How to Test an AI-Generated App for Bugs, Security, and Accessibility

By Freedom251 Team · · 10 min read

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

An AI-generated app needs the same independent verification as any production software. Start with expected behavior and risk, then combine functional and regression tests, code and dependency review, security testing of the running app, and accessibility evaluation against a stated WCAG 2.2 target. Treat AI-written tests and clean scanner output as evidence from limited checks—not as proof that the app is correct, secure, or accessible.

Keep a human owner for every AI-assisted change. That person reviews the implementation and its tests, approves the release, and remains accountable for fixes and maintenance.

1. Define what must work before you run a tool

Generated code often follows an accidental interpretation of a vague prompt. Write the intended behavior separately from the implementation so the app is tested against requirements rather than against its own output.

Describe user-visible acceptance criteria

  • List the tasks users must complete, such as creating an account, signing in, uploading a file, submitting a form, or completing a purchase.
  • Specify the expected result, validation message, permissions, data changes, notifications, and recovery path for each task.
  • Record supported browsers, devices, locales, account types, and network conditions if they affect behavior.
  • Include failure expectations: what should happen with missing, malformed, duplicated, oversized, expired, or out-of-order input.

Map risk and trust boundaries

Identify where data crosses from a browser into an API, from one service into another, or from one user or tenant to another. Mark authentication, authorization, file handling, payment or personally identifiable information, administrative functions, secrets, and integrations with external services. Threat modeling at this stage can reveal design flaws that ordinary unit tests cannot. NIST IR 8397, published October 6, 2021, treats threat modeling as one part of a broad verification baseline and cautions that its techniques are not the totality of verification.

Set release priorities

Rank failures by likely user harm and exploitability. A cross-account data leak, authentication bypass, remote code execution path, or inability to submit a required form belongs ahead of cosmetic defects. Define which findings block release and who can accept residual risk.

2. Build a test matrix instead of relying on one suite

A useful plan assigns each requirement to several kinds of evidence. The same feature may need a unit test, an end-to-end journey, a source review, and an accessibility check.

Area Evidence to collect Questions to answer
Functional behavior Unit, integration, and end-to-end tests Does the feature produce the specified result for valid and invalid inputs?
Regression Existing suite plus tests for historical defects Did the change break a previously working path?
Implementation quality Diff review, static analysis, secret checks Are security-sensitive operations implemented and configured safely?
Dependencies Package, library, service, and lock-file review Are included components supported, patched, and actually needed?
Runtime security Black-box cases, fuzzing, authenticated tests, web scanning where applicable Can an attacker bypass controls or crash, corrupt, or expose data?
Accessibility Automated checks plus manual page, keyboard, process, and assistive-technology evaluation Can people complete important tasks, and does the implementation meet the selected WCAG target?

NIST’s recommended minimum standards page, created July 7, 2021 and updated March 12, 2025, describes these techniques as a baseline. Use the matrix to expose untested requirements rather than to claim that every possible defect has been covered.

3. Verify functional behavior and regressions

Run the existing tests, then inspect the test changes

Run the project’s normal test commands in a clean, reproducible environment. Capture the version of the runtime, operating system, database, browser, feature flags, and test data. A passing result is meaningful only when the suite actually exercises the changed code and its real dependencies.

Review the generated test diff line by line. OWASP’s Secure Coding with AI guidance warns that an agent may delete a failing test, weaken an assertion until almost any result passes, replace a real dependency with a mock, or write a test that simply asserts the buggy behavior it generated. Check that assertions verify exact business outcomes, authorization decisions, persisted data, emitted events, and user-visible errors—not merely that a function returned without throwing.

Cover normal, negative, and boundary cases

  • Normal use: valid accounts, expected formats, ordinary record counts, and successful integrations.
  • Missing and malformed input: blank fields, invalid encodings, unexpected types, truncated payloads, and unrecognized enum values.
  • Boundaries: minimum and maximum lengths, zero and negative quantities, date and time-zone edges, large files, pagination limits, and rate limits.
  • State and timing: retries, duplicate submissions, expired sessions, concurrent updates, interrupted uploads, delayed callbacks, and partial failures.
  • Authorization: each role, tenant, object owner, and unauthenticated state attempting the same operation.
  • Error paths: unavailable databases, rejected third-party calls, malformed responses, timeouts, and recovery after a failed transaction.

Preserve historical bug tests

Every fixed defect should become a regression case that fails on the old behavior and passes on the corrected behavior. Keep the original reproduction data and expected result where it is safe to do so. This prevents a later prompt or refactor from reintroducing the problem.

