How to Triage and Fix Vulnerabilities Found by an AI Code Scanner

Treat an AI code-scanner alert as an investigation lead, not a confirmed exploit. Read the rule and data-flow path, verify that an attacker can reach the operation in the deployed product, assess impact and evidence, assign an owner, apply a narrowly reviewed fix, then run tests and a fresh scan. If you dismiss the alert or accept the risk instead of changing code, record the evidence, rationale, owner and review date.

What an AI scanner finding actually tells you

An AI-assisted scanner may identify a suspicious pattern, infer a source-to-sink data flow, suggest a proof of concept, or report behavior observed during execution. Those are different evidence levels. A high-severity label does not by itself prove that the vulnerable path is reachable or exploitable in your product.

Evidence level What it means How to use it in triage
Theoretical or static reasoning The tool matched a pattern or inferred a possible flow from an input source to a sensitive operation. Inspect the code, framework behavior, guards and deployment before treating it as a vulnerability.
Reproducible proof of concept A controlled test demonstrates that crafted input reaches the operation and produces the suspected effect. Record the test, prerequisites, environment and observed impact. Keep reproduction isolated and safe.
Demonstrated runtime evidence Instrumentation or production-like execution shows the behavior occurring under defined conditions. Preserve logs and scope. Confirm that the observation represents the shipped configuration rather than a test-only path.

Microsoft’s SARIF guidance uses distinctions such as theoretical, proof-of-concept and demonstrated evidence to describe confidence. Use those labels to communicate what is known; they do not replace engineering analysis. A scanner can be directionally right while being wrong about the exact location, missing a guard, or misunderstanding a framework’s encoding and authorization behavior.

Step 1: Read and capture the alert

Before changing code, save enough context that another engineer can reproduce your reasoning. Capture the following in the issue or pull-request task:

  • Rule name, weakness category and the scanner’s explanation.
  • Repository, branch, commit or build in which the alert appeared.
  • File, function, line or code range, including the surrounding operation.
  • Reported severity and any confidence or evidence indicator.
  • Source of untrusted data, sensitive sink and the path between them, if the finding is flow-based.
  • Suggested fix, generated patch or links to related alerts in the same flow.
  • Whether the finding is new, recurring or already covered by an existing issue.

Do not rely on a screenshot alone. Copy the alert’s stable identifier and commit reference, because line numbers move as code changes. If the tool provides a source-to-sink trace, preserve every step: request parameter, message, file read, database value, deserialization boundary, template, command, query or other operation that the tool claims is dangerous.

Understand the operation named by the rule

Translate the rule into a plain-language claim. For example, the scanner may be saying that request data reaches a SQL query, a shell command, an HTML response, a file path, a redirect, a serialization routine or an authorization decision without the expected control. The claim is narrower than “this application is hacked.” Identify the exact operation and the security property that should protect it: parameterization, output encoding, allow-list validation, authentication, authorization, path confinement, cryptographic verification or safe deserialization.

Step 2: Trace the data flow in the real code

Open the affected code at the reported commit and follow the value in both directions. Start at the source and continue to the sink; then inspect every transformation that may alter trust or meaning.

Identify the source

Ask where an attacker could control the value. Common sources include HTTP query and body parameters, headers, cookies, uploaded files, webhook payloads, messages from queues, imported records, environment variables supplied by a deployment system and values read from a database that another user can influence. A value that is user-controlled in one endpoint may be trusted in another, so verify the actual entry point rather than relying on a variable name such as input or data.

Identify the sink

Find the operation that could turn the value into impact. Examples include string-built SQL, process execution, template rendering, HTML or JavaScript output, filesystem access, URL redirects, XML or object deserialization, dynamic evaluation, access-control checks and calls to privileged internal services. Confirm that the reported line is the operation that matters; helper functions and wrappers can move the real sink elsewhere.

Inspect controls between source and sink

Document validation, normalization, canonicalization, escaping, parameter binding, type checks, authorization checks and framework defaults. Check their order. A check performed before URL decoding, Unicode normalization or path canonicalization may not protect the final value. Escaping for HTML does not make a string safe for a shell command, SQL statement or JavaScript context. A generic “sanitize” function is not evidence until you know which context it protects and whether every route uses it.

