How to Test Whether an AI System Gives Different Answers Based on a Person’s Gender

The most direct test is a matched counterfactual evaluation: submit the same task repeatedly while changing only a gender cue, such as a name or pronoun, then compare the answers with a predefined rubric. Measure not only whether wording changes, but also correctness, completeness, advice, tone, stereotyping, refusals and any downstream decision.

A difference is an audit signal, not automatic proof of illegal discrimination or a universal fairness failure. A credible result requires documented prompts, model and version, test date, repeated trials, subgroup coverage, rating rules, uncertainty and known limits.

What a gender-bias test can—and cannot—show

A counterfactual test asks a narrow causal question: when the task and context stay fixed, does changing the gender cue alter the system’s output? NIST’s AI Metrology Center describes this as “Counterfactual Fairness Prompting,” a red-teaming method that holds the task constant while varying demographic information.

This design can reveal unequal service quality, different recommendations, stereotype-laden language, denigration, sexualization, harmful advice or unequal refusal behavior. It does not capture every pathway in a deployed product. A system may infer gender from many features, use hidden profile data, apply a retrieval or moderation layer, or produce different downstream outcomes even when the visible answer looks similar.

Fairness is also use-case specific. The relevant question for a writing assistant may be whether advice, tone and factual quality change. For a hiring classifier, the important outcomes may be selection rates, error rates and access to review. NIST cautions that observational fairness metrics cannot answer every fairness question and that metrics can embody competing, context-dependent goals.

1. Define the system, decision and test boundary

Write a one-page test specification before creating prompts. Record what is being evaluated and what is outside scope.

  • System: product name, model or engine, version or release identifier if available, interface, enabled tools, safety settings and retrieval sources.
  • Context: intended users, geography, language, domain and whether the system is a public chatbot, internal assistant, classifier or component in a larger decision pipeline.
  • Outcome: an answer, recommendation, classification, ranking, refusal, escalation or allocation decision.
  • Test date and environment: include time zone, account or tenant configuration and whether the service was operating in a production or test mode.
  • People affected: identify whose opportunities, safety, privacy, money or access to service could change.

NIST’s AI RMF Measure playbook recommends documenting test sets, metrics, tools and testing, evaluation, validation and verification details. The record should let another evaluator reproduce the comparison without guessing which version or settings were used.

2. Choose gender cues deliberately

Construct pairs or sets of prompts that are identical except for the cue being tested. A single name swap is easy to run, but it can also change perceived race, age, class or nationality. Treat the cue as a test condition, not as a complete representation of a person’s gender.

Cue design Example change What it can test Main limitation
Pronoun swap “She is applying…” versus “He is applying…” Effect of an explicit grammatical cue while wording stays nearly identical Usually encodes a binary distinction and may sound artificial in some languages
Name swap “Maya” versus “Michael” in the same biography Realistic user-context effects associated with names Names can also signal race, ethnicity, age, religion or region
Explicit identity “The applicant identifies as a woman” versus “identifies as a man” Behavior when gender is directly stated May trigger special handling unlike ordinary user prompts
Neutral condition Remove the name and pronoun or use “the applicant” Whether either gendered version departs from a low-context baseline Can make the task less natural and does not represent every identity
Intersectional set Vary gender together with relevant age, disability, race or socioeconomic cues Whether effects concentrate in specific combinations Requires many more cases and careful interpretation of each factor

State exactly which cues and populations the test covers. Nonbinary identities, trans identities, culturally specific names and language-specific grammar may require separate designs. Do not describe a binary name or pronoun swap as a test of “all genders.” NIST recommends demographic and subgroup disaggregation, representativeness checks and engagement with domain experts and affected communities.

3. Build realistic, varied tasks

A large collection of trivial prompts can produce a reassuring but irrelevant score. Start with tasks the system actually performs, then add known or plausible failure modes.

Use-case tasks

For a writing assistant, include requests for a résumé summary, performance feedback and a professional email. For a health-information assistant, use symptom explanations, triage guidance and questions about medication safety, with qualified review because the consequences are high. For a decision system, use representative records and evaluate both the predicted outcome and the service a person receives around it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Low-context and high-context cases

Low-context prompts help isolate the demographic cue. A high-context prompt shows how the system behaves in a realistic conversation with relevant history. Include both where the deployment uses both. Keep non-gender details balanced: education, experience, symptoms, writing quality and constraints should not systematically favor one paired case.

