A large AI bug queue becomes manageable when every report follows the same risk-based path: validate the evidence, check that the surrounding system was healthy, classify and merge duplicates, rank consequences and urgency, assign an accountable owner, then fix and retest. Keep a human responsible for acceptance and priority decisions, especially for safety, security, privacy and business-critical failures.

Do not begin by asking which model is at fault. Begin by asking whether the failure is real, reproducible and important, and whether a connector, permission, rate limit, configuration or evaluation problem produced it. That order prevents wasted engineering time and keeps severe issues from being buried under low-value tickets.

1. Set up one repeatable triage record

Every report should be useful to an engineer who did not witness the failure. Make the following fields mandatory before an item enters the ranked queue.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Field What to capture Why it matters
Model and version Model name, release or deployment identifier, system prompt version and relevant feature flags Behavior can change after a model, prompt or configuration update.
Input evidence Exact prompt, files, tool calls, retrieved context, locale and any redacted user data Paraphrased prompts often cannot reproduce the failure.
Expected and actual result Expected policy or product behavior beside the observed output, action or omission Reviewers need a testable definition of “wrong.”
Time and environment Timestamp with time zone, region, client, deployment, dependency versions and experiment flags Incidents may be limited to one release, region or traffic path.
Telemetry Request ID, trace, evaluation ID, logs, tool responses and relevant screenshots Correlated evidence separates a model response from an orchestration failure.
Impact Number and type of affected users, workflow, business consequence and whether sensitive data was involved Impact drives priority more reliably than how dramatic a single output looks.
Reproduction Exact steps, frequency, minimum input and whether a second operator reproduced it Reproducibility determines whether a fix can be verified.

Use an intake form or issue template that blocks submission when the model version, input, expected result, actual result or timestamp is missing. Permit a fast emergency report for an active safety or security event, but assign someone to complete the record immediately.

Gemini CLI’s documented issue workflow is a useful pattern: labels identify the area, issue kind and P0–P3 priority, while missing information and reports against old versions are flagged for completion or retesting. Apply the same principle in whichever tracker you use. Keep the user-impact issue separate from an implementation pull request so the reason for the priority remains visible after the code changes.

2. Verify the system before blaming the model

Many apparent model failures are failures around the model. Before comparing outputs or changing prompts, check the path that supplied data and executed the response.

Dependency checks

  • Confirm that retrieval indexes, knowledge sources and document permissions were available at the request time.
  • Check connector, API and tool-call errors, including timeouts, malformed responses and partial results.
  • Verify authentication, authorization, quota and rate-limit status.
  • Compare configuration, routing, model alias and feature flags with a known-good request.
  • Check whether the evaluator itself was healthy: missing fixtures, stale expected answers, failed parsers and unavailable test data can create false failures.

Microsoft’s AI troubleshooting playbook puts these checks ahead of model triage. If a dependency was unhealthy, repair it and rerun the evaluation or production replay before opening a model-quality investigation. Record the dependency result in the ticket; otherwise the same false failure will be counted again during the next queue review.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Separate three kinds of evidence

  • Model behavior: The model receives the intended context and still produces an unsafe, incorrect, irrelevant or inconsistent result.
  • Product behavior: The application formats, routes, stores or displays a valid model result incorrectly.
  • Infrastructure behavior: A service, connector, identity layer, evaluator or deployment prevents the intended request from completing.

A single user-visible symptom can contain more than one category. Tag the observed failure and the suspected root cause separately so an infrastructure fix does not erase a remaining model-quality risk.

3. Classify failures and merge duplicates

Classification makes a large queue searchable and exposes root causes that individual ticket titles hide. Use one primary failure class and any number of secondary labels.

Class Typical examples Immediate question
Safety or compliance Disallowed advice, missing refusal, regulatory control bypass Could this cause physical, legal or policy harm?
Security or privacy Prompt injection, data leakage, unauthorized tool action Can an attacker exploit it or access another user’s information?
Reliability Timeouts, crashes, intermittent tool failures, nondeterministic routing How often does the workflow fail, and is there a safe fallback?
Quality Incorrect facts, poor reasoning, wrong extraction or classification Is the output wrong in a consequential workflow or merely suboptimal?
Data Missing records, biased coverage, stale or malformed training or retrieval data Is the issue localized to a source or systemic across inputs?
User experience Confusing citations, unusable error messages, inaccessible interaction Can users understand and recover from the behavior?
Infrastructure Broken deployment, permissions, API, queue or evaluator Is the model receiving and returning what the product expects?

Cluster before you rank

Search by model version, prompt or feature flag, trace signature, error code, input pattern and failure class. Link duplicate reports to one canonical issue rather than closing them as unrelated tickets. Preserve the count of affected reports and users on the canonical issue; that count is evidence of reach and recurrence.

Group recurring failures even when the wording differs. A low-pass-rate evaluation, a repeated refusal gap and dozens of similar support tickets may share one root cause. Fixing a systemic issue can remove more risk than fixing the most recently filed individual report.