Check reachability and deployment differences

Determine whether the path can execute in the product that users receive. Look for feature flags, dead code, disabled routes, authentication requirements, tenant boundaries, network policy, build-time exclusions and platform-specific branches. Compare the scanned commit with the deployed artifact. A finding in a test fixture, example application or unreachable administrative route may have a different priority from the same pattern on a public endpoint handling sensitive data.

Step 3: Validate exploitability safely

After tracing the code, state the attack preconditions precisely. A useful statement names the required access, input, configuration and outcome: for example, “an unauthenticated request to endpoint X must supply value Y, the feature flag must be enabled, and the value must reach operation Z.” If any prerequisite is absent in the production deployment, explain why and preserve the evidence.

Use a controlled reproduction when it is safe

  1. Create an isolated branch or disposable environment that matches the relevant runtime, configuration and dependency versions.
  2. Use a harmless test payload designed to demonstrate the boundary, not to damage data or access systems. Seed synthetic records and disable external side effects.
  3. Observe whether the input reaches the sink, is rejected by a guard, is transformed safely or produces the suspected effect.
  4. Capture the request, configuration, logs, test result and exact commit. Record negative results as carefully as positive ones.
  5. Delete test data and confirm that credentials, logs and artifacts from the exercise cannot be reused against real systems.

Do not attempt exploitation against production merely to raise confidence. If reproduction is impractical, classify the alert using the code and deployment evidence you do have. “Not reproduced” is not the same as “false positive”; it means the current evidence is incomplete.

Write an evidence-based conclusion

Use a short conclusion with four parts: the reachable path, the missing or effective control, the plausible impact and the evidence level. Example: “A tenant administrator can supply the report filter; the value reaches a string-built query without parameter binding; the query can read other tenant rows; this is confirmed in an isolated integration test.” If a guard blocks the path, name the guard and test that supports the conclusion instead of simply writing “scanner error.”

Step 4: Prioritize findings with context

Start with the scanner’s severity, but do not sort a queue by that label alone. OWASP’s vulnerability-management guidance recommends risk-based prioritization using multiple signals. EPSS can add an exploitation-likelihood signal on a 0–100% scale and is updated daily, but it is not a guarantee that a particular code path is exploitable.

Decision axis Questions to answer Effect on priority
Impact Could exploitation expose personal, financial or credential data, change records, execute code, cross tenant boundaries, interrupt service or bypass a security control? Higher consequence generally raises priority, especially when recovery is difficult.
Evidence Is this a static pattern, a reproducible proof of concept or demonstrated runtime behavior? Stronger evidence increases urgency, while weak evidence still warrants review when impact is high.
Attacker position Does the attacker need an account, a specific role, network access, a malicious file, a compromised dependency or another prerequisite? Fewer prerequisites usually means broader exposure.
Reachability and exposure Is the path public, internet-facing, internal, disabled, restricted to a test build or protected by another service? Public and reachable paths deserve faster action than unreachable code.
Affected assets and data Which service, tenant, device, secret, database or operational process is involved? Critical assets can justify escalation even when the scanner’s label is moderate.
Mitigation and fixability Can a safe code fix, configuration change, access restriction or compensating control reduce risk quickly? A fast, reliable mitigation may be appropriate while a larger redesign is prepared.
Ownership and age Who can change the code, and how long has the alert remained open? Unowned or aging findings need explicit escalation so they do not disappear in the queue.

Use the same axes when comparing alerts from different repositories. A medium-severity, internet-facing injection with a working proof of concept may outrank a high-severity pattern in unreachable sample code. Record the reason for the ordering so a later reviewer can challenge or update it when deployment conditions change.

Step 5: Assign an owner and choose a disposition

Route a valid finding to the team that owns the affected service or component, and track it in the established issue or pull-request workflow. OWASP states the operational rule plainly: “Findings without owners do not get fixed.” Ownership means a named team or accountable engineer, not merely a product label.

