The most useful preparation for a preconception appointment is a short, accurate record of your migraine pattern and a complete list of everything you take. Record when each attack starts and ends, its symptoms and severity, every acute medicine dose and time, and all preventive medicines, vitamins, minerals and supplements. Take those records to an obstetric clinician or headache specialist before trying to conceive. A diary supports that conversation; it cannot decide whether you should stop, continue or switch a medicine.
Why tracking matters before conception
Pregnancy planning changes the question from “What stops this attack?” to “What is the safest effective plan for attacks, prevention and timing if I become pregnant?” The answer depends on the exact product, dose, frequency, other health conditions and whether you are trying to conceive, newly pregnant or further along.

The American College of Obstetricians and Gynecologists (ACOG) advises discussing headache symptoms, frequency, acute treatments and preventive treatments at a prepregnancy visit. The American Migraine Foundation (AMF) similarly recommends planning before trying and reviewing prescription medicines, over-the-counter products, vitamins and supplements. A record made over several weeks gives the clinician usable evidence instead of relying on memory.

Do not use a diary as permission to stop a prescription suddenly. A clinician may recommend continuing, changing, reducing or stopping preventive treatment, depending on your individual situation. Ask for a plan before you make a change.

Set up a diary that captures decisions, not just pain
Use paper, a spreadsheet or a phone diary. The NHS migraine treatment pathway mentions printable and smartphone diary options but does not establish that one format is better. Choose the format you can complete consistently and export or print for an appointment.
| Entry | What to write | Why it helps the appointment |
|---|---|---|
| Start and end | Date and time the attack began and ended. If it continued through sleep, record the best estimate. | Shows frequency and duration and helps distinguish a single prolonged attack from separate attacks. |
| Severity | A simple scale you can repeat, such as 0–10, plus whether normal activities were possible. | Shows functional impact rather than pain alone. |
| Symptoms | Pain location and quality, nausea or vomiting, light or sound sensitivity, aura, and anything noticeably different from your usual pattern. | Highlights changes that may require assessment and helps confirm whether the pattern is typical for you. |
| Acute treatment | Medicine name, dose as prescribed, time taken, and whether it helped or symptoms returned. | Shows how often medicines are used and whether the current plan is working. |
| Preventive treatment | Name, prescribed dose and schedule, plus any clinician-directed change. Keep this separate from attack-by-attack medicines. | Lets the clinician review prevention independently from rescue treatment. |
| Context | Optional notes on sleep, meals, hydration, caffeine, stress, menstrual timing or other circumstances. | May reveal patterns worth discussing; it does not prove that a particular factor caused an attack. |
MedlinePlus specifically advises noting when headaches occur and when sumatriptan is taken. The broader fields above are practical tracking suggestions, not a universal medical form. If an entry is uncertain, label it as an estimate rather than filling in a precise time you do not know.
Use one row per attack
A compact row prevents a busy month from becoming an unreadable narrative. For example: “12 March, 07:30–15:00; pain 8/10; visual aura and nausea; sumatriptan 50 mg at 08:00 as prescribed; partial relief, pain returned at 13:00; unable to work.” Add a separate line for a second attack on the same day.
Count an attack according to the way your clinician normally counts it. Do not invent a new definition to make the monthly total look lower or higher. Bring the raw entries; the clinician can interpret clusters, prolonged symptoms and medication-use days.
Keep a separate, complete medication and supplement list
Make a list for the appointment even if a product appears unrelated to migraine. Include prescriptions, over-the-counter pain relievers, combination products, vitamins, minerals, herbal products and dietary supplements. Include medicines used only occasionally. MedlinePlus recommends bringing a written list of medicines, vitamins, minerals and supplements to medical appointments, and AMF specifically includes over-the-counter products and supplements in pregnancy planning.

| List item | Details to record |
|---|---|
| Prescription medicines | Exact name, strength, prescribed dose, schedule and reason for use. |
| As-needed medicines | Name, dose, maximum directions on the label or prescription, and typical number of days used per month. |
| Nonprescription products | Full product name and formulation. Combination products can contain caffeine or another pain reliever that is easy to overlook. |
| Vitamins and supplements | Name, amount per serving, number of servings and brand-free product details from the label. |
| Recent changes | What changed, when, who advised it and what happened afterward. |
Take photographs of labels if that is easier than typing them. Keep the list dated and update it whenever a dose, product or schedule changes. A list made several months ago may be incomplete by the time of the visit.
Build a preconception review step by step
- Start before you begin trying. Choose a review date early enough to discuss changes, alternatives and monitoring. AMF notes that harmful effects from some medicines can occur early, sometimes before a person knows they are pregnant. Dr. Robbins of AMF explains: “Much of the injury that can be done to a developing baby by medications that are taken by a mother who is newly pregnant can occur early on, often before a mother even knows she’s pregnant.” This is a reason to plan, not a reason to stop treatment without advice.
- Collect several weeks of entries. Record every attack and acute dose, including uneventful days. A short run is still useful; tell the clinician how many days you tracked and where estimates were necessary.
- Reconcile the list. Compare your diary with prescription bottles, pharmacy records and supplement labels. Mark anything discontinued, newly started or taken differently from the prescribed directions.
- Book the appropriate clinician. Start with your obstetric clinician or primary-care clinician; include a headache specialist if your attacks are frequent, disabling, changing or difficult to control.
- Bring the records in a readable form. Print a one-page monthly summary or export the diary, and bring the full medication list. Keep the original entries available if a clinician needs detail.
- Leave with a written contingency plan. Ask what to do for an attack while trying to conceive, after a positive pregnancy test and if the usual treatment fails. Ask when to call and which symptoms require urgent assessment.
Questions to ask at the appointment
- “Is my current medication safe for pregnancy?”
- “Do I need to stop taking medication before trying to get pregnant?”
- “When should I stop taking my medication if I’m planning on getting pregnant?”
- Which medicines are preventive, which are for an individual attack, and what is the plan for each?
- If a change is recommended, what is the alternative, when should the change occur, and how will we judge whether it is working?
- What should I do if I have an attack before I can reach the clinic or before pregnancy is confirmed?
- How should I handle a missed dose, vomiting after a dose or an attack that lasts longer than usual?
Ask the clinician to specify whether advice applies while trying to conceive, during a particular trimester or after delivery. “Pregnancy-safe” is not a single category that applies identically to every medicine, dose or stage.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsWhat pregnancy may do to your migraine pattern
Migraine does not follow one predictable pregnancy course. AMF reports that up to 80% of women have fewer or less intense migraine attacks by the second trimester, weeks 13–26. That is a population-level estimate, not a promise for an individual. Some people worsen, develop new symptoms or experience different patterns in different pregnancies.
Continue tracking after conception unless your clinician recommends another method. Improvement does not automatically mean a preventive medicine should be changed, and worsening does not identify the cause by itself. Record the new pattern and contact the clinician about treatment rather than self-adjusting.
Medication information that requires jurisdiction and timing
ACOG guidance in the United States
ACOG says acetaminophen may be used for headaches during pregnancy. It also says acetaminophen combined with caffeine has been shown to help migraine pain and advises keeping total caffeine from all sources at or below 200 mg per day. Count coffee, tea, energy drinks, chocolate and medicines that contain caffeine.