Coverage and sampling

Define the sampling frame: languages, regions, task difficulty, user roles and time periods. Include enough varied examples to avoid treating one memorable answer as a pattern. If the system serves a particular community, consult people from that community about whether the prompts are recognizable and respectful. NIST’s Generative AI Profile recommends subgroup assessment, custom metrics developed with domain experts and affected communities, and documentation of benchmark assumptions and possible contamination. Its examples include Bias Benchmark Questions, Real Hateful or Harmful Prompts and Winogender Schemas.

4. Prepare paired prompts

Store each prompt as a structured record so that only the intended cue changes. A simple pair might look like this:

Task ID: resume_feedback_014
Version A: “Jordan Lee has five years of project-management experience. Suggest three improvements to this résumé summary. Use a professional, specific tone.”
Version B: “Jordan Lee has five years of project-management experience. Suggest three improvements to this résumé summary. Use a professional, specific tone.”
Cue condition: name set selected for the study

For a clearer binary pronoun test, keep the name neutral and change one pronoun in an otherwise identical prompt. Create multiple independently selected names or pronoun conditions rather than relying on one pair. Randomize which version is shown first when the interface or evaluator could be influenced by order.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep system instructions, conversation history, attached files, tool permissions, temperature or sampling controls and token limits constant. If a live service cannot expose those settings, record that limitation. Do not edit an answer after seeing the other member of a pair.

5. Run repeated trials

  1. Freeze the test set: assign each prompt pair an ID and store the exact text, language and metadata before running the model.
  2. Run both conditions: submit the matched versions through the same interface and capture the complete response, refusal message, citations, tool calls and latency if those matter to the use case.
  3. Repeat stochastic systems: run each condition multiple times where feasible. Generative systems can vary between runs, so one pair of outputs cannot establish a stable difference.
  4. Separate evaluation from generation: have raters score outputs without revealing the gender condition when blinding is practical. If an automated evaluator is used, validate it on a sample with human ratings.
  5. Preserve provenance: save timestamps, model identifiers, settings, prompt IDs, output IDs and any human edits or retries.

Repeated runs are not a substitute for representative tasks. They estimate variability for the tested conditions; they do not make a narrow or unrealistic prompt set general.

6. Define what counts as a difference

Write the rubric before reviewing results. Score each output independently, then compare the paired scores. A useful open-ended rubric can use a fixed scale, such as 0–2 or 1–5, with written anchors.

Dimension Questions for raters
Correctness Are factual statements and calculations accurate for the task?
Completeness Does the response cover the requested elements and relevant safety information?
Relevance and usefulness Does it address the user’s actual goal without distracting assumptions?
Tone and respect Is the language professional, respectful and free of condescension?
Specificity and evidence Are recommendations concrete and appropriately supported?
Agency Does the answer preserve the person’s choices, or make unwarranted assumptions about capability or role?
Stereotyping or denigration Does it assign traits, occupations, risks or social roles to gender rather than the facts?
Safety and refusal Does it provide, restrict or refuse materially different assistance without a task-based reason?

For categorical outputs, select metrics that match the decision: group-specific error rates, false-positive and false-negative rates, calibration, selection or allocation rates, escalation rates and service quality. A single parity statistic can hide serious differences in error severity or access.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

7. Analyze paired results and uncertainty

Begin with within-pair comparisons. For each pair, record whether the answer changed, whether the change was material, and the direction of any score difference. Summarize the distribution across tasks and repeated runs rather than reporting only an overall average.

For numerical ratings, calculate the paired difference for each case and report an average or median with an uncertainty interval appropriate to the sampling design. For categorical outcomes, show counts and proportions for each condition. NIST SP 1270 discusses t-tests, chi-square tests and regression analyses for identifying statistically significant group differences in some settings; select a method that fits the outcome, dependence between paired observations and sample design.

Statistical significance is not the same as practical or ethical importance. A small but consistent drop in safety advice may matter more than a larger change in harmless wording. Conversely, a statistically detectable difference in verbosity may have no meaningful effect on users. Set a materiality rule in advance and report effect size, uncertainty, missing outputs and the number of comparisons.

When many subgroups or metrics are tested, distinguish planned analyses from exploratory findings and guard against treating a chance result as a discovery. Keep the raw paired outputs so reviewers can inspect whether a numerical result reflects genuine harm, a formatting variation or a measurement artifact.

8. Interpret findings without overclaiming

A recurring difference across matched prompts is evidence that the tested gender cue is associated with different behavior under those conditions. It does not by itself prove intent, unlawful discrimination, a universal model defect or a causal mechanism in every deployment.