For a valid vulnerability

Create a work item containing the evidence summary, impact, prerequisites, affected versions, priority, proposed next action and due date. Link the change that will address it, but keep the original alert record so the decision history remains auditable. Escalate immediately when the issue affects active credentials, regulated data, broad tenant access or a public exploit path.

For a false positive or non-applicable alert

Use the scanner’s supported dismissal category and write a concise rationale tied to code or configuration evidence. State why the reported source cannot reach the sink, why the operation is safely constrained, or why the file is not shipped. Include the commit or test that supports the decision. Avoid dismissing an alert merely because it is inconvenient or because a different tool reports no issue.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For a real risk that cannot be fixed now

Record the compensating control, residual risk, accountable owner, business reason and review or expiry date. Examples include temporarily disabling an endpoint, restricting network access, rotating exposed credentials, adding monitoring or placing a service behind a stronger authorization boundary. A risk exception should be visible and time-bound; it is not a permanent substitute for a code fix.

Step 6: Make a targeted fix

Choose the smallest change that removes the unsafe behavior without weakening the intended feature. Replace string construction with the API designed for the context, enforce authorization at the service boundary, constrain file paths after canonicalization, encode output for its actual context or upgrade a vulnerable dependency to a supported fixed version. If the issue is in a third-party component, verify the package identity, fixed version, transitive dependencies and license or compatibility implications before upgrading.

Review an AI-generated patch as untrusted code

An AI suggestion is a draft, not a security decision. GitHub documents risks that include incorrect syntax or location, semantic changes, misleading or partial fixes and newly introduced vulnerabilities. Review it with the same rigor as any contribution.

  • Confirm that the patch changes the real source-to-sink path identified during triage.
  • Check that validation or encoding matches the output context and occurs after the necessary normalization.
  • Look for bypasses through alternate endpoints, error handlers, batch jobs, background workers and legacy code paths.
  • Inspect every dependency addition, version change, lockfile update and generated file.
  • Compare authorization behavior before and after the change, including anonymous, low-privilege and cross-tenant cases.
  • Read the complete diff rather than accepting only the lines highlighted by the assistant.

Keep the security change focused. Refactoring unrelated code in the same patch makes review and later incident analysis harder. If the safe solution requires a larger redesign, split preparatory refactoring from the change that closes the vulnerability when your release process permits.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 7: Verify the fix and close the alert

A code edit or a dismissed alert is not proof of remediation. Verification should cover behavior, regression risk and scanner state.

  1. Run unit tests for the validation, authorization or encoding logic, including boundary and malformed inputs.
  2. Run integration or end-to-end tests that exercise the original entry point, realistic authentication and the affected data store or service.
  3. Run security-focused tests or a controlled proof of concept that previously demonstrated the issue. Confirm that it now fails safely and that legitimate use still works.
  4. Inspect the final diff, dependency graph and generated artifacts. Ensure the fix is present in the artifact that will be deployed.
  5. Run a fresh scan on the changed branch or commit using the relevant rules. Review both the original alert and any new findings.
  6. Check logs, performance and error handling for regressions, then obtain the required code-owner or security approval.
  7. Record test commands, environment, commit, scan result and reviewer decision before closing the work item.

GitHub documents that a new pull-request analysis can close an alert when the issue is no longer detected. Treat that status as one verification signal, not as a replacement for tests and code review. A scanner may stop reporting the original pattern while a logically equivalent bypass remains elsewhere.

When a rescan still reports the vulnerability

The code change is not in the analyzed commit

Confirm the branch, repository, build trigger and commit analyzed by the scanner. Generated code, vendored copies or a stale pull request can leave the old path in the scan.

The source-to-sink path still exists

Reopen the flow and identify the remaining unsafe operation. The first patch may have added validation at one entry point while another route reaches the same sink. Centralize the control or cover every caller.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The rule needs configuration or a justified suppression

If the operation is intentionally safe, document the framework guarantee, wrapper contract or test that proves it. Use a narrow, reviewed suppression tied to the exact location and reason; do not disable the rule globally to clear one alert.

