A useful record keeps each result’s date and units so you and your clinician can compare changes over time.
What kidney results should you track?
Kidney monitoring commonly includes two different lab measures: a blood test for estimated glomerular filtration rate (eGFR) and a urine test for albumin, often reported as an albumin-to-creatinine ratio (ACR or uACR). They answer related but distinct questions. An eGFR reflects how well the kidneys filter blood; urine albumin can indicate kidney damage. One number alone does not give a complete picture. NIDDK explains these kidney tests, and the National Kidney Foundation offers a dated GFR and albuminuria tracking resource.

If your care team recommends home blood pressure checks, record those too. Blood pressure is relevant to kidney care, but a cuff measures blood pressure—not eGFR or urine albumin. Ask your health professional what your personal blood pressure goal is and how often to check it; there is no one schedule that fits everyone. NIDDK’s guidance on high blood pressure and kidney disease recommends discussing both with your health professional.

How to set up a useful phone record
- Choose a place you can maintain. Use a secure phone note, spreadsheet, or health-record feature. Consider whether it has privacy controls and whether you can share or export the record in a way that works for you and your clinician.
- Make a dated entry for each result. Include the test name, result, units shown by the lab, test date, and any reference information on the report. Keep the lab’s wording and units rather than casually converting a value.
- Log home blood pressure only if advised. Record the date, time, and reading, following the checking schedule your care team gave you. If you use a home blood pressure monitor, use an appropriately sized upper-arm cuff and follow its instructions; a cuff is for blood pressure readings only.
- Add questions as they occur to you. A note beside the latest results can help you remember what to ask at your next visit. NIDDK provides a kidney disease management guide with a test-results tracking document and appointment questions.
- Review trends with your care team. Compare results by date and ask what a change means for you and whether follow-up is needed. Do not treat a single result or app interpretation as a diagnosis or as a reason to change medicines, diet, or monitoring frequency on your own.
What your phone can—and cannot—tell you
A phone is useful for record keeping and appointment preparation. The official patient resources cited here describe tracking results and discussing them with a provider; they do not establish that a phone can measure eGFR or urine albumin. Those measures come from blood and urine testing.

Some kidney-related apps provide reference or educational information rather than measurements. The National Kidney Foundation describes its eGFR calculator app as a tool for medical professionals to estimate kidney function. Its relative-risk, monitoring and referral app information describes category-based reference guidance. These are not phone sensors or a substitute for an individualized plan from your clinician. App availability can change, so check the current listing and platform support before relying on a particular app.
Questions to take to your next appointment
NIDDK suggests asking questions such as:
- “What is my GFR? What does that mean?”
- “Has my GFR changed since last time?”
- “What is my urine albumin? What does it mean?”
- “Has my urine albumin changed since the last time it was checked?”
- “Is my kidney disease getting worse?”
- “Is my blood pressure where it needs to be?”
You can also ask how often you need blood and urine tests, how often to check blood pressure at home if recommended, and what change should prompt you to contact the care team. NIDDK advises patients: “Each time you get checked, ask your provider how the test results compare to the last results.” See NIDDK’s guidance and visit questions.

Why dated tracking matters
Kidney care relies on how results change over time, interpreted in context—not on an isolated phone entry. NIDDK says treatment goals include keeping GFR stable and urine albumin the same or lower; your clinician can explain what your own results mean and what action, if any, is appropriate. NIDDK’s CKD treatment information discusses these goals.
