How to Treat Keratosis Pilaris: A Gentle, Evidence-Based Routine

Keratosis pilaris (KP) is harmless, so treatment is optional. If the rough bumps bother you, start with short warm showers, a mild unperfumed cleanser and regular moisturizer. Add only one skin-softening ingredient at a time, follow its label and ease off if your skin becomes irritated.

What keratosis pilaris looks and feels like

KP forms when dead skin cells plug hair follicles, creating tiny rough bumps. They commonly appear on the upper arms and front of the thighs, though other areas can be affected. The bumps may resemble goosebumps or small pimples, and their color varies with skin tone. Dry weather and winter can make them more noticeable.

KP is not infectious. It may itch, often improves in summer and worsens in winter, and can persist for years before clearing on its own. Other skin conditions can look similar, so an article cannot confirm a diagnosis. If the bumps are unusual, inflamed or especially itchy—or you are unsure what they are—ask a healthcare professional or pharmacist.

Start with a low-irritation daily routine

Keep bathing short and gentle

  • Use warm or lukewarm water, not hot. The American Academy of Dermatology (AAD) advises bathing no more than once a day and keeping a shower or bath to 20 minutes or less; the NHS also recommends cool or lukewarm bathing.
  • Choose a mild, unperfumed cleanser. Avoid drying or perfumed soaps, and do not pick, scratch or rub the bumps.

Moisturize after bathing

Pat skin so it remains slightly damp, then apply moisturizer. The AAD advises applying it within five minutes of bathing and choosing a thick, oil-free cream or ointment. Reapply when skin feels dry. A cream containing urea or lactic acid can combine moisturizing with a skin-softening effect.

Consider one optional treatment ingredient

If roughness persists despite moisturizing, a product with a keratolytic ingredient may help loosen or soften the plugs. Dermatology guidance lists urea, lactic acid, salicylic acid, glycolic acid, other alpha-hydroxy acids and retinoids as options. General guidance does not establish one over-the-counter ingredient or brand as best for everyone.

Ingredient group Role in a routine Practical note
Urea or lactic acid Can soften rough skin while providing a moisturizing step. Often available in creams; follow the product label.
Salicylic acid, glycolic acid or other alpha-hydroxy acids Help loosen dead skin cells. Use as directed; too much can irritate skin.
Retinoids Medicine options that can help with plugged follicles. They have additional cautions. Ask a clinician whether they are appropriate, especially during pregnancy or nursing.

Use one active product at a time, in the amount and frequency on its label or prescribed by a dermatologist. Avoid starting by layering several strong actives together. If skin becomes raw, unusually dry or irritated, stop the medicine for a few days. Restart only cautiously if appropriate; ask a clinician if you are unsure.

Keep physical exfoliation gentle—or skip it

A soft washcloth can be used gently, but exfoliation is optional. Forceful scrubbing, rough body scrubs, aggressive brushes and attempts to extract plugs can irritate skin and worsen symptoms. More exfoliation is not necessarily more effective.

Set realistic expectations and know when to ask for help

Improvement is gradual, not a guaranteed cure. If you follow a treatment plan and see no improvement after 4 to 6 weeks, the AAD advises telling a dermatologist. Some people need to try different options, and maintenance may be needed to preserve improvement; a clinician may suggest reducing medicine frequency or switching to a non-prescription moisturizer.