4. Review generated code, secrets, and dependencies

Inspect security-critical changes directly

Read the diff for authentication, authorization, session and token handling, input validation, output encoding, file paths, database queries, deserialization, cryptographic operations, logging, and error handling. Confirm that checks occur on the server or other trusted boundary; a client-side control is not an authorization barrier. Look for fail-open defaults, broad exception handlers, privilege decisions based on user-supplied fields, and sensitive values written to logs or responses.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Run static and secret analysis

Use static analysis appropriate to the language and framework to find common defects, unsafe data flows, injection risks, and unreachable or duplicated logic. Run heuristic secret checks against source, configuration, build artifacts, and commit history where permitted. Investigate every finding: scanners can report false positives, while unusual encodings or newly invented environment variables can evade simple patterns.

Account for included code and services

Inventory direct and transitive packages, generated modules, container images, APIs, hosted services, and build plugins. Review lock files and integrity metadata, remove unused components, and verify that versions receive security updates. NIST specifically calls for attention to included code such as libraries, packages, and services, not just the files an AI agent edited. Check default configuration, exposed ports, permissions, telemetry, and data retention for each dependency.

5. Attack the running application

Source review cannot show how deployed components behave together. Test a build that matches production configuration as closely as possible, with synthetic data and explicit authorization for any external target.

Exercise black-box behavior

Interact through the same interfaces an untrusted user can reach. Try invalid and expired credentials, altered identifiers, missing anti-replay values, unexpected content types, oversized requests, duplicate actions, and requests sent out of sequence. Compare responses, status codes, timing, and side effects between an authorized account, another user’s account, and an anonymous client. Verify that errors do not disclose stack traces, secrets, internal identifiers, or data belonging to another tenant.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fuzz suitable input surfaces

Fuzz parsers and input boundaries where malformed data could cause a crash, excessive resource use, memory corruption, query manipulation, or inconsistent validation. Start with isolated staging services and conservative rate limits. Preserve the exact seed, payload, environment, and response for each reproducible failure; fuzzing without triage creates noise rather than assurance.

Use a web application scanner when it fits the deployment

For an internet-connected web app, a scanner can identify issues visible from its test position. Authenticate it only with dedicated, least-privilege accounts and exclude destructive actions. Reproduce important findings manually, determine affected versions and data, and test the fix. A clean report means only that the scanner’s checks found nothing under those conditions; it is not a security certification.

6. Test accessibility against a declared WCAG 2.2 target

Choose the conformance level—A, AA, or AAA—and document which WCAG 2.2 success criteria apply to the product. WCAG 2.2 is a W3C Recommendation published October 5, 2023. Conformance is evaluated on complete pages and complete processes, not on an isolated component or a favorable screenshot.

Cover full pages and important journeys

Include the states needed to finish a meaningful task: initial and populated forms, validation errors, authentication, permission denials, loading and empty states, dialogs, confirmation pages, and recovery after a failure. For a sign-in, form submission, or checkout flow, test every page required to complete it. A single inaccessible step can make the process unusable even when its individual widgets pass an automated scan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use automation for detectable conditions

Run an accessibility evaluation tool over representative routes and states. Automation is effective for some machine-detectable conditions, such as missing structural relationships or certain contrast and naming problems, but W3C’s guidance on selecting evaluation tools states that no tool can check every accessibility aspect automatically. Record the tool version, ruleset, URL, viewport, account state, and exclusions so results can be reproduced.

Perform human evaluation

  • Navigate core tasks with a keyboard alone. Check visible focus, logical focus order, no keyboard traps, usable skip mechanisms, and operable menus, dialogs, and drag alternatives.
  • Inspect whether alternative text conveys the purpose of meaningful images and whether decorative images are treated as decorative.
  • Confirm that labels, instructions, error messages, and status changes communicate purpose and state, including for screen-reader users.
  • Check headings, landmarks, table relationships, language, zoom, reflow, text spacing, pointer targets, and orientation at the sizes and layouts your product supports.
  • Use more than one browser and, for core journeys, a screen reader or other assistive technology used by your audience.
  • Ask people with disabilities to perform realistic tasks when feasible. W3C recommends usability testing in addition to functional conformance testing because conformance alone does not establish that a product is usable.

Do not describe an app as fully compliant solely because an automated report is clean. State the target, pages and processes examined, exceptions, manual methods, and unresolved issues.

7. Choose tools and review depth deliberately

There is no universally best scanner or test stack. Compare candidates against the application and the evidence you need to preserve.