The dependency remains vulnerable transitively

Inspect the lockfile and dependency tree. Updating a direct package may leave an older transitive package in the shipped artifact. If no fixed version exists, apply a compensating control and time-box the exception.

Build a repeatable triage record

A compact record prevents the same investigation from being repeated during audits or incidents. Include:

  • Alert identifier, rule, repository and analyzed commit.
  • Source, sink, data-flow path and relevant code locations.
  • Reachability, attacker prerequisites, deployment exposure and affected assets.
  • Evidence classification and reproduction details or the reason reproduction was unsafe or impossible.
  • Impact assessment, priority rationale and accountable owner.
  • Disposition: fix, dismiss, or risk acceptance with compensating control and expiry date.
  • Patch or dependency version, review participants and test results.
  • Fresh scan result, remaining findings and closure date.

Improve the process after the alert is closed

Review open findings by severity, age, team and evidence strength rather than looking only at a current total. Repeated false positives can indicate a rule, framework model or repository configuration that needs improvement. Repeated valid findings in one pattern may justify a safer shared library, a secure coding standard, a test harness or a pipeline gate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep monitoring dependencies after the initial review. NIST’s recommended minimum standard for vendor or developer verification says: “The components of your software must be continually monitored against databases of known vulnerabilities; a new vulnerability in existing code may be reported at any time.” Schedule dependency and container checks, define who receives newly disclosed alerts and verify that the escalation path still works when teams or services change.

Track remediation time and reopening rates internally, but do not turn a scanner score into a promise of exploitability or safety. The useful outcome is a defensible chain from alert, through evidence and ownership, to a tested change or a documented, expiring exception.

Common triage mistakes

  • Equating severity with exploitability: a label is a starting signal; deployment reachability and attacker prerequisites determine practical risk.
  • Trusting the highlighted line: the real control may be in a caller, wrapper, middleware layer or configuration file.
  • Calling “not reproduced” a false positive: absence of a safe reproduction leaves uncertainty; explain what was and was not tested.
  • Accepting an AI patch without reading the diff: partial fixes and semantic changes can create a quieter vulnerability.
  • Closing after a clean local test: verify the shipped artifact and rerun the scanner on the actual change.
  • Leaving exceptions open-ended: name an owner, compensating measure and expiry or review date.
  • Disabling a noisy rule globally: narrow the suppression and address the underlying modeling or coding problem.

A practical decision sequence

  1. Understand: capture the rule, location, flow, severity and analyzed commit.
  2. Trace: identify the attacker-controlled source, sensitive sink and every control between them.
  3. Validate: check reachability, deployment conditions and safe reproduction evidence.
  4. Prioritize: weigh impact, evidence, attacker position, exposure, assets, mitigation and age.
  5. Own: assign the service owner and create a tracked task with a clear next action.
  6. Disposition: fix, dismiss with evidence, or accept risk temporarily with controls and an expiry.
  7. Review: inspect the targeted patch, dependencies and complete diff, especially any AI-generated code.
  8. Verify: run tests, rescan the changed commit, inspect new findings and record the result.
  9. Learn: monitor components and use recurring patterns to improve rules, libraries and team practice.

Frequently Asked Questions

Is a high-severity AI scanner alert automatically a confirmed vulnerability?

No. It is a signal that must be checked against the actual source-to-sink path, guards, deployment reachability, attacker prerequisites and evidence level.

What should I do when I cannot reproduce the reported behavior?

Record the attempted reproduction and environment, then inspect code and configuration for reachability and effective controls. Do not label the alert a false positive solely because a safe test did not reproduce it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can I merge an AI-generated security fix without a security review?

No. Review the complete diff, dependency changes, alternate paths and behavior, then run relevant tests and a fresh scan before merging.

When is risk acceptance appropriate?

Only when the risk is understood and a code fix is not currently practical. Document compensating controls, an accountable owner, residual risk and a review or expiry date.

What proves that a vulnerability is fixed?

Evidence should include a targeted change, tests covering the original path, review of the shipped artifact and a fresh scan showing the original finding is resolved without introducing regressions.
