Use verified measure results to find gaps, distinguish missing care from missing documentation or access barriers, and close each gap with evidence that satisfies the current specification.
Keep three views separate: CMS Star Ratings are contract-level results, HEDIS is a performance framework collected for Medicare Special Needs Plans (SNPs), and CAHPS reports beneficiary experience. They inform one improvement program, but they are not interchangeable scores.

Start with the correct measurement year and contract
A Medicare Advantage quality workflow is only reliable when its scope is explicit. Record the measurement year, the Star Ratings publication year, contract type, and the exact CMS files and technical notes used. A measure, weight, cut point, reporting treatment, or specification can change between cycles.
The 2027 Star Ratings fact sheet, published by the Centers for Medicare & Medicaid Services (CMS) on October 8, 2026, describes up to 43 measures for MA-PD contracts, up to 32 for MA-only contracts, and up to 13 for standalone prescription drug plan (PDP) contracts. Those are maximum program counts, not a promise that every measure applies to every contract.

- Read Before You Buy — No Video Output: These adapters support charging and USB 2.0 data transfer, but cannot transmit video signals. Except for standard USB webcams (which use USB data only), they are not compatible with HDMI/DisplayPort cables, video-capable USB-C hubs, or docking stations with video output.
- Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
- Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
- Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
CMS says, “The cut points are recalculated each year based on performance during the measurement period.” Consequently, a prior year’s star threshold is not a safe target for the next cycle. Use the current Part C and D performance-data index for the current data tables, technical notes, and measure files, and check it again when CMS posts updates.
Know what each quality source actually measures
Teams often place Star Ratings, HEDIS, and CAHPS in one dashboard and then treat their values as if they describe the same thing. Their units and uses differ.
| Source | Unit of analysis | What it contributes | How a team uses it |
|---|---|---|---|
| CMS Star Ratings | Medicare contract | One- to five-star results assigned with CMS cut points | Review contract-level program performance and the requirements for the applicable rating year |
| HEDIS for Medicare SNPs | Applicable SNP measure population | Clinical and process performance information used to find gaps and monitor improvement | Prioritize SNP interventions, compare results over time, and set realistic internal targets |
| Medicare Advantage and Part D CAHPS | Surveyed plan enrollees | Beneficiary experience, including access, appointments, communication, coordination, and plan ratings | Find experience and access problems that may require operational improvement |
CMS describes HEDIS results as useful for identifying performance gaps, monitoring quality-improvement initiatives, tracking progress, comparing performance, and establishing targets for SNPs. The CMS HEDIS overview was last modified September 10, 2024; verify current CMS and NCQA specifications before implementation.

Build a measure inventory before looking for gaps
Do not begin with a generic checklist of screenings or diagnoses. Start with the current specification for every measure that applies to the contract. The inventory is the control document that tells analysts, care managers, vendors, and auditors what is being measured and which evidence is acceptable.
- Confirm applicability. Match the contract type and reporting year to the CMS measure files. Record whether a measure is included, excluded, newly added, respecified, or subject to a different reporting treatment.
- Capture the eligible population. Document enrollment, age, clinical, service-use, and other inclusion conditions exactly as defined in the current measure specification. A member can appear in one denominator and be excluded from another.
- Define numerator evidence. List the event, result, service, or documentation that satisfies the numerator. Do not assume an outreach attempt, appointment request, referral, or member promise is numerator evidence.
- Record exclusions and exceptions. Put every allowed exclusion, required exclusion evidence, and timing rule in the inventory. Avoid applying an exception simply because it seems clinically reasonable.
- Specify the performance period. Include the start and end dates, look-back windows, result date rules, and the data-finalization date used by the team.
- Identify the source system. Name the claims, encounter, pharmacy, laboratory, electronic health record, chart-review, survey, or other source expected to provide evidence. If multiple sources are permitted, document the reconciliation order.
- Assign accountability. Give each measure a business owner, data owner, clinical or operational reviewer, and escalation path. This is an internal operating design, not a CMS assignment.
| Inventory field | Decision it supports |
|---|---|
| Reporting year and specification version | Whether the rule set is current for the intended Star Ratings cycle |
| Contract and measure applicability | Which members and results belong in the calculation |
| Denominator definition | Who is eligible during the performance period |
| Numerator evidence | What proves completion or the required result |
| Exclusions and exception evidence | Which records may be removed and why |
| Data source and refresh date | Where the evidence comes from and how current it is |
| Owner, due date, and escalation | Who must act when a member or process is at risk |
CMS’s performance-data resources identify the authoritative technical notes, data tables, and measure files. They do not replace measure-level specifications for coding, inclusion, exclusion, or evidence decisions.