When there are “15+” failures in one evaluation or incident cluster, Microsoft’s operational guidance says not to triage every failure independently. Identify higher-leverage groups first, then sample individual cases to confirm that the cluster is coherent. This is a playbook rule, not a universal industry threshold; choose a different trigger if your traffic or evaluation size makes 15 inappropriate.

4. Rank by consequence, likelihood and urgency

Priority is a decision about risk, not a synonym for technical difficulty. NIST’s AI Risk Management Framework describes treatment as being prioritized by impact, likelihood, available resources or methods and organizational risk tolerance. Apply those dimensions consistently, and document why a report moved up or down.

Use these scoring axes

  • Consequence: Safety, legal, privacy, security, financial, operational and reputational harm if the behavior occurs.
  • Population: Number of affected users and whether they include children, patients, employees, customers or other vulnerable groups.
  • Workflow criticality: Whether the failure blocks a core business process or an optional feature.
  • Likelihood: Observed occurrence rate, exposure and ease of triggering.
  • Reproducibility: Whether independent operators or automated tests can produce the failure.
  • Systemic reach: Number of models, products, tenants, regions or data sources sharing the cause.
  • Exploitability: Whether an attacker can intentionally trigger or amplify the behavior.
  • Time sensitivity: Active incident, contractual deadline, regulatory obligation, seasonal event or rapidly expanding exposure.
  • Workaround: Whether users can safely complete the task another way, and whether the workaround is practical.
  • Mitigation effort: Time and resources for containment, a durable fix and verification. Effort should influence sequencing among similarly risky items, not excuse severe exposure.

Define priority bands

Priority Use when Typical action
P0 Critical journey blocked, serious safety, security or privacy exposure, or broad outage Declare an incident, contain harm immediately, assign an on-call owner and review continuously.
P1 High-impact failure in a core scenario, significant recurring degradation or a credible exploit without a safe workaround Schedule the next available engineering capacity, set a near-term review date and track mitigation separately from the permanent fix.
P2 Meaningful but contained degradation, limited population or a usable workaround Plan in normal prioritization, monitor occurrence and re-score if reach or severity changes.
P3 Low-impact defect, cosmetic problem, edge case or improvement request Keep visible in the backlog and address when it offers better value than higher-risk work.

Cirq’s guidance uses inability to install and a security vulnerability as P0 examples and notes that priority can change as expectations evolve. Treat the bands as policy, not permanent labels. A P2 can become P1 after a model release increases exposure; a P0 can be downgraded only after evidence shows that containment reduced the risk.

Do not invent a universal numeric formula. The authoritative guidance does not establish one. A lightweight internal score can help sort a queue, but the written rationale and human review remain the decision record.

5. Decide whether a model failure or product bug comes first

Use a decision sequence rather than arguing over ownership in a meeting.

  1. Can the request be reconstructed? If not, mark the report as evidence-incomplete, request the missing trace or input, and keep it out of definitive model-quality statistics.
  2. Did the intended context and tools arrive? If retrieval, permissions, routing or tool execution failed, open or link an infrastructure/product issue and rerun the case on a healthy path.
  3. Does the model fail with controlled, known-good context? If yes, classify it as a model behavior issue and attach the minimal reproducible case.
  4. Is the model result valid but the application wrong? Route to product engineering while retaining any user-impact priority on the canonical issue.
  5. Does the failure occur across inputs or versions? A broad pattern suggests systemic remediation; a narrow pattern may need a targeted data, prompt or evaluator fix.

Ownership can be shared, but accountability cannot be vague. Name the team that owns the next experiment and the person who accepts the verification result.

6. Assign an owner and a next action

Each canonical issue should contain five operational commitments:

  • Accountable team: The group with authority to change the affected model, product or dependency.
  • Next experiment or mitigation: For example, disable a risky tool, add a retrieval check, roll back a version, tighten a policy or create a regression test.
  • Review date: The date when someone must reassess progress, even if the permanent fix is not ready.
  • Verification owner: A person independent enough to confirm the expected behavior and inspect side effects.
  • Exit condition: The exact test, pass rate, safety check or production observation required to resolve the issue.

Keep severity, priority, status and root-cause confidence as separate fields. “Investigating” does not mean low priority, and “fixed in code” does not mean verified in production.

7. Contain harm before pursuing root cause

During a major incident, restore safe service before perfect diagnosis. Google SRE’s guidance is deliberately blunt: “Your first response in a major outage may be to start troubleshooting and try to find a root cause as quickly as possible. Ignore that instinct!” Make the system work as well as possible under the circumstances, while preserving logs, prompts, model outputs and timelines for later analysis.

Safe containment options

  • Disable the affected capability or tool for the smallest necessary population.
  • Route traffic to a known-good model or earlier configuration when its limitations are understood.
  • Require human approval for high-impact actions.
  • Restrict sensitive data, retrieval sources or output channels until access is verified.
  • Show a clear fallback or pause processing rather than silently returning an untrusted answer.