Decision axis What to check
Technical coverage Supported language, framework, runtime, browser, database, API style, and deployment model.
Inspection position Whether the tool examines source, dependencies, secrets, configuration, a running service, or several of these.
Workflow integration Pull-request and CI support, baseline handling, policy gates, and behavior on generated files.
Evidence quality Stable rule identifiers, source locations or request traces, severity rationale, export, and reproduction instructions.
Data and deployment constraints On-premises operation, retention, access controls, handling of proprietary code, and safe staging credentials.
Accessibility scope WCAG criteria covered, page states visited, authentication support, manual-review prompts, and process-level reporting.

Use several complementary techniques rather than selecting a tool because it reports a high issue count. A tool that cannot reach an authenticated state or dynamic error message may miss the most important parts of your app.

8. Triage, fix, and retest every finding

Record reproducible evidence

For each issue, record the affected requirement or success criterion, environment and build, account or role, exact steps or request, observed and expected results, evidence such as logs or screenshots, severity, user impact, and owner. Keep sensitive test data out of tickets. A clear reproduction path lets a developer verify both the defect and the fix.

Fix in risk order

Block release for critical security defects, cross-user exposure, authentication or authorization bypasses, data loss, exploitable injection, crashes in essential paths, and failures that prevent a required user group from completing a core task. Fix lower-severity findings according to impact, reach, and effort, and document an explicit risk decision when an issue remains.

Retest the fix and nearby behavior

  1. Reproduce the original failure on the affected build.
  2. Apply the smallest safe change and add or update a regression test.
  3. Rerun the focused test, then the relevant integration and end-to-end suites.
  4. Repeat the security or accessibility check that found the issue and inspect adjacent states for regressions.
  5. Run the broader release suite and preserve results, tool versions, configuration, and commit identifiers.

Keep test history so a later AI-assisted change can be compared with a known baseline. Assign a human reviewer to approve the final diff, including generated tests and configuration.

9. Add safeguards when the product itself contains AI

Ordinary application testing remains necessary. If the product includes a model, agent, retrieval system, prompt workflow, or AI decision, add controls for that AI-enabled behavior rather than assuming the generator’s testing covers it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OWASP’s AI Security Verification Standard (AISVS) version 1.0, released in June 2026, is a catalogue for AI-enabled systems across lifecycle stages. It contains 191 requirements across 12 chapters and three appendices, with each requirement assigned verification level 1, 2, or 3. Use the level that matches the system’s risk and treat the catalogue as an additional security-verification reference, not a substitute for application, infrastructure, privacy, or accessibility testing.

10. Common failure modes and how to recover

The generated suite is green but the app is wrong

Inspect assertion strength and test-data realism. Restore deleted cases, replace inappropriate mocks with integration coverage, and write tests from the acceptance criteria and negative cases rather than from the implementation.

A scanner reports many issues with no usable proof

Capture the rule, request or code location, configuration, and reproduction steps. Deduplicate related findings, validate the highest-impact items manually, and tune exclusions only after confirming that the excluded pattern is safe.

The accessibility score is high but users cannot finish a task

Test the complete process with keyboard and assistive technology. Check focus movement, labels, error recovery, dynamic announcements, and states the automated crawl did not reach. Reframe the result as partial evidence rather than a conformance claim.

Security testing was done only on source code

Exercise the deployed interfaces with role-separated accounts, malformed requests, expired credentials, and concurrency or replay cases relevant to the feature. Source analysis cannot establish runtime configuration or cross-service behavior.

No one owns the decision to ship

Name a human owner for each AI-assisted change, require attributable review of security-critical code and tests, and make unresolved risk acceptance explicit. Accountability cannot be delegated to a generator or a dashboard.

11. A repeatable release checklist

  • Requirements, acceptance criteria, trust boundaries, data classes, and release-blocking risks are documented.
  • Normal, negative, boundary, state, authorization, error, and historical-defect tests exist and pass.
  • Generated test diffs were reviewed for deleted cases, weakened assertions, excessive mocking, and assertions of buggy behavior.
  • Security-sensitive code, static-analysis findings, secrets, dependencies, configuration, and included services were reviewed.
  • Runtime black-box, fuzzing, authentication, authorization, and applicable web-scanner checks were completed and triaged.
  • A WCAG 2.2 level is stated; complete pages and processes were assessed with automation and human evaluation.
  • Keyboard operation, focus, labels, alternative text, dynamic states, zoom or reflow, and assistive-technology behavior were checked for core tasks.
  • Critical findings are fixed or explicitly blocked; every fix has a regression test and a retest record.
  • A named human reviewed and approved the release, with evidence retained for the build and tools used.