Turn performance results into a care-gap work queue
A care gap is not merely a low percentage. It is an actionable difference between the required evidence and the evidence currently available for an eligible population. Build the work queue from the latest validated result, then classify why the gap exists.
Separate four common gap causes
- Care need: The member is eligible and the required service, test, medication review, or follow-up has not occurred.
- Access barrier: Transportation, appointment availability, language, network capacity, cost, or other practical barrier prevents completion.
- Documentation gap: The care occurred, but the plan lacks the record, code, result, date, or other evidence required by the specification.
- Data-quality or attribution problem: A duplicate, stale enrollment record, wrong provider assignment, delayed encounter, or mapping error places the member in the wrong queue.
Classifying the cause prevents a plan from sending repeated reminders to a member whose care is complete but not documented, or from closing a record administratively when a genuine clinical need remains.
Prioritize without inventing a universal threshold
Use a transparent prioritization policy approved by the plan. Typical factors include imminent measure deadlines, clinical risk, the number of eligible members, repeated failed outreach, access barriers, and the time required to obtain acceptable evidence. Do not present an internal priority score as a CMS rule, and do not substitute a local target for the current CMS cut point.
Assign a specific next action
Every open gap should have one next action, one accountable owner, and one due date. Examples include verifying a missing result, arranging an in-network appointment, resolving a transportation barrier, requesting an exchange of medical records, or escalating a provider attribution error. Record the attempt, outcome, and evidence location so another team member can continue the case without restarting the investigation.
Use a closure workflow that proves the numerator
Care management activity and confirmed measure completion are different metrics. Track them in separate fields and reconcile them before reporting a closure.
- Open: The member is in the current denominator and qualifying evidence is absent.
- Engaged: Outreach or coordination has occurred, with the date, channel, and result recorded.
- Scheduled or referred: A service, appointment, test, or follow-up is arranged; this status is not yet numerator completion unless the specification explicitly allows it.
- Completed pending validation: The event appears to have occurred, but the required record or result has not passed validation.
- Closed with evidence: The record satisfies the current numerator and documentation rules, and the source and date are stored.
- Exception or unable to close: The case has a documented reason, any permitted exception evidence, and a follow-up or escalation decision.
Reconcile the final status against the authoritative data source. For example, an outreach system may show that a member agreed to obtain a test, while a laboratory or encounter record is needed to establish completion. The exact evidence depends on the current measure specification.

Monitor leading activity and confirmed outcomes separately
A dashboard should show whether the team is doing the work and whether the work produces valid measure evidence. Combining the two hides failure modes.
Leading workflow indicators
- Eligible members loaded into the work queue
- Members contacted and contact attempts by channel
- Appointments or referrals scheduled
- Barriers identified and resolved
- Records requested and received
- Cases waiting for provider, laboratory, pharmacy, or claims data
Outcome indicators
- Validated numerator completions
- Remaining eligible members without acceptable evidence
- Documented exclusions and exception records
- Results by provider, population segment, geography, or care team where permitted by the measure design
- Changes from the prior reporting extraction using the same specification version
Use a refresh timestamp and specification version on every dashboard view. A rising outreach count can coexist with a flat completion rate; that pattern points to an access, documentation, attribution, or evidence problem rather than proof that the intervention worked.
Use HEDIS deliberately for SNP improvement
For Medicare SNPs, HEDIS results provide a structured way to identify performance gaps and monitor quality-improvement initiatives. Use the HEDIS result to select a population or process for intervention, establish a realistic internal target, and follow the trend through subsequent extracts.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Keep the HEDIS denominator and numerator definitions tied to the current specification. The CMS overview does not provide one consolidated, permanent coding guide for every measure; implementation teams must use current CMS and NCQA materials for inclusion, exclusion, timing, and evidence rules.
A practical SNP review combines the measure result with the work-queue classification:
- Compare the eligible population with the members lacking evidence.
- Break the open population into care need, access, documentation, and data-quality causes.
- Assign an intervention appropriate to the cause rather than sending the same message to everyone.
- Re-run the measure logic after the evidence refresh, not merely after outreach activity.
- Document the intervention, owner, date, and observed change so the quality-improvement team can distinguish progress from seasonal or data-lag effects.
Use CAHPS to find experience and access problems
CAHPS adds the beneficiary perspective that claims, encounters, chart review, and HEDIS cannot provide on their own. Review results for obtaining needed care, getting appointments promptly, communication with providers, care coordination, and overall plan ratings. These domains can reveal why a clinically appropriate intervention is not reaching members even when the plan’s internal workflow appears active.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
Use CAHPS findings to form operational questions: Are members unable to find an available provider? Are instructions understandable? Do handoffs between the plan and clinicians leave members unsure about the next step? Are appointment delays concentrated in a service area? Then connect the answer to an owner and an intervention, while keeping the survey result separate from clinical numerator performance.

