Running shoes could be contributing to foot or knee pain if symptoms began after a shoe change, a pair feels tight or rubs, or the soles show uneven wear. Those clues do not prove the shoes are the cause: training changes, hills, surfaces and injuries can also trigger pain. Check your shoes without running through pain, ease off activity that hurts, and seek medical advice for severe, persistent or worsening symptoms.
Could my running shoes be causing my foot pain?
Possibly, but pain location alone cannot identify the cause. Heel or sole pain can occur after a sudden increase in running, uphill running, or wearing shoes that are worn or do not support you comfortably. The NHS describes heel pain as sharp pain when putting weight on the heel; plantar fasciitis can also cause gradual, lower-grade heel pain. Pain around the ball of the foot is another pattern seen in running problems. These descriptions can help you explain symptoms to a clinician, but they do not show that footwear caused them.

Could my running shoes be causing my knee pain?
A shoe change is one possible contributor to pain around the kneecap, but knee pain has many causes. AOSSM lists increased mileage, a change in terrain and a running-shoe change among possible contributors to patellofemoral pain. Hospital for Special Surgery describes patellofemoral pain around the kneecap, patellar tendon pain below it, and iliotibial (IT) band pain often on the outside of the knee. Where it hurts is useful information, not proof that your shoes are responsible.

What clues can help you assess your shoes?
Make a timeline
Write down when the pain started, which pair you were wearing, when you switched shoes, and roughly how much you have used that pair. Also note recent changes in weekly distance, pace, hills, running surface, or your return-to-running schedule. A close timing link makes the shoes worth checking, but it cannot establish cause: AOSSM also identifies rapid mileage changes, hills and interval training as common sources of running injuries.

Check fit without running through pain
A shoe should feel comfortable from the start. Your toes should be able to move, the toe box should have room, and the heel should stay secure without slipping. Walking or taking a few comfortable steps can help you notice pressure or rubbing; do not keep running to test a pair if that brings on pain. FootCareMD’s athletic-shoe guidance covers fit and shoe selection.

Place both shoes on a flat surface and compare them. Look for uneven wear and creasing in the midsole. Cushioning and support materials can wear down even when the outside does not look obviously damaged, while aches do not automatically mean a shoe is worn out. Northwestern Medicine’s running-shoe fit guidance discusses material wear, and Nationwide Children’s Hospital’s shoe guidance describes wear clues.

How many miles do running shoes last?
Published recommendations differ, so mileage is a rough inspection prompt, not a replacement deadline or a way to diagnose pain.
| Source | Suggested range | Qualification |
|---|---|---|
| FootCareMD / American Orthopaedic Foot & Ankle Society | 300–500 miles | Year not stated on the source page. |
| Northwestern Medicine | 300–500 miles | Year not stated on the source page. |
| AOSSM | 500–600 miles | Year not stated on the source page. |
How quickly a pair wears depends on its use, running surface, body weight and shoe design. New aches or visible uneven wear are reasons to inspect the shoes, but neither proves they caused the pain.

What should I do if I suspect my shoes are involved?
- Stop or modify running that hurts. Do not provoke symptoms to compare shoes.
- Review recent changes. Consider footwear alongside mileage, pace, terrain, surface and time away from running.
- Check fit and condition. Look for pressure, rubbing, poor heel hold, limited toe room, uneven wear or midsole creasing.
- Avoid changing several things at once. If you are able to run without pain and it is safe for you to do so, returning to a previously comfortable pair may offer a clue, but it is not a validated diagnostic test.
- Get medical advice when needed. Do not keep switching shoes in place of an assessment for persistent, worsening, focal or severe pain.
There is no home test established by these sources that can prove an individual shoe caused an injury. Evidence also does not support confidently claiming that a particular cushioning, stability or minimalist feature will prevent or cause an individual runner’s injury. FootCareMD says data do not establish whether cushioned or minimalist shoes are better. A 2022 review discusses continuing uncertainty in footwear and injury research, including limited evidence about upper components and the need for more research on insoles and outsoles: Agresta et al., “Running Injury Paradigms and Their Influence on Footwear Design Features and Runner Assessment Methods”.
When should I stop running and seek care?
Stop running if it causes knee pain. The NHS advises contacting a GP or physiotherapist if knee pain remains after a week of rest, and seeking assessment for heel pain that does not go away. Substantial swelling around the heel or sole warrants prompt care. AOSSM recommends immediate medical treatment for severe pain, swelling, loss of motion or a change in running form. If symptoms are persistent, worsening or severe, seek medical evaluation rather than trying to solve them by repeatedly changing shoes.

For fit, footwear can be personal: Northwestern Medicine sports-medicine physician Steven E. Mayer, MD, says, “Your foot structure, the way you move and the activities you do all play a role in determining which shoe will feel best and help keep you comfortable.” A suitable pair should meet observable needs—comfortable fit, toe room, secure heel hold, appropriate use and acceptable wear—rather than a presumed ability to prevent injury.