Record who approved the mitigation, when it began, what users were affected and what new risks it introduced. A workaround that prevents one failure but creates data loss or accessibility problems is not a complete resolution.

8. Retest, measure and recalibrate

Resolution requires evidence, not a merged change. Re-run the original reproduction, the relevant evaluation set and nearby regression cases. Test the healthy dependency path separately from the model behavior so a passing result has a clear explanation.

Track these fields on the canonical issue:

  • Occurrence rate before and after mitigation
  • Severity and priority history
  • Time to first response and time to repair
  • Whether the workaround was available and usable
  • Reopen and recurrence rate
  • Verification result, test version and verifier
  • Residual affected users or scenarios

NIST recommends maintaining records of errors, near misses, incidents, impacts, severity and responses, along with software-quality measures such as bug occurrence, severity, response time and repair time. Re-score a report when the model version, data, exposure, business criticality or user population changes.

9. Automate clerical work, not accountability

Automation is valuable when it reduces sorting time without hiding risk. Safe candidates include:

  • Extracting model, version, timestamp, trace and environment fields from logs.
  • Suggesting area, failure-class and P0–P3 labels.
  • Clustering probable duplicates by trace, prompt pattern, error code and output signature.
  • Detecting recurrence and low pass rates across evaluations.
  • Reminding owners about stale, unverified or missing-evidence reports.
  • Generating a weekly queue view from the tracker.

Use confidence thresholds and an audit trail for every automated suggestion. A human must review clusters that contain safety, security, privacy or high-impact reports. Never allow an AI system to silently close, downgrade or merge such an issue without an authorized reviewer seeing the evidence.

NIST DevSecOps guidance allows AI to recommend work items, assignments, priorities and remediation, while stating that “acceptance, prioritization, and execution remain the responsibility of authorized human stakeholders.” Configure automation accordingly: recommendations may be automatic; final decisions must be attributable.

10. Run a cadence that prevents backlog decay

Continuous review

Review P0 and P1 items continuously while an incident or active exposure remains. Check that mitigations still work, evidence is preserved and the owner has a current next action.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Scheduled backlog triage

At a regular interval, review older P2 and P3 items, unassigned reports, duplicate clusters and tickets awaiting verification. Reopen an item when a model release or traffic change makes its old workaround unsafe.

Weekly queue view

Publish counts by priority, age, owner, product area, failure class, affected users, recurrence and verification state. Add trend lines for occurrence rate, median time to first response, median time to repair, reopen or recurrence rate, duplicate rate and the percentage of reports with reproducible evidence.

Use the dashboard to find process problems as well as product problems. A rising duplicate rate indicates weak search or intake labels; many unverified “fixed” issues indicate that ownership or exit conditions are unclear; a growing age distribution in P2 may mean your priority bands are too broad or capacity is misallocated.

11. Worked example: turning a noisy queue into a decision

Suppose a support team receives 40 reports after a model deployment. Twenty-two describe answers that omit a required policy citation, nine report intermittent timeouts, six show users seeing another tenant’s retrieved document, and three are cosmetic formatting complaints.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Validate the records and find that the nine timeout reports share one connector error. Merge them under an infrastructure issue and rerun evaluations after the connector is healthy.
  2. The six cross-tenant document reports are grouped by trace and treated as a security and privacy cluster. They receive P0 handling, access is restricted, and an authorized security owner approves the containment.
  3. The 22 citation reports are sampled after dependency recovery. If the model receives complete context and still omits the citation, retain them as a systemic quality or compliance issue rather than 22 independent bugs.
  4. The three formatting reports remain P3 unless the formatting prevents a critical workflow or accessibility requirement.
  5. Each canonical issue gets an owner, mitigation, review date, verification owner and explicit exit test. The queue now represents three causes and four operational priorities instead of 40 equally weighted tickets.

12. Common triage mistakes and their fixes

Treating every report as unique

Fix: cluster by evidence and preserve affected-count data on one canonical issue.

Ranking by who complained loudest

Fix: document consequence, likelihood, reach, exploitability and workaround, then apply the same priority bands to every team.

Blaming the model before checking dependencies

Fix: verify retrieval, connectors, authentication, rate limits, configuration and evaluator health first.

Using “fixed” as the exit criterion

Fix: require a reproducible retest, regression coverage and a named verifier.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Letting automation close sensitive issues

Fix: require authorized human acceptance for safety, security, privacy and other high-impact reports.

Leaving old priorities untouched

Fix: re-score when versions, data, exposure, business criticality or expectations change.

Bottom line

A large AI backlog should behave like a risk-management system, not an inbox. Validate evidence, prove the surrounding system is healthy, classify and deduplicate, rank by consequence and urgency, contain active harm, assign accountable owners, and close only after independent verification. Automation can accelerate the clerical work, but people must remain responsible for what gets accepted, prioritized and shipped.