ACOG describes nonsteroidal anti-inflammatory drugs (NSAIDs), including ibuprofen and naproxen, as an option only in the second trimester and for 48 hours or less at a time. It notes uncertainty in the first trimester and possible birth defects with third-trimester use, and advises discussing planned use with an ob-gyn. This is general ACOG guidance, not a prescription for your situation.

The NHS migraine treatment pathway states that topiramate is contraindicated in pregnancy for migraine prevention under the cited UK framework and is subject to a pregnancy prevention programme for women of childbearing potential. That policy is jurisdiction-specific. If you live outside the UK, ask your own clinician or regulator for current local requirements; do not transplant the UK wording as universal law.
Drug-specific information about sumatriptan
MedlinePlus tells people who take sumatriptan to tell their clinician if they are pregnant or planning pregnancy. It also says not to take sumatriptan on more than 10 days in a month and to contact a clinician if it is needed for more than four headaches in a month. Those instructions are specific to that medicine and do not establish that sumatriptan is appropriate for every person. Record each dose so your clinician can compare your use with the directions.
Never infer safety from a friend’s medicine, an old prescription or a supplement label. Bring the exact product and ask for advice that matches your location, pregnancy stage and medical history.
Recognize symptoms that should not wait for a diary review
A diary is not a diagnostic test. Seek prompt clinical assessment for a new, severe or persistent headache, especially during pregnancy. ACOG notes that a severe headache that will not go away can signal preeclampsia. Possible accompanying symptoms include swelling of the face or hands, vision changes, upper-abdominal or shoulder pain, and nausea or vomiting in the second half of pregnancy.

- Tell the clinician that you are pregnant or trying to conceive and provide your medication list.
- Do not delay assessment while waiting to complete missing diary fields.
Choose a format you can maintain and protect
Paper journal
A paper calendar or notebook works when you want a fast entry and no account or device is involved. Keep a spare page with your current medicine list and photograph completed pages before the appointment. Store it somewhere private because migraine notes can contain menstrual, pregnancy and other health information.
Spreadsheet
A spreadsheet makes monthly counts easier. Use one row per attack and columns for start, end, severity, symptoms, acute medicine, response and notes. Add a separate sheet for medicines and supplements. Do not use formulas to manufacture a diagnosis; use totals as a prompt for discussion.
Phone diary
A phone app can provide reminders and quick timestamps. Before entering health information, check its privacy policy, export options, account requirements and deletion controls. The NHS pathway mentions smartphone diaries but does not verify the privacy practices, efficacy or current features of any particular app. A plain notes app may be sufficient if you can keep consistent fields.
Troubleshoot common tracking problems
“I forget to record attacks until the next day.”
Enter a short note immediately—start time, approximate severity and medicine time—then fill in details when you can. Mark estimates clearly. A consistent approximate record is more useful than a few perfect entries.
“My medicine is a combination product.”
Copy the complete label, including each active ingredient and caffeine amount. Ask a pharmacist or clinician to reconcile duplicate ingredients across products.

“I take a preventive medicine and rescue medicine with similar names.”
Put them in separate sections and write the purpose beside each name. Log preventive doses on the medication list and attack medicines in the attack row.
“My attacks changed after I started tracking.”
Tracking can make patterns more visible, but it does not prove that recording caused a change. Report the timing, symptoms and any medicine or life change to your clinician.
“I had a positive pregnancy test before the appointment.”
Contact the clinician or pharmacist promptly with your current list and the date of your last doses. Do not abruptly stop a prescribed medicine unless a clinician who knows your situation tells you to do so. Ask what to take for an attack while the plan is being reviewed.

A useful preconception record has two parts: an attack diary and a current medicine-and-supplement list. Together they show frequency, duration, disability, symptoms and actual medicine exposure. Bring both to a clinician before trying to conceive, use the clinician’s plan for any medication change, and keep tracking because pregnancy may improve, worsen or otherwise alter migraine. Persistent or severe headache with concerning pregnancy symptoms needs prompt assessment, not a more detailed diary.