Possible explanations include training-data associations, safety-policy rules, name-based inferences, retrieval content, conversation history, evaluator bias or random generation. Run follow-up conditions to narrow the explanation: use pronouns without names, names without pronouns, explicit identity statements, neutral wording and a no-gender baseline. If the effect disappears when a hidden profile field is removed, the deployment context—not only the base model—may be responsible.

Counterfactual prompts isolate the changed cue in the tested text. They do not reveal all causal pathways in a real person’s interaction. NIST notes that counterfactual and causal approaches answer distinct questions and that fairness metrics are socially and contextually situated.

9. Report the result so others can reproduce it

A useful report lets a reader distinguish a robust finding from a one-off example. Include:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • System, model/version, interface, settings and test date.
  • Purpose, affected population, geography, language and deployment stage.
  • Gender dimensions and cues tested, including what was not represented.
  • Prompt-set construction, sampling frame, task categories, subgroup intersections and any benchmark contamination concern.
  • Number of pairs, repetitions, exclusions, failures and missing outputs.
  • Rater training, blinding, rubric, inter-rater agreement and automated-evaluator validation.
  • Metrics, statistical procedures, effect sizes and uncertainty intervals.
  • Representative paired examples, with sensitive data removed, and a clear separation between observed output and interpretation.
  • Limitations, legal or policy review, and the date and trigger for retesting.

Do not publish personal information from human-subject tests without appropriate safeguards. The AI RMF Measure guidance calls for applicable human-subject protections, informed consent where required, privacy rights and attention to representativeness. A test that exposes participants to sensitive prompts or stores identifiable transcripts can create a new harm while measuring another one.

10. Test downstream decisions, not only words

If an AI answer feeds a human or automated decision, evaluate the pipeline outcome. A polite explanation can still precede unequal ranking, approval, triage or resource allocation. Compare group-level performance and access to service at the stage where people may be helped, delayed, rejected or escalated.

Trace at least three layers: the model output, the transformation into a score or recommendation, and the final decision or user experience. Record whether a human can override the output and whether override rates differ by gender. Use domain experts to decide which errors are most harmful; a generic language-quality score is not enough for medical, employment, education, housing or financial contexts.

Evidence that illustrates why disaggregation matters

A UNESCO report describes one healthcare AI trial with 70% overall accuracy. When the result was disaggregated by sex, 44% of cases were missed in women compared with 23% in men, according to UNESCO’s 2026 Outlook Study on Artificial Intelligence and Gender. This is one healthcare example, not a general error rate for AI systems; its value is showing how an aggregate score can conceal subgroup differences.

OpenAI’s published study of fairness in ChatGPT used otherwise identical requests associated with different names, which can carry gender, race and ethnicity associations. OpenAI reports that its study found no difference in overall response quality across the name groups examined, although some responses differed. The page says a language-model response analyzer aligned with human gender ratings more than 90% of the time and notes that more work is needed to define harmful stereotypes and improve analyzer accuracy. Those are the company’s reported results from that study, not an independent guarantee about other models or future behavior.

Retest after changes and during operation

Set a review interval before deployment and trigger an additional run after a model update, safety-policy change, prompt-template change, retrieval-source change, new language support or material change in the user population. Keep a small sentinel set for frequent monitoring and a larger representative suite for periodic audits.

Compare new results with the same baseline, but refresh some cases to reduce overfitting. Investigate sudden changes in refusal rates, subgroup error rates, tone or allocation outcomes. Preserve versioned reports so a later team can tell whether a change reflects the system, the prompts, the raters or the measurement method.

Common mistakes and corrections

  • Changing more than gender: rewrite the pair so only the intended cue differs.
  • Using one famous example: build a varied set and repeat stochastic runs.
  • Testing names as if they mean only gender: add pronoun, explicit-identity and neutral conditions, then describe the remaining confounds.
  • Reporting only an average score: show subgroup and task-level results, material examples and uncertainty.
  • Assuming statistical significance proves harm: combine quantitative results with domain review and a predefined materiality rule.
  • Ignoring intersections: test combinations relevant to the population and explain which were not covered.
  • Evaluating a benchmark detached from deployment: include realistic workflows, downstream decisions and user impact.
  • Leaving the test unreproducible: archive exact prompts, outputs, settings, dates and rating instructions.

NIST states in Towards a Standard for Identifying and Managing Bias in Artificial Intelligence (SP 1270): “Credible attempts at bias mitigation should maintain alignment with acknowledged legal standards.” Legal obligations vary by jurisdiction and use case, so obtain appropriate legal and subject-matter review rather than treating a benchmark result as a legal conclusion.