Refresh the process for 2027 changes
The 2027 Star Ratings include measure additions and specification changes that make a prior inventory unsafe to reuse without review. CMS reports these changes in its 2027 Medicare Advantage and Part D Star Ratings fact sheet.
- New measures: Concurrent Use of Opioids and Benzodiazepines and Polypharmacy: Use of Multiple Anticholinergic Medications in Older Adults.
- Restored measure: Care for Older Adults—Functional Status Assessment returned after specification changes.
- Respecified measure: A Colorectal Cancer Screening measure moved into the ratings after its specification was revised.
- Future timing: Part C Depression Screening and Follow-Up begins with measurement year 2027 and appears in the 2029 Star Ratings.
The Contract Year 2027 Medicare Advantage and Part D Final Rule fact sheet, dated April 2, 2026, provides the finalized measure-change and timing information. At the start of each cycle, compare the new measure files with the prior inventory, update data mappings and work queues, and retrain staff on changed evidence rules.

- Version control: Store the CMS file name, specification version, extraction date, and calculation logic with each result.
- Denominator reconciliation: Investigate unexpected population changes before interpreting a rate increase or decrease.
- Evidence review: Sample closed cases to confirm that the recorded document, code, result, and date satisfy the measure rule.
- Exception control: Require the evidence permitted by the specification; a free-text explanation alone may not support an exclusion.
- Source latency: Label records that may be incomplete because claims, encounters, laboratory results, pharmacy data, or medical records have not arrived.
- Role separation: Give data calculation, clinical review, outreach, and final validation clear responsibilities so the person recording an outreach attempt is not the only person deciding that a numerator is met.
- Change review: When CMS updates a technical note or measure file, assess whether existing cases, mappings, dashboards, and reports must be recalculated.
These controls are internal operating practices designed to apply CMS requirements consistently. They do not change the official specification or create a new CMS reporting rule.

Common failure modes and corrective actions
Using last year’s cut points
Symptom: A team labels a result as on track using a prior star threshold. Correction: Load the current year’s cut points and performance-period definition from CMS before setting targets.

Applying every measure to every contract
Symptom: Analysts report measures that do not apply to the contract type or reporting year. Correction: Reconcile the contract inventory to the current CMS measure files and retain the applicability decision.
Counting activity as completion
Symptom: Outreach, scheduling, or referral counts are reported as numerator closures. Correction: Keep activity statuses separate and require the evidence named by the current specification.
Mixing member-level gaps with contract ratings
Symptom: A care manager treats a contract star result as an individual member status. Correction: Use member-level denominators and evidence for case work; use Star Ratings for contract-level review.
Ignoring documentation and data latency
Symptom: The plan repeats outreach for completed care or declares success before records arrive. Correction: Add a pending-validation state, source refresh date, and reconciliation step.
Treating CAHPS as a clinical measure
Symptom: Survey responses are substituted for service or outcome evidence. Correction: Use CAHPS to diagnose experience and access problems, then monitor the applicable clinical or process measure separately.
A repeatable annual operating cycle
- Cycle launch: Confirm the measurement year, Star Ratings year, contract scope, and current CMS files.
- Specification build: Update populations, evidence, exclusions, timing, source mappings, owners, and due dates.
- Baseline: Run the validated logic and reconcile unexpected denominator or data-source changes.
- Gap stratification: Classify open records by care need, access, documentation, and data quality.
- Intervention: Assign targeted actions and monitor leading workflow indicators.
- Validation: Reconcile completed work with the authoritative evidence source and close only qualifying records.
- Experience review: Analyze CAHPS findings for access, communication, coordination, and plan-experience improvements.
- Outcome review: Compare verified results with the prior period using compatible specifications and explain material changes.
- Refresh: Incorporate CMS updates, changed measures, new timing rules, and lessons from the completed cycle.
This cycle keeps quality measurement, care management, and beneficiary experience connected without collapsing them into one misleading number. The result is a year-aware process that can show what was measured, who needed action, what was done, and which evidence demonstrated closure.
