Asking someone else whether they notice it, changing rooms, and tracking when it recurs can offer clues—but none rules out a real odor or an environmental hazard. If smoke, gas, or a chemical release is plausible, follow local safety instructions and get to safety rather than staying to test the smell.
What “phantom smell” means—and what it does not mean
Phantosmia is smelling an odor that is not present in the environment. The perception is real as an experience; the term does not mean someone is pretending or that the sensation is “imaginary.” It describes a smell perception for which no odor source is present.

Two related terms describe different changes. Parosmia is when an odor is present but smells different from how it normally does. Hyposmia is reduced smell, while anosmia is complete loss of smell. These terms classify the kind of smell change; they are not a home diagnostic test. The National Institute on Deafness and Other Communication Disorders (NIDCD) explains these smell disorders.
What clues can help you investigate an odor?
Use observations to describe the experience, not to declare it phantom. Another person may not notice an odor that is real but localized, intermittent, or below their detection threshold. A smell that follows you between rooms or recurs without an apparent source may be worth mentioning to a clinician, but neither clue proves its cause.

- Ask another person whether they notice the odor, while remembering that a different answer does not settle whether a source exists.
- Notice whether the smell changes when you move to a different location or situation.
- Record when it happens, how long it lasts, and whether it recurs.
Do not rely on smell alone to check for danger. The NHS guidance on lost or changed smell warns that smell loss can make it harder to detect gas leaks, fires, or spoiled food. Use appropriate independent alarms, professional checks, and local emergency or utility instructions when relevant.
What phantom smells can be like
Phantom odors may be pleasant or unpleasant, occasional or persistent, and noticed in one nostril or both. The smell’s character does not identify its cause. A particular odor, by itself, cannot establish whether the experience is phantosmia or point to a specific medical condition, according to Mayo Clinic and Cleveland Clinic.

Possible causes—and why a list is not a diagnosis
Possible causes include recent respiratory or sinus illness, head injury, aging, certain medicines, and neurological conditions. Mayo Clinic also lists temporal lobe seizures, inflamed sinuses, brain tumors, Parkinson’s disease, and COVID-19 among possible causes of phantosmia. NIDCD discusses a broader range of causes of smell disorders, including smoking, nasal growths, hormonal disturbances, dental problems, chemical exposures, radiation treatment, and nervous-system conditions.

When to contact a healthcare professional
Seek medical advice for a persistent or concerning smell change. The NHS advises seeing a GP if smell has not returned to normal in a few weeks. Cleveland Clinic advises contacting a healthcare provider if phantom smells last more than three weeks. These are recommendations from different sources, not a single universal cutoff.
NHS Scotland’s clinician guidance says isolated phantosmia without focal neurological symptoms or signs does not by itself call for neurological investigation or treatment, and describes phantosmia as nearly always benign. If a gas leak, fire, chemical release, or poisoning is credible, prioritize immediate safety and follow local emergency and utility instructions. Do not stay in a potentially dangerous place to find out whether other people can smell the odor.

What a clinician may do
Assessment typically starts with questions about the smell and health history, followed by an examination of the ears, nose, and throat. A health professional may also conduct a supervised smell test. Depending on the circumstances, a clinician may consider nasal endoscopy or CT or MRI imaging; these tests are not necessary for everyone. Before an appointment, note when episodes began, how often they occur and how long they last, whether one or both nostrils are involved, and whether the odor changes by location or situation. Include recent illness, head injury, dental or sinus symptoms, medication changes, possible chemical exposure, and any other symptoms. These details can help a clinician understand the pattern; they do not diagnose it on their own.
